Clinics in diagnostic imaging (91). Subarachnoid haemorrhage.

Computed tomography (CT) is the initial radiological investigation of patients with an acute neurological event. A 64-year-old woman presenting with generalised weakness and headache for two days was diagnosed on CT to have subarachnoid haemorrhage. Digital subtraction angiography confirmed the cause to be a ruptured posterior communicating artery aneurysm. The patient was treated by neuroradiological intervention using occlusive coils. The CT features of subarachnoid haemorrhage are discussed. Accurate CT interpretation is essential to direct appropriate investigations and management in patients with stroke, particularly as acute cerebral infarction may occasionally mimic subarachnoid haemorrhage. The role of magnetic resonance (MR) imaging in evaluation of cerebral infarct is also discussed.

American Dietetic Association's Standardized Nutrition Language: Project logic model and current status.

Standardized terminology and digital sources of evidence are essential for evidence-based practice. Dieticians desire concise and consistent documentation of nutrition diagnoses, interventions and outcomes that will be fit for electronic health records. Building on more than 5 years of work to generate the Nutrition Care Process and Model as a road map to quality nutrition care and outcomes, and recognizing existing standardized languages serving other health professions, a task force of the American Dietetic Association (ADA) has begun to develop and disseminate standardized nutrition language. This paper will describe the group's working logic model, the Nutrition Care Process, and the current status of the nutrition language with comparisons to nursing process and terminology.

[Aneurysm of the popliteal artery: diagnostic and therapeutic considerations].

The authors settle the roles of the most recent diagnostic methodologies useful to frame the popliteal aneurysm pathology namely digital arteriography, N.M.R., and doppler-flowmetry. They emphasize, in an observed case, the adopted therapeutic criteria, that is basically interventional. The thinness of saphenous veins draws toward the employing of "Goretex".

Clinical application of percutaneous transluminal angioscopy. Comparison of findings in percutaneous transluminal angioplasty, thrombolysis, thrombus-extraction and stent-application.

Since the introduction of ultrathin angioendoscopes into clinical application, it has been possible to subject the findings of conventional or digital angiography to objective verification. In dogs and in patients, this new angioscopic method for visualization of arteries without surgical intervention has been carried out. The approach is transfemoral. Endoscopy is combined with angiography and, when indicated, percutaneous transluminal angioplasty (PTA), thrombolysis, thrombus extraction and insertion of a new type of stent. An ultrathin endoscope with an outer diameter of 1.6 mm and a working channel of 0.35 mm is used. Guidewires, contrast media and drugs for local thrombolysis can be employed through this channel. The endoscope is placed in the region of interest under fluoroscopy and direct visualization is obtained by using 0.9% NaCl for decreasing blood flow. Image recording is achieved by video or high-speed camera. With this method, the intraluminal events of dilatation, recanalization, local lysis, stent application and thrombus extraction can be visualized.

Analysis of zygapophyseal joint cracking during chiropractic manipulation.

OBJECTIVES: To determine if there is a relationship between the side of head rotation and the side of joint crack during "diversified" rotatory manipulation of the cervical spine. DESIGN: Randomized experimental study. SETTING: Macquarie University, Centre for Chiropractic, Summer Hill, New South Wales. SUBJECTS: Fifty asymptomatic subjects were recruited from the students and staff of the above college. INTERVENTION: Single, unilateral "diversified," high velocity, low amplitude, rotatory thrust technique. MAIN OUTCOME MEASURES: Joint crack sound wave analysis of digital audio tape (DAT) recordings, taken from two skin mounted microphones positioned on either side of the cervical spine. RESULTS: All 50 subjects exhibited at least one audible joint crack sound during manipulation. Forty-seven subjects (94%) exhibited cracking on the ipsilateral side to head rotation (95% confidence interval, 83.5% to 98.7%). One subject exhibited joint cracking on the contralateral side only, while two subjects exhibited bilateral joint crack sounds. There was a statistically significant lower rate of exclusively ipsilateral joint cracking in subjects with a history of neck trauma (80% vs. 100%, p = .023). CONCLUSIONS: This research suggests that during the "diversified" rotatory manipulation of the cervical spine utilized in this study, there is a higher occurrence of the joint crack on the ipsilateral side to head rotation.

