[Relationship between coronary angiography and syndrome differentiation type of TCM].

OBJECTIVE: To understand the relationship between the coronary Angiography (CA) reflected degree of coronary artery lesion and the TCM Syndrome Differentiation-type (TCM-SDT). METHODS: Patients TCM-SDT were differentiated adopting the standard of Syndrome Differentiation for coronary artery diseases revised in 1990, and the comparison of TCM-SDT with the findings in CA was carried out. RESULTS: CA examination showed that 29 patients (39.73%) were of one-artery lesion, 9 patients (12.33%) two-artery lesion and 35 patients (47.95%) three-artery lesion; the stenosis degree of them were: 54 arteries were mild stenosis, 38 moderate stenosis and 31 severe stenosis; 29 arteries were completely occluded, the sum total of affected artery was 152 (69.41%), and the other 67 arteries (30.59%) had no lesion. TCM-SDT showed that all the 73 patients had secondary Excess Syndrome, among them 73 patients (100%) with blood stasis Syndrome, 39 patients (53.4%) with Phlegm-Turbid Syndrome, 16 (21.0%) with Qi-stagnation Syndrome and 7 (9.6%) with Cold condensation Syndrome. 63 patients (86.3%) with obvious primary Deficiency Syndrome, among them, 57 (78.1%) were Qi-Deficiency, 18 (24.7%) Yang-Deficiency, 28 (38.4%) Yin-Deficiency and 5 (6.8%) Yang collapse. All patients had blood stasis of various degrees, manifesting on tongue proper prominently. In the 80.8% patients who had angina pectoris, 74.0% had their mouth, lips and gum dark red, dark purple or light dark in color. The comparison between TCM-SDT and Blood-Stasis score revealed that patients of Cold condensation-blood stasis Syndrome and Yang collapse-blood stasis Syndrome were serious. Patients of Qi stagnation-blood stasis Syndrome and Yin Deficiency-blood stasis Syndrome were mild cases, while patients of Phlegm Turbid-blood stasis Syndrome and Qi Deficiency-blood stasis Syndrome were moderate severity cases. The number of affected artery in Cold condensation patients was the most, and in Yin-Deficiency patients was the least. The coronary artery stenosis degree in patients of Qi-stagnation and Yin-Deficiency were milder than in those of Yang-collapse and Yang-Deficiency patients. CONCLUSION: The more the number of arterial lesion, the severer the degree of stenosis and the higher the blood stasis score, then the more serious the degree of stasis.

[Relationship between apo-protein in renal disease patients with diagnosis and TCM syndrome differentiation].

UNLABELLED: The relationship between the level of apo-protein in 132 renal disease patients with diagnosis and TCM Syndrome Differentiation was analysed. The serum apo-protein level of 132 patients group and 135 healthy adults group were monitored with radioimmunodiffusion assay. The patients were divided into three groups. Spleen Qi and Kidney Qi Deficiency (SQKQD), both Qi and Yin Deficiency (QYD), and the Liver Yin and Kidney Yin Deficiency (LYKYD) group. All patients were divided into with-Dampness and without-Dampness group according to their clinical symptoms and signs. Comparison was also made between different groups. RESULTS: Renal diseases including nephrotic syndrome, chronic nephritis, chronic renal failure and patients with SQKQD, QYD, LYKYD, the levels of apo-B and apo-B/apo-A-I were higher than that of normal (P < 0.01) in various degree, while the level of apo-A-I, was not changed or lower than the normal. In TCM Syndrome differentiation, apo-B and apo-B/apo-A-I in the SQKQD group were higher than those in QYD and LYKYD group (P < 0.05). Moreover, these two values in cases with Dampness were much higher than those without Dampness (P < 0.01). Therefore, it is important to treat this secondary abnormality of lipid metabolism for reducing the further deterioration of renal diseases. When primary renal disease was treated, elimination and removal the dampness might reduce the further damage of hyperlipidemia in renal diseases.

[Effects of di-zhen granules on catecholamine transmitter and gonadotropin of clamacteric rats with deficiency of yin brings about the interior heat-syndrome].

