On-Line Myocardial Tissue Characterization with a New Commercially Produced Software.

Myocardial tissue characterization has been performed using various ultrasonic techniques, one of which is the cyclic variation of integrated backscatter, a method that analyzes the acoustic properties of the myocardium using backscattered radiofrequency signals to provide information about myocardial structure and function. Previous studies using prototype equipment have demonstrated a reduction in the cardiac cycle variation of integrated backscatter in various pathologic states. Recently, a commercially produced software package that allows online analysis of cyclic variation of integrated backscatter has been made available for testing by various investigators. To evaluate this new commercially produced software, we compared integrated backscatter results in three groups of patients: a control group; an end-stage cardiomyopathy group; and a heart transplant recipient group. Integrated backscatter of the septum and posterior walls in the parasternal long axis and 12, 3, 6, and 9 o'clock regions in the short axis was performed using a commercially produced program (Hewlett-Packard Sonos 1500). In the control group, the mean cyclic variation of integrated backscatter was 5.04 +/- 1.60 dB in the septum and did not significantly vary from the rest of the regions studied. In comparison, cyclic variation of integrated backscatter in every region studied was reduced in the cardiomyopathy and heart transplant groups. Intraobserver variability, interobserver variability, and reproducibility over a 3-month interval was found to be 6.5%, 5.7%, and 7.5%, respectively. These results indicate that: (1) online analysis of cardiac cyclic variation of integrated backscatter is possible utilizing commercially produced software; (2) results obtained are consistent with a low intraobserver and interobserver variability and are reproducible over time; and (3) as observed in the comparison between the transplant and control groups, this information may detect changes in cardiac structure even in the absence of changes in function. Retinal thickness in healthy and diabetic subjects measured using optical coherence tomography mapping software.

PURPOSE: To define the normal retinal thickness in healthy subjects using optical coherence tomography (OCT) mapping software and to assess the ability of OCT to detect early macular thickening in diabetic patients. METHODS: Six radial scans centered on the fixation point were done on 60 healthy eyes and 70 eyes of 35 diabetic patients without macular edema on biomicroscopy. Retinal thickness was measured automatically with OCT mapping software. Mean retinal thickness was compared in subgroups of healthy patients based on age, sex, and eye, and in the eyes of diabetic patients and healthy subjects. Thickening was diagnosed if mean retinal thickness of an area was greater than the mean thickness + 2SD in the corresponding area in healthy subjects; or if the difference between right and left eye exceeded the mean difference + 2 SD in a given area in healthy subjects. RESULTS: In healthy subjects, mean retinal thickness in the central macular area 1000 microm in diameter was 170+/-18 microm. There was no significant difference according to age, or left or right eye, but central macular thickness was significantly greater in men than women (p=0.0139). No difference was observed between the eyes of healthy subjects and diabetic patients without macular edema on biomicroscopy, but OCT detected early macular thickening in 12 diabetic eyes. CONCLUSIONS: In this study average retinal thickness and mean local variations in a normal population were defined using a commercially available mapping software. OCT seems a sensitive tool for detecting early retinal thickening.

SLIMarray: lightweight software for microarray facility management.

BACKGROUND: Microarray core facilities are commonplace in biological research organizations, and need systems for accurately tracking various logistical aspects of their operation. Although these different needs could be handled separately, an integrated management system provides benefits in organization, automation and reduction in errors. RESULTS: We present SLIMarray (System for Lab Information Management of Microarrays), an open source, modular database web application capable of managing microarray inventories, sample processing and usage charges. The software allows modular configuration and is well suited for further development, providing users the flexibility to adapt it to their needs. SLIMarray Lite, a version of the software that is especially easy to install and run, is also available. CONCLUSION: SLIMarray addresses the previously unmet need for free and open source software for managing the logistics of a microarray core facility.

Two software tools for plane-wave scattering on nonspherical particles in the German Aerospace Center's Virtual Laboratory.

