A case was 77-year-old male with left back pain who had received artificial pneumothorax therapy 40 years ago due to lung tuberculosis. He had a spindle shaped mass in his left chest wall which had connection to the chronic tuberculous cavity. Because malignant cell was not detected by biopsy, it was therapied as tuberculous abscess. Then the mass grew rapidly into 20 cm in diameter. Although surgical operation was carried out, it was impossible to remove it all. After the surgery, the pathological findings was liposarcoma. In spite of the radiotherapy, the patient died 4 months after the surgery. We should always keep in mind the early diagnosis of malignant tumor associated with chronic tuberculous empyema.
Thoracoscopy, an endoscopic examination for pleurisy, can be performed using general anesthesia, usually without intubation, or using a neuroleptic, after induction of artificial pneumothorax if required. At the end of surgery, a thoracic drain is inserted to expand the lung. Complications are rare and mortality is below 0.017%. The main indication for thoracoscopy is diagnosis of pleural effusion, where its sensitivity and specificity are clearly superior to that of needle pleural biopsy and (or) to pleural fluid cytology. In diffuse pulmonary diseases, thoracoscopy also allows lung biopsy. Some therapeutic measures are mainly feasible by this route. Pleurodesis is performed in case of recurrent pleural effusion of for pneumothorax.
OBJECTIVE: To study the mechanism of severe hypoxemia associated with acute unilateral atelectasis (AUA). METHODS: Six dogs with acute left side atelectasis were studied during the period of rest and exercise. The pulmonary shunt and PaO2 were measured at different levels of ventilation of right lung. RESULTS: After the left side ateloctasis during resting condition, the right lung could compensate to produce only mild hypoxemia (PaO2 9 kPa). But with further increased ventilation by exercise the shunt increased (from 19% to 29%) and led to severe hypoxemia (PaO2 8 kPa). When an artificial pneumothorax was instituted on the left lung, the shunt decreased (from 29% to 18%) and PaO2 increased (from 8 kPa to 9 kPa). CONCLUSIONS: After AUA, whether severe hypoxemia would occur is determined by the volume of ventilation of the normal side lung. The excess increase of ventilation of the normal side lung by any reason may result in severe hypoxemia.
A chronic expanding hematoma in the thoracic cavity is very rare and is clinically included in a category of chronic hemorrhagic empyemas as a special type of chronic empyemas in Japan. In this paper, we report a case of this disease. The patient was a 69-year-old female, who was underwent bilateral artificial pneumothorax for pulmonary tuberculosis 35 years ago. On 25. Nov. 1996, she admitted into Nagoya University Hospital for a diagnosis of intrathoracic tumor. On 2. Dec. 1996, we removed the tumor with combined resection of left lower lobe. The tumor was diagnosed pathologically as an intrathoracic chronic expanding hematoma. This disease is benign, but progressive clinically. In progressive case it is dangerous to remove the tumor for a risk of excessive bleeding and hard adhesion to the chest wall. So we think the earlier is a surgical treatment, the better is a management for this disease.
A high incidence of non-Hodgkin's lymphoma of the pleural cavity has developed in Japanese patients with long-standing pyothorax (38 years on average) resulting from artificial pneumothorax for the treatment of pulmonary tuberculosis or tuberculous pleuritis. Patients with pyothorax-associated lymphoma (PAL) have long been exposed to antituberculous drugs, antibiotics, bacterial or viral products, and frequent diagnostic radiation for the confirmation of pneumothorax and pyothorax. We analyzed p53 mutations on paraffin-embedded specimens from 21 patients with PAL by PCR-single-strand conformational polymorphism followed by direct sequencing. An unusually high frequency of p53 mutations (14 of 21 cases, 67%) was detected in the PAL specimens, and mutations consisted of 13 nucleotide substitutions and 1 deletion. Furthermore, 10 of 13 substitutions (77%) occurred at dipyrimidine sites (CC:GG to CT:GA substitution). Such specificity has not been reported, except for solar light-related skin cancer and AIDS-related lymphoma in some parts. An UV light mimetic agent may be produced in the long history of chronic inflammation in tuberculosis or immunodeficient patients.
Pyothorax-associated lymphoma (PAL) is a newly-described entity developing several decades after artificial pneumothorax treatment for pulmonary or pleural tuberculosis. It is known to be associated with Epstein-Barr virus (EBV) with constant expression of the two latent membrane proteins: latent membrane protein (LMP)-1 and EBV-associated nuclear antigen (EBNA)-2. We are reporting three new cases of PAL. All of the tumours were of B-cell lineage and classified as large-cell diffuse lymphomas according to the International Working Formulation for the Classification of Lymphomas. The EBV genome was detected in two of the cases with LMP-1 and EBNA-2 expression. No EBV could be detected in the third case suggesting that different mechanisms may be involved in the pathogenesis of the disease. Body cavity-based high grade lymphomas (BCBL) represent a new disease, developing mainly in human immunodeficiency virus (HIV) infected patients: the tumoural cells often contain both human herpes virus (HHV)-8 (or Kaposi's sarcoma herpes virus) and EBV genomes, suggesting that these viruses might co-operate in the pathogenesis of the disease. The pleural location and the association of EBV have led to speculation that PAL could also be related to HHV-8 infection. However, no HHV-8 genome could be detected in any of the 14 tested cases already reported in the literature nor in the two cases we studied (one EBV-positive and one EBV-negative), suggesting that PAL and BCBL are two different entities.
