Effects of thermal application on dysphagia after stroke.

The influence of thermal application on dysphagia for liquid boluses after multiple cerebrovascular accidents was measured. A month-long trial of thermal application organized according to the guidelines of a single-subject withdrawal or ABAB design was replicated across 7 male subjects. After baseline testing, 6 subjects were randomly assigned to begin the study with a week-long period of thermal application (B stage), and 1 subject was randomly assigned to begin the study with a week-long period of no thermal application (A stage). Subsequent week-long A and B stages were then alternated until each subject had completed the 4-week interval of this study. Progress testing occurred at the end of each stage and at follow-up testing 1 month after the study's completion. The influence of thermal application on eight duration and four descriptive measures was determined by three judges using visual inspection of data displays. Two of three judges agreed that 2 of the subjects demonstrated decreased Duration of Stage Transition in the absence of any change in the occurrence of aspiration or penetration. Overall, this study failed to reveal strong evidence that 2 weeks of thermal application alternating with 2 weeks of no thermal application improves dysphagia following multiple strokes. Further replications are needed.

Pharmacokinetics of a contraceptive patch (Evra/Ortho Evra) containing norelgestromin and ethinyloestradiol at four application sites.

AIMS: To determine the pharmacokinetic profile of norelgestromin (NGMN) and ethinyloestradiol (EE) following application of the contraceptive patch, Evra/Ortho Evra, at each of four anatomic sites (abdomen, buttock, arm, and torso). METHODS: Thirty-seven healthy, nonpregnant women aged 20-45 years participated in this open-label, four-period crossover study. Subjects were randomized to one of four treatment (site of application) sequences. Each patch was worn for 7 days, with a 1 month washout between treatments. Blood samples were collected before and at various times up to 240 h after application of each patch. Serum samples were assayed for NGMN and EE by validated methods. RESULTS: The serum concentration reference ranges for NGMN and EE are 0.6-1.2 ng ml-1 and 25-75 pg ml-1, respectively, based on studies of the mean Cave of oral norgestimate 250 microg and EE 35 microg. For all application sites, mean concentrations of NGMN and EE remained within these ranges during the 7 day wear period. Absorption of NGMN and EE during patch application on the buttock, arm, and torso was equivalent. Absorption of NGMN and EE during patch application on the abdomen was approximately 20% less than observed for the other three sites, although mean serum concentrations were still within reference ranges. A previous study demonstrated therapeutic equivalence of patches worn on the abdomen vs other sites. CONCLUSIONS: Serum concentrations of NGMN and EE from the contraceptive patch remain within the reference ranges throughout the 7 day wear period, regardless of the site of application (abdomen, buttock, arm, or torso).

Ablation of idiopathic ventricular tachycardia by bipolar radiofrequency current application between the left aortic sinus and the left ventricle.

BACKGROUND: Failure to ablate idiopathic ventricular outflow tract tachycardia by radiofrequency current is not uncommon and suggests that non-standard approaches may be required to map and suppress idiopathic ventricular tachyarrhythmias in some patients. METHODS AND RESULTS: Left and right ventricular activation and pace mapping proved inadequate for radiofrequency application in a patient with idiopathic ventricular outflow tract tachycardia. Presystolic activity was recorded at the left aortic sinus of Valsalva, and the QRS complex recorded at this location during pacing showed few differences compared with that recorded during tachycardia. Radiofrequency current application at this site transiently suppressed the tachycardia. Following new mapping of the left ventricle outflow tract, radiofrequency application just below the aortic valve in close proximity to the previous aortic application site transiently abolished the arrhythmia. Finally, bipolar radiofrequency application between the distal electrode of the aortic catheter and the distal electrode of a second catheter placed in the left ventricular subaortic area permanently suppressed the tachycardia. CONCLUSION: Bipolar radiofrequency application between the aortic sinus of Valsalva and the left ventricle could be an alternative approach in occasional patients with idiopathic ventricular outflow tract tachycardia resistant to conventional left ventricular and aortic root unipolar radiofrequency application.

