[The sphenoid: tumoral pathology].

The interest and difficulty of sphenoid tumor imaging relate mainly to its great diversity. Detection may be difficult due to variable tumor size and often misleading clinical data. Because the sphenoid is located at the center of the skull base, tumor extension may be quite variable and difficult to assess, requiring sufficient knowledge of anatomy and imaging features to detect tumor spread, especially retrograde perineural spread, since this has a negative impact on prognosis.

[Magnetic resonance imaging in the diagnosis of nonorganic hydatid disease].

This article evaluates MRI diagnostic value in discovering of the non-organic hydatid disease. MRI data of 21 patients, suffering from parasite pathology of liver (n = 12), liver and peritoneum cavity (n = 2), liver and retroperitoneal space (n = 2), liver and thigh's muscles (n = 1), peritoneum cavity (n = 2), retroperitoneal space (n = 1), spine and paravertebral area (n = 1) were analyzed. Based on histopathological results, features of unusually localized hydatid cysts (HC) MRI- semiotics are described in detail and compared with liver echinococcosis. MRI technique for identification of some hydatid cyst's structures is shown. The authors discuss the MRI reliability in differential diagnosis of non-organic HC and several disorders (non-parasite congenital and acquired cysts, hematoma, abscess, metastasis) of the same anatomical region. They underline some MRI advantages in GD disclosing comparing with ultrasonography and computed tomography. However, serological tests are needed for diagnosis verification. The authors also postulate the importance of clinical data being taken into account for radiological conclusion.

[X-ray computed tomography in pleuropulmonary asbestos pathology].

Results of computed tomography and conventional radiology were compared during investigation of 17 patients with pleuropulmonary lesions from confirmed asbestosis. Computed tomography images appeared to provide improved data for detection and analysis of pleural plaques, particularly in the costovertebral grooves; false pleural images (associated shadows, fatty tissue) can be eliminated. The CT scan should occupy a place of choice for surveillance of subjects in contact with asbestos and presenting pleural anomalies.

[Therapeutic maneuvers in the treatment of paroxysmal positional vertigo: can they indicate a central genesis?].

Typical Paroxysmal Positional Nystagmus (PPNy) is a highly specific clinical finding that indicates the existence of a specific peripheral lesion: Paroxysmal Positional Vertigo (PPV). However, besides this typical nystagmus of high topodiagnostic significance, atypical PPNy's are also frequently found. Such atypical findings can lead one to suspect a central vestibular system pathology and often require additional diagnostic testing. Of 1805 patients with complaints of vertigo, 305 proved affected by PPV. The present study evaluated how patients affected by the posterior semicircular canal PPV (278/305, 91.2%) responded to canalith repositioning maneuvers. In 246 patients (88.5%) the Semont maneuver was performed as simplified by Toupet, while the remaining 32 subjects (11.5%) were treated with the Epley maneuver as modified by Herdman. The Semont maneuver provides the relief of symptoms in 96% of patients while the Epley maneuver resolved 78% of cases. The patients that did not respond to the maneuvers, even after several attempts, were subsequently treated using "habituation"-based rehabilitation techniques (the Brandt-Daroff technique and Vestibular Habituation Training). The presence of atypical PPNy was found in 57 subjects. Of these, the liberatory maneuver always resolved the PPV. In the present study, the choice to perform a neuroradiological study was based on the lack of response to physical therapy, whether performed with liberatory maneuvers or with habituation techniques. In 2 of the 3 cases of PPV which remained unchanged despite treatment, imaging revealed a severe expansive CNS pathology. In these patients the dizziness complaints disappeared after neurosurgery: this fact appears to confirm the central origin of these PPNy cases. These data clearly indicate that the results of PPV treatment must be carefully evaluated. In cases of PPV of the PSC, when either typical or atypical PPNy persists one month after diagnosis, with no appreciable period of remission, neuroradiological tests should be performed in order to rule out any CNS pathology.

Pathologic features of spinal disorders in patients treated with long-term hemodialysis.

