[Differential diagnosis of neoplasms of the lacrimal gland using ultrasonic scanning].

The paper describes results of ultrasound scanning in 32 patients with new formations in the lacrimal gland area, using a device "Ophthalmoocuscan, Model 200 A" (USA), with ultrasound fluctuation frequency--10 mHz, exposition time of probing impulse--0.2 microseconds. It was found that in case of lacrimal gland pathology the highest information value is received when the probe is placed at the superior external margin of the orbit and its scanning surface is directed on the site of the lacrimal gland projection. Characteristics of scanograms of different histologic types of new formations in the lacrimal gland area described: those of pleomorphic adenoma, adenocarcinoma, lymphoma, inflammatory pseudotumor, retention cysts. Typical scanograms were obtained in 78.1% of patients. The obtained characteristics of scanograms are recommended to be used in a complex with other methods for differential diagnostics of lacrimal gland new formations of different genesis.

Lacrimal gland and fossa lesions: role of computed tomography.

The lacrimal gland region can be involved in a wide spectrum of orbital pathology, including tumors and inflammatory and lymphoid lesions. This article focuses special attention on benign mixed tumors because of the excellent prognosis for the patient when it is completely excised at first surgery. The appearance of these lesions on CT is detailed in the illustrations of this article, and the findings of experts in the field are presented as well.

Dacryocystitis↗

[Echographic diagnosis in tumors of the paranasal sinuses with invasion of the lacrimal ducts].

In 16 patients the histologically proven diagnosis of a lacrimal sac tumor was made; in 9 of them a paranasal sinus tumor had invaded the lacrimal pathways, in 7 patients existed a primary tumor of the lacrimal sac, A- and B-scan sonography allow a clear-cut differentiation of both these entities. Furthermore primary and tumor-related inflammations of the lacrimal pathways can be distinguished by echography, which thereby provides the decisive diagnostic basis for the different therapeutic procedures in each case.

Dacryocystitis↗

Nasolacrimal disorders and their treatment.

Dysfunction of the excretory part of the nasolacrimal system may be congenital in otherwise normal infants or associated with congenital facial anomalies. It may also occur as a result of soft-tissue trauma to the face and naso-orbital fractures. Dacryocystorhinostomy and conjunctivodacryocystorhinostomy are discussed as treatment techniques.

[Digital radiology in the study of the lacrimal ducts].

The authors emphasize the usefulness of digital dacryocystography (DCG), as compared with various current techniques. Utilizing a radiographic unit equipped with a video-fluoroscopic system and interfaced to a digital video-processor, several digitalized images are acquired before, during and after the injection of contrast medium. Final images are obtained by subtraction of suitable pairs of source frames. Twenty-six patients affected by epiphora have been examined so far. In 21 cases digital subtraction DCG allowed an accurate visualization of the lacrimal system; in the other five patients the amount of information was acceptable. This methodology allows the assessment of both the normal anatomy of the lacrimal passages and their pathological patterns, such as obstructions, stenoses, fistulas, chronic dacryocystitis, lacrimal stones. The examination is easy and quick to perform, with no discomfort for the patient. Digital subtraction DCG proves thus to be a very valuable technique thanks to its possible electronic elaboration--i.e. the subtraction and the magnification of images--to its better contrast resolution, and to the possibility it yields of dynamic studies under radioscopic control.

Dacryocystorhinostomy without flaps.

In a series of 50 consecutive cases, a modification of the Kasper technique of dacryocystorhinostomy without flaps resulted in a success rate of 90%. The success rate was 92.5% in patients with nasolacrimal duct obstruction alone, but only 80% in those with both nasolacrimal duct obstruction and canalicular disease. The results of this study indicate that equivalent results are achieved with or without the use of flaps. The elimination of flaps shortens and simplifies the procedure.
