The development, present status and future trends in the use of computers in urology in the Federal Republic of Germany are reviewed. The hardware, software required for hospital and private practice and the staff needed are discussed. Proposals are given for the installation and stepwise upgrading of computer systems, from simple text processing units to complex hospital communication systems. Finally new technologies that might considerably change the use of computers in urology are presented.
Several possible procedures are discussed for computerizing clinical research and evaluated with regard to available performance and installation effort, the ratio of which is seen to determine efficiency. Furthermore, a short discussion is dedicated to the interface between clinicians and software experts, an issue of central importance in the achievement of satisfactory solutions.
Data management in hospitals used to be the data management of the hospital-management in the last 15 years, not however the data management the doctors performed. The acceptance for electronic means slowly rising more software is used in the clinics today but still by the doctors privately. Software specialists have registered the growing market available for their products. That is why the danger to accept even unusuable software rises every day, as unexperienced software-beginners as doctors mostly are often relied on offers without being told about their dignity and usefulness.
A computer programme for the digitalis therapy developed by us was tested in 282 patients with heart insufficiency. It helps to control the course of the therapy, to establish the optimum fully effective dose of the individually determined glycoside sensitivity and to decrease significantly the danger of an overdosage.
Several possible procedures were discussed for computerizing clinical research and basic documentation. All approaches were evaluated with regard to available performance and installation effort. Additionally examples for personal computer aided data collection in the field of perinatology, oncology, endocrinology and expert systems were presented.
