The ciliary body in accommodation.

The ciliary muscle ring contracts about 0.8 mm in radius during maximum accommodation and this change in radius does not significantly alter as the eye ages between 15 and 45 years. Despite this constant movement of the ciliary muscle ring, the force of contraction steadily increases over the same age period from 0.8 to 1.2 gms. The force of contraction of the ciliary muscle per dioptre--the myodioptre--changes both during the act of accommodation and as the eye ages. In the first case as the amplitude of accommodation increases, the myodioptre also increases in direct proportion to the amplitude. In the second case, as the eye ages an additional increase occurs at all amplitudes of accommodation. This increase is initially quite small but between the ages of 40 and 45 it becomes very much greater. As age advances this increased force of contraction is matched by a decrease in movement of the equator of the lens so that the zonule which is attached to both the ciliary body and the lens becomes increasingly stretched during accommodation and so can transmit the increased force of contraction. The zonular fibres between the ages of 15 and 45 do not appear to change in their extensile properties so the elasticity modulus remains constant with a value of 3.5 X 10(5)Nm-2. This Young's modulus is some 10 to 900 times less than the elasticity modulus respectively of lens capsule or tendon collagen but almost the same as aortic elastin.

Alteration of the vascular supply in the rabbit ciliary body by transscleral diode laser cyclophotocoagulation.

BACKGROUND: Besides the direct destruction of the non-pigmented ciliary epithelium by cyclodestructive procedures, further mechanisms are responsible for the decrease of intraocular pressure. This study evaluates the alteration of the ciliary body vascularization by contact transscleral diode laser cyclophotocoagulation in rabbit eyes. METHODS: Pigmented chinchilla bastard rabbits were used. Preliminary experiments were conducted to determine the parameters for diode laser cyclophotocoagulation of the pars plana or pars plicata. Then, treatment of the pars plicata (three rabbits) or pars plana (three rabbits) was performed in the right eye of six rabbits. After 2, 6 and 12 weeks histologic and transmission electron microscopic studies were performed. Furthermore, three rabbits received pars plicata cyclophotocoagulation of the right and pars plana cyclophotocoagulation of the left eye. After 2, 6 and 12 weeks, vascular casts of the ciliary body were investigated by scanning electron microscopy. RESULTS: Histologic and transmission electron microscopic studies showed a marked coagulation necrosis with subsequent ciliary body atrophy, destruction of the ciliary epithelium, pigment dispersion in the ciliary body stroma and peripheral anterior synechiae. Examination of vascular casts of the ciliary body revealed a marked rarefication of the capillary network within the treated areas of the ciliary body in all eyes and at every time of investigation. Anterior to the laser burns the capillary network was not markedly affected in the eyes with cyclophotocoagulation of the pars plana. After 3 months short vessel sprouts were seen, but regeneration was mostly incomplete. CONCLUSIONS: The vascular casting technique is an excellent method for the investigation of changes in ciliary body vascularization after cyclodestruction. This study is the first to demonstrate a marked rarefication of the ciliary body vascularization after diode laser cyclophotocoagulation using vascular casts. The results suggest that alteration of vascularization probably acts as a strong synergistic mechanism in the decrease of intraocular pressure after cyclophotocoagulation of the pars plicata.

Ultrasound biomicroscopic study of ciliary body thickness in eyes with narrow angles.