STUDY DESIGN: Pathologic features of hemodialysis-associated spinal disorders were evaluated using preoperative radiographic images and histologic findings of the spinal lesions resected during surgery. OBJECTIVES: To investigate the pathology of hemodialysis-related spinal disorders and to determine the role of amyloidosis in the establishment of severe destruction of the spine. SUMMARY OF BACKGROUND DATA: The pathologic events leading to hemodialysis-associated spinal disorders are poorly understood. The distribution of amyloid deposits in the spine also has not been clarified. METHODS: Twenty patients with hemodialysis-associated spinal disorders were investigated regarding pathologic features of neural compression and spinal destruction. Preoperative radiographic images such as plain radiography, tomography, computed tomography, magnetic resonance imaging, and scintigraphy were assessed for the existence of an intracanal mass, hypertrophy of the ligamentum flavum, and destructive changes of the spinal components. Histologic examination also was conducted by light microscopy and scanning electron microscopy to determine the distribution pattern of amyloid deposits in the spinal components. RESULTS: Six patients with no destructive changes in the spine showed spinal canal stenosis. In the cervical spine, a main factor associated with spinal canal stenosis was the presence of intracanal amyloid deposits in three patients. In the lumbar spine, a main factor associated with spinal canal stenosis was hypertrophied ligamentum flavum in three patients. Destructive changes of the facet joints, intervertebral disc, and vertebral body were seen in the other 14 patients. Amyloid deposits were densely distributed at the enthesis of capsular fibers to the bone and in anular tears in the intervertebral discs. Vertebral end plates were destroyed by penetration of amyloid granulation into the vertebral body. Osteoclast activity in the destroyed vertebral bodies was enhanced, with no evidence of new bone formation. CONCLUSIONS: Amyloid deposits played an important role in the progression of spinal destruction and severe instability.

[Magnetic-resonance imaging in the diagnosis of overload injuries of bones].

A comparative evaluation was done of quality of roentgenological and magnetic-resonance imaging (MRI) data for the early diagnosis of overload injuries of bones. 57 servicemen were examined presenting with the above changes at different stages of the pathological process in the bones of the foot, shank, and thighbone. MRI permits detecting changes in stress-induced injuries of bones 2-3 weeks earlier than X-ray does. MRI has been shown to allow a higher significance judgement about changes taking place in soft and paraossal tissues of the extremity in stress-induced fractures. MRI is an alternative in early diagnosis of stress-related injuries, the repeated roentgenography and MRI help in interpreting plain rhoentgenograms promoting the accuracy of diagnosis in the pathology above mentioned.

Alpha EEG activity and subcortical pathology in HIV infection.

Quantitative electroencephalographic (QEEG) investigations of patients with HIV-1-infections were made to detect early signs of HIV brain involvement. QEEG data were recorded in a prospective controlled cohort study together with standardised clinical, immunological and neuropsychological tests. Subject groups were HIV-seronegative controls, HIV-seropositive subjects with asymptomatic infection, and HIV-seropositive subjects with symptomatic infection exclusing AIDS defining illnesses. Marked increase in background alpha amplitude preceded cognitive and neurological impairment in the symptomatic stage of infection. Elevation of alpha amplitude was also associated with change in psychiatric status. Antiretroviral medication suppressed this alpha elevation, supporting the usefulness of QEEG in monitoring drug CNS effects. In the context of clinical, neuropathological and brain imaging data it is suggested that these changes in the alpha rhythm are the earliest, albeit unspecific, signs of HIV brain involvement, reflecting the predominantly subcortical prominence of the pathological process.

Computational Pathology for Accurate Prediction of Breast Cancer Recurrence: Development and Validation of a Deep Learning-Based Tool.

Accurate recurrence risk stratification is crucial for optimizing treatment plans for breast cancer patients. Current prognostic tools like Oncotype DX offer valuable genomic insights into hormone receptor-positive and human epidermal growth factor receptor-negative patients but are limited by cost and accessibility, particularly in underserved populations. In this study, we present Deep-Breast-Cancer-Recurrence (BCR)-Auto, a deep learning-based computational pathology approach that predicts breast cancer recurrence risk from routine hematoxylin and eosin-stained whole slide images. Our methodology was validated on 2 independent cohorts: The Cancer Genome Atlas Program breast cancer data set and an in-house data set from The Ohio State University. Deep-BCR-Auto demonstrated robust performance in stratifying patients into low- and high-recurrence risk categories. On The Cancer Genome Atlas Program breast cancer data set, the model achieved an area under the receiver operating characteristic curve of 0.827, significantly outperforming the existing weakly supervised models (P = .041). In the independent The Ohio State University data set, Deep-BCR-Auto maintained strong generalizability, achieving an area under the receiver operating characteristic curve of 0.832, along with 82.0% accuracy, 85.0% specificity, and 67.7% sensitivity. These findings highlight the potential of computational pathology as a cost-effective alternative for recurrence risk assessment, broadening access to personalized treatment strategies. This study underscores the clinical utility of integrating deep learning-based computational pathology into routine pathological assessment for breast cancer prognosis across diverse clinical settings.

