[Removal of calculi located in the navicular cavity by means of a Zeiss loop].

The authors present a case of a patient with recurring stones suffering from disectasia of the vesical neck. Two of the stones situated in the navicular cavity were easily removed by means of Zeiss' loop without any complications and therefore the authors think that this simple method may be used when attempts to remove the stone by means of forceps or other techniques have failed and before proceeding to an external meatotomy which in always more traumatic and this system may even be applied as the initial and best method in these patients. After having made a review of the bibliography at their disposal, the authors have found no descriptions of any case in which this method was used, as in the cases of ureteral lithiasis.

[Bibliography of technology for germfree animal research--1977 supplement (author's transl)].

Thirty-two references were added to the Bibliography of Technology for Germfree Animal Research which was published in this journal; Experimental Animals 24 (4), 229--253, 1975 and 25 (3), 223--225, 1976. The literature consists of useful technical reports concerning germfree mammalian and avian research classified as follows; 1: Isolator, 2: Diets, 3: Sterilization, 4: Obtaining germfree infants and hand rearing, 5: Animal care, management and transportation, 6: Experimental methods, 7: Sterility tests and contamination, and 8: Operating and patient isolators. Abstracts and brief description with no data have been excluded.

An analysis of comprehensive health promotion programs' consistency with the systems model of health.

Purpose. The purpose of this article is to report a review and analysis of the concordance between current comprehensive corporate health promotion programs as described in the published literature and the systems model of health and to explore emerging trends in the field of health promotion. Search Methods. MEDLINE, BIOSIS, and PsycINFO searches were conducted from 1985 to 1991, and the bibliographies of articles thus obtained were back searched for additional descriptions of corporate health promotion programs. Inclusive criteria included "comprehensive" corporate programs, published in peer-reviewed journals or books, and descriptions adequate enough to permit coding in the majority of analysis matrix categories. Out of 63 identified programs, 16 met the inclusion criteria; 47 were excluded. A common reason for rejection was the limitation imposed by inadequate program descriptions in the published literature. Major Findings. On average, the comprehensive corporate programs reviewed were initiated between 1984 and 1987 and set in the context of a manufacturing firm with over 10,000 employees. A minority of programs (12.5%) consistently satisfied systems model criteria. The most common category of programs were those which were inconsistent (44%), meeting some of the criteria of a systems model of health promotion, but not all. The mechanistic medical and public health models predominated strongly (63%) with the preeminent goal being individual risk factor modification. Conclusions. The limitations of the published literature do not permit strong conclusions about the number or degree to which current corporate comprehensive programs are concordant with the systems model of health. Although mechanistic models of health predominated, there is evidence that a number of comprehensive programs were inconsistent with the mechanistic model, meeting some of the criteria, but also meeting some systems model criteria. To continue the advancement of health promotion with clarity and focus, further research is needed to clarify outcomes across different "world view" models of health promotion. Health promotion specialists need to carefully scrutinize programs emanating from different "world view" models as they design, develop, implement, and evaluate corporate programs.

[Proposal for a new descriptive psycho-physiopathological model of schizophrenia].

We present the conclusions of a study of pattern recognition in an intermittent luminous stimulation. This stimulation was stable on the one hand and on the other hand on fixed time basis (S.L.I. pulsations emitted by two flickers) and other associated tests (reaction time, Rorschach Test, etc). We have compared a population of 30 schizophrenics (French classification) and a reference group of 53 adult subjects of both sexes. We have not divided the patients into subclasses according to their symptoms. We have noticed in a significant manner the following signs: great vigilance at the beginning, decreasing very quickly, low attentiveness, a slowness of perception and motion, a weakness in the elaboration of decision processus, certain difficulties in defining the classification of objects, a modified perception of colours, a very feeble distinction of pertinent signals and of noise, an invasion by the internal stimulus, unbalanced compared to "outside", as in sensory deprivation, a great weakness in the processus of habituation and learning, a loss of the redundancy and the constancy of the outside world (or troubles of the internal coherency, as defined by Varela). The closure, the temporal troubles (historical and present) of the integration of signals are sufficient to explain these results. A model explains the deduced hypothesis on two levels: 1) historical: the troubles of habituation and learning prevent the formation of the inner stimulus (or image, representation); 2) present: closure, ambivalence (Gödel), troubles of associations, difficulties in detecting what is pertinent, hallucinations (a "delirious" internal stimulus). The specific brain-channels seem to be normal, on the contrary the non-specific channels and the channels of integration seem troubled. The temporal trouble of "present" seems to be located on a precocious precategorical iconic level. This descriptive model does not prejudge the etiology of the disease (bibliography).

Descriptive cataloging for collections of medical art.