Changes in cardiovascular disease knowledge and behavior in a low-education population of African-American and white adults.

OBJECTIVE: Racial differences in secular changes in cardiovascular disease risk factor knowledge and behaviors were assessed among adults with low levels of education throughout a community-wide cardiovascular disease prevention program. METHODS: Four independent cross-sectional telephone surveys were conducted with the random-digit-dialing technique in 1987, 1988, 1989, and 1991 in a biracial South Carolina community. Community-wide cardiovascular disease intervention programs were initiated in 1988 and continued through 1990. Changes in the prevalence of cardiovascular risk factor knowledge, dietary fat intake, leisure-time physical activity, smoking, and cholesterol screening behavior were compared between African-American and white respondents in a population subset with less than 12 years of education using analysis of covariance regression techniques. RESULTS: Mean intake of high fat foods was lower in 1991 than in 1987 among both white and African-American respondents; the trend for lower mean intake began in 1989 among African-American adults. Prevalence of the correct exercise knowledge was higher in 1988 than in 1987 for both groups, but this trend was maintained only among white respondents. However, prevalence of leisure-time physical activity did not change significantly between 1987 and 1991. Prevalence of cholesterol level knowledge and screening behavior increased over time among both groups; however, greater increasing trends between 1987 and 1991 were observed among white adults. CONCLUSIONS: Favorable secular changes in fat intake, exercise knowledge, cholesterol level knowledge, and cholesterol screening behavior were observed among both race groups during a time period that coincided with community-wide intervention efforts and messages. Greater changes in most of these behaviors and knowledge were observed among white adults suggesting that health behavior messages may not have reached all segments of this community.

Binocular three-dimensional perception through stereoscopic generation from rotating images.

RATIONALE AND OBJECTIVES: The authors evaluated the clinical utility and potential applications of a binocular three-dimensional (3D) image display in diagnostic radiology. MATERIALS AND METHODS: Rotating video displays of computed tomographic (CT), magnetic resonance (MR) angiographic, and digital subtraction angiographic (DSA) image data were used to generate stereoscopic image displays with a 3D appearance. Eight physicians viewed and scored eight skeletal and eight vascular-interventional studies with a planar display mode and a cathode ray tube. Each physician then viewed the 3D display of the same data and assessed the change in image findings, as well as any corresponding changes in level of diagnostic confidence. RESULTS: Image findings changed in 78 (61%) of the 128 studies after viewing the 3D displays. In 94 (73%) of all 128 studies, the interpreters reported increased confidence in their perception of the findings. Results for the vascular-interventional and skeletal cases were generally very similar. CONCLUSION: Binocular 3D stereoscopic displays from rotating images were reported to provide better image conceptualization and a higher degree of confidence in the findings on the images.

Genotyping of Mycoplasma gallisepticum and M. synoviae by Amplified Fragment Length Polymorphism (AFLP) analysis and digitalized Random Amplified Polymorphic DNA (RAPD) analysis.

Mycoplasma gallisepticum (MG) and M. synoviae (MS) are the cause of considerable economic losses in the poultry industry. Molecular differentiation of avian Mycoplasma strains may be helpful in tracing infections and in the evaluation of implemented intervention strategies. Amplified Fragment Length Polymorphism (AFLP) has shown to be a powerful typing technique but the application for poultry Mycoplasma strains is very limited. The aim of this study was to evaluate the reproducibility and discriminatory power of AFLP HindIII/HhaI and AFLP BglII/Mfel for the inter- and intraspecies differentiation of avian mycoplasmas and to compare these test characteristics with digitalized Random Amplified Polymorphic DNA (RAPD) analysis. The reproducibility of RAPD, AFLP HindIII/HhaI and AFLP BglII/Mfel was 50-100, 97-98 and 86-99%, respectively. RAPD and both AFLP enzyme combinations were able to differentiate between five avian Mycoplasma species. For AFLP, five MG and four MS clusters could be identified. The phylogenetic tree for both enzyme combinations was comparable. For RAPD, four MG clusters could be identified. For MS, however, due to the poor reproducibility of the RAPD technique, no clear genogroups could be identified. On basis of the results of this study it can be concluded that AFLP is a powerful technique for the genotyping of avian mycoplasmas and that, although AFLP HindIII/HhaI generated patterns with less fragments, the final results showed homologous results.

