In vitro cytotoxicity of a composite resin and compomer.

AIM: This work was designed to investigate the potential cytotoxicity of two of the newer dental restorative materials. Spectrum composite resin and Dyract AP compomer. METHODOLOGY: Cultured human endothelial cells (ECV-304) were exposed to each of the restorative materials through a 70-microm dentine barrier to simulate the in vivo clinical situation. Cell viability was measured by the MTT (3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyltetrazolium bromide) assay and lactate dehydrogenase release assay. The effects of different extents of light-curing were also examined by microscopic examination of stained human promyelocytic leukemia cells (HL-60). Caspase-3 activation was determined as a measure of apoptotic cell death. RESULTS: Assessment of cellular viability indicated that both materials cause cell death, with Spectrum being the more toxic. The cytotoxicity was considerably increased in the absence of the dentine barrier. Direct exposure to Spectrum for 12 h resulted in the death of 69% of the cells after full light-curing (78% of total death was by apoptosis) and 96% after partial light-curing (73% of total death was by necrosis). Assessment of caspase activation, in the absence of the dentine barrier, showed that longer curing-times resulted in an increase in the proportion of the cells dying through apoptosis, rather than necrosis, for both materials tested. CONCLUSIONS: These results indicate the restorative materials to be potentially toxic, particularly if the degree of light-cure is inadequate.

A biomechanical evaluation of anterior and posterior tibialis tendons as suitable single-loop anterior cruciate ligament grafts.

PURPOSE: Because allograft tendons used to replace a torn anterior cruciate ligament are in short supply, it is useful to explore other possible graft sources. The purpose of this study was to determine whether a graft formed from a loop of either an anterior or posterior tibialis tendon has structural, material, and viscoelastic properties similar to those of a double-looped semitendinosus and gracilis (DLSTG) graft. TYPE OF STUDY: Completely randomized design. METHODS: Four structural and 3 material properties were determined for each type of graft (n = 10) by measuring the cross-sectional area, looping the tendon(s) over a post, gripping the free ends of the tendon(s) with a freeze clamp, and pulling the graft to failure by using a materials testing system. Two viscoelastic properties were determined for each type of graft (n = 10) by measuring the decrease in load under a constant displacement (i.e., stress relaxation test) and the increase in displacement under a constant load (i.e., creep test). RESULTS: For grafts 95 mm in length, the ultimate load and ultimate displacement of a loop of anterior (4,122 N, 12.0 mm) and posterior tibialis (3,594 N, 12.5 mm) tendon were either similar to or significantly greater than those of the DLSTG graft (2,913 N, 8.4 mm) (P =.204 for the posterior tibialis ultimate load and P < or =.007 for the remaining quantities). The stiffness and cross-sectional area of the anterior (460 N/mm, 48.2 mm2) and posterior tibialis (379 N/mm, 41.9 mm2) grafts were similar to those of the DLSTG graft (418 N/mm, 44.4 mm2) (P > or =.283). The tensile modulus, stress at ultimate load, and strain at ultimate load of the anterior tibialis and posterior tibialis grafts were either similar to or significantly greater than those of the DLSTG graft. The decrease in load of the anterior tibialis and posterior tibialis grafts was either greater than or similar to that of the DLSTG graft for the relaxation test (P < or =.066). The increase in displacement of the anterior tibialis (0.3 mm) and posterior tibialis (0.4 mm) grafts was minimally but significantly greater than that of the DLSTG graft (0.2 mm) for the creep test (P < or =.004). CONCLUSIONS: The structural, material, and viscoelastic properties of a single loop of anterior tibialis and posterior tibialis tendon are either better than or similar to those of a DLSTG graft. Consequently, single-loop grafts formed from tibialis tendons should function well as a replacement for a torn anterior cruciate ligament.

Comparative study of the configuration and the cleanliness level of root canals prepared with the aid of sodium hypochlorite and bis-dequalinium-acetate solutions.

