[Study on relationship between Deficiency Syndrome of TCM and the biological behavior in patients of esophageal carcinoma].

OBJECTIVE: To investigate the relation between Deficiency Syndrome (DS) of TCM and the invading depth, lymphnode metastasis, TNM stage and prognosis prediction of esophageal carcinoma. METHODS: According to Syndrome Differentiation of TCM, 101 patients of esophageal carcinoma were evaluated before operation and divided into two groups, the DS group and the non-DS group. RESULTS: Thirty-eight cases in the 101 patients were DS, among them, 14 were Qi-Deficiency, 10 Yin-Deficiency, 10 Qi-Yin Deficiency, 2 Qi- and blood Deficiency, 1 Yang-Deficiency and 1 Yin-Yang Deficiency. There were significant differences between the DS group and non-DS group in the invading depth (deeper in DS), metastasis of lymphnode (severer in DS) and TNM stage (later in DS) of esphageal carcinoma, (P < 0.05). CONCLUSION: There is a significant relation between DS and the biological behavior of esophageal carcinoma. The prognosis of DS patients is poorer than that of the non-DS patients.

[Histocytopathological study on gastric mucosa of spleen deficiency syndrome].

OBJECTIVE: To explore the pictures of gastric mucosa of Spleen deficiency syndrome, and the relationship between the degree of pathological changes of gastric mucosa and types of Spleen deficiency syndrome. METHODS: One hundred and eighty-eight Spleen deficiency patients with organic diseases of heart, lung, liver, gallbladder, pancreas, intestine and kidney excluded were examined their gastric mucosa. Tests of histopathology and subcellular ultrastructure were conducted by using optical microscope, electron scanning microscope, electron transmission microscope and histochemical staining methods. RESULTS: The gastric mucosa of Spleen Qi deficiency and Spleen Yang deficiency patients could either be affected with organic lesion (type G) or unaffected (type F); the gastric mucosa of Spleen Yin deficiency and Spleen deficiency with Qi stagnation were both affected by gastric lesions, and the degree of their lesions was more serious than that of Spleen Qi deficiency of type G and Spleen Yang deficiency of type G, P < 0.05-0.01. CONCLUSION: Pathological changes did not necessarily occur in the gastric mucosa of Spleen deficiency patients; the degree of pathological changes was relevant with the evolution of Spleen deficiency types.

[Observation on left ventricular diastolic function of patients with heart-deficiency syndrome of cardiovascular diseases].

In this paper, left ventricular diastolic function (LVDF) of 99 Heart-Deficiency Syndrome (HDS) patients was observed with pulse Doppler echocardiogram and was compared with normal group. The results showed that different LVDF abnormalities occurred in the HDS patients. When E, Ei DC were decreased and A, Ai, A/E, Ai/Ei, IRT increased, the degree of HDS deteriorated, the pattern was Heart Qi-Yin Deficiency > Heart Yang Deficiency > Heart Qi Deficiency > Heart Yin Deficiency. In conclusion, the above parameters of LVDF could be important indices for evaluation of HDS.

[Experimental study on the effect of yiyuan shengxue capsule on gamma-interferon and tumor necrosis factor-alpha in patients with chronic aplastic anemia].

OBJECTIVE: To explore the effect of Yiyuan Shengxue capsule (YYSXC) on peripheral gamma-interferon (gamma-IFN) and tumor necrosis factor-alpha (TNF-alpha) in patients with chronic aplastic anemia (CAA). METHODS: Applying the assay of microcell lesion inhibitory and L595 cytotoxicity test to detect gamma-IFN and TNF-alpha level 36 thirty-six cases with CAA before and after treatment with YYSXC. RESULTS: To compare the test group with the normal group, the difference was significant before treatment (P < 0.05, P < 0.01), but after treatment, the difference was insignificant between these two groups (P < 0.05). To compare the group of Kidney Yin Deficiency with Kidney Yang Deficiency, both the difference of before and after treatment were significant. For the therapeutic effective rate, the group of Kidney Yang Deficiency was higher than that of Kidney Yin Deficiency. CONCLUSION: YYSXC could treat CAA through reducing the level of peripheral gamma-IFN and TNF-alpha of CAA patients, and the effect was through regulating the dysfunctional immune state.

[Syndrome differentiation in traditional Chinese medicine in chronic aplastic anemia; T-lymphocyte subpopulation in peripheral blood and erythrocyte C3b receptors].

