The influence of training on tinnitus perception: an evaluation 12 months after tinnitus management training.

Sixty-five subjects were reviewed 12 months after tinnitus management training, which had been comprised variously of information, relaxation training and a therapeutic noise strategy. Seventy-four per cent of subjects reported increased habituation to tinnitus (n = 48), 65% reported reduced tinnitus annoyance (n = 42), and 52% reported an increased ability to cope with tinnitus (n = 34). Twenty-five per cent of subjects reported deterioration in coping ability (n = 16), 23% reported reduced habituation to tinnitus (n = 15) and 8% reported increased tinnitus-related annoyance (n = 5). None of the management strategies were found to be significantly more effective than others in facilitating improved coping or habituation to tinnitus. Subjects who reported reduced coping and habituation to tinnitus experienced greater levels of general life stress than subjects who reported increased habituation and coping ability. The use of relaxation therapies as applied in this study did not appear to influence the level of tinnitus distress or the level of life stress. Thirty-seven per cent of subjects given long-term low-level white noise (LTWN) stimulation reported benefit. However, LTWN stimulation did not significantly alter tinnitus awareness or the minimum masking level (MML) of tinnitus. Long-term low-level white noise stimulation appeared to influence cognitive reaction to tinnitus rather than its physical perception. Subjects who initially had low ability to cope with tinnitus and preferred a more active coping style reported significantly greater benefit from LTWN stimulation than subjects whose primary approach to coping was to regulate the emotional impact of tinnitus.

Training for hospital security: an alternative to lawsuits for training negligence.

Training for the management of violent behavior, on the part of both hospital security officers and other staff members, is essential if violence and inappropriate behavior, and their consequences, are to be successfully dealt with. The laws and practices centering around the concept of training negligence are discussed fully in this article.

Change and challenge in health care education and training: ASHET (American Society for Healthcare Education and Training) national membership survey results.

Education and training are vital and yet rapidly changing components of the health care delivery system. Understanding and adapting to these changes are critically important to all health care professionals who represent the leadership in the industry. A major objective of the American Society for Healthcare Education and Training is the periodic measurement, assessment, and reporting of these changes. Toward this objective, ASHET commissioned a survey of its entire national membership in the fall of 1988. The following are the results of this survey.

[Patient training in peritoneal dialysis: 1. Training in self care, cognitive study].

Authors consider education for dialytic patients a tool for patients' self-care and better quality of their life. In our Country the peritoneal dialysis involve 10% of the total dialytic population where the patient assumes a quite emerging role. The educational process is called training and the responsibility of the nurse is primarily important due to the trust and the consequential nurse-patient relationship. The article is divided in two parts: in the first, we explore the importance of the training as teaching methodology and in the second, we present the results of a national survey to better understand the role of the nurse and the methodology utilised in different Centers.

The efficacy of problem-solving communication training alone, behavior management training alone, and their combination for parent-adolescent conflict in teenagers with ADHD and ODD.

Two family therapies were compared using teens with attention-deficit/hyperactivity disorder. Ninety-seven families were assigned to either 18 sessions of problem-solving communication training (PSCT) alone or behavior management training (BMT) for 9 sessions followed by PSCT for 9 sessions (BMT/PSCT). Both treatments demonstrated significant improvement in ratings of parent-teen conflicts at the midpoint but did not differ. By posttreatment, both produced improvement on ratings and observations but did not differ. Significantly more families dropped out of PSCT alone than out of BMT/PSCT. At most, 23% of families showed reliable change either by midpoint or by posttreatment, with no differences between therapies. Yet 31%-70% of families were normalized. Group-level change and normalization rates support treatment efficacy, whereas indices of reliable change are less impressive.

[Laboratory medicine in the obligatory postgraduate clinical training system--common clinical training program in the department of laboratory medicine in our prefectural medical university hospital].

