[Primary leiomyosarcoma of the heart, report of a case and review of the literature].

Primary cardiac neoplasms are unusual and are found in only about 0.0017% of autopsies. Approximately 25% of primary cardiac neoplasms are malignant. A case of primary leiomyosarcoma arising from the right ventricle and the pulmonary artery is reported. A 37-year-old man had sudden dyspnea and chest pain. Angiocardiography and MRI revealed moving masses throughout the right ventricle and the pulmonary arteries. An emergent surgery was performed. The multiple tumors were observed and incomplete resection was done. The patient was survived with the aid of a veno-arterial bypass as an assisted circulation on the first postoperative day. However, he gradually became deteriorated again and died 8 days after the operation due to the right ventricular failure. Eleven cases of the primary cardiac leiomyosarcoma including our case in the Japanese literature were reviewed briefly.

Leiomyosarcoma of the thoracic aorta.

This case involves a patient with aortic leiomyosarcoma. A patient clinically suspected of type IIIa dissecting aortic aneurysm underwent surgery. The descending thoracic aorta was found to be filled with a soft, yellow tumor and was replaced with a woven Dacron graft. Microscopy of the surgical specimen revealed large, atypical spindle cells with numerous mitoses in bundles intersecting at 90 degrees, suggesting leiomyosarcoma. This diagnosis was confirmed immunohistochemical study. The postoperative course was uneventful and the patient was discharged on postoperative day 40; however, the patient developed bronchopneumonia due to malignant pleuritis 3 months postoperatively and died on postoperative day 103.

Primary leiomyosarcoma of the heart subsequent to double carcinomas of the thyroid and lung.

A 63-year-old woman underwent surgical operations for left lower lung cancer and for thyroid cancer. Nine months later, a third cancer developed in her heart and this tumor was removed by open heart surgery. A pathologic study revealed that the tumor was primary leiomyosarcoma of the heart and thus independent from the previous lung and thyroid carcinomas. This case was regarded as a triple carcinoma including a primary leiomyosarcoma arising from the left atrium. Reports in the literature on primary malignant tumors of the heart are reviewed briefly.

Collision tumour in the pelvic cavity: rectal leiomyosarcoma and prostate adenocarcinoma.

We report a rare and, to our knowledge, as yet undescribed type of collision tumour - rectal leiomyosarcoma and prostate adenocarcinoma. Our study also provides the first data on molecular alterations [polymerase chain reaction/loss of heterozygosity (LOH) analysis] of the APC, NF-1, DCC, p53, nm23-H1 and BRCA-1 genes in the two components of the collision tumour. None of the genes examined in this study expressed LOH in the prostate carcinoma component of the collision tumour. By contrast, in the leiomyosarcoma component, LOH was found at the DCC and p53 genes, proving that these two tumours did not arise from the same stem cell but represent two different neoplastic growths.

[Weakly malignant leiomyosarcoma of the inferior vena cava. A case report].

Primary vascular leiomyosarcomas of the great vessels are rare and the most common localization is the inferior vena cava. Due to nonspecific clinical signs, diagnosis is often made during operation or autopsy. At present, a radical tumor resection is the first choice of treatment. This is a case report of a 49-year-old woman who suffered from abdominal pain for 10 years because of a weakly malignant leiomyosarcoma of the inferior vena cava. A radical tumor resection and reconstruction with a prosthetic vascular graft were performed. Diagnosis, therapy, and prognosis are discussed.

[Leiomyosarcoma of the inferior vena cava].

Leiomyosarcomas of the inferior vena cava are rare and the clinical symptoms unspecific. We report a case of leiomyosarcoma of the inferior vena cava in an 82-year-old woman presenting with weight loss and abdominal pain. Following elaborate preoperative examinations, surgical resection was performed and the inferior vena cava was reconstructed. Clinical signs, diagnosis, therapy, and prognosis are discussed.

[Leiomyosarcoma of the splenic artery].

Leiomyosarcomas of vascular origin are a very rare group of tumors developing from smooth vascular muscle cells. The most common location are in large vessels, e.g. vena cava, saphenous vein or pulmonary artery, but fewer than 200 cases are reported in the literature. We report a case of a patient with a leiomyosarcoma of the splenic artery. This location has not been described previously.

[Painful cutaneous leiomyosarcoma].

Leiomyosarcomas are derived from smooth muscles and only rarely involve the skin. A 73-year-old male presented with progressive tumor painful on the chest which had been present for 3 years and was tender to palpation, a finding more often associated with benign leiomyomas. immunohistochemical staining demonstrated multiple nerve fibers entrapped by atypical myofibrils, perhaps explaining the tenderness. Since 1996, we have excised five painless cutaneous leiomyosarcomas, in which entrapped nerves could not be demonstrated.

[Cutaneous metastases from a leiomyosarcoma of the testicular tunica albuginea].