[Clinical features of pathologically confirmed metastatic bone tumors--a report of 390 cases].

BACKGROUND & OBJECTIVE: With the development of diagnostic techniques of imaging and pathology, early diagnosis of metastatic bone tumors has been greatly improved, but the clinical characteristics which are essential for diagnosis are rarely reported. In this article, the clinical features of pathologically confirmed metastatic bone tumors were analyzed for further improvement of early diagnosis and treatment. METHODS: Clinical data of 390 patients with pathologically confirmed metastatic bone tumors, treated from 1980 to 2003 at The First Affiliated Hospital of Sun Yat-sen University, were reviewed respectively to summarize the clinical features, including disease history, predilection sites, clinical manifestation, and imaging presentations. RESULTS: Of the 390 patients, the ratio of men to women was 2.12:1; the median age was 55.7 years, and 81.5% of the patients were over 41 years old. The primary tumors were lung cancer (21.8%), prostate cancer (13.1%), breast cancer (7.4%), liver cancer (6.4%), gastrointestinal cancer (5.7%), and unknown cancers (24.6%). The common metastatic sites were spine (47.7%), pelvis (18.2%), femur (15.4%), and rib (12.6%). Multiple metastases occurred in 20.5% of the patients. The main symptoms were skeletal pain (53.3%), pathologic fractures (10.3%), dysfunction (4.9%), and paraplegia (2.1%). Primary tumor detected before metastasis accounted for 29.7% of the patients with a median metastatic time of 319 days, and the metastatic intervals were uncertain in 70.3% of the patients. Osteolytic types accounted for 80.7% of the cases in radiographic patterns, followed by osteosclerotic (10.5%) and mixed types. CONCLUSIONS: Metastatic bone tumors most frequently occur in patients older than 41 years, and commonly originate from lung, prostate, breast, and liver. Vertebrae, pelvis, femur, and rib are the most common sites of metastases. The clinical manifestation is extensive and nonspecific. Most lesions present osteolytic patterns. Metastases with unknown origin account for 24%. In spite of complexity, the clinical features should be mastered for early diagnosis and treatment.

[Past, present and future of digital pathology].

The four main aspects of applied medical information technology, which change the traditional systems of the entire health service are signal and data processing, digital modelling and interface optimisation. The information technology serving individual clinical specialties including clinical histopathology is changing at each of the four levels resulting in transformation of the communication paradigms. The object of investigation in histopathology is the digital slide, which is accessible throughout the world with no time or geographical limits. It permits the digital modelling of routine histological and/or cytological slide and it also allows measurements by using image analysis or stereology software packages. The electronic slide can be viewed, examined and diagnosed on a computer connected to a microscope, a new interface in diagnostic histopathology. This study describes the main theoretical and practical aspects, including challenges, of digital pathology and it also discusses the conditions required for successful information management. It reviews the experiences of the last one and half decades gained in the field of pathological information technology in Hungary including its main episodes and milestones of development. Introducing its present state, this paper describes the concept and the mode of investigation of the digital slide. It shows the development and use of a virtual microscope in Hungary. Based on know-how including the British experience this review describes the possible uses of digital slides, which by improving communication could have a positive effect on the entire health care system. It summarises the possible and necessary components of a digital pathology laboratory, which may include the new Slide Archive and Communication System (SACS). Using experimental data it mentions the possibility of generating primary digital pathological sample and producing the so called optical biopsy with no need for removing tissue from the patient.

Advanced magnetic resonance imaging of articular cartilage.

MRI is one of the most accurate imaging methods available to diagnose disorders of articular cartilage. Conventional two-dimensional and three-dimensional approaches show changes in cartilage morphology. Newer and substantially faster three-dimensional imaging methods show great promise to improve MRI of cartilage. These methods may allow acquisition of fluid-sensitive isotropic data that can be reformatted into arbitrary planes for improved detection and visualization of pathology. Unique MRI contrast mechanisms also allow clinicians to probe cartilage physiology and detect early changes in cartilage macromolecules.

Evaluation of stress-related anterior lower leg pain with magnetic resonance imaging and intracompartmental pressure measurement.

