Sustaining Oncology Studies--SOS: information and support for training, research and exploitation in cancer and related biomedical disciplines.

This paper outlines the development of the Sustaining Oncology Studies Information Resources (SOS Europe), a multimedia World Wide Web (WWW) prototype providing support to experimental and clinical cancer researchers, general practitioners, industry personnel, and university students in the field of oncology in Europe and Italy. The system utilizes applications developed for the WWW and is designed in the most easily understandable approaches possible. The prototype now structures oncology-related information available on the Internet and also places resources maintained locally at users' disposal. The system utilizes a WWW browser as a design platform and HTML to build its Home and subpages and to create hyperlinks to internal and external resources.

SELDI ProteinChip array in oncoproteomic research.

Research into the causes, early detection and treatment of cancers is a primary focus of the health care industry and proteomic-based methodologies are providing an increasingly important role in addressing these issues. The ProteinChip Array technology forms the basis of a clinical proteomics platform designed to expedite the discovery, validation, and characterization of cancer biomarkers at all stages of cancer progression. Being able to detect cancer progression early in turn allows for the possibility of more effective treatment. This short review serves to introduce the technology by highlighting specific examples related to cancer biomarker discoveries.

Julius--a software framework for computer-aided-surgery.

In the paper we introduce Julius--an extendable cross-platform software framework for medical visualization and surgical planning. Julius features a modular, cross-platform design using Qt and Vtk libraries and comes with a set of image analysis components, like semi-automatic segmentation, registration, visualization and navigation. We also present a 3D surface generation pipeline used in Julius for generating surfaces from volume data. The pipeline consists of image based filtering, marching cubes surface extraction algorithm, surface decimation and surface smoothing steps. We use this approach within different medical applications like craniofacial surgical planning and will also show the overall software framework within the paper.

Progressive stent technologies: new approaches for the treatment of cardiovascular diseases.

The CYPHER (Cordis, Johnson & Johnson) sirolimus-eluting stent and the TAXU (Boston Scientific) paclitaxel-eluting stent have been extensively evaluated and have been proven to be significant novel tools for the treatment of coronary artery disease. Several sirolimus derivatives have already emerged, receiving CE Mark approval. However, in the future, it is likely that drugs presently under investigation will address additional mechanisms associated with neointimal formation, either as single agents or in combination with antiproliferative compounds. Concurrently, alterations on stent platform design (helicoidal, open-closed cell), coatings (biodegradable, bioabsorbable, nanoporous) and polymers are being explored.

[Methodology for the implementation of e-Health services for chronic patient monitoring and control].

A methodology is presented for a smooth, orderly implementation of specific e-Health services for monitoring chronic patients outside of the hospital setting. Identified as a stage-gate model for the management of the overall implementation process, this methodology is presented formally structured into three steps: a) exploratory examination (pilot project stage); 2) in-depth evaluation (clinical trial stage); and 3) deployment (guided use stage). In the first stage, controlled by the R+D team, the predominant criiteria are the functionality and usability of the technologies involved. In the second stage, controlled by an associated Health Technology Evaluation Agency, the predominant criterion is the scientific aspect related to the results obtained in the clinical testing. The third stage is controlled through decisions made by the health administrations as to the implementation of new technologies and the financing thereof. A description is provided as to the requirements of the technological platform designed to serve its the medium for the projects and tests from stage 1 and 2. As an example of what is done in stage 2, a description is given of a trial related to hypertension.

The effect of sudden inversion stress on EMG activity of the peroneal and tibialis anterior muscles in the chronically unstable ankle.

Residual symptoms resulting from recurrent episodes of inversion-type ankle sprains may be attributed to a decreased neuromuscular response of the peroneal or tibialis anterior muscles, thereby increasing the probability for reinjury. The purpose of this study was to examine the electromyographic (EMG) response time of the peroneal and tibialis anterior muscles in response to sudden plantar flexion/inversion stress in the chronically functional unstable and normal ankle. Subjects for this study consisted of 13 athletically active individuals (five males and eight females, mean age = 19.2 +/- 1.51 years) with a previous history of a unilateral inversion-type ankle sprain. A specially designed platform that allows each foot to drop into plantar flexion/inversion from a standing neutral position was used. Reaction time in milliseconds for the peroneal and tibialis anterior muscles to sudden plantar flexion/inversion was measured via surface EMG. A paired t test was performed with the Bonferroni-Dunn correction factor to determine differences between the peroneal and tibialis anterior as well as between the chronically unstable and contralateral normal ankle. The results indicated no significant differences between the stable and unstable ankles for the peroneal or the tibialis anterior muscles. The results also indicated no significant differences existed between the tibialis anterior and peroneal muscles in either the stable or unstable ankles. The findings from the present study suggest that self-reported functional ankle instability may not result in a diminished reflex response time of the peroneal and tibialis anterior muscles to sudden plantar flexion/inversion stress.

