[Value, concept and efficacy of medical psychological examinations. Comments on the critique by A. Müller].

The authors discuss the re-orientation regarding the withdrawal of driving licences, aptitude tests, re-training of drivers and therapy for DUI offenders as proposed by Müller in "Blutalkohol" in 1993. The authors note that Müller's analysis refers to an examination practice which is outdated and no longer compatible with the current development of examination and consultation techniques used by officially recognised medico-psychological examination centres in Germany. Methodological misunderstandings and miscalculations in Müller's analysis are pointed out. In contrast, a diagnostic strategy is presented aiming not only at improving traffic safety but also at optimising justice for each individual case. This also aims at giving advice to DUI offenders on how to improve their fitness for driving using appropriate measures. Special emphasis is put on procedures which enable DUI-offenders to use their time of suspension to genuinely improve their driving fitness with the help of diagnostic and advisory bodies in the medico-psychological examination centres.

[Driver's licence: responsibility, ethics and deontology].

The new law enacted on march 23rd 1998 regarding driving-licence, which became effective on october 1st 1998, modifies the method for obtaining the different types of driving-licences. Its 6th annex enumerates a list of physical impairments and pathologies implying the denial, the limitation or the withdrawal of the driving-licence; it determines if a medical advice, and if the case which kind of medical advice, is required. This leads to new situations, for which no jurisprudence has been established to date, where not only the responsibility of the licence's applicant but also that of the examining physician could be suspected. The physicians are also confronted to ethical an deontological problems on which the medical authorities have not always given an opinion. This paper examines them in order to give some bases for reflexion and discussion.

[Perimetry findings and driving performance. "How much visual field" does a motorist need?].

OBJECTIVE: On 01-01-99 the new driving licence regulations came into force. Are the visual field criteria mentioned there sufficient for an adequate evaluation of driving fitness? METHODS: Typical (binocular) visual field defects were digitally superimposed onto photographs of traffic scenes to show the influence of such scotomata on visual perception. RESULTS: Exemplary cases clearly show that evaluation of visual field borders alone is not sufficient for evaluating driving fitness. They also prove that binocular scotomata affecting the (para-)central region of the visual field are of special importance to driving performance. The actual guidelines given by the traffic commission of the German Ophthalmological Society (DOG) and the legally defined limits will be critically discussed. CONCLUSION: From the ophthalmological point of view the actual visual field criteria of the new driving licence regulations do not seem to be sufficient for the evaluation of driving fitness: An additional consideration of (para-)central scotoma is mandatory. This publication provides recommendations for execution and interpretation of perimetric examinations for traffic ophthalmological purposes.

Survey on health status of heavy vehicle drivers in Klang valley.

A survey was carried out using a medical examination format that was prepared by the Malaysian Medical Association. The findings of the survey show that of the 266 cases surveyed, 64 drivers (24% of cases surveyed) are either totally unfit to drive or temporarily unfit to drive heavy goods and passenger vehicles. This is clear indication that the current format that is being used by the Road Transport Department is inadequate and needs to be reviewed. It must also be stressed that all the above 64 drivers have been certified fit using the existing Road Transport Department format and are currently driving in our highways and roads. Heavy vehicle goods and passenger vehicle drivers if not properly examined and medically certified are not only be endangering their own lives but also that of others. It is therefore recommended that based on the data available from this survey, the Road Transport Department should seriously consider adopting the medical examination format that was formalised by the Malaysian Medical Association and used in this survey.

[Transport medical aspects of comprehensive medical care of ageing citizens (author's transl)].

The transport medical aspects in providing comprehensive care for ageing citizens are discussed from the point of view of a maximum degree of integration of aged and ageing people under the socialist conditions prevailing in our country. In this connection the conditions and services to be provided by society are mentioned and the norms of behaviour in public passenger and road traffic to be observed in ageing people owing to their diminishing performance are discussed. Suggestions are put forward as to how the traffic environment can be adapted to the diminished performance of old people. Apart from this, geromedically relevant passages of the guidelines for medical and psychological examination and evaluation of vehicle operators are elucidated (Verfüngungen und Mitteilungen des Ministeriums für Gesundheitswesen, No. 18, 1973, from 19, Nov. 1973).
