Free fat transplant prevents osseous reunion of skull defects. A new approach in the treatment of craniosynostosis.

Free fat tissue transplant to correct premature epiphyseal fusion of long bones has been successfully used to prevent re-ossification after bone bridge resection. In the present study we have applied this principle in the prevention of osseous reunion of the bone defect in the calvaria of the rabbit. In two-week-old animals standard bone defects were made on both sides of the sagittal suture to the temporal bone. The dura was left intact. On the right side a free autogenous fat transplant was placed to fill the bone defect. On the left side the control defect was left without fat interposition. The skull defects were examined postoperatively at three weeks, three months, and eight months. The control defects reossified within three weeks. The bone defects with fat tissue transplant were found to be open and of original size at eight months postoperatively. On the transplanted side histological examination revealed living fat cells filling the gap. Osteogenesis was inhibited and reunion of the bone edges was prevented. All the control bone defects were filled by lamellar bone. The use of free fat tissue transplant can be a useful alternative, serving as a biological interposition material in the surgical treatment of craniosynostosis.

The emerging role of adoption reunion registries: adoptee and birthparent views.

Since the 1980s, government-sponsored adoption reunion registries have emerged as key access points for individuals separated by adoption to secure information and initiate efforts to be reunited with their birth-families. Despite the prevalence of such registries, little information is available about their operational effectiveness. This article presents data from a survey of adult adoptees and birthparents who used the services of a statewide reunion registry.

Adoption↗

[Adoption and reunion.].

Following the phenomenon of reunions instigated by the newspaper Le Soleil of Quebec, the author provides us with his reflections on this subject. He explains that there exists already in parents and children of a natural family, an adoption phenomenon whose bases he indicates. From this finding he discusses the six (6) important facts that the reunion phenomenon has uncovered, and concludes with this question : does the search for biological parents represent a search for a real alternative to the child's actual parents, as religion is at times, or, is it more an effort aimed at the retrieval of the complete history of his birth and the first days of his life?

[Incidence of endemic goiter on Reunion Island. A search for etiological factors].

In an epidemiological study carried out in Reunion Island, 1,686 randomized school children aged from 11 to 15 years were examined for goitre by cervical palpation. A detailed questionnaire was fully completed by each child and his parents. The iodine level was measured in 168 urinary samples and in the salt and water consumed in the various places investigated. The overall incidence of goitre was 8.2 percent, rising up to 19.7 percent in the mountainous part of the island. The mean urinary iodine level was 40.2 +/- 2.7 micrograms I/g creatinine (m +/- SEM) and fell to 20.0 +/- 3.7 in the highlands. Water and salt contained little iodine. A significant relationship was noted between the presence of goitre on the one hand and sex, familial incidence of goitre, cassava consumption and distance from the coast on the other hand. This study demonstrates that endemic goitre and iodine deficiency are present in a limited area of Reunion Island.

[Seroepidemiological study of human leptospirosis at Reunion Island].

In order to evaluate the prevalence of human leptospirosis in Reunion island and to identify possible risk factors, a study was realised on a representative population sample of 3.338 persons. The prevalence of leptospirosis, diagnosed by ELISA and confirmed by Micro Agglutination Test, was 1.1%. Male predominance and higher prevalence in rainy parts of the island, were confirmed. It has not been possible to display risk factors such as housing conditions or professional exercise. The serological repartition showed not only icterohaemorrhagiae serovar, but also canicola, panama and sejroe, especially in women. These results, compared with clinical studies (showing nearly exclusive male repartition, in agricultural workers, due to icterohaemorrhagiae serovar), confirm the double look of human leptospirosis in Reunion island: clinical leptospirosis, severe, concerning males, often countrymen, due to icterohaemorrhagiae serovar, and infraclinical leptospirosis, concerning principally females, which is a domestic illness, due to other serovars.

Empirical classification of infant-mother relationships from interactive behavior and crying during reunion.