Estradiol and estrone plasma levels during application of three strengths of a 7-day estradiol transdermal patch.

BACKGROUND: A new estradiol transdermal patch was developed for a once weekly application, with the aim to achieve an optimum practicability and to improve long-term compliance with estrogen replacement therapy. The pharmacokinetics of estradiol (CAS 50-28-2) and of estrone (CAS 53-16-7) during a 7-day application of the new patch is reported in this publication. METHODS: Unconjugated estradiol and estrone were assayed in plasma in a three-way crossover study on 18 postmenopausal women during and after a 7-day application of 3 strengths of the new patch, with daily release rates of 25, 50 and 75 micrograms of estradiol. RESULTS: During the 7-day application of the transdermal patches the concentration in plasma of unconjugated estradiol increased from less than 5 pg/ml, typical of postmenopause, to average concentrations of 26, 49 and 66 pg/ml under the patches with the release rates of 25, 50 and 75 micrograms/day of estradiol, respectively. The increases were linearly related and proportional to the strength of the patches. Upon removal of the patches, the estradiol concentrations returned to the basal postmenopausal values in 8-24 h. Retarded with regard to estradiol, there was also an increase of unconjugated estrone, from basal concentrations of 24 pg/ml to average concentrations of 39, 54 and 62 pg/ml, respectively. Estrone returned to its basal concentrations 24-48 h after removal of the patches. The estradiol/estrone ratio from the low pre-treatment values of 0.15-0.21 typical of postmenopause increased to average values of 0.51, 0.92 and respectively 1.09 during the application of the patches with the three strengths. The ratios are in the range of those of unconjugated hormones during the fertile age of women. The patches were well tolerated by the skin, with rare mild and transient reactions that disappeared spontaneously and did not cause interruption of treatment. Also the systemic tolerability was good, with occasional mild or moderate side effects typical of estradiol found especially under the application of the two higher strengths, i.e. with release of 50 and 75 micrograms/day of estradiol. CONCLUSIONS: The effective pharmacokinetic performance over the 7-day application, combined with the good general and local tolerability and the need to apply the patches only once weekly confer to the new patches a favorable practicability for the long-term estrogen replacement therapy needed to control the most severe postmenopausal disorders.

Prospective evaluation of hemoclip application with injection of epinephrine in hypertonic saline solution for hemostasis in unstable patients with shock caused by upper GI bleeding.

BACKGROUND: It is difficult to arrest severe upper GI bleeding with any of the available hemostatic modalities in unstable patients who are in shock, and the rates of persistent bleeding and mortality in this group remain high. This prospective study evaluated hemoclip application alone and in combination with injection of hypertonic saline solution with epinephrine in this subgroup of patients with GI bleeding. METHODS: Twenty-two patients in shock because of upper GI bleeding were enrolled and divided into 2 groups based on the response of systolic blood pressure to rapid infusion of 1000 mL of lactated Ringer's solution: an unstable shock group, in which systolic blood pressure did not stabilize at greater than 90 mm Hg, and a stable shock group, in which systolic blood pressure stabilized at greater than 90 mm Hg. Emergency endoscopy was performed in both groups; those in the stable group were treated by hemoclip application alone and those in the unstable group were treated by hemoclip application combined with injection of hypertonic saline solution with epinephrine. The following parameters were compared: vital signs on admission and after infusion of lactated Ringer's solution, hemoglobin concentration, endoscopic classification of type and site of bleeding, number of hemoclips required to arrest bleeding, volume of hypertonic saline solution with epinephrine injected, initial hemostatic rate, rate of recurrent bleeding, the need for additional preventive therapy (hemoclip application), and mortality. RESULTS: The rate of initial hemostasis was 92% in the stable shock group and 100% in the unstable shock group. Bleeding did not recur in either group. The volume of packed red cells transfused and the endotracheal intubation rate were significantly greater in the unstable shock group. Preventive application of hemoclips was performed at endoscopic follow-up 12 times in 10 patients in the stable shock group and 9 times in 7 patients in the unstable shock group. There were no deaths in either group. CONCLUSION: Endoscopic injection of hypertonic saline solution with epinephrine combined with hemoclip application provides effective hemostasis in unstable patients in shock caused by severe upper GI bleeding. The hemostatic result is comparable with that achieved by hemoclip application alone in patients with bleeding but less severe shock.

