[Organizational and methodological reorganization of diagnostic services in the polyclinics].

Suggestions are made of organizational and methodological restructuring of diagnostic polyclinic service which make it possible to substantially increase its diagnostic and economic effectiveness. The major elements of such restructuring should include: 1. introduction of special programmes on risk group selection according to separate diseases; 2. The application of standardized complex ++fluoro-roentgenographic techniques while using apparatus available in the majority of regular polyclinics. The programmes elaborated permitted to detect a great number of different diseases (tumors and chronic non-specific lung diseases, tumors and gastric ulcer, tumors of the colon, breast tumors, etc) the proportion of which in the general pattern of diseases in great. According to the authors' opinion the introduction of the proposed diagnostic programmes into the work of polyclinics would sharply increase the effectiveness of this service.

[Electroroentgenoscanning of the thyroid gland].

A significant drawback of the radioisotopic scanning of the thyroid gland is the absence of a distinct knowledge of topographo-anatomical associations of this gland with other organs and tissues. To eliminate the mentioned drawback the method of electroscanning (combined electroroentgenography and radioisotopic scanning in two interperpendicular projections) was introduced into practice of the regional hospital. Electroscannograms were prepared on paper by means of the apparatus made in the USSR: roentgenograph ERGA-MP, gammatopograph GT-2, and roentgenodiagnostic unit ARD-2. The principle of the method consists in recording the scannogram of the gland on the preliminarily prepared electroroentgenogram of the cervical area and of the upper part of the chest in the horizontal position of the patient. Lead labels attached to definite areas of the patient's skin were used for matching the scannogram with the electroroentogenogram. This method differs from routine seannography of the gland by the fact that it provides an additional topography-anatomical information; it can be used in any radioisotopic diagnostic laboratory equipped with a scanner.

[A method for modernization of intraoral roengenography].

Experience in the use of PARDUS X-ray diagnostic apparatuses for intraoral roentgenography of maxillodental system is described. A method providing better uniformity of amplification in roentgenographic images of maxillodental system is suggested.

Response of condylar growth cartilage to induced stresses.

The endochondral growth apparatus of the mandibular condyles, in contrast to epiphyseal plates of long bones, reacts positively to mechanical stimulation. Roentgenographic and histologic analyses of the joint structures of two experimental rhesus monkeys, when compared with untreated controls, showed obvious morphologic and structural transformations of the condylar heads as the result of the instituted treatment. Corollary differences were found in the ontogenetic and histogenetic pattern, as well as in the hormonal control, of these condylar growth centers.

Excursion of the flexor digitorum profundus tendon: a kinematic study of the human and canine digits.

The most common problem following primary flexor tendon repair is the failure of the tendon apparatus to glide, secondary to the formation of adhesions. Early motion following tendon repair has been shown to be effective in reducing adhesions between the tendon and the surrounding sheath. Therefore, it is important to determine the amount of flexor tendon excursion along the digit during joint motion. In this study, the excursion between the flexor digitorum profundus (FDP) tendon and the sheath was examined in both human and canine digits. Based on roentgenographic measurements and joint kinematic analysis, the motion of the bones, the FDP tendon, and the sheath were measured with respect to joint rotations. It was found that the canine flexor tendon apparatus behaved similarly to that of the human for the motions studied. The amount of tendon excursion was very small in regions distal to the joint in motion (approximately 0.1 mm/10 degrees of joint rotation). There was little displacement of the sheath (0.2-0.3 mm), except at the metacarpal joint region during metacarpophalangeal (MCP) joint motion and at the proximal interphalangeal (PIP) joint region during PIP joint motion. Tendon excursion relative to the tendon sheath was the largest in zone II during PIP joint rotation (1.7 mm/10 degrees of joint rotation). These results suggest that PIP joint motion may be most effective in reducing adhesions following tendon repair in zone II.

[Stages of development of aspergillus-mycetom and their roentgenographic signs (author's transl)].

A series of 13 cases of aspergillus-infection in the respiratory tract are reviewed. A case of rapid development of an intrapleural aspergillus-mycetom is presented. A complete series of chest roentgenograms and operation-photos allowed to demonstrate like with a quick-motion apparatus 3 stages of development of an intrapleural aspergillus-myceton: I. Infection through Aspergilli of a residual intrapleural cavity by the bronchial way. Pleural thickening as reaction to the irritation. II. Development of fungus-lawn and mycotic layers on the cavity-wall. III. Scaling off from the wall and moulding to the fungus-ball through movement of human body. Each of the three stages can be coordinated to a roentgenographic sign. The detection in an early stage of development makes it possible to treat the aspergillus-mycetom also by conservative methods, f. i. with a Pimafucin-Suspension (Natamycin) by aerososl.

[Nonstandard equipment for intracavity gamma-therapy cabinets].

Carrying out the intracavitary radiotherapy requires strict control over accurate disposition of endostates relative to a pathological focus and checking the proper position of the patient during the whole irradiation course. For this purpose, a double-tube X-ray localizer, based on the X-ray apparatus RUM-10 and a new therapeutic table were designed and accepted for clinical practice. The table construction ensures the localizer application in both the roentgenographic and roentgenoscopic modes and permits to employ a TV-system with amplification of X-ray patterns.

Reflex sympathetic dystrophy with prominent involvement of the nail apparatus.

After closed hand trauma, a 17-year-old boy had acute inflammatory changes that resembled bacterial whitlows of the third and fourth right fingers. Clearing of the inflammatory changes was followed by the development of cyanosis, hyperhidrosis, and roentgenographic evidence of patchy osteoporosis in the involved extremity. Findings of a biopsy specimen revealed that the inflammatory lesions in the proximal nail folds were caused by proliferation of capillary vessels embedded in edematous loose connective tissue. This is the first report of cutaneous histopathologic findings in the first stage of reflex sympathetic dystrophy, although similar features have been described in synovial and bone biopsy specimens of patients with reflex sympathetic dystrophy.