Multiple discriminant function analysis (MDFA) was conducted with data from 255 Strange Situations conducted and scored by Ainsworth and her colleagues. Cross-validated discriminant functions and classification weights were obtained, allowing attachment classifications (A, B, C) to be assigned directly from scores on interactive behavior and crying during reunion episodes. In the past, classification agreement within laboratories has often been used as a training criterion. Unfortunately, this does not insure that classification criteria agreed upon within a laboratory are comparable across laboratories, nor does it insure that agreed upon criteria will yield the same classifications that would have been assigned by the researchers who developed the scoring system. The present results enable researchers who have mastered the scoring systems for reunion behavior and crying to obtain attachment classifications directly from scores on these variables. Alternatively, this procedure may be used to guide the training of, and validate classification decisions by, local judges.

Celebrating life: lessons learned from a bone marrow transplant reunion.

The number of persons undergoing bone marrow transplantation (BMT) has increased dramatically over the last decade to more nearly 15,000 each year throughout the world. Addressing the physical and psychosocial needs of this particular population is of utmost importance as greater numbers of persons face the challenges associated with surviving BMT. Most of the research to date has been with survivors of allogeneic transplants. The increasing number of autologous transplant survivors presents a new constituency to evaluate. A reunion was held at a regional cancer center to bring together former BMT patients, their families, and the staff who had cared for them. The purpose was to provide attendees with a forum in which to ask questions, to offer them a historical perspective on this treatment, and to begin to gather long-term data on their transplant experiences. The latter was done initially by mail questionnaire and continued in the day's breakout sessions. The information gained provides a strong incentive to increase research efforts on long-term effects of BMT. The reunion surpassed everyone's expectations both in terms of attendance and overall success of the program content and networking, which has continued.

[Iodine deficiency in the Cirque of Salazie (Reunion Island). Analysis of nutritional causes and study of their consequences].

Randomized subjects (n = 294), (35 young children, 6-30 months old--91 children 8.6 +/- 0.7 years, 116 teenagers 13.2 +/- 1.9 y--52 young women 28.5 +/- 6.1 y) leaving in a mountainous area of Reunion Island (Cirque de Salazie) were evaluated according to the following: a questionnaire related to food intake, cervical palpation for thyroid size evaluation and collections of urine and foods (natural and mineral water, breast and commercial milk and salt) for iodine measurement. Neonatal TSH of 308 new borns in Salazie were compared to TSH of 300 new-borns of Lille (France). The prevalence of goiter increases with age (0% in young children, 12.1% in children, 23.3% in teenagers, 38.4% in women) whereas median urinary iodine (microgram/dl) decreases in the same groups (12-7.8-5.6-4.2). The intake of milk and dairy products significantly decreases from children to adults. Neonatal TSH values are significantly higher in Salazie than in Lille (p < 0.005). Natural and mineral water, breast milk and salt contain little iodine. In conclusion iodine deficiency is present in this area of Reunion Island with a significant increment of neonatal TSH values.

[Acute leptospirosis in children in Reunion Island: a retrospective review of 16 cases].

