[A case of sudden unexplained death following retro-sternal esophagoplasty (author's transl)].

A recent case of deadly syncope, overcome five days after a colic retro-sternal esophagoplasty for caustic stenosis of the esophagus, and remaining unexplained even at the post-mortem examination, reminds the same cases formerly published. Its report is done for the purpose of inciting other authors to publish similar facts that they could have observed. These collected cases would lead to consider the possible responsibility of the retro-sternal way and put its indications back into question.

[Peptic ulcer of the cologastric anastomosis in esophagoplasties].

Two cases of cologastric anastomotic peptic ulcer after esophagoplasty for stenosis from caustic burns are reported, one developing 2 1/2 years and the other 12 years after operation. One case was treated by complex gastrocolic resection and the other by distal gastrectomy with ulcer excision and retrimming of the cologastric anastomosis. Long-term postoperative results were good. The different surgical procedures reported in the literature are discussed, and the value of combined gastrotomy and exploratory colotomy for detection of lesion extension emphasized. The preferred method is complex cologastric resection with conservation of the stomach in the digestive circuit.

[Radiological examination of esophagoplasty interventions for esophageal cancer].

The experience in the radiological study on the esophagoplasty for esophageal cancer is reported. In the various types of surgical operations: gastroplasty: 284 cases, colonplasty: 28 cases, jejunoplasty: 10 cases, substitution of the cervical esophagus: 74 cases, the radiological contribution is analysed both in the early anastomosis check: after 7 days the intrathoracic and after 10 days the cervical ones, and in the distance check. For this last aspect, particularly the functional pattern, it is also suggested the use of paraphysiologic meals.

[Architectonics of the vessels of small and large intestinal grafts in esophagoplasty].

In experiments on 30 cadavers the structure of the small and large bowel vessels with the concern in esophagoplasty was studied. The angiography proved that in case of multiarcade structure of the small bowel vessels, more than in 50% of cases the great bowel showed a trunk type of the division of the vessels. In 70% of cases with a favourable structure of the small bowel vessels good conditions for taking a graft from the large bowel were found. In case of a relatively unfavorable or an unfavorable structure of the small bowel vessels (26.7%) there was the possibility to get a graft from the left side of the large bowel and in 50%--from the right one.

[Effect of hyperbaric oxygenation on local tissue blood flow to a small intestine transplant intended for esophagoplasty].

The time course of the local tissue blood flow in the small intestinal graft used for esophagoplasty was studied in 54 acute experiments on rabbits exposed to hyperbaric oxygenation (1 hour, 2 ata). It has been shown that hyperbaric oxygenation prevents alterations in the local tissue blood flow in the small intestine. This fact provides evidence in favour of hyperbaric oxygenation application under clinical conditions.

[Step-by-step mobilization with upwards transposition of the intestinal complex in total esophagoplasty using the right half of the large intestine].

An experience of performing total esophagoplasty with high esophagus strictures and 30 anatomo-experimental investigations on corpses enabled the authors to develop a new method of extraction of a transplant which was called a step-by-step mobilization with a displacement of the intestinal complex with fixation of the small intestine mesentery root to the posterior wall of the rectus abdominis sheath. The essence of the step-by-step mobilization with a displacement of the intestinal complex upwards is the following: if the first step of usual mobilization of the right half of the large intestine with a process of ileum proves to be not sufficient for making high anastomosis, the second step should be fulfilled consisting in mobilization of the small intestine mesenterium root which gives more opportunity to displace the large intestine transplant. If this measure still seems to be insufficient the third step is necessary such as a mobilization of the terminal portion of the duodenum. It allows the transplant to be made as high as the pharynx practically in all cases.

[Colonic esophagoplasty in the treatment of postburn esophageal stricture].

In patients aged from 2.5 to 67 years 56 esophagoplasties were done. The authors prefer the left half of the colon application with the blood supply through the middle colonic artery and with antiperistaltic transplant position. In 53 of 56 patients the transplant was conducted to the neck in retrosternal tunnel. General mortality is 7.1%. Literary data and own views on the problem of possible esophageal malignancy origin in the late period after burn occurrence, the surgical tactics versions are adduced.

Left thoracotomy for coronary revascularization after esophagoplasty with substernal colon interposition.

