Pelvic venous changes after tubal sterilization.

We studied pelvic venous patterns after tubal ligation and compared them after different tubal sterilization procedures. To this end transuterine pelvic venography was performed preoperatively and after the first and sixth periods postoperatively in 56 women. All procedures that interrupt or interfere with the blood supply were analyzed in terms of uterine congestion, uterovaginal and ovarian varicosities and the incidence of pelvic venous stasis. There is a compelling need for a sterilization method that does not interrupt or damage the pelvic circulation; ideally it should be nonsurgical.

No increase in arteriolosclerotic retinopathy or activity in tests for circulating immune complexes 5 years after vasectomy.

On the basis of studies in monkeys, the hypothesis has been proposed that vasectomy induces the formation of circulating immune complexes (CICs), which--via activation of the complement system--may result in immune injury of the endothelium and thereby exacerbate atherosclerosis; the same mechanism has been suggested to cause retinal arteriolar changes in vasectomized men. We compared 46 men, 5 years after vasectomy, with 46 age-matched control subjects and found no difference in the distribution of arteriolosclerotic retinopathy gradings evaluated by ophthalmoscopy and fundus photography. Blood samples from the two groups were collected and handled identically, and no significant difference in activity was found in four different tests for CICs and two tests for split products of complement factor C3. Thus, the results do not support the hypothesis that changes mediated by CICs occur in vessels after human vasectomy.

[Fertility following abortion, using Boero's method (author's transl)].

Follow-up studies were conducted into the conditions of ten women whose pregnancies had been terminated by means of Boer's method, in the second trimenon. Two of them had lost any capability of conception. Another four conceived without problems. One was pregnant, following three years of treatment for sterility. Two subsequent pregnancies of another woman ended in spontaneous abortion. Two women remained sterile.

Endocrine profile of patients with post-tubal-ligation syndrome.

The endocrine profile of the midluteal phase was assessed in 29 patients with the post-tubal-ligation syndrome, consisting of pain, bleeding and premenstrual tension. Compared to normal controls, the patients had a high serum estradiol and a low serum progesterone level. This abnormal luteal function may be responsible for the symptoms observed and may also explain the failure to conceive following successful reversal of tubal ligation. It is recommended that patients seeking sterilization reversal be screened for abnormal luteal function preoperatively. Selection of sterilization procedures that minimize alteration in luteal function should be given high priority.

Vasectomy: subclinical pathologic changes.

The nature of the local histopathologic changes secondary to vasectomy is described in tissue excised at vasovasostomy in 37 secondarily infertile patients. Segments of surgically removed vasa were also studied in four patients with primary infertility who underwent scrotal explorations and subsequent vasoepididymostomy. Except for infertility, all patients were asymptomatic. Three, often concurrent, inflammatory or proliferative changes were found in 50 of 76 segments of vasa deferentia. These included suture granuloma, sperm granuloma, and vasitis nodosa, the last being a ductular proliferation originating from the central vas lumen and extending into the perivasal soft tissues. Vasitis nodosa occurred in 66% of the patients, and although it was often found with a sperm granuloma, it did occur by itself and is a lesion which should be recognized.

Cancer risk among women sterilized with transcervical quinacrine hydrochloride pellets, 1977 to 1991.

OBJECTIVE: To determine whether a cluster of eight cancers among 572 women who had received transcervical quinacrine hydrochloride was a random occurrence or evidence of an increased risk of cancer. DESIGN: Retrospective cohort study using interviews and reviews of medical records. Cancer cases were evaluated using cohort analyses and space-time cluster methods. SETTING: Santiago and Valdivia, Chile. SUBJECTS: Fourteen hundred ninety-two women who received transcervical quinacrine pellets for sterilization between 1977 and 1989. MAIN OUTCOME MEASURE: Age- and site-specific incidence of invasive cancers. RESULTS: Eight hundred two women were interviewed. From 1 to 14 years of data were available on 600 of the noninterviewed women from clinic records. During 7,852 women-years of follow-up, 17 invasive cancers were identified, compared with 11.8 expected, based on age-specific rates from the Cali, Colombia cancer registry. Five cases of cervical cancer were observed, compared with 3.96 expected. Only one other uterine cancer was observed, a leiomyosarcoma, compared with 0.2 or 0.3 other uterine cancers expected. CONCLUSIONS: The occurrence of an unusual cluster was confirmed, but no evidence was found of excess cancer risk associated with quinacrine pellet sterilization. However there was a single provocative observation (the leiomyosarcoma), and surveillance of the cohort is continuing.

[Effect of inhaled steroid on bone metabolism in the treatment of bronchial asthma].

