[Late management of medial canthal deformities after naso-orbital-ethmoid fractures].

OBJECTIVE: To evaluate the surgery methods and effectiveness of medial canthal deformities after naso-orbital-ethmoid fractures (NOE) and analysis the long-term results. METHODS: The clinical data and following-up results from 83 patients (83 eyes), which were treated in the Department of Ophthalmology, Ninth People's Hospital, Medical School of Shanghai Jiao Tong University from January 1999 to January 2004, were retrospectively analyzed. All the patients had unilateral medial canthal deormities after NOE fractures, and they were on operations of medial canthoplasty and fixation of the canthal ligament. Based on the severity of the canthal deformities and local scar of the area, different skin flaps were utilized for canthoplasty, such as "Y-V" flap, "Z" flap, "Mustard" method and rotation flaps. According to the fracture pattern and severity of the medial canthal tendon's injury, different medial canthopexy were performed such as suturation, fixation with titanium nail, drilling a role across the bone, and reposition the "central segment" using titanium miniplates. The surgery to repair the obstruction of the lacrimal system were accompanied with canthoplasty at the same time, while the orbital surgery for repositon of the enophthalmos was often performed at the first stage, and then the deformities of medial canthus were corrected after more than six months. The follow-up time varied from 12 to 30 months. RESULTS: 72 cases obtained satisfied contour of the medial canthal area, the position of the canthus was symmetry with the normal side, especially the formation of the canthal fossa. Recurrence of deformities occurred in 11 cases in six months after canthoplasty, among them 6 patients were re-operated for the second time. CONCLUSION: According to the fracture pattern and severity of the medial canthal deformity, different operative designs and canthoplasy can produce satisfied results.

[Bilateral "crocodile tears syndrome" associated with Melkersson-Rosenthal syndrome--case report].

We present a rare case of bilateral crocodile tears syndrome (CTS) in the course of Melkersson-Rosenthal syndrome. Melkersson-Rosenthal syndrome is characterised by a triad of recurrent orofacial swelling, relapsing facial paralysis, and fissured tongue. The classic triad is infrequent and oligosymptomatic variants are seen more frequently. CTS is a rare complication of facial nerve paralysis characterised by inappropriate lacrimation on the side of the palsy in response to salivary stimuli. It results from aberrant reinnervation of the lacrimal gland by salivary parasympathetic fibres. The therapeutic approach for an acute bout of Melkersson-Rosenthal syndrome consists mainly of steroid administration. CTS management is composed of anticholinergic drugs and surgical procedures. Botulin toxin injection into the lacrimal gland is the most modern therapeutic option. In the case presented CTS developed in a 50-year-old man after 5 incidents of facial palsy due to Melkersson-Rosenthal syndrome. The case deserves attention due to the rarity of the observed symptoms and signs.

Mucoepidermoid carcinoma of the lacrimal gland. Report of a case with oncocytic features arising in a patient with chronic dacryops.

A 62-year-old woman had recrudescence of growth of a lacrimal gland fossa cystic mass present for more than 40 years. Results of histopathologic examination of the excised specimen showed infiltrating nests of epidermoid and mucin-producing cells, consistent with a mucoepidermoid carcinoma. Also present were numerous large oncocytic cells partially lining the lumens of the multiloculated cyst and penetrating the adjacent stroma. To the authors' knowledge, this is the first reported case of mucoepidermoid carcinoma with prominent oncocytic features arising in a previously documented chronic cystic lesion of the lacrimal gland. Because of the population of oncocytes (a metaplastic phenomenon encountered in ductular epithelial cells) among the cells lining the cyst, the authors conclude that the carcinoma arose from a long-standing dacryops. This conclusion was also supported by the electron microscopic demonstration of duct-type secretory granules within the mitochondria-rich oncocytes.

Review of lacrimal gland lesions.

A review of 142 lacrimal gland biopsies performed during a 25 year period revealed that 78% of lacrimal gland lesions were of nonepithelial origin and only 22% were primary epithelial neoplasms. The nonepithelial lesions included inflammation (64%) and lymphoid tumors (14%), while the epithelial lesions included dacryops (6%), pleomorphic adenoma (12%), and malignant epithelial tumors (4%). These results contradict the much quoted teaching that 50% of lacrimal gland lesions are primary epithelial tumors and 50% are nonepithelial lesions.

[Ultrasound diagnosis of the lacrimal sack].