Fifteen-month-old Sprague-Dawley rats were given with some kinds of hot property of herb to produce model of Deficiency of yin brings about the interior heat-syndrome. The weight, water drinking volume, and body temperature of model rats were observed at the process of producing model and the treatment of di-zhen granules. Hypothalamic NE, DA and plasma NE, E concentration were determined by high-performance liquid chromatography, and serum E2, FSH, LH levels were determined by radioimmunoassay. The results were that after having been given the hot property of herbs for 2 weeks, the model rats increased water drinking volume and body temperature, which is similar to the appearance of patients with Deficiency of yin brings about the interior heat syndrome, and after having been treated with di-zhen granules, the water drinking volume and temperature of model rats decreased significantly (P < 0.01). The hypothalamic NE, DA, plasma NE, E, and the serum FSH, LH of the group treated with di-zhen granules were significantly lower than that of the model group and the normal group(P < 0.01). The results show that our model mentioned above possesses some characteristic features of the human climacteric syndrome which exhibits Deficiency of yin brings about the interior heat-syndrome. Di-zhen granules may regulate the function of GnRH neurons and other neurotic nucleus in hypothalamus, and then rectify the endocrine disorders of climacteric.

Expression of bcl-2 oncogene in gastric precancerous lesions and its correlation with syndromes in traditional Chinese medicine.

AIM: To observe the protein and mRNA expression of bcl-2 oncogene in gastric precancerous lesions (GPL) and to analyze its correlation with syndromes in traditional Chinese medicine (TCM). METHODS: Sixty-seven patients with GPL confirmed by gastroscopy and pathology were studied, including 39 cases of moderate gastric mucosal dysplasia, 19 cases of severe gastric mucosa dysplasia, 9 cases of incomplete colon metaplasia. In syndrome differentiation of TCM, 17 cases belonged to the syndrome of qi and yin deficiency of the spleen and stomach complicated by qi stagnation, 21 cases belonged to the syndrome of qi and yin deficiency of the spleen and stomach complicated by stomach heat, 29 cases belonged to the syndrome of qi and yin deficiency of the spleen and stomach complicated by blood stasis. Protein and mRNA expression of bcl-2 oncogene were detected by labeled streptavidin biotin (LSAB) immunohistochemistry and in situ hybridization respectively. RESULTS: Abnormal expression of protein and mRNA on bcl-2 oncogene was found in GPL, which increased gradually with the course of lesions. In moderate and severe gastric mucosal dysplasia and incomplete colon metaplasia, there was no difference in the expression of bcl-2 oncogene (P>0.05). In different accompanying syndromes, the expression of protein and mRNA on bcl-2 oncogene increased gradually in the following order: deficiency of both qi and yin of the spleen and stomach accompanying qi stagnation-->stomach heat-->blood stasis. In GPL, compared with accompanying blood stasis, there was an obvious difference in the expression of bcl-2 oncogene between the syndrome of qi and yin deficiency of the spleen and stomach and accompanying stomach heat, so did accompanying qi stagnation (the level of protein: chi(2) = 8.45, P<0.05; the level of mRNA: chi(2) = 7.35, P<0.05). CONCLUSION: Apoptosis-associated bcl-2 oncogene is abnormally expressed in GPL, which correlates with different accompanying syndromes in TCM.

[Study on relationship between cellular immunity and TCM typing in patients with minimal residual leukemia].

