[Spinal puncture in suspected purulent meningitis].

Today, CT scan of the brain is performed as a routine a the first diagnostic procedure in cases of suspected acute subarachnoid haemorrhage. Subarachnoid blood is demonstrated in a high percent of these cases provided that the scan is carried out within the first few days after the bleeding. Furthermore, the scan may also disclose localised intracranial haematoma or other expansive lesions with brain shift. In such cases, lumbar puncture may be dangerous, due to the risk of herniation. In cases of suspected acute bacterial meningitis or other infectious disease of the central nervous system, performance of a CT scan before lumbar puncture is not a well established procedure. Two case histories are described. Both patients had brain abscesses and deteriorated dramatically after lumbar puncture. The role of brain CT scanning in the management of such patients is discussed.

[Headache after spinal puncture. Treatment alternatives: epidural blood patch].

Headache is one of the most frequent complications of the subarachnoid puncture. Physiopathology and the different clinical factors involved in this type of headache are reviewed. The treatment is based in two types of procedures: those increasing cerebrospinal fluid production and those trying to decrease its leakage, like the epidural blood patch.

[Effect of tiapride on the side effects of cerebrospinal fluid depletions in spinal puncture, pneumoencephalography and air myelography].

The authors report their experience of the treatment post-cerebrospinal fluid loss syndrom, following subtraction by lumbar puncture, pneumoencephalography and air myelography. The study of 70 patients, divided in two equal groups, one receiving classic therapy, one receiving tiapride, a new molecule of original conception, lead to emphasize the interest of this drug. Tolerance in general is good, despite the administration of high doses and effects on the syndrom are positive and remarkable in preventive and curative treatment.

[Spinal vascular malformation in a child, revealed by lumbar puncture].

Spinal vascular malformations are exceptional in children. We report a case in 1 year-old patient due to a spinal vascular malformation which was revealed by paraplegia after lumbary punction. The diagnosis was made by MRI. Spontaneous outcome was characterised by the persistence of paraplegia.
