[Ultrasound examination in the surgery of hepatic echinococcosis].

It is expedient to perform treatment of patients with the hydatic liver with special reference to the stage of the development of the parasite, the state of the fibrous capsule which can be established with the help of ultrasound examination both before and during the operation. The developed ultrasound classification of liver Echinococcosis allows taking into account all the stages and forms of the disease which is necessary for planning the optimum method of treatment. Using computed tomography and serological methods is justified in the situations more complex for diagnosis. In all the cases of exogenous development of echinococcal cysts the most radical method is thought to be maximum ablation of the fibrous capsule under control of intraoperative ultrasound investigation.

[Protoscolecides in the surgical treatment of hepatic echinococcosis--myth or reality].

Traditionally protoscolicidal agents have been used during conservative surgery for cystic liver hydatidosis to sterilize the cyst cavity and to avoid dissemination. Many surgeons questions the effect of the procedure due to lack of objective data and possible complications for their use. Recently introduced minimally invasive techniques--percutaneous (PAIR), endoscopic and laparoscopic--have reestablished the need for the evaluation of protoscolocidal agents.

[Contribution to the surgical and cytostatical treatment of multiple abdominal and hepatic echinococcosis].

This is a report on 5 patients with multiple liver and abdominal echinococcosis. Besides surgical treatment the cytostatic Cyclophosphamide (=Endoxan) has been used in all cases. With this combined treatment there has been no recurrence. At this time the cytostatic-surgical combination treatment represents the therapy of choice in multiple echinococcosis. It remains to be seen whether a specific drug therapy will displace these methods in the near future.

[Diagnostic and therapeutic approach in hepatic echinococcosis complicated by mechanical jaundice].

The article presents the results of the treatment of 43 patients with echinococcosis of the liver the course of which was complicated by mechanical jaundice (MJ). In 11 patients MJ arose as a result of cystic content break into the bile ducts, in 32 patients, due to cystic compression of the lobular bile ducts. An effective scheme of the patients' examination is proposed which involved present-day ultrasound, X-ray and endoscopic methods. The system of objective evaluation of the patients' condition is outlined as designed for planning surgical policy. In non-severe disease the patients are operated after standard preparation within 24 hours after hospitalization. If the disease is more severe, the patients demand preparation including extracorporeal detoxication and cholangiostomy. In serious conditions the operations are made to remove the cyst content and cholestasis. This differentiated diagnostic and therapeutic policy diminished postoperative lethality from 15.2 to 6.9%.