Enhancing adherence among older African American men enrolled in a longitudinal cancer screening trial.

PURPOSE: The purpose of this study was to enhance adherence among older (aged 55 years and older) African American men enrolled in a cancer screening trial for prostate, lung, and colorectal cancer. For this study, we defined adherence as completing the trial screenings. DESIGN AND METHODS: We used a randomized trial design. Case managers contacted intervention group participants (n=352) at least monthly by telephone and provided information and referral services. The control group included 351 participants. RESULTS: Among participants with low income, those in the intervention group had higher screening adherence rates than did participants in the control group for (a) prostate-specific antigen test for prostate cancer (74.3% vs 53.0%, p=.001), (b) digital rectal exam for prostate cancer (66.2% vs 46.1%, p=.011), and (c) chest x-ray for lung cancer (70.9% vs 51.3%, p=.012). We found no statistically significant differences in adherence rates for flexible sigmoidoscopy screening for colorectal cancer. In contrast, among participants with moderate-to-high income, we found no statistically significant differences in adherence rates between intervention and control group participants for any of the screening tests. IMPLICATIONS: The case management intervention was effective in enhancing adherence among participants with the lowest income, who in many studies are the most difficult to retain.

Short- and long-term outcomes of adult audiological rehabilitation.

OBJECTIVE: To examine short- and long-term subjective benefits of providing a counseling-oriented audiological rehabilitation (AR) program as an adjunct to hearing aid intervention for individuals with adult-onset hearing loss. DESIGN: One hundred six veterans (68 men and 38 women), fit binaurally with digitally programmable analog hearing aids, participated. The Communication Profile for the Hearing Impaired (CPHI; ) was administered to all participants before hearing aid fitting. Half the patients were randomly assigned to receive hearing aids alone (i.e., control); the other patients were assigned to participate in a 4-wk group AR program in conjunction with receiving hearing aids (i.e., HA + AR). At the end of the AR program, the CPHI was again administered to all participants to assess short-term benefit and at 6 mo and 1 yr after hearing aid fitting to assess long-term benefit. RESULTS: A separate repeated-measures version of the general linear model was used to examine short- and long-term benefits for the CPHI factor scores (communication importance, communication performance, adjustment, interaction, and reaction) and for individual importance ratings and scale scores. Hearing aid use improved both short- and long-term self-perception of communication performance, with no additional benefits from participation in the AR program. Consistent with the goals of a counseling-oriented AR program, differential short-term treatment effects were found for communication strategy usage, which led to differential short-term benefits for the interaction and reaction factors. Although failing to reach strict criteria for statistical significance, there was an observable difference in short-term outcomes between the two groups for the adjustment factor, with greater improvements occurring for the HA + AR group. Over the course of the year, benefits measured for the HA + AR group remained stable, whereas scores for the control group continued to increase, resulting in no differences in factor scores between groups at 1 yr after intervention. CONCLUSIONS: The finding of a short-term differential treatment benefit for AR in terms of interaction and reaction, and possibly for adjustment, was important, as better outcomes in these areas may be important in the decision to keep hearing aids. If this is the case, then the data support the inclusion of a counseling-oriented AR program. Differential treatment effects in interaction and reaction appeared to result from communication strategy use, indicating that the AR program is meeting many of its goals in this area. The lack of long-term differential effects appeared as the result of continued changes in adjustment, interaction, and reaction with continued hearing aid experience.

Perceived benefits: a predictor of participation in prostate cancer screening.

