Best evidence topic reports. Gastric lavage in paracetamol poisoning.

A short cut review was carried out to establish whether gastric lavage is better than activated charcoal in cases of poisoning with paracetamol. Altogether 63 papers were found using the reported search, of which four presented the best evidence to answer the clinical question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these best papers are tabulated. A clinical bottom line is stated.

Efficacy of gastric emptying: gastric lavage versus emesis induced with ipecac.

A prospective study was done to compare the efficacy of gastric emptying achieved by gastric lavage (GL) with that of ipecac-induced emesis (IE) in victims of drug overdose. Thiamine was used as a marker of recovery in gastric samples, as measured by ion exchange/ion pair high-performance liquid chromatography. There were 51 patients in the IE group and 37 in the GL group. GL produced a higher mean per cent recovery of thiamine than did IE (90% +/- 34% compared with 50% +/- 35%). There was a significant difference between the two groups (P less than .001). Recovery of thiamine was more than 70% in 28% of the IE patients, and exceeded 70% in 75% of GL patients. When gastric emptying is desired for management of the adult overdose victim, the use of GL maximizes the chance of recovering unabsorbed liquid drugs from the stomach.

Simple gastric lavage cytology in carcinoma stomach; its relevance to developing countries.

Fifty patients suffering from stomach carcinoma and other non-malignant abdominal disorders, comprising the study group and the control group respectively, were subjected to simple saline lavage for obtaining exfoliated cells from the gastric mucosa. A positive cytological diagnosis was made in 40% cases of carcinoma stomach while the control group gave only 5% false postive results.

Use of vancomycin in the culture of Mycobacterium tuberculosis from gastric lavage.

BACKGROUND & OBJECTIVES: Earlier studies from the Tuberculosis Research Centre, Chennai, on culture of Mycobacterium tuberculosis from gastric lavage (GL) specimens in selective Kirchner's medium (SK) resulted in a loss of 60 per cent culture results due to contamination with aerobic spore bearers (ASB). Addition of vancomycin to SK (SKV) effectively reduced the contamination rate to 20 per cent. The objective of the present study was to further reduce the contamination by collecting the specimens in bottles containing vancomycin, thus providing continuous exposure of the sample to the drug, which is bactericidal to ASB. METHODS: One thousand GL specimens collected from children in vancomycin containing bottles were decontaminated and cultured in SK medium, with and without vancomycin, subcultured on Lowenstein Jensen (LJ) medium and the culture results compared. RESULTS: The contamination of cultures in SK and SKV was 15 and 4 per cent respectively when the specimens were collected in bottles containing vancomycin compared to 60 and 20 per cent contamination reported in the earlier studies. INTERPRETATION & CONCLUSION: The reduced contamination in SK and SKV is most likely due to the collection of samples in vancomycin containing bottles. Although a concurrent comparison of samples processed in vancomycin free conditions would have been ideal, it could not be done due to practical difficulties. The study thus confirms the value of vancomycin as a major deterrent for contamination due to aerobic spores and better results can be obtained if vancomycin is used in sample collection bottles, transport media and liquid culture media used in mycobacteriology laboratories particularly in humid and tropical environment.

Gastric lavage is better than bronchoalveolar lavage for isolation of Mycobacterium tuberculosis in childhood pulmonary tuberculosis.

We compared the sensitivity of gastric lavage (GL) with bronchoalveolar lavage (BAL) for isolating Mycobacterium tuberculosis (Mtb) from 20 children with a presumptive diagnosis of primary pulmonary tuberculosis. GL was performed on three consecutive mornings after an overnight fast. BAL was performed on the same day as the last GL. Specimens were submitted for smears and culture for Mtb. None of the acid-fast stained smears was positive. Cultures of BAL fluid on 2 patients (2 of 20 or 10%) were positive for Mtb. Cultures of the gastric aspirates from the same 2 patients were also positive for Mtb. Eight additional patients had positive GL cultures with negative BAL cultures, resulting in a total of 10 of 20 (50%) patients with positive GL cultures for Mtb. The results of our study indicate that GL done on three consecutive days is better than BAL for the bacteriologic diagnosis of childhood pulmonary tuberculosis.

Best evidence topic report. Gastric lavage in aspirin and non-steroidal anti-inflammatory drug overdose.

A short cut review was carried out to establish whether gastric lavage was better than activated charcoal alone at reducing toxicity after aspirin or other non-steroidal anti-inflammatory drug (NSAID) overdose. Altogether 72 papers were found using the reported search, of which one presented the best evidence to answer the clinical question. A further relevant paper was found on scanning the references of papers identified. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these best papers are tabulated. A clinical bottom line is stated.

Gastric lavage in accident and emergency.

There are few specific treatments available to neutralise the effect of ingested toxic agents. For most overdose patients presenting to accident and emergency (A&E) departments, therefore, the aim of treatment is to limit the systemic absorption of the drug or drugs taken. Gastric lavage is a traditional intervention for managing overdose patients in A&E and the author outline how this procedure may be performed.