PURPOSE: To determine the ciliary body thickness and other biometric findings in eyes with narrow angles. METHODS: Eighteen otherwise normal eyes with narrow angles in 18 Japanese patients and 18 normal control eyes with open angles in 18 age-matched and sex-matched Japanese patients were studied. A-scan ultrasonography was performed to measure anterior chamber depth, lens thickness, axial length, and relative lens position. Ultrasound biomicroscopy was also performed to obtain measurements of the anterior ocular structures, including anterior chamber depth and ciliary body thickness at sites 1 mm and 2 mm posterior to the scleral spur (positions 1 and 2, respectively). RESULTS: Compared with normal control eyes, the narrow-angle eyes showed a shallower anterior chamber (narrow angle, 1.87 +/- 0.27 mm; control, 2.69 +/- 0.26 mm; P <.0001), a thicker lens (4.97 +/- 0.49 mm, 4.26 +/- 0.53 mm; P <.0001), a more anteriorly located lens (2. 21 +/- 0.13, 2.35 +/- 0.14; P <.0001), a shorter axial length (22.70 +/- 0.97 mm, 23.41 +/- 0.86 mm; P =.012), and a thinner ciliary body (position 1: 454 +/- 107 microm, 602 +/- 86 microm; P <.0001; position 2: 203 +/- 50 microm, 321 +/- 68 microm; P <.0001). Lens thickness was significantly correlated with ciliary body thickness at positions 1 (R(2) = 0.34; P =.0001) and 2 (R(2) = 0.43; P <.0001). Anterior chamber depth was significantly correlated with ciliary body thickness at positions 1 (R(2) = 0.48; P <.0001) and 2 (R(2) = 0.56; P <.0001). CONCLUSION: Thinning of the ciliary body may be one of the important factors associated with the anterior location of the lens, the increased lens thickness, and the decreased anterior chamber depth in eyes with a narrow angle.

[Medulloepithelioma of the ciliary body. A case report].

Medulloepithelioma of the ciliary body is an uncommon intraocular tumor occurring during the first year of life. Malignant degeneration may occur. We report the case of a 4-year-old child who presented medulloepithelioma of the left eye disclosed by oesotropia at 2 years of age. Clinically, there was oesotropia, positive light perception and cataract with vascular membrane spreading to the nasal side of the irido-corneal angle. CT scan and ultrasound B revealed a ciliary body tumor involving the sclera and orbital fat. After exenteration, the pathology study reported malignant medulloepithelioma of the ciliary body with scleral extension. No local recurrence or metastasis has been observed at 8 months follow-up. We discuss the clinical, radiological and therapeutic features of this uncommon tumor.

Evaluation of ciliary body detachment in hypotony.

PURPOSE: This article describes an anatomic classification for the hypotonous eye and links this classification with approaches to the surgical treatment of this condition. The classification is based on very-high-resolution ultrasound scans and three-dimensional reconstruction of planar ultrasound scans. METHODS: Standard 10-MHz B-scans and very-high-resolution (40-60 MHz) scans were performed with a planar motor system using the immersion method. The anatomic changes in the hypotonous eye were classified and correlated with clinical and ultrasonographic findings at the time of therapeutic intervention and after resolution of the hypotonous condition. RESULTS: Hypotony is defined as low intraocular pressure most commonly due to ciliary body detachment or ciliary body dysfunction. With ciliary body detachment, three anatomically related causative types were identified: tractional hypotony, which has ultrasonographically demonstrable proliferative vitreociliopathy with membrane attachments between the ciliary body, iris, and formed vitreous; dehiscence hypotony, which has a modification in the scleral acoustic architecture consistent with a scleral break or wound dehiscence; and primary type hypotony, which has ciliary body detachment but no ultrasonographically demonstrable tractional component or scleral anatomic modification, but may have an iris scleral separation. CONCLUSIONS: Each type of ciliary body detachment hypotony may have a different management approach, so high-resolution ultrasound, particularly when shown with sequencing or three-dimensional displays to demonstrate the extent of detachment, can aid in the selection and implementation of appropriate therapy.

Ciliary body naevus.

PURPOSE: To describe clinical, ultrasound biomicroscopy (UBM), and histopathologic characteristics of benign melanocytic tumors of the ciliary body. DESIGN: Consecutive case series. METHODS: Six patients with a pigmented ciliary body tumour underwent complete ophthalmic examination and UBM, with histopathologic examination carried out on three cases. RESULTS: Six patients presented with a pigmented iridociliary mass, with central displacement of iris root. UBM revealed a stromal mass arising in pars plicata and/or pars plana in all six with a cyst in three cases (intrinsic=1 and extrinsic=2). Iridocyclectomy was performed because of documented growth in three cases, and all three cases proved to be ciliary body spindle-cell naevus. The other three patients have remained stable. CONCLUSIONS: On clinical basis and with available ancillary studies, ciliary body naevi cannot be reliably differentiated from ciliary body melanocytoma and ciliary body melanoma. Even with clinically documented growth, the lesions may prove to be ciliary body naevi.