Prostate cancer is the most frequently diagnosed major cancer and the second cause of cancer-related deaths among men. With early detection through screening and timely treatment, 9 out of 10 men will survive a minimum of 5 years. However, with late diagnoses, only 3 out of 10 men will have a 5-year minimum survival rate. Guided by a conceptual map, this correlational research examined perceived benefits as a predictor of participation in free prostate cancer screening. Perceived benefits are the personal belief and valuing of screening for early detection of prostate cancer. All subjects received one of four educational interventions: traditional, peer educator, client navigator, or combination. Participation in prostate cancer screening was measured by compliance with the American Cancer Society's Guidelines, which included a digital rectal exam (DRE) and/or a prostate-specific antigen (PSA) blood test. The purposive sample (n = 1,522) of men, ages 40 to 70 years, was recruited from randomly selected churches, barbershops, industries, housing projects, and car dealerships in a southeastern state. Seventy-two percent of the sample was African American. Predictors of participation in free prostate cancer screening were these: perceived benefits, being white, having at least a high school education, being married, and receiving the client navigator or combination educational intervention. The Benefits Scale was significant (p = 0.013, odds ratio (OR) = 1.059) as a predictor for participation in screening when all demographic variables and educational interventions were controlled. Practice implications for nursing are discussed and recommendations for future research are presented.

Preventing pressure ulcers: a systematic review.

CONTEXT: Pressure ulcers are common in a variety of patient settings and are associated with adverse health outcomes and high treatment costs. OBJECTIVE: To systematically review the evidence examining interventions to prevent pressure ulcers. DATA SOURCES AND STUDY SELECTION: MEDLINE, EMBASE, and CINAHL (from inception through June 2006) and Cochrane databases (through issue 1, 2006) were searched to identify relevant randomized controlled trials (RCTs). UMI Proquest Digital Dissertations, ISI Web of Science, and Cambridge Scientific Abstracts were also searched. All searches used the terms pressure ulcer, pressure sore, decubitus, bedsore, prevention, prophylactic, reduction, randomized, and clinical trials. Bibliographies of identified articles were further reviewed. DATA SYNTHESIS: Fifty-nine RCTs were selected. Interventions assessed in these studies were grouped into 3 categories, ie, those addressing impairments in mobility, nutrition, or skin health. Methodological quality for the RCTs was variable and generally suboptimal. Effective strategies that addressed impaired mobility included the use of support surfaces, mattress overlays on operating tables, and specialized foam and specialized sheepskin overlays. While repositioning is a mainstay in most pressure ulcer prevention protocols, there is insufficient evidence to recommend specific turning regimens for patients with impaired mobility. In patients with nutritional impairments, dietary supplements may be beneficial. The incremental benefit of specific topical agents over simple moisturizers for patients with impaired skin health is unclear. CONCLUSIONS: Given current evidence, using support surfaces, repositioning the patient, optimizing nutritional status, and moisturizing sacral skin are appropriate strategies to prevent pressure ulcers. Although a number of RCTs have evaluated preventive strategies for pressure ulcers, many of them had important methodological limitations. There is a need for well-designed RCTs that follow standard criteria for reporting nonpharmacological interventions and that provide data on cost-effectiveness for these interventions.

[Interventional radiologic management for early post-transplant perfusion failure of renal allografts].

PURPOSE: Open surgery for correction of early vascular complications in allogenic kidney transplantation carries the risk for increased morbidity and graft loss. The question was raised whether modern interventional radiologic techniques, especially the use of vascular stents, could play an alternative therapeutic role in those complicated cases. MATERIAL AND METHODS: 3 patients with early postoperative perfusion failure of their renal grafts were referred to the radiologist after Doppler sonography. Immediate digital subtraction angiography was initiated and during the same session percutaneous transluminal angiography and placement of a vascular stent were performed to treat the vascular lesion. Anticoagulation was started with low-dose aspirin 100 mg/day. RESULTS: In all 3 patients an intimal dissection of the renal artery with formation of a relevant stenosis was found. Stenosis was corrected by angioplasty and stenting. In one case an accidentally found renal vein thrombosis was additionally treated by transcatheter thrombo-aspiration. No complications related to the interventions occurred. In 2 patients diuresis returned immediately and renal function remained stable in the longterm (follow-up 6 month). In one case massive peripheral arterial thrombosis had to be diagnosed in spite of successful recanalization of the renal artery. After explantation of the organ histology revealed extended parenchymal necrosis. CONCLUSIONS: The timely use of angiography after Doppler sonographic diagnosis for renal transplant perfusion failure is of help not only for exact diagnosis but also for immediate treatment. Radiologic interventional techniques should be regarded as potentially effective and safe for the treatment of early vascular complications after renal transplantation.