We present the methodological background, the range of applicability, and the on-line usage of two software packages, MIESCHKA and CYL, which we have developed for light-scattering analysis on nonspherical particles. MIESCHKA solves Maxwell's equations in a rigorous way but is restricted to axisymmetric geometries, whereas CYL is an approximation for finite columns with nonspherical cross sections. We have established an easy on-line access to both of these programs through the Virtual Laboratory. Its generic software infrastructure was designed to simplify the web-based usage and to support the intercomparability of scientific software.

[Infantile nutritional evaluation with the software EPI INFO (version 6.0), taking into consideration collective and individual approaches, degree and type of malnutrition].

OBJECTIVE: A systematization for an anthropometric infantile evaluation is proposed, employing the software Epi Info, version 6.0, considering the collective and individual approaches, as well as the degree and type of malnutrition. METHODS: The software Epi Info, sponsored by the World Health Organization, provides an infantile nutritional evaluation, which is based on three anthropometric indicators: weight-for-age, height-for-age and weight-for-height. In order to quantify the observed data of each individual, three scales can be used. Among them, the Z score, which discriminates more efficiently the severe cases, is employed as a first choice. The reference pattern of the NCHS is used. Many cutoff points between eutrophy and malnutrition can be chosen, but the -2 Z score is the most employed. The child can be classified in three diagnostic categories: stunting, for height-for- age deficits; wasting, for weight-for-height deficits; underweight, for weight-for-age deficits. Besides this, in a collective approach, the Epi Info software calculates the proportion of individuals in each populational group which have their anthropometric data deviated from the reference curve, constituting the percentage of observed abnormality (called standardized prevalence). RESULTS: In this paper, in which the analysis of a data file is presented as an example, the organization of the information provided by Epi Info is proposed, with the characteristics described below. In order to increase the diagnostic sensitivity, the collective approach is valued. Concerning the type of malnutrition, the English term "stunting" is substituted by "chronic malnutrition", "wasting" by "acute malnutrition" and "underweight" by "isolated weight deficit". Concerning the degree of malnutrition, the -2 and -3 Z scores are employed as cutoffs in order to define, respectively, a threshold between the moderate and severe forms. CONCLUSIONS: The proposed approach takes into account the identification of all forms of malnutrition, from the mildest to the most severe cases, takes into consideration both height and weight deficits and if the process is acute or chronic. Therefore, it is a tool for organizing data available with the use of the computer.

Characteristics of the software for computer applications in medicine.

The requirements of clinical medicine which have tended to make the design and implementation of software for hospital computer systems more difficult than that elsewhere, are discussed in this paper. Specific constraints on the software for selected computer-assisted activities in a hospital environment are examined in considerable depth. It is shown that since some of these activities have counterparts elsewhere, hospital computing can benefit from the accumulated experience in dealing with similiar problems in business and scientific environments. The argument is put forward that developing countries, with their characteristic problem of acute shortage of skilled manpower in both medicine and computing, should initially concentrate on applying computers to these activities alone. Furthermore, medical education in such countries should incorporate programmes relating to computer technology in general and the software aspects in particular.

[Evaluation of therapeutic effects of Chinese materia medica by tongue image analysis software 1.0 based on tongue colors].

OBJECTIVE: To evaluate the therapeutic effects of Chinese materia medica in treating patients with different syndromes by tongue image analysis software 1.0 based on tongue colors, and to discuss the feasibility of applying this computer science-based techniques into drug evaluation. METHODS: The tongue colors and the areas of tongue fur were examined and analyzed by the tongue image analysis software 1.0 in healthy persons and the patients with different syndromes before and after treatment. The parameters of tongue colors consisted of the followings: the hue (H), the lightness (L), the saturation (S), and the values of red (R), green (G) and blue (B). RESULTS: Obvious differences could be revealed in tongue color index between the healthy persons and the patients in five groups of different syndromes. There also existed some significant differences in those index between patients before and after treatment. CONCLUSION: The tongue image analysis software 1.0 based on tongue colors is helpful to evaluate the therapeutic effects of Chinese materia medica.

Optimization of analysis of skeletal ossification of laboratory animals by means of digital radiography software options.