INTRODUCTION: Leptospirosis is a worldwide zoonosis caused by pathogenic species of the genus Leptospira. This infectious disease known with a high incidence in Reunion island (French overseas territories in Indian Ocean) is in state of endemia especially during rains season. OBJECTIVES: The aim of our work was to identify the mains epidemiologic, clinical, biological, and therapeutic features of leptospirosis in children. POPULATION AND METHODS: We conducted a retrospective analysis of children hospitalized in pediatric unit with the diagnosis of leptospirosis from January 2001 to June 2004 in general hospital of Saint-Denis. We found out 16 cases (mean age 14 years, range 9-17), mainly boys (ratio 7:1). RESULTS: The patient sera reacted most strongly with Leptospira interrogans serovars canicola (66%), icterohaemorrhagiae (17%), and sejroe (17%). Epidemiologic data indicated contact with contaminated water in most cases (68%). Jaundice was present in 43% of the patients, increased transaminase levels in 56%, renal failure in 50%, meningitis in 25%, ECG abnormalities in 6%, respiratory manifestations in 6%, systemic manifestations in 12% and thrombocytopenia in 56%. Death rate was zero, but renal failure is likely to induce life prognosis. DISCUSSION: The diagnosis' traps are numerous, leading to an underestimation and underdiagnosis of the leptospirosis, more over there is a lack in specific, reliable, and quick biological test to make the diagnosis. A negative polymerase chain reaction analysis (PCR) do not exclude the diagnosis, and the microagglutination test (MAT) remains the reference of the undoubtly diagnosis of leptospirosis. It appears that it is more often the conjunction of epidemiologic data (young boy, swimming or fishing in river, rains season), with clinical and biological data that lead to the diagnosis. Diagnosis was not evoked at the emergency room in 37% of the patients. CONCLUSION: Leptospirosis should be considered face to an influenza like illness especially during rains season.

Plants from reunion island: evaluation of their free radical scavenging and antioxidant activities.

A total of 274 crude methanolic and dichloromethane extracts of endemic or indigenous plants from Reunion Island (75 species belonging to 35 families, 43 among them being used in the local traditional pharmacopoeia) have been qualitatively screened for their free radical scavenging (2,2-diphenyl-1-picrylhydrazyl radical) and antioxidant (beta-carotene) activities. In this study, various species with appreciable activity were identified as potentially novel sources of free radical scavenging and antioxidant compounds.

Effects of industrial outfalls on tropical macrobenthic sediment communities in Reunion Island (Southwest Indian Ocean).

Temporal changes in the composition of soft bottom macrobenthic assemblages at Reunion Island (Southwest Indian Ocean) were studied in the context of a long-term environmental monitoring programme studying the impacts of effluents of industrial sugar cane refineries that are transferred to shallow and deep coastal environments by different pathways: surface discharge and deep underground injection. Seven stations (between 20 and 160 m depth) were surveyed between 1994 and 2003 on the industrial zone. One additional station was surveyed on a reference site. Spatio-temporal changes in the composition of macrobenthic communities were assessed using several diversity indices, ABC curves, MDS and associated ANOSIM tests and biotic indices. Among the 171 taxa recorded, polychaetes were dominant (89 species), followed by crustaceans and molluscs. The analysis of spatial changes in the composition of macrobenthos showed the existence of distinct benthic communities along the depth gradient. Temporal changes in macrobenthos composition were most prominent at the shallowest station. They mainly corresponded to the decline of several initially dominant taxa and the increase of the Eunicid polychaete Diopatra cuprea. This station further showed increasing macrofaunal abundance, biomass and sediment organic content over time, concomitant with decreasing sediment grain sizes. In deeper environments, temporal changes were much smaller. Macrofaunal abundance and species richness increased progressively, suggesting a moderate impact on benthic ecosystems resulting from slight enrichments due to effluents rich in organic matter. Our results highlight an original response to disturbance pattern involving opportunistic Eunicidae species (D. cuprea) not previously described. Moreover, they allow for the comparison of the impact on macrofauna caused by industrial effluents exported by two distinct and different pathways in a tropical coastal high-energy marine environment.

Analysis of Variance↗

The use of multilevel models to evaluate sources of variation in reproductive performance in dairy cattle in Reunion Island.

