Serologic evidence that a herpes-type virus is the etiologic agent of heterophile-positive infectious mononucleosis.

The antibody activity against herpes-type virus (HTV) of heterophile-positive infectious mononucleosis sera and "normal" sera from humans was determined with immunoferritin. The antiviral activity of the mononucleosis sera was associated with the IgM antibodies, while the antiviral activity of the "normal" human sera was associated with the IgG antibodies. It was concluded from these findings that the appearance of antibodies to herpes-type virus in heterophile-positive sera represents a primary immunogenic exposure to this virus, or to a serologically related virus. This conclusion, in turn, suggests that herpes-type virus or a serologically related virus is the etiologic agent of heterophile-positive mononucleosis.

[Cytophotometric studies of DNA contents in peripheral blood leukocytes in patients with infectious mononucleosis].

Cytophotometric determinations of DNA were carried out with a pulse cytophotometer in peripheral blood leucocytes in 25 patients with infectious mononucleosis and 30 patients with other infectious diseases. DNA cytograms of patients with mononucleosis showed presence of a high percent of cells in phase S and G2+M of the cell cycle. This is an evidence of active proliferation of these cells persisting for about 6--8 weeks. The authors believe that pulse cytophotometry may be used for detection and investigation of states with lymphocytes stimulation in vivo, e.g. in certain infectious diseases, during immunotherapy of neoplasms, rejection reaction etc.

Double diffusion in gel tests with Paul-Bunnell antibodies of infectious mononucleosis sera.

Double diffusion tests in gel were employed for studies of reactions between infectious mononucleosis sera and extracts of bovine, sheep and equine erythrocyte stromata. The extracts were obtained by ultrasonication of stromata prepared from trypsin-digested erythrocytes. The reaction with bovine stroma extract was composed, in many instances, of two lines. A single line was observed in reactions with sheep and equine stroma extracts. This line merged into a reaction of partial or complete identy with one of the lines formed with bovine stroma extract. Evidence was obtained that some infectioous mononucleosis sera may contain heterophile (Paul-Bunnell) antibodies belonging to IgG class in addition to those of IgM nature.

Infectious mononucleosis test performance on CAP survey specimens.

The participant performances on 1977 and 1978 infectious mononucleosis survey specimens are tabulated. The various methods utilized gave similar results on negative specimens. Commercial reagent kits incorporating differential absorption performed somewhat better on positive specimens than kits without differential absorption. Many of the commercial reagent kits performed very poorly on the false postitive specimen.

Fusobacterium necrophorum septicemia following Epstein-Barr virus infectious mononucleosis.

We report herein a case of a 20-year-old previously healthy man who presented, 25 days after the onset of clinically and serologically confirmed Epstein-Barr virus (EBV) infectious mononucleosis, Fusobacterium necrophorum septicemia. The diagnosis of postanginal septicemia was confirmed by repeated demonstration of fusiform, obligate anaerobic, Gram-negative bacilli in anaerobic blood cultures, identified as F. necrophorum 15 days after admission. This case report aims at underlining the need of taking into consideration the possibility of severe Fusobacterium septicemia in previously healthy patients following EBV infectious mononucleosis in order to prevent increased mortality and morbidity.

Infectious mononucleosis: observations on transmission.

Epsten-Barr virus oropharyngeal shedding has been demonstrated in infectious mononucleosis patients many months after acute illness and long after the disease hallmarks, atypical lymphocytes and heterophile antibody, have disappeared. Extracellular virus is present more frequently in saliva than in other oropharyngeal samples. Prolonged excretion of EBV in asymptomatic carriers explains the difficulty in tracing case-to-case spread and increased transmissibility in age groups in which salivary exchange is high.

Infectious mononucleosis and Epstein-Barr virus. 2. Clinical picture, diagnosis, management.

The diagnosis of infectious mononucleosis is confirmed by hematologic, biochemical, and serologic data. An absolute increase in peripheral mononuclear cells to at least 4,500/cu mm is an essential feature. The serologic hallmark of the disease is the presence of heterophile antibody. The age of the patient may affect the clinical presentation and laboratory findings. Therapy is largely supportive, with steroids being used only in the presence of certain severe complications.

[Brachial plexus neuropathy (amyotrophic neuralgia) and infectious mononucleosis: report of a case].

A case of brachial plexus neuropathy (BPN) associated with infectious mononucleosis (IM) in a 13 year old boy is presented. The essential clinical picture of BPN is discussed and the main points are: acute pain at onset across the top of the shoulder-blade followed by weakness and atrophy on muscles supplied in most cases by superior brachial plexus. The prognosis is good and most patients began to note improvement within months. The main neurological complications of IM are lymphocytic meningitis, peripheral neuropathy and encephalomyelitis and they can occur without any classical findings as pharyngitis, adenopathy or splenomegaly. The association of BPN and IM was described previously in only 7 cases and as the present case all of them had a complete recovery.