The purpose of this work was to evaluate stress-related anterior lower leg pain with clinical examination, magnetic resonance imaging, and measurement of anterior tibial compartment pressure findings. All medical data were gathered from 24 conscripts with stress-related anterior lower leg pain. Twenty exhibited bilateral symptoms. In 22 of the 44 cases, the intracompartmental pressure was pathological. Symptoms were exhibited for longer periods by patients with chronic exertional compartment syndrome (CECS) than by other patients (p < 0.01). At rest, magnetic resonance imaging revealed no abnormal findings in the soft tissues of the legs but showed bone abnormalities in 35 symptomatic legs. Thirty-three exhibited bone stress injuries, and two exhibited leg traction periostitis. On magnetic resonance imaging, there was no difference in bone findings between patients with and without CECS (p > 0.05). Stress-related anterior lower leg pain can be related to CECS, bone stress injury, and traction periostitis. Clinical diagnosis is unreliable. CECS and bone stress injury or traction periostitis can occur separately or together.

Metastatic breast carcinoma confined to bone: portrait of a clinical entity.

BACKGROUND: The current study was performed to study metastatic breast carcinoma that remains confined to bone. METHODS: The medical notes of 2514 breast carcinoma patients who were treated in 2 academic units over a 20-year period were screened and patients who fulfilled the following criteria were selected: 1) clinical manifestation and imaging confirmation of bone metastases, and 2) metastatic disease remaining confined to bone for a minimum of 24 months. Available clinical and pathologic data were recorded and analyzed. The objective of the current study was to describe this clinical entity and investigate possible correlations between clinicopathologic parameters and clinical outcome. RESULTS: A total of 104 patients (4% of the total screened patient population) fulfilled the study criteria. The majority of patients were postmenopausal, with a median age of 58 years; 44 of the patients were found to have metastases at the time of presentation (M1) and 60 patients developed metastases at a median of 38 months (range, 8-160 months) after surgery for the primary tumor. Metastases remained confined to bone for a median of 50 months. Survival after the diagnosis of bony metastases was 72 months and was similar in the 2 groups (66 months vs. 78 months). Of the patients treated, 80% responded to hormonal therapy, and 76.5% responded to chemotherapy. There was no association noted between survival and tumor grade, anatomic distribution, or disease extension. CONCLUSIONS: Bone-confined metastatic breast carcinoma has an indolent clinical course that alleviates the need for vigorous follow-up and calls into question aggressive therapeutic approaches in these patients. Translational studies are warranted to map the molecular profile, leading to the development of targeted therapies in this group of patients.

Cystic nephroma: cytologic findings in fine-needle aspiration cytology.

This report presents the fine-needle aspiration cytology (FNAC) findings of a multicystic renal tumor found in a 3-year-old child. The smears contained benign epithelial cells isolated or arranged in sheets of uniform cells strongly suggesting the lining of the cysts. The combination of the imaging data with the FNAC findings favoured the diagnosis of cystic nephroma (CN), a benign renal tumor that is cured by surgery. Surgical pathology confirmed the diagnosis. CN should be added to the list of tumors of the kidney in infancy that appear to be diagnosable by FNAC/biopsy.

Progress towards automated diabetic ocular screening: a review of image analysis and intelligent systems for diabetic retinopathy.

Patients with diabetes require annual screening for effective timing of sight-saving treatment. However, the lack of screening and the shortage of ophthalmologists limit the ocular health care available. This is stimulating research into automated analysis of the reflectance images of the ocular fundus. Publications applicable to the automated screening of diabetic retinopathy are summarised. The review has been structured to mimic some of the processes that an ophthalmologist performs when examining the retina. Thus image processing tasks, such as vessel and lesion location, are reviewed before any intelligent or automated systems. Most research has been undertaken in identification of the retinal vasculature and analysis of early pathological changes. Progress has been made in the identification of the retinal vasculature and the more common pathological features, such as small aneurysms and exudates. Ancillary research into image preprocessing has also been identified. In summary, the advent of digital data sets has made image analysis more accessible, although questions regarding the assessment of individual algorithms and whole systems are only just being addressed.

Pretreatment laparoscopic surgical staging in locally advanced cervical cancer: preliminary results in Korea.