Macromorphometric study on ciliary body location in canine eyes.

This macroscopic study firstly examined the precise locational information of the canine ciliary body, i.e., the ciliary crown and the ciliary ring in the beagle. The safe and effective transscleral laser photocoagulation technique requires the accurate location of the ciliary body. In both sides of the eyeball in 10 beagle dogs, the width of the ciliary ring and the distance from the limbus to the ciliary ring were measured with calipers using a stereomicroscope at the 8 points. The widest portion of ciliary body was found at the dorsal to ventro-temporal area of the lateral canthus (lateral portion of the eyelid; ear side). In contrast, the narrowest portion was seen at the ventro-nasal to nasal area of the medial canthus (medial portion of the eyelid; nasal quadrants). Use of transscleral photocoagulation at the present narrowest area of ciliary body may carry a high risk of destruction of the optic portion of retina.

Endoscopy for evaluation and treatment of the ciliary body in hypotony.

PURPOSE: To determine the clinical utility of the intraocular videoendoscope for evaluation of the ciliary body in eyes with hypotony and as a visualization tool for surgery of the ciliary body. METHOD: A gradient index of refraction rod endoscope was used to evaluate the ciliary body in 14 patients. Videotapes and charts were reviewed retrospectively to correlate the appearance of the ciliary body and to analyze the clinical findings and surgical results. Videoendoscopic surgery to remove fibrous tissue from the ciliary processes was performed in nine eyes. RESULTS: The endoscope produced a clear detailed image of the ciliary body in a state that is undisturbed by scleral depression or other forms of mechanical distortion. In eyes without hypotony the normal ciliary processes are uniformly pigmented with a velvet-like surface at high magnification. White surface changes in the peak and crest of individual ciliary processes (white-caps) were seen in 11 of 14 patients with hypotony. At high magnification the white surface changes had a brush-like texture integral to the ciliary processes. Other findings in hypotony were fibrous covering of the ciliary body in 10 patients, loss of pigment epithelium in 5, traction elongation of the ciliary processes in 8, vascular loops in 4, pigmented sheets in 3, and areas of missing ciliary processes in 1. White caps were present in seven of the nine patients who had surgery for fibrous traction of the ciliary processes and hypotony and three patients with hypotony not due to other endoscopically visible abnormalities of the ciliary processes. Endoscopically visualized dissection of ciliary body in 9 patients resulted in normal or elevated tension immediately postoperatively in 7 (78%). At the most recent visit only three of the patients had normal tension, but for these nine patients the pressures were marginally, significantly (P = 0.08) higher compared to the preoperative pressures. CONCLUSION: The evaluation and management of hypotony is enhanced by the use of intraocular videoendoscopy. Clinical findings of whitecaps and traction elongation of the ciliary processes were associated with ocular hypotony. The endoscope facilitated surgery for dissection and removal of fibrous tissue over the ciliary processes. Further studies are needed to better understand the pathophysiology of ocular hypotony.

Microvascular loops and networks as prognostic indicators in choroidal and ciliary body melanomas.

BACKGROUND: Malignant melanoma of the ciliary body and choroid of the eye is a tumor that disseminates frequently, and 50% of the diagnosed patients die within 10 years. We investigated the hypothesis that, by histopathologic analysis of the arrangement of microvessels (i.e., small blood vessels) in loops and networks, we might be able to differentiate better those patients with a favorable prognosis from those with a poor prognosis. METHODS: We conducted a population-based, retrospective cohort study of melanoma-specific and all-cause mortality for 167 consecutive patients who had an eye surgically removed because of malignant choroidal or ciliary body melanoma during the period from 1972 through 1981. Microvascular loops and networks were evaluated independently by two pathologists who were unaware of patient outcome. RESULTS: Microvascular patterns could be assessed in 134 (80%) of 167 melanoma specimens. The 10-year probability of melanoma-specific survival was worse if microvascular loops (0.45 versus 0.83; two-sided P<.0001) and networks (0.41 versus 0.72, two-sided P<.0001) were present. In multivariate Cox regression analysis of melanoma-specific survival, the hazard ratios were 1.66 (95% confidence interval [CI] = 1.19-2.30) for the presence of loops and networks as a combined three-category variable, 2.36 (95% CI = 1.37-4.05) for the presence of epithelioid cells, 1.11 (95% CI = 1.03-1.19) for the largest basal tumor diameter (evaluated as a continuous variable), and 2.14 (95% CI = 1.25-3.67) for ciliary body involvement. CONCLUSIONS: Patients with malignant uveal melanoma who have a favorable prognosis can be distinguished from those with a poor prognosis by histopathologic analysis of microvascular patterns in uveal melanoma tumor specimens.

