[Paracetamol and other antipyretic analgesics: optimal doses in pediatrics].

Acetaminophen is a safe antipyretic therapy prescribed in children. The dosage usually applied in France ranges from 20 to 30 mg/kg/d, which is low and probably with little efficiency in many cases. From the data of the literature the optimal dosage seems to be 65 mg/kg/d using a 25 mg/kg loading dose, followed by a 12.5 mg/kg every 6 hours maintenance dose. The analgesic effect of a 15 mg/kg oral dose has been also demonstrated. Aspirin is equally effective, especially for rheumatic disorders, but its use is limited by adverse reactions. The other nonsteroidal anti-inflammatory drugs have a limited interest.

N-substituted 4-carboxy-1-phenyl-1H-pyrazole-5-propanamides with antiinflammatory, analgesic, antipyretic and platelet antiaggregating activities.

The synthesis of a series of N-substituted 4-carboxy-1-phenyl-1H-pyrazole-5-propanamides by reaction of 1-phenyl-1H-oxepino[4,3-c]pyrazole-4(8H),6(7H)-dione with aromatic primary amines is described. Some amides showed a platelet antiaggregating activity in vitro superior or comparable to that of acetylsalicylic acid, as well as moderate antiinflammatory, analgesic and antipyretic activities in rats or mice.

Use of antipyretic analgesics in the pediatric patient.

Fever and pain are the most common issues in pediatric patient management. Acetaminophen, aspirin, and dipyrone are the most commonly used drugs and are equivalent in their efficacy. Dipyrone, used in many parts of the world, but not in the United States, is an effective agent; however, it has been implicated in producing agranulocytosis and anaphylactic shock. The salicylates have anti-inflammatory effects making them appropriate for the treatment of patients with juvenile rheumatoid arthritis, but they are gastric irritants, may impair clotting, and, because of saturable kinetics, may lead to accumulation and toxicity. Acetaminophen is an effective antipyretic and analgesic with few side effects that is toxic only in massive overdose.

[A new anti-inflammatory--analgesic--antipyretic for the treatment of acute disease of the bronchi ].

Sixty adult patients, 31 men and 29 women, aged 44 to 60, and affected by acute bronchitis of probable bacterial aetiology were randomly divided into 3 groups of 20 and treated respectively with: 1) morniflumate (one 700 mg tablet twice a day) + amoxicillin (one 1 g tablet twice a day); 2) feprazone (one 200 mg tablet twice a day) + amoxicillin (one 1 g tablet twice a day); 3) amoxicillin (one 1 g tablet twice a day). Mean therapy duration was 9 days. The action of the drugs under study was assessed by objective chest examination and by evaluating the modifications of cough intensity and frequency, chest pain expectorating difficulty, amount of expectoration, body temperature. The overall assessments were completed by side-effect recording and by laboratory examinations carried out at the beginning and end of the study. Checks were made regularly on admission, and in the 3rd, 5th, 7th and last day of therapy. The above mentioned parameters showed a quicker regression of bronchial inflammation in the subjects treated also with the antiinflammatory drug compared to those treated only with the antibiotic. Furthermore, in the subjects treated with morniflumate such improvement was more rapidly achieved compared to those who received feprazone. The analgesic and antipyretic effects of morniflumate were also remarkable. All tested drugs were well tolerated.

4H-thieno[3,4-c]pyrazole derivatives with antiinflammatory, analgesic, antipyretic and platelet antiaggregating activities.

The synthesis of a series of 1-aryl-1,6-dihydro-4H-thieno[3,4-c]pyrazol-4-ones by cyclization of 3-[(2-arylhydrazino)methylene]thiophene-2,4(3H,5H)-diones, prepared by reacting 3-dimethylaminomethylenethiophene-2,4(3H,5H)-dione with arylhydrazines, is described. The 4-fluorophenyl derivative showed remarkable analgesic, antiinflammatory and antipyretic activities in mice or rats, as well as a platelet antiaggregating activity in vitro comparable to that of acetylsalicylic acid.
