Bronchography in the recognition of congenital cystic bronchiectasis.

Congenital cystic bronchiectasis is an uncommon disease. Most of the reported cases have been in infants and young children. This paper presents four cases of older children and younger adults who had congenital cystic bronchiectasis. Distribution of the disease is lobar or may even involve an entire lung. Bronchography plays an important role in the diagnosis of congenital cystic bronchiectasis. The three important bronchographic signs of congenital cystic bronchiectasis are: (1) absence of contrast filling of the bronchial glands duct; (2) contrast filling of numerous cysts which communicate directly with the bronchial tree; and (3) nonvisualization of normal lung parenchyma within the area of involvement.

Bronchial atresia: three-dimensional CT bronchography using volume rendering technique.

We report a case of asymptomatic bronchial atresia of the anterobasal and laterobasal segments of the right lower lobe. Because of the absence of typical bronchial mucocele on chest radiograph, it was difficult to base the diagnosis on chest radiography alone. Helical CT images demonstrated the characteristic appearance of bronchial atresia, and three-dimensional (3D) CT bronchography using the volume rendering technique helped us to comprehend the spatial relationship between the dilated bronchus distal to the atresia and the more proximal bronchus.

[Bronchography in children].

In spite of improvement in the exploratory techniques of the respiratory tract, tracheo-bronchography preserves numerous indications in the study of children's congenital or acquired bronchopneumopathies. The main indications of this investigation consist in the detection of bronchectasies or in the exploratory study of chronic and relapsing bronchial diseases. The investigation makes possible to precise the severity of the disease, to value the results of the medical management and, at last, to select the patients amenable to surgery. Another indications are to be found in the appraisal of some malformative broncho-pneumopathies, with special reference to lung hypoplasias and sequestrations. The technical conditions of the investigation have largely benefited of short duration general anesthesy and of various material improvements (televised fluoroscopy, orientable catheters of small calibre).

[General anesthesia for bronchoscopy and lipidol bronchography in children and infants].

In 160 cases of children and infants, a technique of general anesthesia is described (penthiobarbital-suxamethonium, then halothane-oxygen with a mask in spontaneous ventilation if the bronchoscopy is followed by lipiodol bronchography). This technique is simple and efficacious and makes the child comfortable, especially in cases of repeated operation, and for the operator whose gestures are more rapid and less traumatising and this permits rapid awakening. No incidents or severe complications were noted.

Delayed films in bronchography.

In bronchography, the oil often does not fill all bronchial branches. Films taken 30 to 60 minutes later frequently complete the opacification of bronchi in lingula, middle lobe and lower lobes. Such delayed films may demonstrate bronchiectasis not shown initially, or may exclude bronchiectasis suspected on the first films.

[Acquired tracheobronchial stenosis of the premature infant under artificial respiration. Value of bronchography and endoscopic balloon dilatation. Apropos of 7 cases].

The authors report about a series of 7 cases of acquired tracheobronchial stenosis in premature infants who were intubated as soon as they were born and ventilated. Two important points can be pointed out: the merits of bedside bronchography, which allows diagnosing the lesions, the merits of balloon dilatation during tracheobronchoscopy on the ENT operating theatre to treat the stenosis. This technique allowed improving ventilation and removing the tube in 6 of 7 cases.

Bronchography↗

[Pneumonia following bronchography].

The extravasation of contrast medium into the alveoli during or following bronchography would appear to be associated with an increased complication rate. The histomorphological changes occurRing may vary in degree, and it may thus be difficult to differentiate such changes from those associated with other forms of pneumonia and pneumoconiosis. This paper reports on a case in which, using energy-dispersive X-ray micro-analysis in the scanning electron microscope, the presence of contrast medium within a region of alveolar pneumonia was observed, thus enabling accurate establishment of the cause of this condition.

[Bronchoscopy and bronchography in newborn infants and infants].

In 20 years (1965 to 1984) 428 infants (604 investigations) in the first year of life were bronchoscoped in our hospital; 109 out of them were newborns (119 investigations). Bronchographies were performed in 94 infants including 2 newborns. In the last 10 years the number of investigations decreased. Most frequent indications of bronchoscopies were stridor, aspiration and atelectasis. The usefulness of bronchological investigations in patients with oesophago-tracheal fistula, tracheal stenosis following long time intubation and bronchopulmonary dysplasia is pointed out. The diagnostic bronchoalveolar lavage can be recommended for newborns and infants too.

Bronchography↗

[Place of bronchography in the study and evaluation of the late results of the treatment of the pulmonary and pleural complications of acute pneumonia in children].

Late bronchography was performed in 93 patients, in 46 patients it was made immediately after cupping the critical period of inflammation. The changes found were dependent on the character and size of the process. It has been established that in non-operated children contrasting the bronchi is indicated in late terms of chronic pneumonia in order to specify the residual changes and to choose further treatment.

Anaesthesia for bronchography in small children.

General anaesthesia is infrequently requested for bronchography in small children. The clinical indications, recommendations for the anaesthetic technique, and problems associated with the investigation are discussed.

[Pulmonary aspergilloma clearly demonstrated by digital subtraction bronchography].

A 69-year-old woman was admitted because of persistent productive cough, low-grade fever and abnormal pulmonary infiltrates. Chest roentgenograms revealed right pulmonary cavitary lesions and infiltrates in both upper pulmonary fields. Digital subtraction bronchography (DSBG) was performed to detect for the presence of pulmonary cavitary lesions. Double-contrast DSBG images clearly revealed marked bronchiectasis in the right pulmonary cavity with aspergilloma in the cavitary lesions. Aspergilloma was not obvious in routine chest roentgenograms and computed tomograms. Asperillus fumigatus was cultured from sputum materials, and aspergilloma of the pulmonary cavity was clinically diagnosed. The infiltrative lesions were thought to be associated with invasive aspergillosis because the patient exhibited persistent low-grade fever and positive inflammatory reactions. Anti-fungal chemotherapy was highly effective in treating these lesions. DSBG is a new, less-invasive bronchographic technique for the evaluation of bronchial trees, and in this case proved useful for the detection of small aspergilloma in pulmonary cavities.

Bronchography test studies at the angiography station of the VEPP-3 storage ring.

The K-edge dichromography method was applied for preliminary bronchography test studies at the angiography station on the VEPP-3 storage ring. Images of xenon distribution over the test sample of Plexiglass were acquired. The obtained results demonstrate that thicknesses of xenon as small as 0.5 mm can be visualized.
