Fourteen patients presenting with a primary bone tumour of the knee have been assessed, in parallel with other imaging techniques, by examination using MRI with surface coils. The first examinations were made before any biopsy was made or chemotherapy given so as not to impair the interpretation of the images and to provide guidance as to the correct site for biopsy. Two sequences were made - a longitudinal sequence made with echo-spin, echo-time = 30 ms, repetition-time = 500 ms, with four impulses and a transverse sequence made with echo-spin, echo-time = 40 ms, repetition-time = 1300 ms with two impulses (+/- multi-echo). Thanks to the excellent spatial resolution using a surface aerial, we have been able to determine not only the intramedullary extent of the tumour and its relationship with healthy muscles and vessels but also, in a much more precise way than previously, evidence of extension to the skin, to the epiphysis, to the articular cartilage and to the synovium, thus allowing an objective assessment to be made of the possibility of a closed block-excision of the joint. A second MRI examination with cranial coil was made pre-operatively at the end of adjuvant chemotherapy to determine the precise progress of the tumour, and in particular, to discover the localisation of the tumour in the femoral and tibial shafts, the investigation being aided by systemic scintigraphy.

A case report of a malignant eccrine poroma metastasizing to long bones is described. This rare tumor of intraepidermal eccrine sweat duct origin usually metastasized to visceral organs. This is the second reported case of bone metastasis. Although a hip disarticulation was performed for ipsilateral femoral and tibial metastases presenting seven years after resection of the primary tumor, this 43-year-old man died six months later, from pulmonary metastases.

AIM: The femur is the most common long bone involved in metastatic breast cancer. Several studies have been published on the surgical management of metastatic disease of the femur. However, only few studies have been published specifically on the outcomes following reconstruction of femoral metastasis from breast cancer using a third generation intramedullary nail. The aim of this study is to review the outcomes after intramedullary surgical stabilization of femoral metastases from breast cancer. This is often associated with significant bone destruction. METHODS: A retrospective study of 18 femoral metastatic lesions in 15 patients treated with a Long Gamma Nail over a 6-year period was carried out. Pain relief, mobilization status and implant related complications were the main outcome measures analyzed. RESULTS: Thirteen out of 15 patients had complete pain relief and all patients regained their preoperative mobilization status with or without walking aids. There were no implant failures or perioperative deaths. Four (26%) patients developed minor complications. Ten patients died with an average survival of 9 months and five patients are alive with an average survival of 32 months. CONCLUSION: Stabilization of femoral metastases due to breast cancer with Long Gamma Nail is a safe and effective method with acceptable risks.
A 27-year-old man had tumors in both the femoral and the tibial metaphyses of the left knee. Although histological examination of the biopsy specimens revealed different findings in the 2 lesions, the final diagnosis was malignant fibrous histiocytoma in both.