Relationship of physical appearance and professional demeanor to interview evaluations and rankings of medical residency applicants.

Faculty members rated 54 applicants to a medical residency training program on their social skills and professional commitment. Independent ratings of the candidates' physical attractiveness, neatness and grooming, and (for 18 of these applicants) professional demeanor also were obtained. Professional demeanor ratings were related significantly to the faculty interview ratings of professional commitment and to the subsequent final rankings of the 10 male and the eight female applicants for whom demeanor ratings were available. Ratings of neatness and grooming were related significantly to the faculty interview ratings of social skills and to the subsequent final rankings of the 21 female applicants but not for the 33 male applicants. Physical attractiveness ratings were not related significantly to the faculty interview ratings or the final rankings of candidates of either sex. These data suggest that physical appearance may have had some effect on the interview evaluations and selection of female applicants, and that professional demeanor had a marked influence on the evaluation and selection of both male and female applicants.

Design and characterization of an intracavitary ultrasound hyperthermia applicator for recurrent or residual lesions in the vaginal cuff.

For evaluating the feasibility of treating recurrent lesions in the vaginal cuff by hyperthermia, a 2-element ultrasound applicator was designed, constructed and characterized. A half-cylindrical transducer (d=1 cm, length=1 cm) was used to construct the 2-element ultrasound applicator. Each element of this applicator was operated at 1.5 MHz and characterized by measuring transducer efficiency and acoustic power distribution. Thermocouple probes were used to measure the temperature rise in the phantom. The element sizes used in this study were selected to be comparable to a high dose rate brachytherapy colpostat applicator. Each element was powered separately to achieve a desired temperature pattern in a target. The acoustic output power as a function of applied electric power of elements 1 and 2 were linear over this 1-40 W range and efficiencies were 32.2 +/- 3.4% and 46.2 +/- 0.8%, respectively. The temperature measurements in the phantom showed that a 6 degrees C temperature rise was achieved 2 cm from the applicator surface. As a conclusion, the ability of the ultrasound colpostat applicator to be used for hyperthermia was demonstrated by measuring acoustic output power, ultrasound field distribution and temperature rise in the phantom. Based on the characteristics of this applicator, it has the potential to be useful for inducing hyperthermia to the vaginal cuff in the clinic.

Effect of imazaquin applications on the growth, leaf chlorophyll and yield of soybean in the Guinea savanna of Nigeria.

Field trials were conducted during the wet seasons of 1989, 1991, 1994 and 1995 to evaluate the effects of pre-emergence and post-emergence applications of four rates of imazaquin (0.00; 0.15; 0.30 and 0.45kg a.i/ha) on the growth, leaf chlorophyll types and grain yield of soybean c.v. SAMSOY 2. Imazaquin applications had no significant effect on the growth of soybean roots on most of the sampling dates, but pre-and post-emergence applications of imazaquin at 0.30 and 0.45kg a.i/ha reduced soybean root nodules at 5 and 7 weeks after planting (WAP). Soybean shoot growth was generally reduced at 5WAP by the pre-emergence and at 7WAP by the higher rates (0.30 & 0.45kg a.i/ha) of post-emergence application of imazaquin. Pre-and post-emergence applications of imazaquin showed a strong tendency to reduce the concentration of chlorophyll a and total chlorophyll at 3 and 5WAP respectively. The concentrations of chlorophyll types in soybean leaves at 9WAP was generally comparable among most treatments especially in 1991. Whole plant fresh weight of soybean at 7WAP was reduced by all rates of post-emergence application of imazaquin. However, there was no significant difference in the whole plant dry weight of soybean at 3 to 7 WAP in 1989 and at 3, 5 and 9WAP in 1991. In each trial, pre-and post-emergence applications of soybean significantly increased the grain yield of soybean compared with the control treatment. This study showed that, inhibition of soybean shoot growth and leaf chlorophyll concentration was transient and that soybean plants require about 6 weeks for complete recovery from imazaquin phytotoxicity.