UNLABELLED: In this paper, the Syndrome Differentiation in TCM and the determination on T-lymphocyte subpopulation and RBC C3b receptor were carried out by using indirect SPA assay and RBC C3b receptor ring test among 51 chronic aplastic anemia patients. RESULT: The OKT3, OKT4 and OKT4/OKT8 ratio in groups of Kidney-Yin Deficiency group were lower, while the OKT8 was higher than that of controls. In the group of both Kidney-Yin and Yang Deficiency, OKT4 and OKT4/OKT8 ratio were lower. and the value of OKT8 is higher than that of controls. The OKT8 in the Kidney-Yang Deficiency group was higher. and the ratio of OKT4/OKT8 was lower than that of controls. The OKT8 in the Kidney-Yin Deficiency group among three groups increased higher than that of other two groups. while the OKT4/OKT8 ratio reduced. The OKT4/OKT8 ratio in the group of both Kidney-Yin and Yang Deficiency was higher than that of Kidney-Yang Deficiency group. Among the three groups, the rosette rates of RBC C3b receptor were lower than controls. The above-mentioned data were statistically significant. These characteristic changes could be used as a reference of microcosmic Syndrome Differentiation and also a guide for how to apply the immunosuppressant and immunopotentiator.

[Exploration on essence of spleen-kidney deficiency in middle-aged patients].

OBJECTIVE: To explore the essence of Spleen-Kidney Deficiency in middle-aged patients. METHODS: Investigation was carried out in 773 cases of 50-69 years old to assay 34 parameters in them, including blood lipid, oxidation and anti-oxidation related substance, sex hormone, liver and renal function, immune function, blood routine, blood pressure and lung vital capacity, etc. RESULTS: Spleen Deficiency Syndrome is closely related with lipid metabolism disorder; Kidney-Qi Deficiency Syndrome is closely related with hypoimmune function; Kidney-Yin Deficiency Syndrome is closely related with lipid metabolism disorder and hypertension, and Kidney Yang Deficiency is closely related with weakness of anti-oxidation capacity, hypoimmunity, the internal environment disorder of sex hormone and aging, manifested as multiple functions abating. CONCLUSION: Spleen Deficiency, Kidney-Qi Deficiency and Kidney Yang Deficiency are different layers of a gradually developed and aggravated pathological process, but Kidney-Yin Deficiency could not be listed into this layer.

[Symptoms complex of traditional Chinese medicine in patients with congestive heart failure].

This paper have carried out clinical study on 65 cases of congestive heart failure (CHF). (1) The insufficiency of Heart-Qi was the main cause of invasion of CHF. Along with aggravation of CHF, the Heart Yin deficiency developed into the Heart Yang deficiency and accompanying symptoms multiplied gradually. (2) Except plasma aldosterone (pALD) (P greater than 0.05), the levels of plasma atrial natriuretic polypeptide (pANP) and plasma angiotensin II (pAII) in the Heart Yin deficiency group was increased obviously compared with healthy persons (P less than 0.05-0.001). In the Heart Yang deficiency group, the levels of pANP and pALD was increased markedly (P less than 0.05-0.005), the level of pAII was decreased obviously (P less than 0.05), as compared with the Heart Yin deficiency group. (3) There were positive correlations between pANP and pALD (P less than 0.005), negative correlations between pAII and pANP, pALD (P less than 0.001).

[Analysis on the traditional Chinese medicine syndromes of the patients with autoimmune thyroid diseases. Changes in the thyroid and immune functions in 109 cases].

Eighty-nine cases of hyperthyroidism and 20 cases of hypothyroidism caused by Hashimoto's thyroiditis were observed in order to analyse the thyroid and immune functions of the patients, and their relationship with the syndromes of TCM. The results showed that, in the patients with Yin deficiency syndrome, the contents of total T4, T3 were higher than normal and TSH lower than normal, while in Yang deficient patients, the contents of total T4, T3 were lower than normal and TSH higher than normal. This results suggested that the states of thyroid functions were closely related to the TCM syndromes. It was also found that the percentage of OKT 4+ cells and the self-recognizing ability of lymphocytes were lower than normal in patients with hyperthyroidism and Yin deficiency. While in patients with hypothyroidism and Yang deficiency, they were higher than normal. These meant that the abilities of lymphocyte autoreaction in Yin deficient patients were in contrary tendency with those in Yang deficient patients. The former had the manifestation of over-inhibition while the latter, hyperaction. Besides, the contents of auto-antibodies were higher than normal in both the patients with hyperthyroidism and hypothyroidism, which manifested itself as a common character of autoimmune thyroid diseases. The results indicated that there were common characters as well as individual characters of thyroid and immune functions between hyperthyroid patients and hypothyroid patients, and these characters might well be the material bases of various syndromes in TCM.