Bis-dequalinium acetate (BDA), a quaternary ammonium compound, was introduced as a bactericidal and chemotherapeutic agent for endodontic use. This study was undertaken to compare the efficacy of this compound to 5.25% sodium hypochlorite in the cleansing and shaping of root canals. One hundred twenty freshly extracted teeth were divided into three groups. Each group was treated with the aid of the test material; saline solution was used as a control. After completion of the endodontic preparation, a silicone imprint of the canals was performed and the teeth were decalcified. The imprints were examined three-dimensionally to determine the amount of debris and canal configuration due to defective preparation and anatomic variation. No significant difference could be observed between the tested materials with respect to cleanliness and the shape of the canals. At the apical third, however, the BDA-treated canals showed more favorable results with respect to cleanliness. On the basis of the present and previous findings, it was concluded that BDA might be a favorable replacement for sodium hypochlorite as an irrigation solution.

The effectiveness of four-cavity treatment systems in sealing amalgam restorations.

Amalgam does not bond to tooth tissue; therefore, restorations using such material are prone to leakage despite the deposition of corrosion products. This study evaluated the effectiveness of four cavity treatment systems placed in vivo in sealing restorations of amalgam. Four cavity treatment systems were investigated in this study: Cervitec, Gluma One Bond, Panavia 21 and Copaliner Dentin Varnish and Sealant. No cavity treatment was placed in an additional group to serve as a control. The teeth were extracted within 15 minutes of restoration placement. The specimens were thermocycled (5-55 +/- 2 degrees C, 500 cycles), immersed in a dye solution, sectioned and scored for leakage. Scanning electron microscopy also examined features of the tooth/restoration interfaces. There were statistically significant differences among the groups regarding leakage scores (p = 0.00). None of the materials tested consistently prevented leakage; however, use of Copaliner Dentin Varnish and Sealant resulted in less overall, occlusal and cervical microleakage than any other systems tested. Significantly more leakage was observed in relation to the cervical portions of the cavities (p = 0.00). No significant differences were identified between the leakage scores obtained for the buccal and palatal (lingual) cavities and the different tooth types (p = 0.52 and 0.83, respectively). A level of significance of 0.05 was selected in all cases. The benefits of the materials tested in this study need to be evaluated using robust, long-term clinical studies. Further work should continue to develop laboratory tests that predict the behavior and performance of cavity sealants in clinical service.

Determination of lead in wine by graphite furnace atomic absorption spectrophotometry: interlaboratory study.

An interlaboratory study of a graphite furnace atomic absorption spectrophotometry (GFAAS) method for the determination of lead in wine was conducted. Seventeen laboratories from France, United States, and the United Kingdom, using a variety of GFAAS instruments, took part in the study. The method incorporated a novel matrix-matching procedure to minimize matrix effects between standards and samples. Six wine test materials were prepared and sent to participants as 12 blind duplicate or split level samples. There was good agreement between results obtained from participants and target values (24-279 micrograms/L) obtained with an inductively coupled plasma-mass spectrometry method. The precision of the GFAAS method was well within the range predicted by the Horwitz equation for the 6 test materials analyzed. Repeatability standard deviations ranged from 3 to 17%. Reproducibility standard deviations were in the range of 10 to 30%. The method is recommended for use for official purposes.

Effects of minimum stimulation settings for the Med El Tempo+ speech processor on speech understanding.

OBJECTIVE: The aim of this study was to assess the effects of variations in the settings for minimum stimulation levels on speech understanding for adult cochlear implant recipients using the Med El Tempo+ speech processor. DESIGN: Fifteen patients served as listeners. The test material included sentences presented at a conversational level in noise (74 dB SPL at +10 dB signal-to-noise ratio), sentences presented at a soft level in a quiet background (54 dB SPL), consonants in "vCv" environment (74 dB SPL re: vowel peaks), and synthetic vowels in "bVt" environment (54 dB SPL re: vowel peaks). The patients' speech processors were programmed with minimum stimulation levels set to behavioral threshold, set to 10% of most comfortable loudness, and set to 0 muA. RESULTS: The level of speech understanding achieved in the behavioral threshold condition was not significantly different from that achieved in either the 10% of most comfortable loudness or 0 muA conditions for any test material. Only 2 of the 15 patients demonstrated performance differences of greater than 10 percentage points between the 0 muA condition and the behavioral threshold condition on more than a single test. CONCLUSIONS: Our results demonstrate that there are no grievous consequences, in terms of speech understanding, for setting minimum stimulation levels below behavioral thresholds. The time savings from setting thresholds to 10% of MCL or 0 muA may be especially useful during the initial device fitting.

