Macronutrient composition and food selection.

Humans can maintain health on diets differing widely in their macronutrient content, and numerous diet recommendations have been made to maintain health and to help weight control. Net adenosine triphosphate yields during the oxidation of carbohydrate, fat, and protein come to 75%, 90%, and 55%, respectively. However, macronutrient proportions can only be varied within limits, and differences in energy dissipation achievable by macronutrient exchanges are minor. In the National Health and Nutrition Examination Survey III data, stature explains 10% to 16% of the variance in fat-free mass in adults (the most significant predictor of resting energy expenditure), but <1% of the variance in the percentage of body fat. Thus, differences in resting energy expenditure cannot be expected to have much effect on adiposity. Recommendations designed to facilitate weight control, therefore, should be based on their potential impact on food consumption and energy intake. They should also reflect the fact that the logic for nutrient selection is not the same during weight maintenance and weight reduction. Glycogen levels, along with inherited traits and exercise habits, influence fat oxidation, and, hence, the size that the adipose tissue mass has to reach for fat oxidation to become commensurate with fat intake. Recent increases in the prevalence of obesity could have been brought about by the effect of changes in the food supply and by further declines in physical activity on habitual glycogen levels. Given that biological evolution led to food intake regulating mechanisms that are more powerful in promoting search for food than in seeking to restrain energy intake, it is not surprising that constant availability of desirable foods would lead to a high prevalence of obesity.

Natural products--a simple model to explain chemical diversity.

A simple evolutionary model is presented which explains why organisms produce so many natural products, why so many have low biological activity, why enzymes involved in natural product synthesis have the properties they do and why natural product metabolism is shaped as it is.

Persistent polyclonal B lymphocytosis with binucleated lymphocytes: a study of 25 cases. Groupe Français d'Hématologie Cellulaire.

Persistent and polyclonal lymphocytosis of B lymphocytes (PPBL) with binucleated lymphocytes is an entity characterized by a polyclonal lymphocytosis. The lymphocytosis is stable for years and binucleated lymphocytes are detected on peripheral blood smears. We previously described +i(3q) as a recurrent chromosomal abnormality in seven PPBL patients. In this study we report a large series of 25 PPBL patients and demonstrated that PPBL was associated with +i(3q) in 77% of cases, premature chromosome condensation (PCC) in 50% and both abnormalities in 41% of cases. Furthermore, we demonstrated that i(3q) was present in a minority of B cells, restricted to B lymphocytes independently of the kappa or lambda light Ig chain expression, and exclusively observed in non-binucleated cells. The benign clinical course of PPBL and the lack of biological evolution in the majority of cases suggest that recognition of these disorders is so important that aggressive therapy in PPBL has to be avoided. Whether this syndrome represents a premalignant or benign disease remains unclear. The persistence of cytogenetic abnormalities after stopping tobacco use suggests no association with cigarette smoking.

The windblown hand - diagnosis, clinical picture and pathogenesis.

The syndrome of the windblown hand deformity is a complex constellation of malformations affecting not only the head and the feet but also the hands in a quite distinct manner. In the hand, it involves congenital bilateral flexion contracture with ulnar deviation of the metacarpophalangeal joints. The thumb is characteristically adducted (reaching the palm; "thumb-in-palm deformity") with flexion of the MP joint and hyperextension of the IP joint. The etiology is basically unknown. We present two theories based on knowledge derived from the disciplines of evolution biology and embryology. We believe that the atavistic appearance of phylogenetically primitive muscle groups in conjunction with an impaired rotation of the extremities during embryological development account for this malformation syndrome.

[Prenatal human life and unwanted pregnancy: our medical responsibility].

In practical life, unwanted pregnancy and prenatal human life are often irreconcilable and hence paradoxical. This paradox is aggravated by the fact that the doctor is aware of the human qualities associated with prenatal human life, as formulated today by researchers in biology and psychology: Any human being "in statu nascendi" is endowed with all the requisite human qualities right from the beginning. Seen from the anthropological point of view, the meaning of biological evolution is a development towards increasing spiritualisation and freedom, liberation and ability to change and to undergo transformation. Mental functions of the foetus are present from the eighth week of pregnancy. Human individuality begins with conception and procreation. The multifacetted meaning of the term "unwantedness" is examined. It is the doctor's burden to bear the antagonism between the sensitivity of prenatal human life on the one hand and the rigid wall of denial represented by unwantedness on the other, and to build a bridge of responsibility between the two.
