Long-term Suprapubic cystostomy drainage in spinal cord injury patients.

Thirty-one spinal cord injury(SCI) patients maintained on suprapubic cystostomy (SPC) for at least 5 years were evaluated by intravenous urography (IVU) and cystourethrography (CUG). Only patients who had had a normal IVU and no reflux before SPC were included in the study. The results showed that SPC maintained for more than 5 years causes as much renal damage as the intraurethral catheter retained for more than 20 years. In SCI patients, therefore, SPC should be used only temporarily after urethral, ureteric or bladder surgery.

Circle cystostomy: technical considerations.

Our experience of 30 selected cases in whom circle cystostomy was used for postoperative drainage of the urinary bladder is described. Technical details and considerations used in the preferential selection of this method of urinary drainage over the standard balloon catheter (Foley) drainage are discussed.

Total replacement of the ureter by a segment of the sigmoid colon in the treatment of giant hydronephrosis: calyco-sigmoido-cystostomy.

Two cases of giant hydronephrosis of a solitary kidney are reported. In both of them correctly performed pyelo-ureteroplasties had not assured satisfactory emptying of the pyelocalyceal system. Its dilatation and infection had not disappeared. In order to liquidate the nephrostomy, the ureters were replaced by a sigmoidal loop (calyco-sigmoido-cystostomy) with favorable results assessed 24 and 19 months after operation. The use of an isolated sigmoid segment for replacement of the ureter is discussed.

Pancreas transplantation with pancreatico-duodeno-cystostomy for exocrine drainage: cystographic findings.

Pancreatic transplantation is becoming accepted therapy for a select subset of patients with type 1 insulin-dependent diabetes. At many centers, whole-organ transplants with pancreatico-duodeno-cystostomy for drainage of exocrine secretions is the preferred surgical procedure. We obtained 21 cystograms in nine patients who underwent this procedure who were suspected of having anastomotic leaks and possible abscess. Positive findings included extrinsic bladder compression (nine patients), filling of the transplanted duodenal segment (three patients), edematous bladder wall (four patients), intraperitoneal (two patients) and extraperitoneal (four patients) leaks, and fistulae (two patients). Intraperitoneal extravasation resulted in abscess formation and cutaneous fistulae, and these transplants were subsequently removed. We conclude that cystography is a useful technique in evaluating the site and extent of anastomotic leaks and fistulae.

A cystostomy cannula for dogs.

A practical, durable cystostomy cannula for the collection of urine samples for prolonged periods in awake dogs with intact bladder or hemibladder has been devised. The specifications, method of implantation, complications, and results of its use in nine dogs are described.

[Effect of total biliary diversion (cholecysto-jejuno-cystostomy) on gut hormone release and pancreatic structure in dogs].

In order to elucidate the effect to total biliary diversion (TBD) on gut hormone release and pancreatic structure, we performed cholecysto-jejuno-cystostomy (CJC) in 6 mongrel dogs using a small interposed intestinal segment between the gallbladder and the urinary bladder with ligation of the common bile duct. Twelve weeks after CJC, CJC was converted into cholecysto-jejuno-duodenostomy (CJD) to normalize luminal bile flow. Fat rich meal loading tests were carried out before and after these procedures. Plasma concentrations of GI hormones (CCK, PP, GIP) were measured by radioimmunoassay and morphological changes of the pancreas were evaluated by light and electron microscopic study. CJC significantly enhanced the basal levels and fat-stimulated release of both CCK and PP, however after CJD, these changes returned to the normal levels. Following CJC, fat-stimulated GIP release was completely inhibited, but recovered after CJD. Following CJC, hypertrophy of pancreatic acinar cells with profuse and dilated rough endoplasmic reticulum was observed, while after CJD this change returned to the pre-operative state. These results suggest that a feedback regulation may exist between luminal bile flow and CCK secretion, and pancreatic hypertrophy after TBD is, at least partly, due to the increased plasma CCK levels.

[Canaliculo-cystostomy using the fibrin glue technic].

In ophthalmology the fibrin sealing technique has been used in corneal, scleral, conjunctival and lens surgery, as well as in oculoplastic surgery, namely in lid surgery. Fibrin sealing of the anastomosis of the canaliculo-cystostomy in combination with silicone intubation is a new indication for fibrin sealing. In seven patients with stenosis of the canaliculus communis due to chronic inflammation this new technique was successfully used. Technique of operation, advantages and results of this method are described.

Complication of percutaneous suprapubic cystostomy.

A patient with urethral stricture presented to us with acute retention. Sequential dilatation over a guidewire was done and a suprapubic catheter was placed percutaneously. Guide wire broke during the procedure, which came to our notice on micturating urethrogram. Laser core through for the stricture and endoscopic removal of the guidewire were done.