A simple one-step hemolytic assay for C2 with C2-deficient human serum.

A simple one-step procedure has been developed for the molecular titration of C2 by utilizing the ability of the test material to restore the hemolytic activity of human serum selectively deficient in C2 (C2D serum). In this assay, equal volumes of EA (10(8) cells/ml), C2D serum (1/20), and a suitable dilution of a source of C2 were incubated at 37 degrees C for 60 min and the fraction of cells lysed was used to calculate the effective molecules of C2/ml test material. The assay can be used to titrate C2 in human, guinea pig, rat, mouse and rabbit sera, but not C2 in dog serum. The assay is simple and reproducible, and comparable in sensitivity to the conventional two-step assay with EAC14 cells and Cgp-EDTA.

Mammalian toxicity of 1,3-dichloropropene.

DCP has been utilized as a soil fumigant for more than 45 yr for the control for parasitic plant nematodes. Injected into soil before planting of crops, the instability of DCP in soil and water and its volatility dictate the principal route of human exposure that may occur, inhalation. Extensive data have been accumulated on the toxicity and metabolism of DCP. DCP is moderately toxic via oral or inhalation exposure, is irritating to the skin and eyes, and has potential to produce skin sensitization. It is rapidly and extensively metabolized. It has a half-life in the blood of rats and humans of only 3-7 min and < 10 min, respectively. Rats and mice excrete approximately 80% of even relatively high oral dosages within 24 hr, primarily as breakdown products of a glutathione conjugate or as carbon dioxide. These products reflect the primary routes of metabolism of DCP, via GSH-conjugative and hydrolytic pathways. An additional pathway based upon the epoxidation of DCP has also been proposed, but this does not appear to occur to any toxicologically significant degree in the presence of normally occurring GSTs. Direct evidence of the latter pathway is only been obtained at dosages of DCP in excess of the reported LD50. Humans also appear to rapidly metabolize DCP and excrete its metabolites. Subchronic toxicity studies of relatively pure DCP in rats and mice via oral or inhalation routes have resulted in portal-of-entry tissue effects that reflect the irritant properties of this chemical to nasal and gastric mucosa. At higher exposure levels in mice, however, toxicity was also identified in a remote tissue, the urinary bladder. Toxicity in dogs ingesting DCP was limited to the formation of a regenerative hypochromic, microcytic anemia. No teratological or reproductive effects were observed in rats or rabbits inhaling DCP vapors. Nonneoplastic changes from chronic dosing of DCP were generally similar to those observed in subchronic studies. Somewhat variable responses, however, have been observed for neoplastic effects, depending on the DCP formulation, route, and species used. Inhalation of a recent formulation increased the benign tumor incidence in the lungs of male mice (only) while ingestion of similar test material by rats and mice resulted in a low incidence of benign liver tumors in rats (only). In contrast, an older formulation containing Epi as a stabilizing agent administered to rats and mice via bolus oral dosing induced a number of malignant or benign tumors: in the forestomach and liver in rats and the forestomach, lung, and urinary bladder in mice. An equally complicated database has accumulated for DCP in vitro and in vivo genotoxicity testing. Genotoxicity has been reported in in vitro assays; however, confounding factors such as low-purity formulations, use of a genotoxic stabilizer, or generation of reactive impurities during attempts to purify test material have complicated interpretation. DCP appears to lack direct DNA reactivity, and a general trend toward decreasing activity with increasing complexity of the assay system and the presence of GST is evident. The weight-of-evidence evaluation of the genotoxicity data base suggests a lack of genotoxicity in vivo. Clearly definable treatment-related effects of DCP suggesting a plausible nongenotoxic mechanism of tumorigenic action, for example, enhanced cell proliferation, have not been in evidence in target tissues of treated animals. Thus, the specific mode of tumorigenesis of DCP in test animals remains to be elucidated but appears to involve a non-DNA-reactive mechanism. In conclusion, DCP-based soil fumigants have maintained an important role in agricultural despite the structural similarity of DCP to known genotoxic carcinogens and its own activity in in vitro genotoxicity assays. This role results from a combination of its use on soils before the planting of crops, its limited environmental half-life, rapid metabolism by animals via GSH conjugation and catabolism to CO2, lack of genotoxicity in in vivo assays, and an extensive toxicological database in animals, including several oncogenicity bioassays. These data, when combined with occupational and environmental exposure information, have provided a scientifically sound basis for the continued safe use of DCP-containing products.