Hypoimmune platforms: from rejection to immune evasion and regulatory implications.

The growing gap between organ demand and clinical availability has renewed interest in immune-evasive graft strategies, yet rejection and lifelong immunosuppression remain major barriers to durable success. Advances in genome editing enable immune-evasive cell platforms designed to avoid immune recognition while replacing missing function in allogeneic settings. This review summarizes current strategies for engineering immune-evasive grafts that simultaneously suppress adaptive and innate immune responses. We discuss how coordinated modulation of antigen presentation and immune checkpoint pathways can protect transplanted allogeneic cells and tissues from T, NK, and macrophage-mediated rejection. We also present the emerging concept of integrating hypoimmune engineering with genetically modified porcine donors, where extensive genome editing has reduced, but not eliminated, xenogeneic immune barriers. Combining donor genome modification with immune-evasive graft design represents a promising conceptual advance toward xenograft survival, though whether full elimination of systemic immunosuppression is achievable remains to be established clinically. We further examine how the regulatory landscape for these products is evolving across major jurisdictions, and how differences in approval pathways, manufacturing standards, and long-term surveillance requirements shape the path to clinical translation. Finally, we outline the safety considerations and remaining limitations in immune evasion that must be addressed to enable clinical implementation.

Integration of data and methods for genome analysis.

The development of genomic and post-genomic technologies has created an explosion in the quantity, diversity and availability of both biological data and methods of analysis. Biologists are currently facing the problem of using all these resources to convert raw data into new valuable knowledge. This review presents software platforms designed to handle data and/or methods in the context of genome analysis.

The evaluation of Game-Based e-learning for medical education: a preliminary survey.

Game-Based e-learning (GBeL) was newly designed platform for education of students with higher education, especially for medical students. The purpose of this study was to preliminarily evaluate the attraction of GBeL, the motivation toward GBeL and the learning effect. We found more than 80% of students thought that GBeL was attractive and more interesting than traditional class. However, the percentage of enhancing interest to learn and learning more complete and deep knowledge were less than 50%.

Importance of a balanced-gap technique in rotating-platform knees.

Rotating-platform knees are constructed of highly conforming polyethylene to reduce contact pressures that potentially cause wear. The mobile nature of the rotating platform can lead to bearing spin-out. The purpose of this study was to determine the prevalence of spin-out in a single series. A total of 426 knees in 393 patients were included in the study. All knees were implanted using a balanced-gap technique. The average follow up was 3.2 years. There were no cases of bearing spin-out. The author concludes that the balanced-gap technique reliably prevents spin-out in mobile-bearing total knee implantation. This implantation technique provides the potential wear benefits of the rotating platform design with a nominal bearing spin-out rate.

Detection of viruses: atomic force microscopy and surface enhanced Raman spectroscopy.

This paper demonstrates the capability of atomic force microscopy (AFM) and surface enhanced Raman spectroscopy (SERS) to function effectively as ultra-sensitive readout tools for chip-scale platforms designed for pathogen detection in complex biological media. AFM allows direct (i.e., label-free) visualization and quantification of nanometer-sized viruses captured on a smooth, selective surface. AFM readout led to optimization of a capture substrate for feline calicivirus (FCV), and yielded a limit of detection of 3 x 10(6) FCV/mL. SERS-based detection of FCV, carried out in a sandwich-type assay, requires labelling of the substrate-bound FCV with a selective extrinsic Raman label (ERL). These studies yielded a limit of detection of 1 x 10(6) FCV/mL. The prospects of these two readout methods as additions to the arsenal of tools in bioterrorism prevention are briefly discussed.

Flight simulator platform motion and air transport pilot training.

