Affective disorders and endocrine disease. New insights from psychosomatic studies.

This is a review of psychosomatic interactions between affective disorders (depressive and anxiety disturbances, irritable mood) and endocrine disease. Particular reference is made to stressful life events in the pathogenesis of endocrine disease, psychopathology of hormonal disturbances, and pathophysiology of hypothalamic-pituitary-adrenal axis function in depression and Cushing's disease. These psychosomatic interactions may lead to appraisal of common etiological mechanisms in endocrine and psychiatric disorders, of the value of retaining the category of organic affective syndromes in psychiatric classification, and of the need for research on quality-of-life measures in endocrine disease. The establishment of "psychoendocrine units," where both endocrinologists and psychiatrists should work, is advocated. Such psychoendocrine units may serve and benefit clinical populations who currently defy traditional medical subdivisions.

Antidiuretic hormone and psychosomatic aspects in Menière's disease.

We studied the relation between psychosomatic profiles of patients with Menière's disease and antidiuretic hormone (ADH). For investigation of the psychosomatic aspects, we used the Cornell Medical Index (CMI), Yatabe-Guilford personality (Y-G) test and the originally produced stress questionnaire. In Y-G test, patients with Menière's disease are classified in normal group. In the CMI test, on the other hand, types III and IV were significantly more often observed in Menière's disease than normal control (chi 2 test, p < 0.05). However, the CMI test results have no correlation to plasma ADH (p-ADH) levels. The survey of our original questionnaire revealed that stress has a close relationship to vertiginous attacks. Further, high p-ADH levels were significantly frequently observed in cases with stress compared with cases without stress (t-test, p < 0.05).

[Psychosomatic aspects of gastrointestinal diseases].

The article covers up-to-date information about the incidence of psychosomatic pathology among patients of gastroenterologic profile including patients with gastroesophageal reflux disease and syndrome of irritable colon. From the point of view of psychosomatic idea the article considers the factors predisposing, permitting and retarding the development of the disease. A detailed description is available in psychosomatic disturbances, modern concepts about their pathogenesis including a break of correlation between limbic and frontal regions of central nervous system and disorder in serotonin metabolism--clinical manifestations, methods of diagnosis and treatment including selective inhibitors and stimulators of serotonin reuptake and neuroleptics.

[Hypnosis in dermatology].

Almost all skin diseases are caused by a variety of factors with psychogenic factors having a great influence on progression and the development of chronic conditions. Behavioural disturbances related to dermatological problems and essential pruritus are also mentioned.

[Psychosomatic aspects of neurologic diseases. Experiences with clinical neurology from the viewpoint of the psychoanalyst].

The great relevance of psychoanalytic-psychosomatic aspects for a more profound understanding of neurological diseases is the topic of this article. In terms of medical history, the development of the discipline of neurology especially in Germany is outlined, whereby neurology and psychosomatic medicine share the fate of a handicapped individuation. On the background of this delayed establishment as branches of their own, the mutual denial of neurology and psychosomatic medicine up to the present is discussed. By means of a survey of literature and case examples of several neurological disorders (hysterical conversion symptoms, selected diseases of the extrapyramidal motor system, lumbago, sciatica, cephalgia, epilepsy) the great importance of psychosomatic medicine and psychoanalysis for neurology is pointed out.

Rôle of psychosomatic factors in peptic ulcer disease.

Evidence indicates that a link between peptic ulcer disease (PUD) and selected psychosomatic factors may exist. A series of 70 consecutive male and female adult patients were categorized by peptic symptoms and divided into four groups: a) controls; b) gastric ulcer (GU); c) duodenal ulcer (DU); and d) chronic non-ulcer dyspepsia (CNUD). All patients were interviewed and asked to answer a questionnaire that included demographics, medical history and the incidence of negative life events. A decreased level of activity was a predominant finding in GU, DU and CNUD patients. Family history of PUD may be correlated with CNUD. Of interest was the finding that DU and CNUD patients presented a higher incidence of negative life events when compared to the other study groups. Negative life events that produce considerable stress may predispose to peptic symptoms in certain patients.