[Collaborative tests of steroid hormone determinations: accuracy and precision of analytic results].

In the years 1977 to 1981, 14 quality-control surveys for the determination of steroid hormones were performed in cooperation with the Deutsche Gesellschaft für Klinische Chemie. Hereby the laboratories participating could in each case analyze the following steroids: aldosterone, cortisol, oestradiol-17 beta, oestriol, progesterone, and testosterone. In the light of the results an investigation was made as to whether, in the course of time, an improvement in the accuracy or in the precision of the determinations had been attained, and to what extent the determinations depend on the qualities of the test material. A clear improvement in the accuracy of the results of the analyses could only be ascertained for oestradiol-17 beta. For aldosterone and cortisol, values were found in pool-plasma whose medians were significantly above the definitive values. An improvement in precision could be noted especially with oestradiol-17 beta and to lesser degrees with cortisol and oestriol. The kind of test material--plasma which contained only the hormones to be analyzed on the one hand, and, on the other hand, pool-plasma, which also contained all endogenous hormones--had no influence on the precision of the results from various laboratories. Low concentrations of the individual steroids led--on the basis of the methodological principle of radioimmunoassays--in almost all cases to a reduced interlaboratory precision in regard to values. The accuracy of the analysis values was considerably impaired only with aldosterone and oestradiol-17 beta by low concentrations: the medians here were in part twice as high as the definitive values.

Antimicrobial activity of amalgams, alloys and their elements and phases.

OBJECTIVES: This in vitro study aimed to evaluate the antibacterial effect of amalgams, alloys, elements and phases against two cariogenic bacteria, Actinomyces viscosus and Streptococcus mutans. METHODS: Test materials comprised: (i) commercial amalgams comprising Amalcap (Vivadent), Cavex Avalloy LC and DP (Cavex), Cupromuc (Merz), Fluoralloy and Synalloy (Dentoria); (ii) Ag-Cu alloy; (iii) gamma, gamma 1, gamma 2 and Cu6Sn5 phases; (iv) pure metal samples and chloride solutions of copper, mercury, tin and zinc; and (v) aqueous sodium fluoride. Bacterial suspensions of each of the two bacteria were grown in the presence of the test materials for 24 h. Antimicrobial effectiveness was assessed by measuring reduction in optical density at 640 nm using a visible spectrophotometer. RESULTS: Cupromuc/Fluoralloy, non gamma 2 amalgams and Amalcap displayed high, moderate and no antibacterial activity, respectively. Antibacterial effectiveness was not related to copper content. Whereas mercury, copper, Ag-Cu alloy, fluoride and zinc showed antibacterial activity (Hg > Cu > F > Zn), tin, gamma phases and Cu6Sn5 showed no such activity. SIGNIFICANCE: Although the fluoride and copper solutions were most effective at 50 micrograms ml-1 concentration, their antibacterial action was still significant, albeit reduced, at 10 micrograms ml-1 concentration. This was not the case for mercury chloride which was just as effective at both concentrations. Our results show that although mercury and copper contribute significantly to the antibacterial properties of amalgams, a high copper content does not necessarily relate to high antibacterial effectiveness. These elements could be useful in conferring antibacterial properties to amalgam although their effects on host cells must be investigated.