[Pathology of syndrome-differentiation types of superficial deficiencies in patients with gastric cancer].

Analysing the relations between TCM syndrome differentiation type and pathology in 312 patients with gastric cancer, the authors put forward the syndrome differentiation type of deficiency of vitality and excess in superficiality. The studies showed that the four syndrome types of deficiency of vitality such as Spleen-Stomach deficiency, Qi-Xue deficiency, Yin deficiency and inner heat, Spleen-Kidney Yang deficiency and the three syndrome types of excess in superficiality such as Liver-Stomach disorder, blood stasis and stagnation of Qi, Phlegm-dampness and stagnation of toxic. The same changes in pathologic characteristic of both were from early to late stage, the infiltration getting deeper, the tumor getting larger and the metastasis of lymph nodes around the stomach from little to much. Their difference was that in the deficiency of vitality syndrome-type, the chief manifestation was in nest or spread infiltration type which became serious by degrees, whereas in excess in superficiality type, the reaction of lymphoidocytes around the cancer was decreasing. The studies also showed that in most syndrome types of Spleen-Kidney Yang deficiency and Phlegm-dampness and stagnation of toxic, the tumor occurred in wide-range and the degree of tissue differentiation was quite low. The authors suggested that there might be morphopathologic basis in TCM syndrome differentiation types and the conclusion has practical value in guiding clinics.

[Relationship between coronary angiography and syndrome differentiation type of TCM].

OBJECTIVE: To understand the relationship between the coronary Angiography (CA) reflected degree of coronary artery lesion and the TCM Syndrome Differentiation-type (TCM-SDT). METHODS: Patients TCM-SDT were differentiated adopting the standard of Syndrome Differentiation for coronary artery diseases revised in 1990, and the comparison of TCM-SDT with the findings in CA was carried out. RESULTS: CA examination showed that 29 patients (39.73%) were of one-artery lesion, 9 patients (12.33%) two-artery lesion and 35 patients (47.95%) three-artery lesion; the stenosis degree of them were: 54 arteries were mild stenosis, 38 moderate stenosis and 31 severe stenosis; 29 arteries were completely occluded, the sum total of affected artery was 152 (69.41%), and the other 67 arteries (30.59%) had no lesion. TCM-SDT showed that all the 73 patients had secondary Excess Syndrome, among them 73 patients (100%) with blood stasis Syndrome, 39 patients (53.4%) with Phlegm-Turbid Syndrome, 16 (21.0%) with Qi-stagnation Syndrome and 7 (9.6%) with Cold condensation Syndrome. 63 patients (86.3%) with obvious primary Deficiency Syndrome, among them, 57 (78.1%) were Qi-Deficiency, 18 (24.7%) Yang-Deficiency, 28 (38.4%) Yin-Deficiency and 5 (6.8%) Yang collapse. All patients had blood stasis of various degrees, manifesting on tongue proper prominently. In the 80.8% patients who had angina pectoris, 74.0% had their mouth, lips and gum dark red, dark purple or light dark in color. The comparison between TCM-SDT and Blood-Stasis score revealed that patients of Cold condensation-blood stasis Syndrome and Yang collapse-blood stasis Syndrome were serious. Patients of Qi stagnation-blood stasis Syndrome and Yin Deficiency-blood stasis Syndrome were mild cases, while patients of Phlegm Turbid-blood stasis Syndrome and Qi Deficiency-blood stasis Syndrome were moderate severity cases. The number of affected artery in Cold condensation patients was the most, and in Yin-Deficiency patients was the least. The coronary artery stenosis degree in patients of Qi-stagnation and Yin-Deficiency were milder than in those of Yang-collapse and Yang-Deficiency patients. CONCLUSION: The more the number of arterial lesion, the severer the degree of stenosis and the higher the blood stasis score, then the more serious the degree of stasis.

[Recent pharmacological study on sanshengwan decoction].