Ciliary body degeneration in the Royal College of Surgeons dystrophic rat.

We have studied the pathological changes of the ciliary body in Royal College of Surgeons (RCS) rats with an inherited retinal degeneration. Morphometric analyses were performed on sectioned ciliary bodies by a computerized morphometry system. Age-matched non-pigmented Sprague-Dawley (SD) rats were used as the control animals. The ciliary body of 26-day-old RCS dystrophic rats showed normal structure. However, the length and height of the pars plicata of the ciliary body became shorter and the area became smaller with increased age. Significant decreases in the values of these three parameters were observed between 26-day-old and 3-month-old RCS dystrophic rats. These parameters also showed significant differences when values of 3-month-old RCS dystrophic rats were compared to those of 3-month-old control SD rats. The same trends were observed in the ciliary body measurements in RCS dystrophic rats up to 1 year of age. Scanning electron microscopic examination demonstrated the progressive thinning of the pars plicata of the ciliary body with age in the RCS dystrophic rats. The total volume of the ciliary process of 6-month-old RCS dystrophic rats appeared to be one-half that of 26-day-old RCS dystrophic rats. Transmission electron microscopy revealed progressive cellular degenerative changes in the non-pigmented and pigmented ciliary epithelium of the RCS dystrophic rats. It was apparent that the pigmented ciliary epithelium had more severe degenerative changes than the non-pigmented ciliary epithelium. Immunostaining for Na+ + K+ ATPase of the ciliary epithelium was found to be less in the RCS dystrophic rats than in age-matched controls. This result suggests a possible dysfunction of ion transport in the ciliary body of the RCS dystrophic rats, which may account for their increased incidence of cataract formation. Although the mechanisms for the ciliary body degeneration in RCS dystrophic rats remain speculative, these findings add a new area of interest in this model of inherited retinal dystrophy.

Evidence for the presence of serotonergic nerves and receptors in the iris-ciliary body complex of the rabbit.

The rabbit iris-ciliary body contains 78 +/- 6 ng/gm serotonin (5-HT) while the amine content in the aqueous humor is less than 0.01 ng/100 microliter. The low levels of endogenous 5-HT in the iris-ciliary body could not be directly detected by immunocytochemistry. However, pretreatment in vivo and in vitro with L-tryptophan and pargyline resulted in the localization of a sparse population of 5-HT fibers. These fibers could not be studied by exposing the tissue to exogenous 5-HT since the amine was taken up by noradrenergic fibers as well. This was confirmed in studies involving superior cervical ganglionectomy. It is concluded that 5-HT is taken up by both serotonergic and adrenergic fibers of the iris-ciliary body. In the presence of lithium, 5-HT stimulated an increase in the 3H-inositol phosphate accumulation in a dose-dependent manner in tissue where the phosphoinositide pool was labeled with 3H-inositol. A variety of agonists and antagonists were used to show that the 5-HT response is mediated, at least partly, by 5-HT2 receptors. The 5-HT-mediated effect on inositol phosphates is unaffected by superior cervical ganglionectomy. The effect of noradrenaline, which also stimulates inositol phosphate accumulation (but via alpha 1-adrenergic receptors in the iris-ciliary body), was elevated following superior cervical ganglionectomy.

[Histological analysis of aging ciliary body].

