We present an unusual case of a primary lumbar disk-space mass that presumably developed secondary to a chronic hyperextension spinal fracture associated with spinal stenosis. This injury resulted in the appearance of a lumbar intervertebral disk-space mass or pseudotumor. The pseudotumor most likely resulted from a prior spinal fracture, leading to a fused hyperextension deformity in a patient with underlying chronic degenerative spinal disease.
We report a case of a secreting retroperitoneal paraganglioma which developed on the organ of Zuckerkandl. Paraganglioma is a rare tumor of the paraganglioma system arising from undifferentiated cells of the neural crest. The originality of our observation comes from the secreting character of the tumor and the malignity proved by the local spreading to the wall of the inferior vena cava and the metastasis. The probable existence of a double primary localisation is also exceptional.
Sarcomatous transformation in chordoma is a very rare condition and has been emphasized as a distinct entity because of its more aggressive clinical course. Here we describe a case of dedifferentiated chordoma arising from the skull base region of an 11-year-old boy, with tumor recurrence within one year. This tumor showed features of pleomorphic cell sarcoma with areas more typical of chordoma. Most of tumor cells expressed cytokeratin, epithelial membrane antigen, vimentin and S-100 protein, thus confirming the diagnosis of dedifferentiated chordoma.
The authors report a case of large retroperitoneal schwannoma with osteolysis of the pedicle of L3, submitted to complete excision by partial vertebrectomy of L3 with a combined approach. Schwannoma is a hamartoma constituted by Schwann's cells, that causes non-specific clinical manifestations. Image diagnosis provides information as to the relationships that the neoformation contracts with the surrounding structures, but a definitive diagnosis is exclusively anatomopathologic. Retroperitoneal localization is rather rare (0.5-5% of cases). Treatment is represented by surgical excision. Because it is a benign, slow-growing neoplasm, when dealing with cases that are difficult to treat surgically or when complete excision means disabling neurovascular lesions, we may opt for the incomplete removal of the mass, as long as the case is carefully monitored over time.
Neck pain is a common complaint of patients seeking care in the outpatient setting, and the cases seen vary widely in severity and cause. A careful history and physical exam, followed by appropriate imaging studies, are essential for the orderly work-up and management of neck pain in the ambulatory patient. Available imaging studies include plain film radiography, computed tomography (CT), magnetic resonance, and CT myelography. The general considerations necessary to select the appropriate imaging study are discussed for a broad spectrum of common disorders.
Neck pain can occur in several circumstances: traumatic, spontaneous, associated or not with motion, with or without head or upper limb irradiations. Each case requires appropriate clinical examination and radiographs. CT and MRI can be used to obtain additional information. Myelography and arteriography are exceptionally used. Cervical discography and facet joint arthrography are used therapeutically. After a brief anatomical review, normal and pathological patterns will be reviewed using radiographs. Each circumstance is studied: traumatic, degenerative, inflammatory and tumoral. It is emphasized that discogenic cervico-brachial neuralgia usually has a favorable spontaneous outcome. A special chapter is dedicated to calcifying and ossifying diseases of the cervical spine. Cervico-occipital neuralgia is also discussed.
Extradural ependymomas arising from filum terminale externa in the sacrococcygeal region are very rare. Since 1937, out of 58 cases reported in English literature, only 13 cases of tumor located presacrally in the retrorectal space, have been reported. The authors report a 38 yrs old male with a primary ependymoma of the sacrococcygeal region causing extensive sacral destruction.
The paper presents the treatment mode of metastasis to the spine. Between 2000 and 2002 5 cases (3 men and 2 women) with metastasis to the lumbar spine were treated surgically. The metastasis derived from primary foci in the prostate (3 cases), in the breast (1 case) and in the kidney (1 case). The average age of the patients was 65 years (ranging from 53 to 74 years). In all cases a vertebrectomy was performed through an anterior approach. Titanium baskets filled with bone cement were implemented. The spine was stabilized using anterior Kaneda instrumentation (3 cases) and the posterior Omega instrumentation (2 cases). This mode of treatment yielded good results. Pain relief was achieved by decompressing the neural elements, leading to better quality of life among the operated patients.