In order to clarify the effects of longterm inhaled steroid therapy on bone metabolism, we examined 72 patients with bronchial asthma treated mainly with BDP (beclomethasone dipropionate). Multiple scanning X-ray photodensitometry was used to evaluate the degree of bone mineral loss. Osteocalcin, alkaline phosphatase (total and type III) was measured as a marker of bone synthesis and urinary pyridinoline, and deoxy-pyridinoline was measured as a marker of bone resorption. There was age related bone mineral loss. Urinary pyridinoline increased with aging. Treatment related bone mineral loss was not observed either in cases treated with BDP or in cases treated with continual oral steroids. Urinary pyridinoline and deoxy-pyridinoline decreased in patients treated with larger doses of for longer periods with BDP. Serum osteocalcin levels were lower in patients on continual oral corticosteroids. We conclude that inhaled steroid do not deteriorate bone metabolism in patients with bronchial asthma, when used appropriately.

The benefit of early treatment with immunosuppressive agents in lupus nephritis.

OBJECTIVE: In a cohort of 87 patients with lupus nephritis, delay between the detection of the onset of renal disease and renal biopsy was a significant predictor at the time of a first renal biopsy for subsequent renal insufficiency (relative risk - 4.9; 95% confidence interval = 1.7 to 14.5; p < 0.001) and death due to lupus renal involvement (relative risk = 6.7; 95% confidence interval = 2.1 to 21.2; p < 0.001). We evaluated the role of lead time bias, 2 variants of prognostic selection bias (length biased sampling), and the benefit of early treatment as explanations for this effect. METHODS AND RESULTS: Evaluation using the time of renal onset rather than the time of renal biopsy for the analysis suggested that lead time bias was unlikely to be an explanation for the effect of duration on renal insufficiency or death due to renal involvement. Identical values of age, serum creatinine and 24 hour urinary protein excretion at renal onset for those with a long duration versus short duration prior to biopsy, suggested that differences in prognostic selection were unlikely to explain the observed results. A 2nd type of prognostic selection bias arising from the failure to include patients who did not undergo a renal biopsy was further assessed by statistical simulation. The results of this approach indicated that prognostic selection bias was not solely responsible for the significant associations. Because treatment with high dose prednisone and immunosuppressive drugs was not instituted until a renal biopsy had been performed, delay in instituting these therapies remained a possible explanation for the increased frequency of renal insufficiency and death due to renal involvement observed in those with longer delays before renal biopsy. In addition, there was significant deterioration in serum creatinine (median change 0.6 mg/dl) and 24 hour urinary protein excretion (median change 2.5 gm) over the period from renal onset to renal biopsy, and significantly higher scores for the activity, chronicity and tubulointerstitial indices on renal biopsy in those in whom therapy was delayed. CONCLUSION: Prompt therapy with prednisone and immunosuppressive agents in lupus nephritis has a beneficial effect on longterm prognosis.

Effects of prolonged aspirin or acetaminophen administration to rats on liver folate content and distribution. Relation to DNA methylation and S-adenosylmethionine.

The study was aimed at determining the effects of longterm administration to rats of aspirin or acetaminophen (ACAP) on liver folate content and distribution as well as whether or not hepatocyte DNA was methylated at the end of the treatments. In addition, S-adenosylmethionine and S-adenosylhomocysteine hepatic concentrations were determined. In liver, aspirin or ACAP administration were not associated with important changes in total folate content or glutamic acid chain length distribution. DNA methylation assessed by comparing the extent to which DNA from livers of control or treated animals could be methylated in vitro using (3H-methyl)S-Ado Met as methyl donor, resulted in similar [3H] methyl incorporation into DNA regardless of the drug. The long-term administration of the analgesics did not result in significant changes in S-AdoMet/S-AdoHcy ratios.

Pregnancy outcome in women with systemic lupus erythematosus treated with immunosuppressive drugs.

OBJECTIVE: The aim of this retrospective study was to assess the impact of immunosuppressive drugs (ISD) on pregnancy outcome in women with systemic lupus erythematosus (SLE). METHODS: Pregnancy outcome, disease manifestations, and medication use were ascertained by interview and review of the medical record. Three hundred thirty-four (88%) of 380 consecutive women with lupus seen during 1979-1989 at the Lupus Diagnostic and Treatment Center at the University of Pittsburgh participated in this study. RESULTS: Adverse pregnancy outcomes were noted in 59 of 113 (52%) pregnancies in patients with pregnancies after diagnosis of SLE and no exposure to ISD (Group 1) and in 10 of 23 (43%) pregnancies in patients with pregnancies after diagnosis of SLE and with ISD use prior to or during pregnancy (Group 2). Only 3 of 23 or 13% pregnancies in Group 2 resulted in a fetal or neonatal loss compared to 27% in Group 1 (NS). No history of malignancy has been noted in the longterm followup of the surviving children in Group 2 (current mean age is 6.1 years, range 1.5-13 years). CONCLUSIONS: Although overall survival was encouraging in Group 2, questions remain about the safety and longterm mutagenic effects of ISD in women with SLE. Judicious use of these medications prior to or during pregnancy is warranted.