The aging changes in human ciliary body were studied by histological analysis. Thirty four autopsy eyes were fixed in 2.5% formalin and 1.0% glutaraldehyde mixture. The age distribution was 12 eyes below 50 years of age 13 eyes between 60 and 70 years of age, and 9 eyes over 80 years of age. Serial sections of the ciliary body were made parallel to the limbus and vertical to the ciliary processes. From four areas (transitional portion between pars plana and pars plicata, posterior portion of pars plicata, middle portion of pars plicata, and anterior portion of pars plicata) a central section was selected. Histological analysis was done with the use of Azur II stained light microscopic pictures. The basement membrane of non-pigmented epithelium below 50 years of age was thinner than between 60 and 70 years of age. This finding may be related with lowering aqueous humor secretion with aging. Non-pigmented epithelium, blood vessels in the stroma, and muscle fibers in muscle tissue showed aging changes. The pigment epithelium, basement membrane, and muscle fibers in muscle tissue did not show aging changes in light microscopical analysis, but study of their fine structure is necessary.

A new B-scan ultrasonographic technique for observing ciliary body detachment.

We developed a new B-scan ultrasonographic technique for observing ciliary body detachment. Our technique uses a water-bath immersion method in which a real-time A- and B-mode contact scanner is employed. A ciliary body detachment can be ultrasonographically observed either as a separation of the ciliary body from the sclera or as a swelling of the ciliary body region. This technique makes it possible to diagnose ciliochoroidal detachment accurately while a pathologic change is still localized in the ciliary body region.

Vitreoretinal endoscope for the assessment of the peripheral retina and the ciliary body after large retinectomies in severe anterior PVR.

PURPOSE: We evaluated endoscopically the changes of the peripheral retina and the ciliary body after large retinectomies. METHODS: The peripheral retina and the ciliary body of 5 patients with anterior proliferative vitreoretinopathy after large retinectomies (> 180 degrees) were visualized endoscopically. RESULTS: The endoscope allowed a complete assessment of the peripheral retina and the ciliary body. Even the 12 o'clock part of the eye could be evaluated with the 30 degrees optic. The peripheral retina, a fibrosis and a detachment of the ciliary body can be visualized and assessed. The cause of the postoperative hypotony after large retinectomies is mainly related to fibrosis and detachment of the ciliary body. Surgeons can expect a postoperative hypotony if fibrosis and a large detachment of the ciliary body is seen during surgery with the help of an endoscope. CONCLUSION: Endoscopic vitreoretinal visualization of the pars plana and ciliary body is a useful additional technique after large retinectomies in the treatment of anterior PVR.

[(3)H]-serotonin release from bovine iris-ciliary body: pharmacology of prejunctional serotonin (5-HT(7)) autoreceptors.

In the present study, we investigated the pharmacological characteristics of electrically stimulated [(3)H]-serotonin release from mammalian iris-ciliary bodies. Isolated bovine and human iris-ciliary bodies were loaded with [(3)H]-serotonin, superfused with Krebs buffer solution and then stimulated with trains of 300 direct current (d.c.) pulses to initiate the release of the transmitter. The modification of this [(3)H]-serotonin release process by various serotonergic agonists and antagonists was studied in order to define the pharmacology of serotonin receptor(s) present in the iris-ciliary body. In bovine iris-ciliary body, electrically-evoked [(3)H]-serotonin release was calcium-dependent, tetrodotoxin-sensitive and was enhanced by serotonin (EC(50) = 200 n M) and 5-carboxmidotryptamine (EC(50) = 4 n M). The rank order of potency of agonists in enhancing field-stimulated [(3)H]-serotonin release was: 5-carboamidotryptamine > m-chlorophenylbiguanide > 2-methyl-5-hydroxytryptamine = 5-methoxy-dimethyltryptamine > serotonin > 5-methoxy-tryptamine > L-694,247 = alpha-methyl-5-hydroxytryptamine > CGS 12066A = 8-hydroxy-2-(di- n -propylamino)tetraline. Serotonin and m-chlorophenylbiguanide also enhanced electrically-evoked [(3)H]-serotonin release from human iris-ciliary bodies with EC(50)s of 3 microM and 30 n M, respectively. The pharmacological profile displayed by serotonin receptor agonists was supported by the potent antagonism of the serotonin-induced enhancement of [(3)H]-serotonin release by 5HT(7)receptor antagonists SB-258718 (IC(50) = 18.6 +/- 1.2 nM; n = 4) and mesulergine (IC(50) = 0.26 +/- 0.05 nM; n = 4). However, antagonists at 5HT(6)and 5HT(3)receptors exhibited a relatively weak blockade of serotonin induced enhancement of field-stimulated [(3)H]-serotonin release. These studies have shown the presence of functionally active prejunctional 5HT(7)autoreceptors regulating the release of [(3)H]-serotonin from bovine iris-ciliary bodies. Excitatory prejunctional 5-HT autoreceptors also exist in human iris-ciliary bodies. It is possible that these serotonin autoreceptors may have relevance to the regulation of aqueous humor dynamics in the anterior uvea.

