For over 20 years the medical literature has carefully documented the undertreatment of all types of pain by physicians. During this same period, as the field of bioethics came of age, the phenomenon of undertreated pain received almost no attention from the bioethics literature. This article takes bioethicists to task for failing to recognize the undertreatment of pain as a major ethical, and not merely a clinical, failing of the medical profession. The nature and extent of the problem of undertreated pain is examined, as well as possible reasons for its disregard by bioethicists. The factors contributing to undertreated pain in the clinical setting are considered, as well as the hazards posed by recent failures to address ethically questionable clinical practices. Finally, suggestions are offered for refocusing the attention of bioethicists to this significant problem.
Different data sources were used to examine hypothesized relations among neighborhood-, family-, and individual-level variables, and perceptions of neighborhood collective efficacy. Data were from 1,105 individuals (56% female, 42% African American, and 58% White) nested within 55 neighborhoods and 392 families, analyzed within a multilevel design using a 3-level model. At the neighborhood level, the study examined relations between Census, police, and neighborhood representative indicators. At the family level, the model examined the influence of marital status and family income. At the individual level, gender and age were examined. Results indicated that age at the individual level, marital status at the family level, and poverty and perceived gang activity at the neighborhood level predicted levels of neighborhood collective efficacy. The study illustrated significant variation across neighborhoods and families, and demonstrates the utility of combining different sources of neighborhood data to examine relations of interest within a multilevel framework.

Using Tinbergen's 4 why questions of causation, development, evolution, and function, the authors examine hypnosis from a larger, evolutionary perspective. Reasoning by analogy, they seek to view hypnosis in terms of an action pattern that represents a self-contained behavioral program although not as rigid as seen in lower organisms. In humans, such a program develops within the context of a long developmental sequence emphasizing social connections, imitation of significant figures, and the use of linguistic symbols to regulate both internal and external processes and actions. In terms of a mechanism, the authors speculate on the involvement of the cingulate cortex in achieving in hypnosis the experience of autonomous actions or analgesic sensory processes. Finally, they point to the fact that hypnotizability is associated with an ability to reduce the experience of pain, modulate the immune system, and achieve greater benefits of psychosocial therapies as a functional significance of the hypnotic experience.