OBJECTIVE: To investigate the pharmacological function of Sanshengwan decoction with modern medical and rheological techniques, and also to discuss the differences between injury blood stasis syndrome and yang deficiency blood stasis syndrome. METHOD: Animal model for injury blood stasis syndrome was found by bruise; Animal model for yang deficiency blood stasis syndrome was done by exhausting the animals in a - 15 degrees C freezer for 4 hours. RESULT: Blood fibrinogen content, platelet aggregation and blood coagulation time were significantly increased, plasma viscosity and blood coagulation slope was significantly decreased in injury blood stasis syndrome group. Blood fibrinogen content, platelet aggregation and blood coagulation time were dropped markedly. Blood coagulation slope was raised noticeably in yang deficiency blood stasis syndrome group. Sanshengwan decoction behaved very good effects on removing blood stasis on both of the two kinds of animal models. CONCLUSION: Though hemorheological index and experimental data in the two kinds of animal models were behaved statistically significance, but the experimental representative indexes were obviously difference. Sanshengwan the tipical herbal medicine for eliminating blood stasis syndrome showed significantly effects on both of the two kinds of animal models, but the effects on injury blood stasis syndrome group was better than the other animal model's.

[Relationship between CD8+ T-cell CD28 expression and TCM differentiation type in patients with chronic aplastic anemia].

OBJECTIVE: To explore the relationship between CD8+ T-cell CD28 molecular expression in peripheral blood and TCM type in patients with chronic aplastic anemia (CAA). METHODS: Using flow cytometry to detect the CD28 expression in 45 in-patients or out-patients and 24 healthy subjects for control. And the relation with TCM type was analyzed from the immunological aspect. RESULTS: (1) The levels of CD8, CD28, CD8+ CD28+ expression and CD8+ CD28+/CD8+ CD28- were all higher in the CAA patients than those in the healthy subjects (P < 0.05 or P < 0.01). (2) The levels of CD28, CD8+ CD28+ expression and CD8+ CD28+/CD8+ CD28- were all higher in the CAA patients of Shen-Yin deficiency type than those in the CAA patients of Shen-Yang deficiency type (P < 0.05 or P < 0.01). CONCLUSION: (1) The abnormal high expression of peripheral blood co-stimulatory molecules CD28 suggested CD28 disorder may play an important role in immuno-pathogenesis of CAA. (2) The levels of peripheral CD28, CD8+ CD28+ expression and CD8+ CD28+/CD8+ CD28- can be taken as an objective indexes for TCM typing of CAA, which was disordered more severe in patients of Shen-Yin deficiency type than in those of Shen-Yang deficiency type.

[Effects of an ever-young pill on decreased intelligence and symptoms associated with aging].

A test was carried out to evaluate the effect of Ever-Young Pill (EYP) on decreased fluid intelligence and symptoms associated with aging. Nootropil and placebo were used as control drugs. Random, single-blind and self-control observation methods were applied. 90 days were spent for the clinical study. 154 older persons were divided into three groups (EYP 63, nootropil 60, placebo 31). The method of measuring intelligence was talking between man and computer. The computerized program was worked out by our department. The measure included eight parameters. A semi-quantitative method was used to evaluate the symptoms of aging. The main results were as follows: In the group of EYP, there were significant differences (P less than 0.01) in speed of calculation (SC, 5.92 +/- 2.5, 5.1 +/- 2.1), speed of digits and pictures (SDP, 24.2 +/- 6.2, 21.7 +/- 6.7), two digits span (2.8 +/- 1.0, 3.2 +/- 0.8) and other two parameters before and after treatment. In the group of nootropil, SDP and SC were also improved significantly. In the normal control's there was not any evident change. In the early declined health group treated with EYP and nootropil separately, four parameters were improved in each group. The best effect of EYP obtained was on Kidney Yang deficiency and Kidney and Heart Qi Yang deficiency Syndromes. In other syndromes, the results were less satisfactory. By using EYP for treating symptoms of aging the total effective rate was 96.8%, nootropil 71.7% and placebo 44.2%. There were noticeable differences in three groups. Those results suggested that EYP was one of the effective therapies for decreased fluid intelligence with aging.

[Influence of different types of syndrome on the rising of excellent response rate in hernia repair with acupuncture anesthesia].

UNLABELLED: In order to raise the rate of excellent response in repair of hernia with acupuncture anesthesia, we valued individual difference in the light of basic theory of traditional Chinese medicine. 70 cases were typed by symptoms and signs before hernia operation with acupuncture anesthesia in which 48 cases were yang-deficiency type and 22 cases yin-deficiency type, besides, 30 cases of peridural anesthesia were as the controls. 1. Acupuncture Group: 1. Low-frequency electro-needling (1/sec) was applied to Zusanli (ST36) and Sanyinjiao (SP 6), while high-frequency electro-needling (10/sec) applied to the incisional edge. 2. The electrowaves and intension of stimulation were changed by regular time to maintain the best needling sensation. 3. Small doses of adjuvants were used together with acupuncture, which shows synergism clearly (fentanyl, 2 micrograms/kg, fentanyl/droperidol, 1/50, the dose depending on the condition of patients in operation). 2. The Controls: Routine peridural anesthesia and adjuvants were used. The same observation was given as the acupuncture group. RESULTS: I. the response rate of yang-deficiency type was 97.9%, the excellent response rate 75%, while the response rate of yin-deficiency type was 90.8% and its excellent response rate 45.4%. The response rate of the groups was no significant difference (P > 0.05), but the excellent response rate of the two differed significantly, the excellent response rate of yang-deficiency type was better than that of yin-deficiency type (P < 0.05). Study on the pathophysiologic basis of classification of 'spleen' deficiency in chronic gastritis.