Conserving our profession's historical record would be aided by creating a guide for recording bibliographical information associated with medical art. A guide of this nature should conform to currently accepted national standards for bibliographical listings, reflect the cataloging needs of these specialized materials, and contain a sample data collection sheet. Such a guide would provide a starting point for new or undocumented collections to begin organizing and listing their holdings. This article presents a cataloging guide that is specific to the needs of collecting medical art and that conforms, in both language and structure, to established guidelines for descriptive cataloging.

[Creation, development and maintenance of the data-base of a computer-assisted diagnostic system (ADM) (author's transl)].

The creation of the data-base for our ADM system begun in 1972, tests on a high productivity type of procedure which we consider should be explained. In organizing and creating our data-bank our routine procedures are: first, the capture of signs, symptoms and pathological data after the text editing of the description of diseases as prepared by medical students under the direction of the Medical Computing Service; second, the control and coding of the data by the personnel of the Medical Computing Service: third, the machine storage of the data and the automated logical controls by the Medical Computing Service; and fourth, the semantic control of the data as well as the availability of each new version. Other related procedures based on the individual theses and various studies are described as examples. From 1972 to 1980 an impressive data-bank was created holding descriptions of 2200 diseases with 22000 signs and symptoms and 3500 bibliographies. It is now foreseen that between 1980 and 1983, the prototype will be placed in service and adapted. The creation and maintenance of such a data-bank with all the problems that it entails, and developed under such economical conditions justifies our presentation.

[Romberg's syndrome. Progressive facial hemiatrophy].

Romberg's syndrome or progressive facial hemiatrophy is a rare affection that may nevertheless be encountered by stomatologists during the course of their professional life. Diagnosis is simple if clinical and radiographic signs are recognized, an effective iconography being of particular value. This report is concerned mainly with the latter feature, and also includes a description of the principal characteristics of the affection and an extensive bibliography.

[An analysis of the articles published in Revista Medico-Chirurgicală over the last 7 years].

INTRODUCTION: As the number of medical journals is continuously increasing, their quality becomes a very reliable tool used by the editors in order to increase readership. As a consequence, the structure and the content of submitted papers are carefully revised before publication. OBJECTIVES: The purpose of our study is to describe structural aspects and the use of descriptive and inferential statistics methods in papers published by the Medical-Surgical Journal between 1990 and 1997. METHODS: Fifteen items were collected for each paper. Ten of them were related to the general structure: type of paper, number of authors, town of authors, professional environment, medical topic covered by the paper, pluridisciplinarity, the existence of a bibliography, the number of titles in the bibliography, language of publication, presence of abstract. The other five were related to data analysis and reporting: description of statistical methods, the existence of significant results without method description, presence of tables and graphics, use of descriptive and inferential statistics. RESULTS: A number of 477 papers were analysed. Original papers represented 57.4%, 71.5% of the authors were involved in medical/non-medical teaching, 58.3% of papers were related to clinical sciences, 29.8% were related to basic medical sciences; pluridisciplinarity was present in 26.8% of papers (significantly increasing trend, p < 0.01), the bibliography was not present in 27.3% of papers (significantly decreasing trend, p < 0.001), romanian was the language of publication in 83% of papers, followed by english (10.9%), french (4.6%) and german (1.5%). Papers without abstract represented 29.9%, the median number of authors is 3(range 1-15) and the mean number of titles in reference list 14.7(SD = 12). Eighteen papers (3.8%) reported significant results without description of methods, 51.4% used tables and graphics, 13.4% used descriptive statistics methods and only 6.9% of the papers have used inferential statistics methods. CONCLUSION: The structural problems of papers published in Medical Surgical Journal should be avoided using a more restrictive editorial policy. The use of such a policy shows positive results for the last 2 years (1995 and 1996). Statistical methods are poorly used in this Romanian medical journal and much progress is to be made before speaking about statistical assessment of submitted papers.

Neuroleptic-induced catatonia: neuroleptic malignant syndrome?

1. The authors describe a case report of catatonia syndrome after administration of depot neuroleptics. 2. Differential diagnosis was made between neuroleptic malignant syndrome or catatonic syndrome complicated by infection. The signs and symptoms observed throughout the patient's course are detailed. 3. A critical review is made of bibliography on the topic, with emphasis on the lack of clear clinical description and poor conceptual definition.

[Dissociation of retrograde memory impairment in a case of herpetic encephalitis].

OBJECTIVE: To report an uncommon case of memory impairment after meningoencephalitis. METHOD: Detailed description of the history, the lesions, the evolution and review of the bibliography. RESULTS: A 33 year-old man showed a loss of verbal anterograde memory and a dissociation of retrograde memory impairment after an herpetic meningo-encephalitis with lesions predominant in the left temporal lobe. The patient had impairment of verbal anterograde memory. Visual memory, procedural one were intact. He had normal performances in frontal tests. Neuropsychological assessment revealed major loss of episodic biographical memory and public events memory, but sparing of semantic biographical memory and professional knowledge. This patient who was amnesic but who had not difficulties with visual memory, procedural memory, frontal abilities and didactic memory conserved ability to work. CONCLUSION: This case report emphasize the multiplicity of the memory systems and the likely links between episodic biographical memory, semantic biographical memory, autobiographic episodic memory, public event memory and didactic memory.