A 66-year-old man was referred to our institution with recurrent angina pectoris caused by 95% stenosis of the left anterior descending coronary artery. Twelve years earlier, he had undergone esophagoplasty with substernal colon interposition for an esophageal burn caused by a caustic substance. A left thoracotomy approach and femoro-femoral bypass were used safely for coronary artery revascularization.

[Development of methods of esophagoplasty in children].

In the Clinic of Pediatric Surgery of the Russian State Medical University 310 operations for esophagoplasty were performed using segments of the small and large intestine. The 50-year progress in the creation of an artificial esophagus is characterized by gradual advancement from prethoracic plasty after Roux-Herzen-Yudin to the refined method of retrosternal coloesophagoplasty. The incidence of the characteristic postoperative complications reduced by 3 to 10 times, necrosis of the transplant was practically not encountered, the mortality reduced to 1.2%, and the functional results improved significantly.

[One-stage esophagoplasty with pathologically changed or previously operated stomach].

The results of esophagus extirpation with subsequent one-stage esophagoplasty by pathologically changed or previously operated stomach in 50 patients are analysed. 2 patients had gastric and esophagus cancer and 48 patients esophagus strictures. 36 patients had been previously operated on their stomach, 10 patients had scar deformation of the stomach after thermal burns, 2 patients had a giant leiomyoma. Isoperistaltic gastric tube was used as an esophagus substitute. The postoperative mortality rate was 2,1%. The optimal method of surgical treatment of the combined gastric and esophagus lesions is recommended.

[The immediate and late results of the endoscopic treatment of strictures of the esophageal anastomoses after esophagoplasty].

The article summarizes 11 years' experience (1986-1997) of endoscopical treatment for scar strictures of esophageal anastomoses in 13 patients after various types of esophagoplasty. In 65% of patients the diameter of the anastomosis did not exceed 5 mm. For distension of the anastomoses bouginage, balloon dilation and both of them were used. All sorts of dilators were introduced through an endoscopically inserted guide. In one patient introduction of the guide was a failure. Endoscopical procedures were performed in 112 patients. The diameter of the anastomosis 14 mm and more was achieved in 40 (35.7%) patients, 10-13 mm--56 (50,0%), less than 10 mm--in 18 (14.3%). In one half of the patients restenosis developed 1-3 months after the treatment. As a prophylactic measure it is necessary to maintain regular treatment in outpatient clinic with increasing interval during 3-6 months using any available dilators of a large diameter.

Bakamjian island flap for patch esophagoplasty of the cervical esophagus.

Esophageal stricture may result from chemical injury, progressive ischemic fibrosis, constriction of a previous esophagocolonic anastomosis site, or vascular insufficiency of the transferred gut. The Bakamjian skin island flap was employed as a patch for esophagoplasty to relieve focal esophageal stricture in 12 patients. One of these patients developed postoperative leakage, which necessitated a secondary revision. The others had a smooth postoperative course and were able to resume oral intake with ease. It is concluded that the Bakamjian skin island flap is a reliable and convenient method for repairing short defects of the cervical esophagus in one stage.

[Pharyngo-esophagoplasty by right coloplasty for the treatment of post-caustic pharyngo-laryngeal-esophageal burns: a report of 13 cases].

We report a new technique of pharyngoesophagoplasty by right coloplasty, indicated in postcaustic severe pharyngeal stenosis, analyzing the anatomical and functional results in terms of respiratory tract and phonation as well as digestive tract outcome. We compared our results with those obtained with other procedures. Between March 1995 and September 1998, pharyngoesophagoplasty by right coloplasty was performed in 13 patients. All had severe hypopharyngeal stricture with total obliteration of the two piriform sinuses and the upper esophageal sphincter. Nine patients underwent emergency esophagectomy or esogastrectomy by stripping and four had a cicatricial esophagus. Eight patients underwent tracheotomy before the pharyngoplasty due to burns of the laryngeal margin or airway infections. There was no perioperative mortality. The retrospective analysis in February 1998 with a follow-up of 22 months disclosed two deaths, one from septic shock following pneumnia and one suicide. For the 11 other patients, respiratory function had been restored successfully as the tracheostomy tube had been removed. Eight of the patients were on regular oral diet and the jejunostomy tube had been removed. Three had mild dysphagia and the oral diet was supplemented by jejunostomy tube feedings. These results were most successful compared with those obtained with 29 other patients. Pharyngo-esophagoplasty by right colonic transposition appears to be the method of choice for the reconstruction of post-caustic pharyngeal and esophageal stenosis.
