Frequency of N. gonorrhoeae, T. vaginalis, and C. albicans in female venereological patients. A one-year study.

The frequency of Neisseria gonorrhoeae, Trichomonas vaginalis, and Candida albicans has been studied over a period of one year in women attending a venereal diseases clinic. A total of 1,347 women were investigated, all coming from the same catchment area. Gonorrhoea was established at the first visit in 506 patients (38 per cent.), who constituted 97.5 per cent. of the total number of cases of gonorrhoea. Trichomonas vaginalis was found in 272 (20 per cent.) and Candida albicans in 233 (17 per cent.). 176 patients (13 per cent.) had more than one pathogen. Of the patients attending, 22 per cent. (292 women) were so-called "named contacts". The frequency of gonorrhoea established at the first visit in these patients (64 per cent.), was significantly higher, but the frequency of symptoms did not differ from that in other gonorrhoea patients. The number of asymptomatic cases was so large that a single compulsory examination is undoubtedly very useful from the epidemiological point of view, but the value of repeated specimen collections for gonorrhoea is debatable. Complications of gonorrhoea were observed in 29 patients (6 per cent.) at the first visit.

Secnidazole. A 5-nitroimidazole derivative with a long half-life.

The therapeutic activity of a single 2 g dose of secnidazole was studied in patients with urogenital trichomoniasis. In 140 patients, 97% were cured and the drug was well tolerated. In the laboratory, tests on sensitivity were made and the minimal inhibitory concentration (MIC) and the minimal trichomonacidal concentration (MTC) were determined on cultures that had recently been isolated at the clinic, and the pharmacokinetic properties of secnidazole in man were compared with those of tinidazole. The therapeutic efficacy of all the metronidazole derivatives was reviewed and a single-dose treatment is recommended. Therapeutic and prophylactic treatment is achieved by products with a long half-life. Secnidazole, with a half-life of 14.3 +/- 1.3 h (women) and 20.2 +/- 3.1 h (men), is particularly suitable for this type of treatment.

Venereal trichomoniasis: role of men.

It has been suggested that high zinc concentrations found in men may prevent Trichomonas vaginalis from being established in the male reproductive tract. In this investigation T vaginalis was readily killed at concentrations of zinc that occur in the prostatic fluid of healthy men (minimum trichomonacidal concentration (MTC) of 6.4 mmol/l). T vaginalis was also shown to be killed by human prostatic extracts as well as by human seminal fluid, even when the zinc content was much lower than the MTC for T vaginalis. It seems likely, therefore, that there are at least two antitrichomonal mechanisms in the male reproductive tract, one being zinc dependent and the other not relating to zinc content. Tritrichomonas foetus, which causes venereal trichomoniasis in cattle, was unaffected by bovine seminal fluid and was killed by zinc only at concentrations far higher than those found in the prostatic fluid in the bull (MTC 200 mmol/l).