OBJECTIVES: The purpose of this study was to evaluate the feasibility and efficacy of pretreatment laparoscopic surgical staging in the treatment of locally advanced cervical cancer. METHODS: Forty-four patients with locally advanced cervical cancer who underwent pretreatment laparoscopic surgical staging between October 2001 and April 2004 were reviewed. The pathological diagnosis after laparoscopic surgical staging was compared with results from preoperative magnetic resonance imaging (MRI), and surgical results with follow-up data were evaluated. RESULTS: The mean duration of surgery was 202.0 (range: 120-300) min and the mean number of harvested lymph nodes (LNs) was 38.7 (range: 20-75). Twenty (45.5%) patients had pelvic LN metastasis and 5 (11.4%) patients had pelvic and para-aortic LN metastasis. Region-specific findings of MRI resulted in sensitivity and positive predictive values of 55.9% and 48.7%, respectively, on a three-region analysis. When compared with MRI, laparoscopic surgical staging was superior in detecting microscopic LN metastasis. The time to commencing concurrent chemoradiotherapy after laparoscopic surgical staging was 8.6 +/- 3.3 (mean +/- SD) days. All the patients received concurrent chemoradiotherapy as scheduled. There were three (6.8%) cases of minor postoperative complications, and no mortality occurred during the follow-up period. The 2-year disease-free survival rate of the current study was 89.7%, and three (6.8%) patients experienced recurrence after treatment. CONCLUSIONS: Laparoscopic surgical staging in locally advanced cervical cancer is a feasible and safe pretreatment method, and can be used as a best guideline for individualized concurrent chemoradiotherapy.

Muscle ultrasonography and electromyography correlation for evaluation of floppy infants.

Floppiness in an infant may have a number of different etiologies from disorders of the brain to spinal cord lesions, neuropathies, neuromuscular junction disorders and myopathies. In this study we aimed to investigate the correlation of muscle ultrasonography (US) and electromyography (EMG) in the diagnosis of floppy infants. The study encompassed 41 floppy infants aged 2-24 months. The muscle US and EMG examinations were performed without awareness of the clinical diagnosis. The final diagnosis was established by molecular genetic tests or muscle/nerve biopsy. The neurogenic group consisted of 16 infants according to their US and EMG findings. Fifteen of them had spinal muscular atrophy proven by genetic analysis and one had polyneuropathy diagnosed by nerve biopsy. Six infants were in the myopathic group according to their muscle US and EMG results. All of them underwent muscle biopsy and microscopic examination revealed five congenital muscular dystrophy and one glycogen storage disease. In two infants the US and EMG data conflicted. Their biopsies were also insufficient for the diagnosis. Seventeen infants had normal US and EMG findings but pathologic cranial magnetic resonance imaging or metabolic/genetic tests. They were considered in the group of central hypotonia. Our results suggest a high concordance of US and EMG findings in the diagnostic work-up of neurogenic and myopathic disorders.

Transcutaneous contrast enhanced sonography of the chest for evaluation of pleural based pulmonary lesions: experience in 137 patients.

PURPOSE: Transcutaneous ultrasound enables visualization of pleural based lesions but with a poor correlation to specific pathology. Ultrasound contrast agents in conjunction with contrast specific imaging techniques are increasingly accepted in clinical use. Up to date there are no data about the use of contrast enhanced sonography (CES) in a large series of pleural based pulmonary lesions. MATERIALS AND METHODS: From August 2004 to August 2005, 137 consecutive patients with pleural based pulmonary lesions on B-mode sonography were studied by CES using a transcapillary second-generation contrast agent (SonoVue(R)). The following CES parameters were retrospectively evaluated. Time to enhancement (TE) of contrast agent after i. v. application was determined and classified as short TE (< = 6 sec) vs. delayed TE (> 6 sec). Extent of enhancement (EE) was evaluated during the arterial phase (2 - 30 sec) and the parenchymal phase (1 - 5 minutes) by using the normal splenic tissue as an in vivo reference, and classified in reduced EE (anechoic/hypoechoic) vs. marked EE (isoechoic/hyperechoic) during both phases. Homogeneity of enhancement (HE) was classified as homogeneous vs. inhomogeneous. 60 patients had histologically confirmed malignant lesions due to central lung cancer (n = 31), and peripheral malignant lesions (n = 29). 77 patients had benign pleural based lesions including pneumonia (n = 32), pulmonary embolism (n = 20), compression atelectasis (17), and other benign pleural based lesions (n = 8). RESULTS: Malignant and benign lesions did not vary significantly regarding TE, EE, and HE. However, there were highly significant differences in the ratio of short vs. delayed TE and reduced vs. marked EE between the six disease groups. Characteristic patterns were short TE with marked EE in all compression atelectasis cases and in 62 % of patients with pneumonia. Delayed TE and reduced EE was seen in all patients with pulmonary embolism and in 62 % of patients with peripheral malignant lesions. Central lung cancer and benign nodules did not present with such specific patterns. No significant differences in HE were seen between subgroups. CONCLUSION: Pulmonary lesions are characterized by different CES-patterns of arterial supply as evidenced by TE and EE which depends on underlying causes, but CES does not allow to distinguish benign from malignant pleural based lesions in general.