BACKGROUND: Most of the studies on traditional Chinese medicine (TCM) 'spleen' deficiency syndrome in the recent 30 years were conducted only on the basis of single functional index, neglecting the study on the pathophysiologic internal relationship between spleen deficiency syndrome and gastric diseases in modern medicine. But it was at the subcellular molecular biological level that we explored the pathophysiologic basis of classification of spleen deficiency in chronic gastritis by detecting the bioactive substances in gastric mucosa nuclei and mitochondria. METHODS: By means of optical microscope, scanning electron microscope (SEM), transmission electron microscopy (TEM) and histochemical staining, we conducted histopathological, subcellular ultrastructural analysis and nuclei and mitochondrial ultrastructural analysis of gastric mucosa of 188 spleen deficiency patients and of 42 voluntary blood donors. At the same time, bioactive substances were measured by means of X-ray energy dispersive analysis system (EDAX) image analysis system, radioimmunoassay method and chemiluminescence method. RESULTS: The content of cAMP, superoxide dismutase (SOD), Zn and Cu in gastric mucosa, and the content of Zn and Cu in mitochondria decreased progressively in order of groups: healthy control (HC), spleen Qi deficiency without organic lesion (F-SQD), spleen Yang deficiency without organic lesion (F-SyangD), disease without symptoms group, spleen Qi deficiency with organic lesion (G-SQD), spleen Yang deficiency with organic lesion (G-SyangD), spleen Yin deficiency (SyinD) and spleen deficiency with Qi stagnation (SDQS), chronic spleen deficiency gastritis (CSG) and chronic atrophic gastritis (CAG); decreased in order of HC, intestinal metaplasia (IM)Ia, IMIb, IMIIa and IMIIb, P < 0.05. The content of DNA, Zn and Cu in nuclei progressively increased in order mentioned above, P < 0.05. CONCLUSIONS: The quantitative changes of gastric mucosal cAMP, SOD, Zn, Cu, of mitochondrial Zn, Cu and of nuclear DNA, Zn and Cu are not only the substance base on which the lesion of gastric mucosa tissue structure occurs, but also the substance base on which spleen deficiency is classified. G-SQD and G-SyangD were more likely to be found in low-grade or middle-grade CSG and CAG, while SyinD and SDQS in middle-grade or high-grade CSG, CAG and IMIIb.

[Urine osmotic pressure and deficiency-excess syndrome of renal disease: analysis of 428 cases].

By means of TCM differentiation of symptom-complexes, the authors tested and analysed the urine osmotic pressure (UOP) and the urine and plasma osmotic ratio (UPOR) for 428 cases of renal disease, with the conclusion that the UOP and the UPOR were within the normal value range for not only the 36 cases lack of clinical symptoms so as to be unable to have TCM classification identified, but also for 24 cases of Wind edema excess syndrome mainly caused by pathogenic Wind's invasion to the Lung. But for 74 cases of damp-heat Kidney impairment and 294 cases with the main symptom being Kidney deficiency [including weakness of Qi of Kidney, Yang deficiency of Spleen and Kidney, Yin deficiency of Liver and Kidney], the value of their UOP and the UPOR had the tendency of reduction (P less than 0.01), among which the value of the patients of Kidney Yang deficiency reduced most obviously. The further observation showed that, for the nocturia patients caused by renal disease, the value of UOP and the UPOR reduced more obviously than usual. Therefore the authors assert that the test on UOP and UPOR will offer an objective index to patients' nocturia and Kidney-Qi weakness. 60 cases with renal disease of Kidney deficiency syndrome and 27 cases of damp-heat Kidney impairment syndrome under the diagnosis and treatment based on an overall analysis of symptoms and signs leads to the following conclusion: With the elimination of pathogenic factors and recovery of kidney, the damp-heat Kidney impairment patients' UOP will be increased. The low UOP of patients caused simply by Kidney deficiency, however, will recover slower.