I propose a postgraduate common clinical training program to be provided by the department of laboratory medicine in our prefectural medical university hospital. The program has three purposes: first, mastering basic laboratory tests; second, developing the skills necessary to accurately interpret laboratory data; third, learning specific techniques in the field of laboratory medicine. For the first purpose, it is important that medical trainees perform testing of their own patients at bedside or in the central clinical laboratory. When testing at the central clinical laboratory, instruction by expert laboratory technicians is helpful. The teaching doctors in the department of laboratory medicine are asked to advise the trainees on the interpretation of data. Consultation will be received via interview or e-mail. In addition, the trainees can participate in various conferences, seminars, and meetings held at the central clinical laboratory. Finally, in order to learn specific techniques in the field of laboratory medicine, several special courses lasting a few months will be prepared. I think this program should be closely linked to the training program in internal medicine.

Mental health evaluations of U.S. Air Force basic military training and technical training students.

A 2000 report by Staal, Cigrang, and Fiedler and a 1998 report by Cigrang, Carbone, Todd, and Fiedler described the attrition of U. S. Air Force basic military trainees due to mental health disorders for the year 1997. This article looks at the population of Air Force basic military trainees and technical training school students located at the same base during the year 2001. In addition, we look at the effect of allowing basic trainees and those in technical school to refer themselves for mental health evaluations as opposed to only evaluating those referred by secondary sources. Primary results of the data analysis suggest that mental health-related separation rates for calendar year 2001 basic military trainees are consistent with past years at 4.2%. For both basic trainees and technical school training students, adjustment disorders and depressive disorders are the top diagnostic categories related to recommendation for separation.

Determinants of repeated-sprint ability in well-trained team-sport athletes and endurance-trained athletes.

AIM: To examine the importance of peak .VO(2) in determining repeated-sprint ability (RSA), we recruited 20 well-trained females (10 team-sport athletes and 10 endurance-trained runners; mean SD peak .VO(2): 3.3+/-0.2 L x min(-1)) who were homogenous with respect to peak .VO(2) (mean difference = 0.05 L x min(-1)). METHODS: Tests consisted of a RSA cycle test (5 x 6-s max sprints every 30 s) and a peak .VO(2) test. Venous and capillary blood was sampled immediately before and after the 5 x 6-s cycle test for the determination of hypoxanthine concentration ([Hx]), lactate concentration ([La-]) and pH; blood buffer capacity (beta(blood)) was also estimated. RESULTS: The team-sport athletes had significantly higher peak power for the 1(st) sprint (P(1); W x kg(-1)), total work for 5 x 6-s sprints (W(tot); J x kg(-1)) and power decrement across the 5 sprints (P(dec)), (p<0.05). There were also significant between-group differences for post-test values of [Hx], [La-] and pH (p<0.05). While there was no significant difference in beta(blood) between the 2 groups (p=0.10), there was a moderate effect (d=0.77). CONCLUSION: These results suggest that factors in addition to peak .VO(2) are likely to be important for RSA.

Training-as-treatment: effectiveness of the Certified Compassion Fatigue Specialist Training.

Mental health and other service professionals who work with trauma survivors often experience the debilitating effects of compassion fatigue and caregiver stress. Leaders within the field have called for effective, empirically supported interventions for professionals who experience these negative effects. In response to the call, this study examines the treatment effectiveness of the Certified Compassion Fatigue Specialist Training (CCFST) for 83 participating mental health professionals. Results show a statistically and clinically significant decrease in participants' compassion fatigue and burnout symptoms and increase in their compassion satisfaction. A "training-as-treatment" effect of CCFST is introduced, described, and evaluated. Discussion of these findings, clinical implications, limitations, and future directions are provided. CCFST appears to be an effective intervention for ameliorating compassion fatigue symptoms in mental health professionals.

Comparison of residency-trained and nonresidency-trained family physicians in North Carolina.