Digital radiography software enables image manipulation and reformatting, thus allowing precise diagnosis and measurements even in cases when exposure parameters are suboptimum and visualized structures are faintly radiopaque. The aim of the paper is evaluation of digital radiography software options in optimization of analysis of skeletal ossification in rats. Wistar at pups at the age 0 to 22 days of postnatal life were radiographed by means of a storage phosphor digital radiography system. In order to optimize viewing conditions as well as density nd linear measurements all available image reformatting options and filters of digital radiography software Digora 2.0 were used and evaluated. Application of digital radiography image enhancement options was found advantageous in optimization and improvement of viewing conditions and it allowed higher accuracy and reproducibility of the measurements. Digital radiography is a novel and precise tool in determination of skeleton ossification and could be used as a supplement to routine staining methods.

[Molfig, a software for displaying the structure and vibration of molecule].

A Molfig software has been developed for displaying the structure and vibrational mode of molecule in our lab. Various functions and a friendly interface are equipped in the software. The testing results showed that the software may enhance our understanding of the relationship between the vibrational behavior and the structure of molecules.

[Comparison of fingerprints using a personal computer and graphic software sold on market].

We compared the fingerprint images using a personal computer and a graphic software sold on market. The fingerprints of thumb of left hand recorded by pressing and rolling on paper were input in computer using image scanner. These two fingerprint images were overlapped by moving and rotation on the computer screen, and were fitted well. This result showed that the fingerprints recorded by any way from one person were fitted. Latent and accused's fingerprints were also input in computer using image scanner. These images were analyzed by the same procedure. These two fingerprints were part by unfitting. It was decided these two fingerprints were not derived from one person. We also considered the possibility that the latent fingerprint was the reversed one. Then, the image was reversed of its color on the screen of computer (black to white and white to black) and compared by the same procedure. In this experiment, two fingerprint images were not fitted in a part. This showed that the reversed fingerprint was also able to be analyzed by this procedure. For the comparison of fingerprints, this report showed the ability of the personal computer and software which can be obtained easily from market. Following the improvement of the hardware and software used in this examination in near future, the computerized identification of fingerprints like this study will become more accurate and convenient.

[Guide to a new data base system of literatures on dental materials and devices. Off-line retrieval system using commercially available software for data base management].

This article describes the off-line retrieval systems for the data base file of literatures on dental materials and devices which was constructed by the Japanese Society for Dental Materials and Devices. The off-line systems employed two kinds of commercially available softwares for data base management on a personal computer. The data base file was stored in the large-scale computer and downloaded into MS-DOS text files. They were supplied through floppy disks. A program was developed for converting the text files into coded data format which could be transmitted to the applied software. The converted data base files could be successfully processed with two commercial softwares for data base management on a personal computer.

Designing prevention-oriented software for workplace health and safety.

Rapid changes in technology and health services organization are propelling information-driven decision making in all aspects of business. The potential of electronic data processing to reduce costs and increase efficiency has been heralded, but disease and injury prevention have received scant attention. Prevention-oriented software for surveillance and disability management must generate population- and case-level measures of magnitude and severity of occupational injury and illness. A public domain program, called WIZARD, was developed as a model. With 12 data elements, it generates, from injury and employment files, injury- and return-to-work rates, avoidable costs, attributable cases using several reference rates, and case lists from screening criteria. In conjunction with an explicit case definition, screening criteria for case follow-up, and guidelines for medical, disability, and health and safety management, software can play a significant role in prevention.

Experience with gated cardiac software phantoms for quality control of applications programmes.

Ten gated cardiac software phantoms, representing normal and abnormal clinical conditions, were transferred to 9 different computer systems and tested with 11 cardiac programmes. Problems of inappropriate data format were encountered when analysing the phantoms on other systems. The global left ventricular ejection fraction (LVEF) values resulting from the different programmes were compared. Significant programme differences were found, the programmes falling generally into two groups. Few LVEF outlier values were identified. Full functional assessment of cardiac phantoms requires a set of different views which together form the phantom. Application of software phantoms for programme assessment and training requires prudence and attention to the acquired data format, frame time, and gating method for the appropriate use of phantoms.