Medical students' professional ethics: defining the problems and developing resources.

An AAMC working group concluded that medical educators have paid insufficient attention to students' development of professional standards and to the immediate ethical dilemmas that students face as students. Its discussion of the primary determinants of medical students' ethical development focused on improving: (1) the evaluation of candidates for admission; (2) the medical school learning climate, including recognition of the challenges inherent in the increasing cultural diversity among students and patients; and (3) communication to students of expectations of professional behaviors and standards. An AAMC initiative is outlined that includes preparation of a resource book featuring cases illustrating common ethical dilemmas that medical students face, descriptions of effective honor systems, suggestions for faculty development, and an annotated bibliography.

[Contribution of one century of X-rays to medicine].

In order to commemorate the centenary of the attribution of Nobel prizes in sciences, we present in this article a short description of the interaction of X-rays with materials and a bibliography of 33 Nobel prize winners (6 for medicine, 18 in chemistry and 9 in physics) whose work is connected to applications of X-ray interactions for medical and pharmaceutical purposes, and additionally of 12 Nobel prize winners in physics for their initiative in the discovery of particles which were recently used for medicine.

[Pulmonary atresia with intact ventricular septum (author's transl)].

Ten cases of pulmonary atresia with intact ventricular septum are presented. The groups in which these patients are classified are described, thinking that the mentioned classification is a simplification of the problem. After discussion of the dates apported by clinic, thorax X-Rays and electrocardiogram, authors come to the conclusion that they are not specific of this cardiopathy. Contribution of hemodinamic and angiocardiographic characteristics, paying special attention to the blood flow to lung vascular tree and in existance of intramyocardium sinusoid are shown. Description of the general tendencies of the most accepted surgical techniques in bibliography for resolution of both groups of patients is made.

REFERENCE UPDATE and REFERENCE MANAGER: personal computer programs for locating and managing references.

REFERENCE UPDATE (a trademark of Research Information Systems, Inc.) is a diskette-based service which provides subscribers with a weekly update on the latest publications in biology and medicine. REFERENCE MANAGER (a registered trademark of Research Information Systems, Inc.) is a microcomputer-based software package developed to manage selected references for quick retrieval and bibliography generation. These two systems allow scientists to build and update a personalized data base of references. The following article gives an overview of REFERENCE UPDATE and REFERENCE MANAGER and provides a description of the various functions each system offers.

[Systematic review of the effectiveness and indications of antidigoxin antibodies in the treatment of digitalis intoxication].

INTRODUCTION AND OBJECTIVES: Cardiac glucoside intoxication is a frequent medical problem given the following: the very narrow therapeutic range, its use in advanced aged patients, in patients with altered renal function, and because of interaction with other drugs. There are two types of digitalis intoxication: one that appears as a complication of the treatment with digitalis, and the other as a result of an accidental ingestion or in suicide attempt. The objective of this study was to review and assess the level of scientific evidence on the effectiveness and the indications of use of Fab fragments of antidigoxine antibodies. METHODS: A systematic bibliographic search in the following databases was made: MEDLINE, The Cochrane Library, The Iowa Drug Information Service, Embase, LMS/R&D Insight, and Indice Médico Español. The selected papers were classified according to their level of scientific evidence. RESULTS: Abstracts of 252 references were reviewed. In the reviewed bibliography no controlled, randomized trials were found. Most of the studies found are descriptions of case series or single cases that were treated with antidigoxin Fab fragments. These types of studies provide little or no scientific evidence to speak of. None of the treatment regimes with antidigoxin antibody Fab fragments so far proposed have proven to be valid in a controlled, randomized clinical trial. CONCLUSIONS: There is a very high level of concordance among the studies reviewed with regards to the efficacy and the indications for the use of Fab fragments in severe acute accidental digitalis intoxication and in suicide attempts. Regarding those intoxications that result in patients undergoing digitalis therapy, usual therapeutic approach is traditional treatment and the monitorization of the severity of the intoxication.

Trends in place preference conditioning with a cross-indexed bibliography; 1957-1991.

The purpose of this work is to present a perspective of the conditioned place preference (CPP) test by offering an overview of the empirical research from 1957-1991. The intent is not to extensively analyze the controversies inherent to any behavioral technique but rather to present a survey of research using a descriptive statistics approach to explore topical issues. The objectives of this work are three-fold: (a) to provide an exhaustive bibliography of the CPP literature including articles, journal abstracts, book chapters and critical reviews; (b) to provide a cross-index of identified key words/drugs tested; and (c) to give an overview of selected procedural issues underlying CPP testing.