Sources of variation in measures of reproductive performance in dairy cattle were evaluated using data collected from 3207 lactations in 1570 cows in 50 herds from five geographic regions of Reunion Island (located off the east coast of Madagascar). Three continuously distributed reproductive parameters (intervals from calving-to-conception, calving-to-first-service and first-service-to-conception) were considered, along with one Binomial outcome (first-service-conception risk). Multilevel models which take into account the hierarchical nature of the data were used to fit all models. For the overall measure of calving-to-conception interval, 86% of the variation resided at the lactation level with only 7, 6 and 2% at the cow, herd and regional levels, respectively. The proportion of variance at the herd and cow levels were slightly higher for the calving-to-first-service interval (12 and 9%, respectively) - but for the other two parameters (first-service-conception risk and first-service-to-conception interval), >90% of the variation resided at the lactation level. For the three continuous dependent variables, comparison of results between models based on log-transformed data and Box-Cox-transformed data suggested that minor departures from the assumption of normality did not have a substantial effect on the variance estimates. For the Binomial dependent variable, five different estimation procedures (penalised quasi-likelihood, Markov-Chain Monte Carlo, parametric and non-parametric bootstrap estimates and maximum-likelihood) yielded substantially different results for the estimate of the cow-level variance.

[Obstetrical risk factors of 365 primiparous adolescent pregnancies in Reunion Island].

OBJECTIVES: Analysis of obstetrical risk factors in teenage primiparous pregnancies in Reunion Island (4% of total births). MATERIALS AND METHODS: Retrospective study, between 2001 and 2002, comparing primiparous adolescents (13-17 years, n = 365), with primiparous controls (18-29 years, n = 2050). The analysis included demographical factors, maternal medical histories, prenatal follow-up, obstetrical risk factors, delivery modes and neonatal characteristics. RESULTS: Adolescents attended on average 8 prenatal consultations, however 4% had poor prenatal care (less than 3 visits, OR 4.2, P < 0.001 vs controls). They presented less gestational diabetes, but there were no differences concerning pre-existing hypertension, hypertensive disorders of pregnancy, medical reasons of hospitalisation between the two groups. Mode of delivery was more favorable in adolescents (half rate of caesarean sections, shorter duration of membrane ruptures). Nevertheless, adolescents presented a higher risk of severe prematurity (<32 Weeks gestation, incidence 3.6% vs 1.6%, OR 2.3, p = 0.008). CONCLUSION: With optimal prenatal care (more than 90% of our cohort), primiparous adolescents present globally a favorable course of their pregnancies and have better deliveries than their young (18-29 years) counterparts. However, there is a significant risk of severe prematurity requiring special care for these pregnancies.

[Study of 178 ante partum deaths in 2001-2004 in the southern part of Reunion Island].

INTRODUCTION: The perinatal mortality rate is 18.5 in the southern part of the Reunion Island (Indian Ocean), of which 2/3 are due to antepartum fetal deaths (APFD). METHODS: During a 4-year period (2001-2004) all APFD from 22 weeks gestation were recorded and analyzed with placental histology, bacteriological samples and autopsies in 27% of cases. The Australasian and New-Zealand classification PSANZ-PDC (2000) was used. Risk factors of fetal death with monofetal pregnancies are determined in comparison with live births. RESULTS: Out of 21.495 total births, 178 APFD were recorded. The main obstetrical risk factors were primiparity (OR 1.6, p = 0.002), maternal age over 34 years (OR 1.6, p = 0.01), hypertensive disorders of pregnancy (OR 3.0, p < .001) and multiple births (OR 2.5, p < 0.001). The great majority of APFD (76%) involved preterm fetuses, of which 61% of very preterm (<33 weeks), and 25% of fetuses were growth retarded (OR 3.9, p < 0.001). Only 8% of cases were considered unexplained. The main etiologies were infectious causes in 26% of cases, vascular fetal growth restriction (18%), specific perinatal conditions (14%) of which one-third were due to cord anomalies, preeclampsia (10%), maternal conditions (8%), congenital anomalies (8%) and ante-partum hemorrhage (7%). We discuss the interests and the limitations of using the Australian and New-Zealand classification PSANZ 2000. Intra-uterine growth retardation is one of the principal risk factors of fetal death. CONCLUSION: Besides well-known obstetrical risk factors such as diabetes, hypertension, multiple pregnancies, all screening of intra-uterine growth retardation in the second trimester of pregnancy should include a special survey in order to minimize the incidence of APFDs.