OBJECTIVE: To analyse the correlation between the cellular immunity and TCM Syndrome typing of patients with minimal residual leukemia (MRI). METHODS: The analysis was performed by detecting cellular immunological parameters in 30 MRI patients, 55 healthy persons and 36 patients with acute leukemia (AL) using three fluoresceine-conjugated monoclonal antibodies and flow cytometer. RESULTS: As compared with those in the healthy persons, universal reductions of various parameters were shown in the MRI patients, including WBC count, absolute value of total T-lymphocytes, T-helper lymphocyte (P < 0.05) and total lymphocytes; the percentage and absolute value of NK cells (P < 0.01); and the percentages of total T-lymphocytes, CD4+ CD29+, T-suppressor cells and T-memory cells (P < 0.05 or P < 0.01), but without any rising of absolute value. As compared with those in the patients with AL, parameters were similar in the two groups with insignificant difference. The disturbances, including the lowering on ratio of T-helper/T-suppressor lymphocytes, in MRI patients of Qi-blood insufficiency type was the severest, that in the patients of Qi-Yin deficiency type was the mildest, and that in patients of Yin-deficiency with excess Fire type located between them. CONCLUSION: The immune function of MRI patients is low, belonging to the TCM Syndrome of vital energy deficiency with evil-lingering. Since the degree of cellular immune disturbance is different in various TCM Syndrome types, therefore, they should be treated with different dosages of different drugs.

[Therapeutic changes in nephrotic syndrome treated with yiqi huoxue jiedu decoction and corticosteroid].

UNLABELLED: Plasmic and urinary TXB2 and 6-Keto-PGF1 alpha of 87 patients of nephrotic syndrome (NS) and 25 healthy subjects were measured by RIA, and the influence of Yiqi Huoxue Jiedu decoction (YHJ) and corticosteroid on above-mentioned parameters was investigated. RESULTS: Plasmic and urinary TXB2 as well as the ratio of TXB2/6-K-PGF1 alpha of both types of NS increased (P < 0.01-0.05). The plasmic TXB2 of II type was higher than that of I type of NS (P < 0.01), 6-K-PGF1 alpha increased as well (P < 0.05). After treatment, plasmic and urinary TXB2 of both types of NS were remarkably lower (P < 0.01), while urinary 6-K-PGF1 alpha increased significantly (P < 0.05). In comparing with pre-treatment investigation, the plasmic 6-K-PGF1 alpha of II type was reduced (P < 0.05). It was shown that YHJ would inhibit the synthesis of TXA2 and regulate the TXA2-PGI2 balance. YHJ was markedly effective in producing TXB2 and 6-K-PGF1 alpha increase, its therapeutical effects was better to patients with Yin Deficiency and both Qi-Yin Deficiency, but not so well with Yang Deficiency Syndrome.

[Clinical observation on treatment of 43 women with polycystic ovary syndrome based on syndrome differentiation].

OBJECTIVE: To investigate the therapeutic effect of treatment based on syndrome differentiation and its influence on the serum levels of testosterone and insulin in women with polycystic ovary syndrome (PCOS). METHODS: Forty-three women, diagnosed with PCOS as well as syndrome of kidney yin deficiency or syndrome of spleen qi deficiency, were enrolled in the study. Twenty-five PCOS women with syndrome of kidney yin deficiency were treated with traditional Chinese herbs for nourishing yin to reduce fire (nourishing yin group), and 18 PCOS women with syndrome of spleen qi deficiency were treated with herbs for invigorating spleen and replenishing qi (replenishing qi group). Body mass index (BMI), waist-to-hip ratio (WHR), waist circumference (WC), and the serum levels of insulin and testosterone before and after treatment were detected. RESULTS: Among the 43 cases, 20 cases had high serum testosterone level, in which 13 cases with syndrome of kidney yin deficiency, 7 cases with syndrome of spleen qi deficiency, but the difference had no statistic significance; 17 cases had high serum insulin level, in which 11 cases with syndrome of spleen qi deficiency, 6 cases with syndrome of kidney yin deficiency, the difference had statistic significance (P<0.05). In nourishing yin group, the serum levels of insulin and testosterone declined after treatment (P<0.05), and BMI, WHR and WC showed no significant changes. In replenishing qi group, the serum level of insulin declined after treatment (P<0.01), and BMI, WHR, WC and the serum level of testosterone showed no significant changes. CONCLUSION: Traditional Chinese herbs for nourishing yin to reduce fire can significantly reduce the serum levels of testosterone and insulin in PCOS women with syndrome of kidney yin deficiency, and herbs for invigorating spleen and replenishing qi can significantly reduce the serum level of insulin in PCOS women with syndrome of spleen qi deficiency.