Cutaneous metastases from a leiomyosarcoma of the testicular tunica albuginea are very rare. Primary leiomyosarcomas in this site are uncommon and furthermore cutaneous metastases develop only in 1,4% of all malignant tumors of the internal organs [10]. A 71 year old patient with such a sarcoma underwent primary excision. Nine months later multiple, firm, nodules were identified as cutaneous metastases. Further staging revealed already extensive, diffuse pulmonary and bony metastases without contiguous or regional lymph node involvement.

Leiomyosarcoma of the kidney in an HIV-infected child.

Spindle cell tumors (leiomyoma and leiomyosarcoma) have been described in immunocompromised children after transplantation and in association with HIV infection. Previous reports have described tumors of respiratory (lung, bronchi), gastrointestinal (stomach, bowel, liver), and subcutaneous origins. We report a case of leiomyosarcoma of the kidney in an HIV-infected child.

Leiomyosarcoma of the popliteal vein.

Primary vascular leiomyosarcomas are rare tumors arising from the media of vessel walls, involving the extremities in about one third of reported cases, and the popliteal vein even less frequently. We report a case of popliteal vein leiomyosarcoma in a 62-year old man who presented with leg pain and edema 4 weeks following spine surgery. Findings on ultrasound, MRI and CT angiography are reviewed, with particular emphasis on the value of CT angiography in fully evaluating the extent of the mass and in assisting percutaneous biopsy.

Leiomyosarcoma of the distal femur in a patient with a history of bilateral retinoblastoma: a case report and review of the literature.

We report a case of primary leiomyosarcoma of the distal femoral shaft arising in a patient who had undergone bilateral orbital enucleation for bilateral retinoblastoma several years previously. Radiography demonstrated an osteolytic, expansive lesion with cortical destruction anteriorly in the distal femoral shaft, and these findings were confirmed on CT. MR imaging revealed an expansive intramedullary lesion with cortical breakthrough and soft tissue extension. The occurrence of a second malignancy in patients with a history of bilateral retinoblastoma is well documented. Many different histological types have been described, with osteosarcoma and leiomyosarcoma occurring with the greatest frequency.

Primary leiomyosarcoma of bone.

We report a case of primary leiomyosarcoma in the right femoral diaphysis. Conventional radiographs revealed an indistinct osteolytic lesion with a very subtle periosteal reaction. MR imaging revealed an intramedullary lesion with a well-defined margin, cortical breakthrough and soft tissue extension. The lesion was isointense relative to muscle on T1-weighted imaging, iso-hypointense relative to fat on fast spin echo T2-weighted imaging, and hyperintense to muscle on fast spin echo T2-weighted imaging with fat saturation. A peripheral rim and intratumoral spoke-wheel-like enhancement in the soft tissue component of the tumor was noted on gadolinium-enhanced T1-weighted imaging with fat saturation. Such an enhancing pattern, which has not been described in the literature previously but has been seen in leiomyosarcomas elsewhere by both MR imaging and computed tomography, deserves further pathological correlation.

Abdominal aortic invasion by leiomyosarcoma.

We describe a case of retroperitoneal leiomyosarcoma with extra- and intravascular involvement. The patient presented with abdominal aortic rupture secondary to tumor invasion and extensive tumor thrombus in the inferior vena cava. The presence of a rupture of the abdominal aorta is an important radiologic diagnosis to make because early endovascular or surgical repair can prevent a catastrophic free rupture. Inferior vena cava involvement by leiomyosarcoma may be locally resected in some cases but has a poor long-term survival rate.

[Leiomyosarcoma of the inferior vena cava. Review of a rare disease].

Primary sarcomas of the great vessels are rare. Their most common site is the inferior vena cava. We report a primary leiomyosarcoma of a 41 year old female patient localised just below the right renal vein. The resected tumour showed the histological signs of a well differentiated, progesterone positive leiomyosarcoma. The case is discussed on the data of the reviewed literature.

A recurrent left atrium leiomyosarcoma.

Cardiac leiomyosarcoma is a rare tumour which can grow either intramurally or extramurally. Its prognosis is dismal. Surgery lengthens survival, but recurrences usually occur in the short term. Gated MRI is useful for the assessment of cardiac masses since it provides fine spatial and contrast resolution. We present the case of a left atrial leiomyosarcoma whose recurrence was diagnosed by gated MRI. Our patient underwent a second resection but died soon after.

Small leiomyosarcoma of the renal capsule: CT findings.

Three unusual cases of small-size leiomyosarcoma of the perirenal space were studied with CT. The renal capsule has been proved to be the origin of this type of tumor. A CT examination is accurate in suggesting the site of origin and excluding a renal cell carcinoma. However, unless evidence of invasion is noted, it is impossible on CT features to discriminate leiomyosarcoma from a benign leiomyoma.