[Clinical study and prevalence of fetal alcohol syndrome in medico-social institutions of the Reunion Island].

BACKGROUND: Fetal alcohol syndrome (FAS) is a major problem in the Reunion Island and the Public Health Authorities decided to determine its prevalence in their medico-social centers on 31 December 1996. MATERIAL AND METHODS: A questionnaire was established to identify affected patients in the 20 medico-social centers in charge of 1320 children. Eighty-eight children were selected and 87 could be analyzed. RESULTS: Sixty-four of 87 (76.3%) were FAS and 23 of 87 (23.7%) had closely alcohol-related diseases. The prevalence was between 7.1 and 14.1% and lower than expected from available data. CONCLUSION: The study allowed to precise the social and familial factors predisposing to alcohol addiction during pregnancy. A TV prevention message will be broadcasted after this study.

Bird pollination in an angraecoid orchid on Reunion Island (Mascarene Archipelago, Indian Ocean).

BACKGROUND AND AIMS: Although numerous angraecoid orchids in Madagascar display typical sphingophilous syndrome (i.e. white, nectariferous, long-spurred flowers, producing a strong scent at the crepuscule that is attractive to moths), three species of Angraecum in Reunion, belonging to the endemic section Hadrangis, have atypical unscented and short-spurred flowers. The aim of the study was to investigate the implication of plant-pollinator interaction on the evolution of floral morphology of these peculiar island floral forms. METHODS: The flower morphology of A. striatum (one of the three section Hadrangis species) was investigated by performing a set of floral measures, and the reproductive biology was investigated by a set of hand pollination experiments. Natural pollinators were observed by means of a digital video camera. Pollinator efficiency (pollen removal and deposition) and reproductive success (fruit set) were quantified once a week in natural field conditions during the 2005 flowering season (i.e. from January to March). KEY RESULTS: The orchid is self-compatible but requires a pollinator to achieve fruit set. Only one pollinator was observed, the endemic white-eye Zosterops borbonicus (Zosteropidae). These birds perched on inflorescences, and probed most fresh-looking flowers on each plant for nectar. Nectar was both abundant (averaging 7.7 microL) and dilute (averaging 9.7 % sugar in sucrose equivalents). Birds were mostly active between 0830 and 0930 h. Visits to plants were extremely short, lasting from 9 to 27 s. At the study site, 60.9 % of flowers had pollen removed, and 46.4 % had pollinia deposited on stigmas. The proportion of flowers that initiated a fruit averaged 20.6 % in natural conditions. CONCLUSIONS: For the first time, a bird-pollinated orchid is described from a sub-tribe that is mainly specialized for moth pollination. This study documents a morphological shift in flowers in response to pollinator adaptations in the insular context of the Mascarene Archipelago.

Juvenile limb-girdle muscular dystrophy. Clinical, histopathological and genetic data from a small community living in the Reunion Island.

A series of patients affected by a muscular dystrophy, similar to the original description of a juvenile scapulo-humeral form by Erb in 1884 and fitting with the criteria used to define limb-girdle muscular dystrophies, was discovered in a small community living in the southern part of Reunion Island in the Indian Ocean. A detailed clinical analysis was conducted over 5 years on a cohort of 20 patients. This community presented a high degree of consanguinity as it was segregated from the majority of the island population for more than a century. In previous molecular genetic studies, the disease locus has been mapped to chromosome 15p. Mutations were recently identified in a gene located in this region encoding for muscle-specific calcium activated neutral protease (CANP3). Clinical, pathological, genetic and complete identification of the mutations are presented here, establishing, for the first time, precise clinico-genetic correlations in this form of autosomal recessive, juvenile, limb-girdle muscular dystrophy (LGMD).