[Evaluation and estimation of entrance skin dose in patients during diagnostic and interventional radiology procedures].

A study was performed to evaluate the total entrance skin dose (ESD) of patients during diagnostic and interventional radiology procedures (IVR) and to estimate ESD with body mass index (BMI) and fluoroscopy time. The study included 26 cases of transcatheter arterial embolization therapy (TAE) for hepatocellular carcinoma (HCC) and 19 cases of diagnostic digital subtraction angiography (DSA) for HCC. The ESD of patients was evaluated with a zinc-cadmium sensor linked to a digital counter (SDM: skin dose monitor). Exposure doses were measured with SDM attached to the front of the X-ray beam-limiting device like a dose area product monitor. ESD was calculated from the measured exposure dose. In 26 TAE for HCC, ESD was 1793.7+/-739.1 mGy, with the mean fluoroscopic time of 23.5 minutes and 4.4 DSA acquisitions. The fluoroscopic dose rate was 52.4+/-11.5 mGy/min. In 19 diagnostic DSA for HCC, ESD was 962.9+/-375.2 mGy, with the mean fluoroscopic time of 11.1 minutes and 4.0 DSA acquisitions. The fluoroscopic dose rate was 32.7+/-12.7 mGy/min. Although 33.2% of ESD was from fluoroscopy in diagnostic procedures, the figure was 68.8% in TAE procedures. It was demonstrated that the increase in ESD during IVR was caused by the rise of fluoroscopy dose rate caused by high-magnification fluoroscopy and the extension of fluoroscopy time. In order to reduce ESD, it is necessary to use a low fluoroscopy dose rate with low-rate pulse fluoroscopy, in addition to shortening fluoroscopy time. Fluoroscopy time was a poor predictor of risk because it did not correlate well with ESD during IVR (diagnostic procedures r(2)= 0.897, IVR r(2)= 0.594). However, ESD correlated well with the product of BMI and fluoroscopy time (diagnostic procedures r(2)= 0.910, IVR r(2)= 0.783). The linear relationship between ESD and the product of BMI and fluoroscopy time provides a simple monitoring mechanism of the ESD delivered to the patient during interventional radiology procedures. This linear relationship needs to be established for other types of interventional procedures.

The effect of 10 : 1 compression and soft copy interpretation on the chest radiographs of premature neonates with reference to their possible application in teleradiology.

The aim of the study was to assess the potential application of teleradiology in the neonatal intensive care unit (NICU) by ascertaining whether any decrease in conspicuity of anatomic detail or interventional devices in the chest radiographs of premature infants is caused by picture archiving and communication system (PACS)-based soft copy interpretation of 10 : 1 compressed images. One hundred digital chest radiographs of low-birthweight infants were obtained in the NICU using a storage phosphor system. Laser-printed images were interpreted and the data set for each radiograph was then irreversibly compressed by a 10 : 1 ratio. Four radiologists with extensive PACS experience used a five-point grading system to score laser-printed hard copy images for the visibility of six parameters of anatomic landmarks and interventional devices in the chest. Compressed soft copy images displayed on 2K PACS workstation were subsequently scored using the same approach. Statistical manipulation demonstrated no loss of anatomic detail in five of the six parameters scored, with minimal difference in one landmark, the retrocardiac lung assessment. While further study is required to assess the clinical impact of the variance noted when evaluating lung parameters, the preservation or improvement of information in the remaining parameters following irreversible compression and soft copy interpretation is promising for the potential use of teleradiology in this population.

Development of intraarterial contrast-enhanced 2D MRDSA with a 0.3 tesla open MRI system.

