[Catheter cerebral angiography in little children. Review of 71 examinations on 59 children under the age of 7 years (author's transl)].

Although the role of the cerebral angiography in the diagnosis of the central nervous system disorders in children is well established, there exists some reluctance to perform angiography in little children, probably due to technical difficulties and possible occurrence of side effects. For the last several years, we have performed catheter cerebral angiography on children with various disorders of central nervous system utilizing Seldinger's transfemoral technique. Our technique has been described in detail with special reference to the dilator catheter and double replacement technique, and the results of 71 catheter angiographies on 59 patients under the age of 7 years have been reviewed. No neurological complication or systemic reaction to contrast medium was noted. Circulatory disturbance at the site of femoral puncture was occasionally noted. Transient circulatory disturbance of several hours' duration was not uncommon in this age group. There were three cases which developed thrombus formation at the puncture site with resultant successful thrombectomy utilizing balloon catheter. The causes of these complications were analysed, and the need for systemic heparinization or heparin coated catheter during catheterization in little children was stressed. Comparison was made with other methods of cerebral angiography applied to children. It has been pointed out that this is a relatively convenient and safe technique applicable to little children, in whom multivessel study is often needed.

Symptomatic subdural hygroma after petroclival meningioma surgery. Case report of a intractable complication.

Although subdural hygroma development after craniotomy or aneurysm surgery is a well-known complication and subdural peritoneal or V-P shunt are commonly successful procedures, there are situations that cannot be treated by available surgical options. We reported a case of a 28-year-old young man who developed a symptomatic subdural hygroma after removal of petroclival meningioma. This hygroma increased in size and became bilateral in spite of implantation of V-P with programmable valve at open pressure variable from 30 to 180 mmH2O, placement of subdural-peritoneal shunt with low-low valve or without valve and external diversion. The occurrence of this case showed that there other factors can play role in hygroma development as disturbance of normal CSF dynamic with shunt of CSF from basal arachnoidal to subdural spaces secondary to surgery and/or slow growth of petroclival meningioma as well as postoperative progressive cerebral atrophy. Actually there are not effective diagnostic tools to detect causes and therefore, there are restricted therapeutic possibilities. This potential and serious complication should be always considered when planning petroclival meningioma surgery because of the severe consequences on functional outcome.

[Image-guided epidural blood patch as effective treatment of intracranial hypotension. A case report].

We report the case of a patient with postural headache. A CT scan revealed bilateral subdural hygroma. Brain MRI showed diffuse pachymeningeal enhancement. A diagnosis of intracranial hypotension was therefore made. Thoracic cerebrospinal fluid leak was proved by radionuclide cisternography and contrast myelography. Conservative medical treatment was ineffective. Two thoracic epidural blood patches with radiographic control were made. We think the blood patch is the most important element for success.