A radial hemolysis method in agarose for the functional assay of properdin.

A simple one-step radial hemolytic assay for properdin has been devised. In this assay, the test material is introduced into a well in an agarose plate containing optimal concentrations of normal human serum immunochemically depleted of properdin (RP), EGTA, magnesium ions and unsensitized guinea pig erythrocytes. Following radial diffusion, the area of the hemolytic zones resulting from the activation of the alternative complement pathway and bystander lysis of guinea pig erythrocytes was directly proportional to the concentration of properdin in the test material. The assay is specific reproducible and sensitive and the correlation with the radioimmunoassay for properdin is very good. The assay has been used to measure properdin activity in animal sera.

Toxicity of a mancozeb containing fungicide formulation and CU-sulphate to chicken embryos after administration as single compounds or in combination.

Environmental pollution of metal modelled by copper-sulphate and a 80% mancozeb containing fungicide formulation (Dithane M-45) were studied on chicken embryos after administration as a single compounds or in combination. The test materials were injected into the air-chamber in a volume of 0.1 ml/egg on day 12 of incubation. The concentration of copper-sulphate was 0.01%. The applied concentration of Dithane M-45 fungicide formulation was 0.2%. Evaluation was done on day 19 of the hatching period. The combined administration of copper-sulphate and the fungicide formulation did not cause a significant reduction in body weight as compared to the control data and the results from individual toxicity study of the test materials. After the combined administration of copper-sulphate and the fungicide formulation the rate of embryomortality was 40%. The incidence of developmental anomalies were sporadic. Light microscopic findings exhibited a degenerative change in the liver tissue of combined administration group. Activities of GPT and GOT enzymes increased markedly in the combined administration group. In summary, it can be established that the interaction of copper-sulphate and an 80% mancozeb containing fungicide formulation (Dithane M-45) caused higher embryomortality with respect to the test of individual toxicity of copper-sulphate and fungicide in our study.

International proficiency testing of analytical laboratories for foods and feeds from 1990 to 1996: the experiences of the United Kingdom Food Analysis Performance Assessment Scheme.

The Food Analysis Performance Assessment Scheme (FAPAS) organized by a Secretariat of the UK Ministry of Agriculture, Fisheries, and Food has checked the proficiency of analytical laboratories for foods and feeds from 1990 to 1996. FAPAS was started for UK laboratories but was expanded worldwide at the request of analysts in other countries who did not have a home-based scheme. Thirteen thousand homogeneity-checked test materials were issued, covering a very wide range of analytes, including pesticides, toxins, veterinary drug residues, trace and nutritional elements, food colors, preservatives, sweeteners, alcohol congeners, fatty acids, nitrate, and proximate analysis. Participants returned 85% of requested data, and 47,000 z-score proficiency assessments were made, of which 81% were satisfactory. Evidence is presented of improvements in overall analytical ability with increased participation in proficiency testing in the areas of proximate analysis; organochlorine pesticide analysis; and lead, mercury, and acesulfame-K analyses. Little improvement was shown in other analytical areas such as calcium analysis. Overall accuracies for analysis of specific pesticides and specific trace elements in the circulated test materials were compared.

Bioassay of captan for possible carcinogenicity.