A parallel software architecture for building intelligent medical monitors.

Intensive care units become more complicated each day as the number of devices developed to monitor various aspects of a patient's status continues to increase. Intelligent monitors attempt to reduce this complexity by interpreting the data and presenting a high level summary to the clinician. We propose an innovative parallel software architecture for constructing intelligent medical monitors: the process trellis. The process trellis is an explicitly parallel structure, and therefore can take advantage of the performance gains available from parallel computing hardware. It does not, however, presuppose any expertise in parallel programming on the part of the application programmer. A prototype cardiovascular monitor has been built using this parallel software architecture. Preliminary testing of the monitor has shown that real-time cardiovascular monitoring, including data calculations, symbolic classification, and interpretation can be accomplished in real-time.

Development of software for pulse oximeter and investigation of its realtime response in clinical environment.

We developed a pulse oximeter software at Thorax Centre, Erasmus University as a joint project with an industry and evaluated it in the clinical environment of thorax anaesthesia using a computerized protocol for realtime data collection during routine clinical procedures. This paper gives an account of the results we obtained from the development project and the clinical study. The paper consists of two parts. First part describes different components of the software module and their influence on different aspects of the clinical behaviour of the oximeter. The second part describes the results of realtime response investigation. The investigation was carried out using a personal computer to collect the data continuously during anaesthesia, surgery and post-operative periods. Two other industry standard oximeters, Nellcor and Ohmeda were also included in our study. We collected data on more than fifty patients on an average of eight hours per patient over a period of four months with major emphasis on low-saturation occurrences. The interpretation of the data was focused more on the realtime response anomalies on random cases than on ensemble statistical data evaluation. We found, that there are few factors in clinical environment which often influences the measurement of a pulse oximeter very strongly. Most often the anomalies were found during low saturation measurement. The main objective of this paper is to make the results available to practising clinicians so that it may be useful to identify these occurrences during routine clinical usage.

Management software for a universal device communication controller: application to monitoring and computerized infusions.

We designed a virtual device for a local area network observing, operating and connecting devices to a personal computer. To keep the widest field of application, we proceeded by using abstraction and specification rules of software engineering in the design and implementation of the hardware and software for the Infusion Monitor. We specially built a box of hardware to interface multiple medical instruments with different communication protocols to a PC via a single serial port. We called that box the Universal Device Communication Controller (UDCC). The use of the virtual device driver is illustrated by the Infusion Monitor implemented for the anaesthesia and intensive care workstation.

Development of stereological analysis software for the calculation of volume, number and surface density.

COSAS, an acronym for COmputerized Stereological Analysis System, is software designed to facilitate the process of quantitative morphological analysis. It is a flexible, reliable, and easy to use software system which provides the biologist with a simple means for performing stereological analysis. It provides for estimation of volume and surface density and allows for direct calculation of diameter information from planimetric data. The latter capability is unique to COSAS and provides the ability to calculate longest and shortest diameters for convex profiles, enabling calculation of number density. Access to all intermediate calculations is possible, so the process of transformation of raw data into the stereological descriptors can be completely exposed. This serves two purposes: (1) it provides a means for tracing results; (2) the intermediate values can be utilized independently. This system was used to analyze peroxisomes in tissues from patients with Zellweger's disease and neonatal-onset adrenoleukodystrophy. The system was also used to facilitate an immunocytochemical analysis of cytochrome P450 topology in the endoplasmic reticulum membrane.

A software engineering approach for medical workstations development.

Multimedia medical workstations represent the natural tool for accessing the hospital information system environment. They are complex medical systems that have to gather, in a single framework, a large collection of components dealing with multimedia medical objects. To remain current with both medical practice and with advances in the computer science field, they have to allow the iterative addition of new functions to the set of existing ones. In this paper, after a survey of commonly required medical workstation functional components, we shall try to discuss how a software engineering approach can streamline the development of a medical workstation. Different software engineering tools needed to build the functional components of a workstation are described. Their integration in a single dedicated environment is considered through four perspectives: data, presentation, communication and control. Benefits and limitations of an object-oriented approach are discussed.