A preliminary examination of the translocation of microencapsulated cyfluthrin following applications to the perimeter of residential dwellings.

Methods have been developed to monitor the translocation of microencapsulated cyfluthrin following perimeter applications to residential dwellings. A pilot study was implemented to determine both the potential for application spray to drift away from dwellings and the intrusion of residues into homes following perimeter treatments. Residential monitoring included measuring spray drift using cellulose filter paper and the collection of soil samples from within the spray zone. In addition, interior air was monitored using fiberglass filter paper as a sorbent medium and cotton ball swabs were used to collect surface wipes. Fortification of matrixes resulted in recoveries of > 90%. Spray drift was highest at the point of application and declined to low but measurable levels 9.1 m from the foundations of dwellings. Soil residues declined to low, but measurable levels by 45 days post-application. No cyfluthrin was measured from indoor air; however, some interior surfaces had detectable levels of cyfluthrin until three days post-application. Findings indicate that spray drift resulting from perimeter applications might contaminate non-target surfaces outside the spray zone. Soil borne residues may serve as persistent sources for human exposure and potentially intrude into dwellings through the activities of occupants and pets. Residues do not appreciably translocate through air and consequently inhalation is not a likely route for human exposure. Surface residues detected indoors suggest that the physical movement of residues from the exterior to the interior might be a viable route of movement of residues following this type of application.

Electrodialytic phenomena and their applications in the dairy industry: a review.

Electrodialysis (ED) is an electrochemical separation process by which electrically-charged species are transportedfrom one solution to another ED is a combined method of dialysis and electrolysis and can be performed with two main cell types: multi-membrane cells for dilution-concentration and water dissociation applications (membrane phenomena), and electrolysis cells for redox reactions (electrode phenomena). The dilution-concentration principle applications in the dairy industry consist mainly of the demineralization of milk or milk by-products. The use of ED with monopolar membrane for protein separation and acid caseinate production, and in bioreactors for organic acid production, is also studied in the dairy industry. The interest of ED as a membrane process has been triggered recently by the development of a new membrane type, bipolar membrane. This membrane carries out the dissociation of water molecules. ED with bipolar membranes was applied very recently to the production of lactic acid from whey product fermentation, production of caseinates, and fractionation of whey proteins. Two principle applications of electrode reactions were published: electrochemical coagulation (EC) to precipitate milk proteins, and electroreduction for the reduction of disulfide bonds in the proteins. It appears in this article that processes using membrane phenomena are more numerous and developed than electrolytic applications. This is the composition of milk and the lack of knowledge of redox reactions of the different food compounds that limit the applications and the development of electrolytic phenomena. Electrodialytic phenomena present a great potential for application in the dairy industry, and more generally, in the food industry; many of these applications have to be discovered.

Effects of several applications of digested sewage sludge on soil and plants.