A bioassay of technical-grade captan for possible carcinogenicity was conducted by administering the test material in feed to Osborne-Mendel rats and B6C3F1 mice. Groups of 50 rats of each sex were administered one of two doses of captan for 80 weeks, then observed for 33 or 34 weeks. The time-weighted average doses for both sexes of rats were 2,525 or 6,050 ppm. Matched controls consisted of groups of 10 untreated rats of each sex; pooled controls, used for statistical evaluation, consisted of the matched controls combined with 75 untreated male and 75 untreated female rats from similar bioassays of six other test chemicals. All surviving rats were killed at 113-114 weeks. Groups of 50 mice of each sex were administered the test material at one of two doses, either 8,000 or 16,000 ppm, for 80 weeks, then observed for 11 weeks. Matched controls consisted of groups of 10 untreated mice of each sex; pooled controls, used for statistical evaluation, consisted of the matched controls combined with 80 untreated male and 80 untreated female mice from similar bioassays of six other test chemicals. All surviving mice were killed at 90-91 weeks. The mean body weights of both low- and high-dose rats and high-dose mice were lower than those of the matched controls throughout most of the study. Mortality rates did not show statistically significant dose-related trends in either sex of either species. In rats, a positive dose-related trend and a difference between incidences of tumors in high-dose and pooled-control groups were found in females when the data for adrenal cortical adenoma were combined with those for adrenal cortical carcinoma (pooled controls, 0/64, low-dose 2/50, high-dose 3/47, P=0.047). There was also a positive dose-related trend for the incidence of C-cell adenoma of the thyroid in female rats (pooled controls 1/66, low-dose 1/49, high-dose 4/44, P=0.035). These endocrine tumors in female rats are believed to have been spontaneous, and not related to treatment. In mice, the incidences of polypoid carcinoma (adenocarcinoma in adenomatous polyp) of the duodenum were statistically significant using tests for a positive dose-related trend both in male mice (pooled controls 0/68, low-dose 1/43, high-dose 3/46, P=0.033) and in female mice (pooled controls 0/68, low-dose 0/49, high-dose 3/48, P=0.022). When the incidences of adenomatous polyp, NOS (not otherwise specified), were combined with those of polypoid carcinoma for statistical analysis, the tests for male mice indicated a substantial increase in significance (pooled controls 0/68, low-dose 3/43, high-dose 5/46, P=0.008). It is concluded that under the conditions of this bioassay, tumors in the duodenum of B6C3F1 mice were associated with treatment with captan, but there was no convincing evidence that the tumors observed in Osborne-Mendel rats were related to treatment.

The influence of automated percutaneous lumbar discectomy (APLD) on the biomechanics of the lumbar intervertebral disc. An experimental study.

Twenty human lumbar motion segments were prepared and tested in an electromechanical materials testing machine in order to investigate the biomechanical changes, i.e. intradiscal pressure, radial extension and height of the intervertebral disc, after automated percutaneous lumbar discectomy (APLD) developed by Onik. The biomechanical data were statistically analyzed with the Friedman test (significance level p < 0.05). The APLD lasted 45 minutes in every segment. The mean weight of material removed was 4.6 g. The removal of 4.6 gram of nucleus pulposus material reduced the height of the disc by an average of 1.42 mm. The intradiscal pressure also decreased by an average of 5.7 bar. The radial bulge increased by an average of 0.45 mm. Our results show that the mechanism improving radicular pain in patients with herniated disc after treatment with percutaneous nucleotomy is still in question. While clinical studies show an improvement of 70% to 85% of patients treated with APLD for herniated disc, this in vitro study showed clearly that radial bulge increases after removal of nuclear material. We postulate that loss of height of the disc and, as a consequence, reduction of tension in the affected nerve root, plays a major role with regard to this improvement.

Biofilm formation by Staphylococcus epidermidis on intraocular lens material.