A case of pseudoadenomatous hyperplasia of ciliary body epithelium successfully treated by local resection.

A case of pseudoadenomatous hyperplasia of ciliary body epithelium was reported in which malignant melanoma of ciliary body was suspected. Partial resection for histopathology was performed in conjunction with cataract extraction, anterior resection and photocoagulation. Histopathology of the tumor identified as pseudoadenomatous hyperplasia of ciliary body epithelium. Partial resection of ciliary body tumor may be an alternative method for its differential diagnosis rather than enucleation and iridocyclectomy.

Isoforms of glucose transporter in the iris-ciliary body.

Isoforms of the facilitated glucose transporter (GLUT) were identified in the iris-ciliary body. Western blot analysis showed that GLUT1 and GLUT4 proteins were expressed in the rat iris-ciliary body. In addition, the content of GLUT proteins in the rabbit iris-ciliary body was estimated by specific [3H]cytochalasin B binding assay. The content of GLUT proteins was found to be more abundant in the iris-ciliary body than in the cerebral cortex. The utilization of glucose in the iris-ciliary body and the transport of glucose into the aqueous humor are suggested to be mediated by GLUT1 and GLUT4 proteins.

Bioinformatics and reanalysis of subtracted expressed sequence tags from the human ciliary body: Identification of novel biological functions.

PURPOSE: The ciliary body is largely known for its major roles in the regulation of aqueous humor secretion, intraocular pressure, and accommodation of the lens. In this review article we applied bioinformatics to re-examine hundreds of expressed sequence tags (ESTs) previously isolated by subtractive hybridization from a human ciliary body library [1]. The DNA sequences of these clones have been recently added to the web site of NEIBank. METHODS: DNA sequence comparisons of subtracted ESTs were performed against all entries in the last available release of the non-redundant database containing GenBank, EMBL, DDBJ and PDB sequences using the BlastN program accessed through NCBI's BLAST services on the internet (NCBI). Sequences were also compared and mapped using the Blast search program provided through the Internet by the Human Genome Project (UCSC). RESULTS: A total number of 284 independent ESTs were classified in 17 functional groups. Analysis of their relationships allowed to define the expression of five major groups of known genes: (i) protein synthesis, folding, secretion and degradation (20%); (ii) energy supply and biosynthesis (12%); (iii) contractility and cytoskeleton structure (6%); (iv) cellular signaling and cell cycle regulation (7%); and (v) nerve cell related tasks (2%), including neuropeptide processing and putative non-visual phototransduction and circadian rhythm control. The largest group contain unidentified sequences, a total of 105 sequences, accounting for 37% of ESTs. The unidentified sequences show similarity to genomic non-coding regions, or genes of unknown function. CONCLUSIONS: The most highly represented EST, correspond to myocilin, a gene involved in glaucoma. The data also confirms the secretory functions of the ciliary epithelium, and its high metabolism; the presence of a neuroendocrine peptidergic system presumably involved in the regulation of the intraocular pressure and/or aqueous humor secretion. Additional genes may be related to a non-visual phototransduction cascade and/or to circadian rhythms. Overall this initial group of subtracted ESTs can lead to uncover novel physiological functions of the ciliary body in normal and in disease, as well as novel candidate genes for ocular diseases.