[Observation on left ventricular diastolic function of patients with heart-deficiency syndrome of cardiovascular diseases].

In this paper, left ventricular diastolic function (LVDF) of 99 Heart-Deficiency Syndrome (HDS) patients was observed with pulse Doppler echocardiogram and was compared with normal group. The results showed that different LVDF abnormalities occurred in the HDS patients. When E, Ei DC were decreased and A, Ai, A/E, Ai/Ei, IRT increased, the degree of HDS deteriorated, the pattern was Heart Qi-Yin Deficiency > Heart Yang Deficiency > Heart Qi Deficiency > Heart Yin Deficiency. In conclusion, the above parameters of LVDF could be important indices for evaluation of HDS.

[Study on relationship among thyroid hormone relativity and Syndrome Differentiation-types of TCM in patients with congestive heart failure].

OBJECTIVE: To study the relationship between the TCM Syndrome Differentiation-types of congestive heart failure (CHF) and thyroid hormones, including triiodothyronine (T3), thyroxine (T4) and thyroid stimulating hormone (TSH), and atrial natriuretic peptide (ANP), as well as cardiac function parameters, including left ventricular ejection fraction (LVEF), mean velocity of circumferentid fiber shortening (mVcf) and A peak/E peak (A/E). METHODS: One hundred patients with CHF were divided into 4 Syndrome Differentiation-type groups, their cardiac function parameters, ANP and thyroid hormones were determined and compared with those in the 23 subjects in the control group. RESULTS: In CHF patients with edema and blood stasis Syndrome type, the level of plasma ANP was significantly higher than that in the control group (P < 0.05); level of T3 was significantly lower than that in the control group and in CHF patients of other three (Xin-qi deficiency, Yin-deficiency and blood stasis) Syndrome groups (P < 0.01, P < 0.01, P < 0.05 and P < 0.01); levels of LVEF and mVcf were significantly lower than those in the other three Syndrome groups (all P < 0.01). Level of T4 in other three Syndrome groups significantly increased than that in the edema and blood stasis Syndrome type. A/E value showed a higher level in patients of all TCM type than that in the control (P < 0.01). Correlation analysis showed that T3 was positively correlated with LVEF and T4 (r = 0.200, P < 0.05, and r = 0.293, P < 0.01), and negatively correlated with ANP (r = -0.263, P < 0.01); T4 was negatively correlated with A/E (r = -0.226, P < 0.05). CONCLUSION: The lowering of T3 and T4 and increasing of ANP may be one of the important reasons for lowering of LVEF in CHF patients with edema and blood stasis Syndrome-type. The decrease of T4 may be one of the important reasons for elevation of A/E and aggravation of left ventricular diastolic dysfunction in CHF patients of all the 4 TCM Syndrome-types.

[Pathomorphologic changes on 150 chronic atrophic gastritis patients by treatment based on syndrome differentiation].

Xiang-sha Warming (XSW) and Shashen Nourishing Decoction (SSN) were used in treating Yang Deficiency Syndrome (YaDS) and Yin Deficiency Syndrome (YiDS) of chronic atrophic gastritis (CAG) respectively. 121 cases with Spleen YaDS, and 29 cases with Spleen YiDS were selected. 26 and 30 specimens were taken from gastric mucosa for observation under the SEM and TEM, including 12 and 12 YaDS cases, 14 and 12 Spleen Qi Deficiency cases respectively, 2 cases of YiDS were also observed with TEM. The specimens were taken from the same site of the gastric mucosa directly under the gastroscope pre- and post-treatment. 43 patients (35.6%) with spleen YaDS and 5 cases (17.2%) in Spleen YiDS showed marked effect after 3-month treatment. It showed that the effect in Spleen YaDS was better than YiDS. The XSW was superior than that of SSN. The effects were related to the syndromes and the degrees of pathologic change. This showed that the recipe could somewhat reverse and restore the abnormal glands of gastric mucosa.