The objective of the present study was to compare the content and medical practice activities of residency-trained (RT) to nonresidency-trained (NRT) family physicians in North Carolina. Interviews and patient ambulatory encounter information were collected from 41 RT physicians and 44 NRT physicians. Significant differences between physicians were found in practice location, number of patient encounters per week, patient age distributions, and rank orders of the ten most frequent diagnosis clusters. The RT physicians tend to work longer hours and see fewer patients, and they are more likely to perform obstetric and pediatric services. Both groups treat a broad range of the patient population, with RT serving more pediatric patients and NRT seeing more geriatric patients.

[International postgraduate training program in medicine: survey of 13 training courses, 1988-1994].

In 1988 an International Postgraduate Training Program in Medicine was established as a joint venture of the Tel Aviv University School of Continuing Medical Education and the Ministry for Foreign Affairs, Division of International Cooperation. During 1988-1994, 359 physicians participated. A survey of 271 graduates was carried out within 1-5 years following the completion of training, and 57% responded to our questionnaire. The program fulfilled the academic expectations of 78% of the trainees, and the practical clinical expectations of 80%. 95% graded the program as good-to-excellent from the sociocultural aspect. 75% and 67.5% of the trainees graded the program as good-to-excellent with regard to improvement in their theoretical and practical skills, respectively. Regretfully, only 45% have continued their ties with the host departments and only 11% have carried out joint research projects. These results imply that further attention should be paid to the trainees' real-time input, in order to increase satisfaction. Personal tutors should be nominated in each department, and should be adequately compensated. More emphasis should be given to initiation of bilateral research projects and maintenance of professional ties between the trainees and their host departments.

The development of visual preferences in art-trained and non-art-trained schoolchildren.

Measures of four dimensions of visual preference (preferences for symmetry, simplicity, uniformity, and expressiveness) and a standardized test of visual-arts achievement were administered to art-trained and non-art-trained children in kindergarten, Grade 3, Grade 7, and high school. The art and non-art groups were matched at the school level on family income, English-language and ethnic backgrounds, school attendance, and school neighborhoods. The results showed effects for age and instruction, but the effects for instruction did not increase after Grade 3. The relative independence of the visual-preference dimensions from visual arts learning suggests they may measure individual differences that take shape early in development and are related to visual arts performance.

Testosterone application influences sympathetic activity of intracardiac nerves in non-trained and trained mice.

Application of testosterone and/or physical exercise causes degenerative and then regenerative patterns of intracardiac sympathetic neurons. Observations in 3 stages (1, 3 and 6 weeks) illustrate the adaptative changes of sympathetic neurons as a response to these stimuli and show that the effects following testosterone application or physical exercise are comparable. Ultrastructural investigations indicate that the sympathetic neurons are more sensitive to testosterone than to physical exercise. The combination of testosterone plus training indicates overlapping effects of these two stimuli. The system of adrenergic nerve fibers seems to be overstimulated. Its reaction pattern is found not only to depend on time but also on the intensity of the stimuli.

Can nursing facility staff with minimal education be successfully trained with computer-based training?

This study compares the effects of computer-based and instructor-led training on long-term care staff with a high school education or less on fire safety knowledge, attitudes, and practices. Findings show that both methods of instruction were effective in increasing staff tests scores from pre- to posttest. Scores of both groups were lower at follow-up three months later but continued to be higher than at pretest. Staff with a high school education increased scores more than those without a high school diploma.

Autogenic regulation training and sickness absence amongst student nurses in general training.

Sickness absence has been a long-standing concern of the nursing profession. In this paper, sickness is construed as illness or the form a complaint takes. Absence is formulated as a form of coping. A longitudinal study using an experimental and control group design was carried out with student nurses using autogenic regulation training (ART). Comparisons of the relationship between ART and sickness absence records were recorded for: total days off; certificated and uncertificated days off; uncertificated days off; certificated days off; total blocks and total wards; first and second ward allocations; and average time sick. It is argued that ART helped significantly to reduce overall sickness absence in the experimental group when compared with the control group subjects. Reasons for this are discussed and issues for further research are raised. It is concluded from the present study that ART has a contribution to make in preparing student nurses to cope with the demands of nursing and sickness absence.
