[Angioscintigraphy in benign tumors of the uterus and adnexa uteri].

Angioscintigraphy was used for examination of 128 patients, of them 91 had tumors of the genital organs, 22 inflammatory adnexal processes and 15 various extragenital diseases. Statistical scintigraphy was performed 5-40 min. after RP intravenous injection at 175-555 MBq. RP injection was followed by dynamic studies, the rate of recording being 1 frame per min. An anterioposterior direct projection with alignment of a gamma-chamber detector over the small pelvic area was used. 99mTc-human serum albumin and erythrocytes labeled in vivo with 99mTc-pyrophosphate served as radioactive markers of the blood pool. A scintigraphic picture in normal and pathology (tumors and inflammatory processes) in the uterus and appendages had characteristics features. Data on a degree of tumor vascularization could be used for recognition of disease as well as for adequate assessment of therapeutic efficacy. The sensitivity of the method was 97%, its specificity--59%, its accuracy--88%.

[Changes in ligated blood-vessel stumps during abdominal hysterectomy including the removal of the adnexa uteri].

Histological changes of ligated blood vessels were studied on 30 women of 41 to 57 years who had been selected according to certain criteria. For this purpose we usually resected the ligg. suspensoria ovarii, parts of the mesosalpinx, lig. latum and rotundum, the vasa uterina, segments of the basale parametrium the plexus vesico-vaginalis and others after 15 to 25 minutes. The abdominal hysterectomy was in most cases indicated due to uterus myomatosus. The histological examinations showed that, in spite of the ligation of the vessels the organism protects itself by forming a closing thrombus preventing a hemorrhage threatening life as well as a secondary hemorrhage at a later stage due to demarcation of the distal stump end. The occlusion consist of merged thrombocytes, coagulated fibrin and other blood elements; the lesion of the endothelial cells isvery important. Proximal, to the centre, the intravasal mutations decrease rapidly, the lumen is in most of the cases empty. Only in 16% of the lumina of arteries or arterioles and in 20% of the veins there were no hemostatic plugs found. The problem of hemostasis during the gynecological operation is only being touched upon, possible complications due to circulation and perfusion lesions are mentioned. A pathological extent of the coagulation in small pelvis is likely to be the reason for an embolism.

[Immunological aspects of the clinical pathogenesis of the inflammatory processes of the adnexa uteri].

Immunological aspects are highlighted of origination and course of chronic inflammatory process. The presented data show that a chronic inflammatory process originates and is maintained at the expense of disturbances in one or several components of the immune system realizing defence of the body against infections, and if these failures are not detected this can be due to inadequate methodological approaches or because of impossibility of identifying the existing inadequacy at the present stage of development of science. The existing congenital failures of immunity compensated by activation of synergic components of the system, with time and under exposure to adverse environmental factors, can bring about a cumulative effect leading to a phenotypical manifestation of the primary defect and to an increased infection case rate, including organs of the female reproductive system.