PURPOSE: The purpose of this study was to develop a new technique for a high temporal resolution two-dimensional MR digital subtraction angiography (2D MRDSA) sequence under intraarterial injection of contrast material to permit the visualization of vascular anatomy and hemodynamics. METHODS: 2D MRDSA was imaged on a 0.3T open MR scanner with a T(1)-weighted fast gradient echo sequence. The phantom study examined vials containing gadolinium (Gd) solutions ranging in concentration from 0.5 mmol/L to 100 mmol/L. Repetition time and echo time were fixed at minimal values in order to achieve high temporal resolution, and only the flip angle was changed in 10-degree increments between 10 and 90 degrees. The in vivo study examined a brachial artery of a human volunteer. MRDSA images were acquired continuously during intraarterial injections of Gd solutions ranging in concentration from 0.5 mmol/L to 100 mmol/L. The subtracted images were displayed on the monitor in real time at a frame rate of one frame per second and evaluated to determine the optimal concentration of contrast material. RESULTS: In the phantom study, a 10-mmol/L Gd concentration with a flip angle of 50 degrees -90 degrees and a 25-mmol/L Gd concentration with a flip angle of 60 degrees -90 degrees showed high signal-to-noise ratios. In the human brachial artery experiment, the forearm arteries were well visualized when solutions of 5-50 mmol/L Gd concentration were used. The 10- and 25-mmol/L Gd concentrations were considered optimal. The palmar digital arteries were also visualized. Higher Gd concentrations showed a paradoxical signal increase when diluted by blood. CONCLUSION: We successfully developed an intraarterial contrast-enhanced 2D MRDSA sequence. With appropriate settings of imaging parameters and Gd concentrations, we obtained acceptable vessel visualization in the human study. The low Gd concentration for optimal visualization permits repeated intraarterial injections. This technique can be a useful tool for investigating the vascular anatomy and hemodynamics required for MR-guided vascular interventions.

Interventions for the treatment of Morton's neuroma.

BACKGROUND: Morton's neuroma is a common, paroxysmal neuralgia affecting the web spaces of the toes, typically the third. The pain is often so debilitating that patients become anxious about walking or even putting their foot to the ground. Insoles, corticosteroid injections, excision of the nerve, transposition of the nerve and neurolysis of the nerve are commonly used treatments. Their effectiveness is poorly understood. OBJECTIVES: To examine the evidence from randomised controlled trials concerning the effectiveness of interventions in adults with Morton's neuroma. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group trials register (searched January 2003), MEDLINE (January 1966 to January Week 2 2003), EMBASE (January 1980 to February Week 2 2003), and CINAHL (January 1982 to February Week 1 2003). SELECTION CRITERIA: Randomised or quasi-randomised (methods of allocating participants to an intervention which were not strictly random e.g. date of birth, hospital record, number alternation) controlled trials of interventions for Morton's neuroma were selected. Studies where participants were not randomised into intervention groups were excluded. DATA COLLECTION AND ANALYSIS: Two reviewers selected trials for inclusion in the review, assessed their methodological quality and extracted data independently. MAIN RESULTS: Three trials involving 121 people were included. There is, at most, a very limited indication that transposition of the transected plantar digital nerve may yield better results than standard resection of the nerve in the long term. There is no evidence to support the use of supinatory insoles. There are, at best, very limited indications to suggest that dorsal incisions for resection of the plantar digital nerve may result in less symptomatic post-operative scars when compared to plantar excision of the nerve. REVIEWERS' CONCLUSIONS: There is insufficient evidence with which to assess the effectiveness of surgical and non-surgical interventions for Morton's neuroma. Well designed trials are needed to begin to establish an evidence base for the treatment of Morton's neuroma pain.

Intraarterial digital subtraction angiography in the diagnosis and treatment of gastrointestinal disorders.

During a period of 18 months, intraarterial digital subtraction angiography (IADSA) was performed in 360 patients with various gastrointestinal disorders. This technique was useful both in diagnostic and interventional procedures. Due to increased contrast resolution, hepatic tumors and portal vein systems were better visualized than with the use of conventional angiography. Also, a significant reduction in the dosage of contrast medium resulted in markedly reduced patient discomfort. Small pancreatobiliary tumors were better evaluated on conventional angiograms due to the decreased spatial resolution of IADSA.