PURPOSE: To compare biofilm formation by Staphylococcus epidermidis on different intraocular lens (IOL) materials. METHODS: The S. epidermidis strains, ATCC 12228 (American Type Culture Collection) and ATCC 35984 (biofilm-producer) were used. Biofilms were cultivated on disks of different IOL materials: silicone, PMMA (polymethylmethacrylate), acrylic, or MPC (2-methacryloyloxyethyl phosphorylcholine) surface-modified acrylic. Biofilms were stained with crystal violet (CV) which served as an index of biofilm formation. The bacterial population was enumerated after biofilm homogenization. Biofilms were also examined by scanning electron microscopy (SEM). RESULTS: Among the four materials tested, the least amount of biofilm formed on silicone. Biofilm production was significantly different between acrylic and MPC surface-modified acrylic lenses at 48 hours (P<0.05-0.01). The bacterial populations were significantly different between acrylic and silicone over 72 hours (P<0.05-0.01). The population on acrylic and MPC surface-modified acrylic continued to increase over 72 hours. The biofilm was recognized after a 24-hour incubation. Rates of biofilm-positive SEM fields, which were defined as being occupied by biofilm over at least half of the area, were significantly different between acrylic and silicone at 72 hours (P<0.05-0.01). CONCLUSIONS: S. epidermidis formed biofilm most intensely on acrylic among the four IOL materials tested. MPC surface-modified acrylic has a preventive effect on biofilm formation.

Changes in speech intelligibility as a function of time and signal processing strategy for an Ineraid patient fitted with continuous interleaved sampling (CIS) processors.

OBJECTIVE: To assess changes in speech intelligibility as a function of signal processing strategy and as a function of time for one of the first two Ineraid patients in the United States fitted with a continuous interleaved sampling (CIS) signal processor. DESIGN: In Experiment 1, the patient was fitted with a CIS processor and measures of speech intelligibility were taken over a period of 4 mo. These data were compared with data collected with the Ineraid. In Experiment 2, three new signal processing strategies were tested. Measures of speech intelligibility were taken at fitting and after a week's use of the processor. In Experiment 3, the number of channels in the processor was reduced to 5, 4, and 3. Each processor was tested at fitting and after a week's use of the processor. RESULTS: In Experiment 1, immediately on fitting, the CIS processor produced better speech intelligibility for consonants, vowels, and the CID sentences than did the Ineraid. Performance improved over periods ranging from 1 to 4 mo depending on the test material. In Experiment 2, two processors produced significantly better speech intelligibility than did other processors. Most generally, performance dropped slightly when a new processor was fitted and then improved over the course of week. All of the processors produced better speech intelligibility than did the Ineraid. In Experiment 3, five channels allowed similar levels of performance as did six channels. The effect of four and three channels varied as a function of test material. Four CIS channels allowed better performance than did the four analogue channels of the Ineraid. CONCLUSIONS: We conclude 1) that CIS processors can provide much better speech intelligibility than can the analogue processor of the Ineraid; 2) that many CIS strategies, not just one, will produce better speech intelligibility than will the Ineraid; 3) that for this patient, five channels can allow as high a level of word intelligibility as can six channels; 4) that when the number of CIS and analogue channels are equated (at four), the CIS strategy provides better speech intelligibility than does the Ineraid; and 5) that speech intelligibility with CIS processors improves over periods as short as a week and as long as several months after fitting of the processor.

Dye leakage of four root end filling materials: effects of blood contamination.

The purpose of this study was to compare the amount of dye leakage (in the presence versus absence of blood) in root end cavities filled with amalgam, Super EBA, IRM, and a mineral trioxide aggregate. After removing the anatomical crowns of 90 extracted human teeth, their roots were instrumented and obturated. Except for their apical 2 mm, the root surfaces were sealed with nail polish. After removal of the apical 2 to 3 mm of each root, a standardized root end cavity was prepared. Five root ends were filled with gutta-percha and no sealer, and another five root ends were filled with sticky wax. These served as positive and negative controls, respectively. The remaining 80 roots were divided into four equal groups and filled with the test materials. For each material, half of the root end cavities were dried prior to placing the filling material. The remaining half were filled after they were contaminated with blood. All 90 roots were then immediately placed in 1% methylene blue dye for 72 h. Finally, the roots were split and linear dye penetration was measured and statistically analyzed (analysis of variance). Presence or absence of blood had no significant effect on the amount of dye leakage. However, the results showed that there was a significant leakage difference between the root end filling materials (p < 0.0001). Mineral trioxide aggregate leaked significantly less than other materials tested with or without blood contamination of the root end cavities.