Sewage sludge production has significantly increased during the last years in European Union (EU) countries, being primarily used for agricultural purposes. In this study, digested sewage sludge was applied to greenhouse soil over a three-year period (2001--2003), with three sludge treatments in the first two years (2, 4, and 6 kg m(-2)) and three more applications using a greater quantity in the last year (6, 8, and 10 kg m(-2)). The effects of sewage sludge application on soil and on a leafy crop (Lactuca sativa L.) were studied. Mineral elements, organic matter, pH, and heavy metals were measured in soil and plant tissues. Pathogen and indicator microorganism dynamics in soil were also determined after each sludge application. Results showed that sewage sludge applications increased organic matter, P, and N Ca content in soil. Furthermore, Zn, Pb, Ni, and Cu content increased in soils, primarily after high doses of sludge. The highest yield value was obtained in the second-year harvest, since the last sludge application did not increase yield. Fecal coliform numbers decreased significantly one month after sludge application. However, total coliforms, Clostridium sulphite-reducers and Salmonella, were present in soils three months after sludge application.

Odor measurements for manure spreading using a subsurface deposition applicator.

Odor emissions during manure spreading events have become a source of concern, particularly where farms are located nearby urban areas. The objective of the present study was to compare odor concentrations and odor emission rates due to pig manure application using two different types of applicators, a sub-surface deposition system and a conventional splash-plate applicator. Air samples were collected using a Surface Isolation Flux Chamber and the "bag-in-vacuum chamber" techniques, at 0.5, 1.5 and 2.5 hours after manure application. A three-station forced-choice dynamic dilution olfactometer was used by an odor panel for determining odor concentration. Preliminary results indicated that with the sub-surface deposition system applicator odor emission rate was reduced by 8% to 38% compared to that of the conventional splash-plate applicator. The highest reduction in odor strength and odor emission rate was observed in the most offensive period after manure application. The sub-surface deposition system may be a solution for hog producers who wish to reduce odor complaints from applying manure without the cost and problems associated with deep injection systems.

An evaluation of microwave diathermy applicators using free space electric field mapping.

An electric field mapping study is described which was designed and executed to study the performance characteristics of various prototype and currently used microwave diathermy applicators operating at 2450 MHZ. A miniature diode detector/dipole probe, developed by the Bureau of Radiological Health, was used to map the 'near' electric fields in the aperture plane of each applicator. From the field mapping results, parameters for use in the evaluation of the radiation safety of various applicators are developed. A thermographic camera was used to determine heating characteristics of the applicators in simulated biological material. The thermal effectiveness of these applicators was also investigated by determining their Specific Absorption Rate, a recently developed parameter. The superiority of a specially designed prototype applicator (in terms of uniformity and effectiveness of energy deposition, and minimal leakage radiation) compared with currently used applicators is demonstrated.

Theoretical and experimental comparison of three types of electromagnetic hyperthermia applicator.

There is evidence that heating of malignant tissue in the treatment of cancer may be beneficial and so the performance of different applicator designs needs to be established. A theoretical model of the dielectric loaded waveguide applicator is compared with models of two other applicators which depend on energy radiated from conductors carrying high frequency current. The latter are exemplified by the compact resonant patch applicator and the lightweight inductive current sheet applicator. It is shown that heating profiles and field penetration of each applicator are similar for equal radiating areas, and these results have been substantiated experimentally. Impedance match as a function of frequency and load is also compared for the three types of applicator.

Theoretical model of internally cooled interstitial ultrasound applicators for thermal therapy.

Interstitial ultrasound applicators for high-temperature thermal therapy are currently being developed for treating cancerous and benign disease. Internally cooled, direct-coupled (ICDC) applicators, composed of a segmented array of cylindrical ultrasound transducers, have demonstrated capabilities of producing controllable and conformal heating distributions along the applicator length and angular orientation. In this study, 2D transient acoustic and biothermal models of ICDC applicators were developed using a mixed implicit and explicit finite difference solution with variable node spacing in cylindrical coordinates for enhanced speed, stability and accuracy. The model incorporates dynamic behaviour of acoustic parameters and blood perfusion as a function of temperature and thermal dose. Acoustic intensity distributions were modelled as a composite of measured and theoretical intensity distributions. The shape and time evolution of temperature contours and thermal lesions for 90 degrees, 200 and 360 degrees angularly directional applicators and multi-transducer applicators were modelled for heating durations between 1 and 5 min. Model parameters were selected to match previously reported ex vivo and in vivo studies of 2.2 mm diameter ICDC devices in thigh muscle and liver (15-30 W cm(-2) applied power density, 0.5-5 min treatment times, 2.8-3.6 cm diameter thermal lesions). The temperatures and lethal thermal dose (600 EM43 degrees C) contours calculated using the models were in excellent agreement with temperatures and thermal lesion dimensions (visible coagulation) determined experimentally. The differences between maximum radial depths of coagulation calculated using the r-z and r-theta models were small, less than approximately 2 mm for 10-15 mm lesions. There was a strong correlation between the calculated 50 degrees C contour and the radial, angular and axial lesion dimensions obtained for 3-5 min heating protocols. The models developed in this study have significant application in design studies and potential future use in treatment planning of ICDC interstitial ultrasound thermal therapy.