The influence of flight simulator platform motion on pilot training and performance was examined in two studies utilizing a B-727-200 aircraft simulator. The simulator, located at Ames Research Center, is certified by the FAA for upgrade and transition training in air carrier operations. Subjective ratings and objective performance of experienced B-727 pilots did not reveal any reliable effects of wide variations in platform motion design. Motion platform variations did, however, affect the acquisition of control skill by pilots with no prior heavy aircraft flying experience. The effect was limited to pitch attitude control inputs during the early phase of landing training. Implications for the definition of platform motion requirements in air transport pilot training are discussed.

[The effect of visual feedback exercises on balance in normal subjects].

The effects of visual feedback exercises on balance was studied in normal individuals, to evaluate the efficacy of visual feedback of postural oscillation in improving stability. A DINAS-CAN dynamometric platform designed by the Valencian Institute of Biomechanics (IBv) was used. Before to commencing training sessions, we determined the following parameters in each individual: (a) static stability, via posturography; and (b) individual ability to displace and voluntarily maintain the center of gravity within the limits of stability. All static measures were poorer after training, but the differences were not significant (p > 0.01). In contrast, the exercises performed via visual feedback of postural control improved significantly (p < 0.01). Training by visual feedback facilitates integration of visual, somatosensory, and vestibular information. In normal individuals, stability improves under excentric conditions, but no improvement is seen in the central resting position.

Cross-platform hypermedia examinations on the Web.

The authors developed a multiple-choice medical testing system delivered using the World Wide Web. It evolved from an older, single-platform, locally-developed computer-based examination. The old system offered a number of advantages over traditional paper-based examinations, such as digital graphics and quicker, easier scoring. The new system builds on these advantages with its true cross-platform design and the addition of hypertext learning responses. The benefits of this system will increase as more medical educational resources migrate to the Web. Faculty and student feedback has been positive. The authors encourage other institutions to experiment with Web-based teaching materials, including examinations.

ECG boy: low-cost medical instrumentation using mass-produced, hand-held entertainment computers. A preliminary report.

A prototype low-cost, portable ECG monitor, the "ECG Boy," is described. A mass produced hand-held video game platform is the basis for a complete three-lead, driven right-leg electrocardiogram (ECG). The ECG circuitry is planned to fit in a standard modular cartridge that is inserted in a production Nintendo "Gameboy." The combination is slightly smaller than a paperback book and weighs less than 500 g. The unit contains essential safety features such as optical isolation and is powered by 9-V and AA batteries. Functionally, the ECG Boy permits viewing ECG recordings in real time on the integrated screen. The user can select both the lead displayed on the screen and the time scale used. A 1-mV reference allows for calibration. Other ECG enhancements such as data transmission via telephone can be easily and inexpensively added to this system. The ECG Boy is intended as a proof of concept for a new class of low-cost biomedical instruments. Rising health care costs coupled with tightened funding have created an acute demand for low-cost medical equipment that satisfies safety and quality standards. A mass-produced microprocessor-based platform designed for the entertainment market can keep costs low while providing a functional basis for a biomedical instrument.

REALISIS: a medicinal chemistry-oriented reagent selection, library design, and profiling platform.

REALISIS is a software system for reagent selection, library design, and profiling, developed to fit the workflow of bench chemists and medicinal chemists. Designed to be portable, the software offers a comprehensive graphical user interface and rapid, integrated functionalities required for reagent retrieval and filtering, product enumeration, and library profiling. REALISIS is component-based, consisting of four main modules: reagent searching; reagent filtering; library enumeration; and library profiling. Each module allows the chemist to access specific functionalities and diverse filtering and profiling mechanisms. By implementing the entire process of reagent selection, library design, and profiling and by integrating all the necessary functionalities for this process, REALISIS cuts the time required to design combinatorial and noncombinatorial libraries from several days to a few hours.

Pediatric ocular photography platform.

We designed a rigid support platform to facilitate ocular photography of infants or small animals. The platform is versatile because it is compatible with either the Zeiss fundus camera or Zeiss Photo Slit Lamp.

PerlPrimer: cross-platform, graphical primer design for standard, bisulphite and real-time PCR.

UNLABELLED: PerlPrimer is a cross-platform graphical user interface application for the design of primers for standard, bisulphite and real-time PCR, and sequencing. The program incorporates highly accurate melting-temperature and primer-dimer prediction algorithms with powerful tools such as sequence retrieval from Ensembl and the ability to BLAST search primer pairs. It aims to automate and simplify the process of primer design. AVAILABILITY: Open-source and freely available from http://perlprimer.sourceforge.net.
