Prolonged retention of fetal bones: intrauterine device and extrauterine disease.

A case of prolonged retention of fragments of fetal bones is reported in a 47-year-old woman. The pieces of bone, retained in utero for 8 years after an elective abortion, were unexpectedly discovered after total hysterectomy with adnexectomy performed for bilateral pyosalpinx, persistent vaginal discharge and severe pelvic pain. The role of this rare complication as a cause of infertility is also discussed.

[Analysis of 138 cases of superior oblique muscle paralysis].

Congenital or early infantile paralysis of the superior oblique muscle is primarily caused by congenital anomaly of the ocular muscles or by birth trauma, while the acquired is mostly caused by trauma, influenza, or acute infection. The diagnosis is done by Lancaster's test, the synoptophore, and the cover test in combination with prisms. For incooperative children, Park's three step test may be used. The angle of strabismus can be measured with the cover test and prisms, or by the synoptophore. For treatment, the patients were divided into (1) the group of prism correction, (2) the group of inferior oblique attenuation in the same eye, (3) the group of inferior rectus attenuation in the other eye, (4) the group of inferior oblique attenuation in the same eye combined with inferior rectus attenuation in the other eye, and (5) the group of superior oblique paralysis complicated with horizontal heterotropia. The immediate and longterm curative effect had been satisfactory. It is important to differentiate congenital cases from the acquired, the primary torticollis from that of ophthalmic origin, and the paralysis of superior oblique in one eye from the paralysis of the superior rectus in the other eye.

Influence of allogeneic bone marrow transplantation on the endocrine system in children.

With increasing survival rates of children grafted for different malignancies concerns about the longterm side effects of this treatment are growing. Therefore, investigations on the function of endocrine systems were conducted in a total 28 patients grafted for various reasons: ALL (N = 18), AML (N = 1), SAA (N = 3), CML(N = 4), neuroblastoma (N = 2). The results can be summarized as follows: 1. The extent of hormonal derangements is primarily dependent on the extent of irradiation prior to BMT. Integrity of hormonal systems was found in cases without irradiation (SAA) or if TBI did not exceed 3 Gy. 2. Primary hypogonadism was present in 18 patients. 3. Primary hypothyroidism was present in 2 patients. 4. Growth impairment was observed in 8 patients. In four of these cases growth hormone deficiency was the cause. In four other cases with graft-versus-host-disease and hepatic involvement SmC/IGF I levels were severely diminished. The data suggest that in most cases BMT itself has relatively few negative effects on the endocrine regulatory system. However, more detailed investigations before and after BMT will be needed to further validate these observations.

Global surveillance and forecasting of AIDS.

The short-term forecasting of future AIDS cases has been attempted by statistical extrapolations of the observed curve of reported AIDS cases. In areas where such reporting is very incomplete or has only recently started, extrapolation is not possible and an epidemiologically-based forecasting model has been developed to estimate the annual number of AIDS cases which may have occurred and to project the annual number and distribution of AIDS cases for up to ten years. This model, which relies on the current understanding of the epidemiology and natural history of HIV infections and on the available HIV serologic survey data, is used to provide estimates and short-term projections of AIDS cases for the USA, Europe, Africa and the world. Because of the very long (mean of 8-9 years) incubation period between HIV infection and the development of AIDS, new cases over the next five years will be mostly derived from persons who became infected with HIV in or before 1987. WHO has estimated that 5-10 million persons worldwide were infected with HIV in 1987. Based on the lower estimate of 5 million, the cumulative number of AIDS cases which can be projected for the end of 1991 is over one million, and for the mid-to-late 1990s could reach 2 to 3 million. HIV/AIDS will therefore be an increasing public health problem throughout the world. Health care systems everywhere will have to be strengthened to respond to this large toll of disease and death due to AIDS.

Arthritis related interests in spinal cord injury.

National concern about traumatic spinal cord injury almost 2 decades ago led federal authorities to establish the Regional Spinal Injury Care Systems and a common database to study this condition. Thirteen years of collaborative data collection have provided valuable experience and information for the planning of spinal cord injury research. This approach may also apply to other categories of patients who have multiple variables associated with the clinical picture, treatment and outcome, and in whom the demonstration of treatment effectiveness requires longterm studies of large numbers of patients. Certain complications seen in patients with spinal cord injury including heterotopic ossification, joint changes, and dermal fibrosis, may provide models for the understanding of certain bone and collagen diseases.

Abortion in adolescence.

Sexual attitudes and behavior of adolescent females have been the topic of much interest over the past decade. Feelings about contraception, conception, and abortion have been described in relation to the adolescents' beliefs about the possibility of becoming pregnant, who will or will not "protect" them, and the influence of significant others on their decision making. This study explores differences in 35 women who had abortions during their teenage years with 36 women whose abortions occurred after the age of twenty. A demographic questionnaire, the Millon Clinical Multiaxial Inventory, and the Beck Depression Inventory were completed by women who were members of a patient-led support group. Premorbid psychiatric histories, the decision-making process itself, and distressing symptoms postabortion are reported. Specific differences in perceptions of coercion, preabortion suicidal ideation, and nightmares post-abortion were found in the adolescent group. Antisocial and paranoid personality disorders as well as drug abuse and psychotic delusions were found to be significantly higher in the group who aborted as teenagers. Hypotheses regarding the influences of adolescent development on mother/child relationships, power struggles, and the use of fantasy as a coping device are explored.