A- und B-scan echography is a useful method for the examination of certain parts of the lacrimal drainage system. In most cases it can substitute for dacryocystography and other expensive methods in the diagnosis of lacrimal sac diseases. Older methods of echography for examination of the lacrimal sac are direct contact, water bath, and rarely transocular contact methods. We have developed a new method, in which a rubber tube filled with fluid is placed between the transducer and the skin surface. Comparison of these methods demonstrates that this is the best method for ultrasound diagnostics of the lacrimal sac because it can be carried out as easily as the direct contact method, and the quality of the echograms is as good as in the water bath method. A special advantage is that causing an infection in the patient with the ultrasound probe can be avoided.

[Fiber optic endoscopically-guided intranasal dacryocystorhinostomy in post-traumatic surgery of the lacrimal ducts].

Increasingly today, dacryocystorhinostomy seems to be the common obligatory path to functional surgery in case of traumatic lesions of the lacrimal ducts. The endonasal approach provides direct access to the nasolacrimal canal, while permitting to avoid the cutaneous approach through tissues that may be coming apart. The use of optic endoscopes renders the operation easier and provides access to the dome of the lacrimal sac, which is impossible under conventional endonasal microscopy. Monitoring and postoperative care are carried out with the same instrument, thereby improving the quality of the end result. The operation usually takes place under potentiated local anesthesia and lasts one hour, on average. The various surgical steps are described and particular attention is given to endonasal localization of the lacrimal ducts and to the technique of marsupialization.

Prevention of radiation-induced stenosis of the nasolacrimal duct.

Epiphora is a recognized complication of radiotherapy of medial canthal tumours. We reviewed the records of 36 patients who underwent prophylactic nasolacrimal duct intubation with silicone tubing before superficial x-ray beam therapy and 42 patients who did not undergo intubation before radiotherapy. The mean length of follow-up was 3.4 years in the intubated group and 7.2 years in the nonintubated group. A total of 31% of the patients in the nonintubated group reported chronic epiphora during the follow-up period, compared with 6% in the intubated group (p less than 0.003).

[Antibiotic therapy in congenital obstruction of the nasolacrimal duct].

Forty-eight children aged from 4 to 11 months, affected by Congenital Nasolacrimal Duct Obstruction (CNDO), treated successfully by local hydrostatic massage, were retrospectively included in two groups: Group A treated by massage and antibiotic eye drops, Group B treated by massage only. The comparison shows a quicker resolution of the CNDO when antibiotic eye drops are administered. The Author stresses the utility of topical antibiotic therapy regardless of the presence of mucopurulent discharge.

[The use of grafts and prostheses in surgery on the lacrimal ducts].

A description is given of the different types of grafts: venous, arterial, skin and conjunctiva, as well as prostheses of plastic material, polyvinyl, silicon, used in the reparatory and reconstructive surgery of the lacrimal ducts. The author gives the indications, advantages and drawbacks of these grafts and prostheses and emphasizes the importance of prosthesis with polyvinyl probes in the reconstructive surgery of the lacrimal ducts over other methods.

[Carcinoma of the lacrimal sac].

The present paper reports on the case of a 63 year-old woman twice operated for chronic suppurated dacryocystitis. At the third operation the lacrimal sac which was enlarged and rigid was removed. Section of the sac revealed a pinkish-grey formation that occupied the entire lumen of the sac. Histologic examination showed a poorly differentiated carcinoma with an islet-alveolar distribution. Stress is laid on the rare occurrence of primary malignant tumours of the lacrimal sac and the diagnostic difficulties in the onset period.

Factitious "calcification" in the lacrimal gland fossa.

Computed tomographic (CT) scans are valuable in the evaluation of lacrimal gland fossa masses. Calcification in this region is considered a sign of malignancy. Silicone implants are widely used in orbital surgery and may give the appearance of calcification on CT scan. These must be distinguished from true calcification by the proper choice of the CT scan window.

[A new method for restoring the patency of the lacrimal ducts in children].

The advantages of the suggested technique are as follows: a ring-shape ligature of silicone capillary is a simple and effective method of treatment; fixation of the capillary terminals end-to-end at the site of exit from the lacrimal points permits an active life of the patient and is cosmetically quite satisfactory; the duration of the intubation may be under 1 year; the method is fairly simple, this helping to reduce the little patients' hospital stay and permitting outpatient treatment for older children.

Avulsion of the canalicular system.

Avulsion of the common canaliculus from its attachment to the lacrimal sac should be suspected in cases involving either deep lacerations on the lateral side of the nose or traumatic telecanthus in midfacial trauma. Blunting of the canalicular angle and rounding of the inner canthus are confirming signs. A canaliculodacryocystorhinostomy can be performed to reanastomize the lacrimal drainage pathways and relieve epiphora.