Highly directional transurethral ultrasound applicators with rotational control for MRI-guided prostatic thermal therapy.

Transurethral ultrasound applicators with highly directional energy deposition and rotational control were investigated for precise treatment of benign prostatic hyperplasia (BPH) and adenocarcinoma of the prostate (CaP). Two types of catheter-based applicators were fabricated, using either 90 degrees sectored tubular (3.5 mm OD x 10 mm) or planar transducers (3.5 mm x 10 mm). They were constructed to be MRI compatible, minimally invasive and allow for manual rotation of the transducer array within a 10 mm cooling balloon. In vivo evaluations of the applicators were performed in canine prostates (n = 3) using MRI guidance (0.5 T interventional magnet). MR temperature imaging (MRTI) utilizing the proton resonance frequency shift method was used to acquire multiple-slice temperature overlays in real time for monitoring and guiding the thermal treatments. Post-treatment T1-weighted contrast-enhanced imaging and triphenyl tetrazolium chloride stained tissue sections were used to define regions of tissue coagulation. Single sonications with the 90 degrees tubular applicator (9-15 W, 12 min, 8 MHz) produced coagulated zones covering an 80 degrees wedge of the prostate extending from 1-2 mm outside the urethra to the outer boundary of the gland (16 mm radial coagulation). Single sonications with the planar applicator (15-20 W, 10 min, approximately 8 MHz) generated thermal lesions of approximately 30 degrees extending to the prostate boundary. Multiple sequential sonications (sweeping) of a planar applicator (12 W with eight rotations of 30 degrees each) demonstrated controllable coagulation of a 270 degrees contiguous section of the prostate extending to the capsule boundary. The feasibility of using highly directional transurethral ultrasound applicators with rotational capabilities to selectively coagulate regions of the prostate while monitoring and controlling the treatments with MRTI was demonstrated in this study.

Determination of ocular toxicity in multiple applications of foscarnet iontophoresis.

This is the first study of multiple applications of drug iontophoresis in the eye. We repeated ocular foscarnet iontophoresis in 10 eyes of 10 rabbits every third day at the same paralimbal site for a total of seven applications over a period of 21 days to determine the efficacy and toxicity of multiple applications of ocular foscarnet iontophoresis. Mean vitreous human foscarnet concentration of 189 +/- 50.6 microM (SD) was achieved four hours after the seventh consecutive iontophoretic application over a period of twenty-one days. These levels were within the therapeutic range (25-800 microM) for the treatment of CMV retinitis and comparable to the intravitreal foscarnet concentrations achieved in eyes treated with a only a single application of ocular iontophoresis. Electroretinography (ERG) and Slit-lamp biomicroscopy responses revealed no evidence of ocular toxicity. Indirect ophthalmoscopy of the retinas and gross examinations of the calottes revealed a single, small burn in the retina and choroid corresponding to the application site of the iontophoresis probe similar to the lesion resulting from a single application of iontophoresis. Light and electron microscopy revealed local tissue injury and fibrosis at the iontophoresis site, but adjacent areas were unaffected.