[Epidemiologic factors in cervical intraepithelial neoplasia III in our experience].

The study of 52 cases of cervical intraepithelial neoplasia seen from 1980 to 1984 shows that early sexual activity increases the relative risk 3.35 times if the intercourse takes place before 20 years of age, and 2.5 times if before 15 years. The use of IUD increases the risk 2.77 times. Parity, civil status and gynecologic infections do not increase the risk. Cervical intraepithelial neoplasia is more frequently diagnosed in women with a higher socio-economic status, due to a better health care.

Sperm autoantibodies as a consequence of vasectomy. II. Long-term follow-up studies.

Thirty-four out of fifty-two vasectomized men studied previously were studied again about 5 years after vasectomy for sperm-agglutinating and sperm-immoblizing antibodies, and for antibodies to human protamine. Only one man lost his already weak antibody activity, whilst one out of eight men, negative at 1 year, appeared to be positive at 5 years. Slightly higher titres in one of the three sperm antibody tests were found in the sera of about 30% of the men. Only in three (9%) was there a rise of two or more steps in more than one technique. Agglutinins and immobilizins were shown to be strongly correlated, as was the case with antibodies to human protamine and head-to-head agglutinins. Seminal plasma sperm agglutinins were detected in the samples of only four (out of thirty) men, in low titres. Circulating immune complexes tested with various techniques were only found in a few sera and not consistently. This prolonged study shows that sperm autoantibodies formed within 1 year after vasectomy are persistent. Their role in remaining infertility after reanastomosis requires further study.

Review of epidemiologic studies of endometrial cancer and exogenous estrogen.

Epidemiologic literature on the association of exogenous estrogen and endometrial cancer is reviewed. Descriptive studies have documented fluctuations in the incidence of endometrial cancer, mainly of localized disease, associated with estrogen use. Etiologic studies have established an association between estrogen use during menopause and the occurrence of endometrial cancer. Although the association appears to be a valid one, several biases may have falsely increased the magnitude of this association. The association also appears to be strongest for local disease and weakest for the most invasive disease, which implies that the etiology for the more invasive endometrial cancers is largely unaccounted for by estrogen use. A need for a prospective study to define other potential risks and benefits of estrogen therapy is clear. However, appreciation of factors known to modify the risk of endometrial cancer from exogenous estrogen can help the clinician to use these preparations judiciously.

Surgical correction of sequelae of pelvic inflammatory disease.

Early diagnosis of pelvic inflammatory disease (PID) and treatment with appropriate doses and types of antibiotics that take into account the polymicrobial etiology of this condition are essential to prevent future infertility. That PID frequently is subacute or relatively asymptomatic argues in favor of a more liberal use of laparoscopy in the diagnosis of this condition. The sequelae of PID that cause infertility can be assessed with the judicious use of hysterosalpingography and laparoscopy. These procedures are complementary. Hysteroscopy is of more limited value. Operative laparoscopy has proven to be useful in restoring fertility in appropriately selected cases. In many instances laparotomy can be replaced successfully with laparoscopic procedures, which offer many advantages. The introduction of microsurgery has significantly improved the success rate associated with reconstructive infertility operations. The successes associated with a new therapeutic modality, in vitro fertilization (IVF) and embryo transfer (ET), although modest, have been improving in the past three years. Reconstructive microsurgery and IVF/ET are complementary treatments for infertility. A thorough investigation of the infertile couple, with meticulous attention given to the various fertility parameters, individualization and a rational approach to treatment with these two modalities will undoubtedly improve the overall results.

Sleep without drugs.

Disturbed sleep is a common problem, particularly among elderly people, and is usually treated with hypnotics. The side effects of longterm administration of hypnotic drugs are well known, but despite this there remains a substantial population of chronic users. These people can be helped to reduce their dependence on hypnotics through psychological techniques. A group of longterm users treated in this manner were shown to reduce their intake of hypnotics significantly more than a group of users who did not receive any psychological treatment. Furthermore, the treated patients did not experience any deterioration in their sleep patterns, and their subjective refreshment from sleep improved significantly. For the patient with sleep problems, psychological techniques are preferable to the longterm use of hypnotics both as a weaning-off agent and as an alternative to drugs.

Acne rheumatism. Report of a case.

A case of acne conglobata with myo-osteoarticular involvement is described. The occurrence of a subacute, symmetrical polyarthritis with radiological changes was the unique feature of this patient. The immunologic work-up demonstrated impairment of delayed hypersensitivity. Control of the systemic symptoms was obtained initially with corticosteroids and during a relapse with indomethacin. Clofazimin was effective in longterm treatment of the acne. Following cessation of all therapy, there has been no evidence of recurrence of either acne or rheumatic symptoms.
