[Postpartum hemorrhage].

Postpartum hemorrhages are observed in 2 to 10% of all deliveries. They are severe in 1% of the pregnancies. However, they remain a major cause of maternal morbidity and mortality. Post-partum hemorrhages are generally separated in 2 categories. Acute hemorrhage occurs in the 24 hours following the delivery and is mainly caused by uterine atony, retained secondines, placenta accreta, birth canal trauma and uterine inversion. The delayed hemorrhage occurs after 1 day to 6 weeks after the delivery and is often related to uterine infection or abnormal involution of the placental bed. The management requires uterine massage and ocytocine or prostaglandins. Surgical or radiological selective ligation or embolization of the internal iliac arteries can be required. The hysterectomy is proposed in the most severe cases.

[Epidemiologic study on the risk factors of postpartum hemorrhage].

The 1:2 matched case-control retrospective study of postpartum hemorrhage (PPH) was undertaken in Suizhou, Hubei Province from 1 January 1990 to 31 December 1991. The subjects consisted of 109 women who had a PPH (blood loss equal to or more than 500 ml) and 218 women whose blood loss at delivery was less than 200 ml. The results showed that multiparity, age younger than 20 or older than 36, long second and third stages of labour, abortion, low income, lack of obstetric care, etc, were identified as risk factors statistically.

[Effects of new dental amalgam fillings in pregnancy on Hg concentration in mother and child. With consideration for possible interactions between amalgam and precious metals].

Amalgam-tooth fillings are suspected to be cause of many different diseases. With special consideration of the problem of newly made fillings, blood samples were taken from 45 affected pregnant women. After birth, newborns and placenta tissue were tested for Hg-contamination. One hundred and twenty one women with older amalgam-fillings were compared to 19 women without amalgam-fillings. The surfaces of the amalgam-fillings were measured in each case in mm2. In 58 women the tooth fillings contained additional precious metal. While in the placentas a retaining function were found of up to ten times higher Hg-values but low correlations to the amalgam surfaces, the Hg-concentrations in the blood samples of mother and child did not correlate to the amalgam-fillings. Newly made fillings during pregnancy had no influence on the Hg-concentration, neither had the contact to precious metals in the approximal and the antagonistic range. The wide-spreading of Hg-values without correlation to the filling-surfaces speaks for a great influence of other environmental factors.

Obstetric hemorrhage and blood utilization.

Patients on a busy obstetric service were prospectively evaluated to determine which ones required blood transfusion. During the period January-April 1990, 5,528 deliveries were performed. Fifty-five patients (0.99%) received blood transfusions during their pregnancy and puerperium. The most common conditions associated with transfusion were trauma due to instrumental delivery (16), uterine atony (15), placenta previa (12), retained products of conception (4), abruptio placentae (4) and coagulopathy secondary to the HELLP syndrome (1). Platelets, fresh frozen plasma, whole blood and cryoprecipitate were administered to 7, 6, 1 and 1 patients, respectively. The transfusion rates by procedure were emergency cesarean hysterectomy for bleeding placenta previa or atony, 7/7 (100%); vacuum extraction, 7/114 (6.1%); forceps delivery, 12/285 (4.2%); uncomplicated cesarean delivery, 10/704 (1.4%); and spontaneous vaginal birth, 19/4,425 (0.4%). The hemorrhage and subsequent need for a blood transfusion were not necessarily due to the procedure except in the case of trauma due to instrumental vaginal delivery. The rate of transfusion of red blood cells for patients undergoing vaginal instrumental delivery was significantly higher than the rate for those undergoing cesarean delivery (relative risk, 2.8; 95% confidence interval, 1.5-5.2). The need for transfusion can be anticipated on the basis of antepartum causes in only 23.7% of patients ultimately receiving blood products.

Hydrallantois in a caprine doe.

Hydrallantois was diagnosed in a 5-year-old Toggenburg doe. Clinical signs included bilateral abdominal distention, anorexia, and recumbency. Ultrasonographically, excessive fluid and live fetuses were detected in the uterus. At the time of cesarean section, the fluid (approx 12 L) was determined to be in the allantoic cavities; concentrations of electrolytes in the fluid were similar to concentrations in allantoic fluid from cows with hydrallantois. Two viable fetuses and 1 nonviable fetus were delivered. The doe retained its placentas for 60 hours, but made a good recovery. Hydrallantois should be considered in the differential diagnosis of abdominal distention in caprine does in late gestation.

Gemeprost in the management of second trimester intrauterine fetal death.

Gemeprost (Cervagem), a synthetic PGE1 prostaglandin analogue was used to induce labour in 20 patients with second trimester intra uterine fetal death. The mean induction abortion interval was 13.8 hours; 17 of the 20 patients delivered within 24 hours. In 17 cases the placenta was retained and the uterus was evacuated under general anaesthesia. Nausea was noted in five (25%) patients, vomiting in two (10%) and diarrhoea in three (15%). No serious side-effects were encountered.

[Caruncle removal in cattle--subsequent treatment and use of a new cervix dilator].

Retention of the fetal membranes in bovines is frequently accompanied by necrosis of the caruncles. This is dependent on the kind of bacterial contamination and seems to be of regional variety. Necrosis of the caruncles requires extirpation which is the only treatment to save life, fertility and economic benefit of the animal. Following extirpation after-treatment over a period of eight to nine days essentially influences the success. Antibiotic therapy is not the main principle of treatment but to handle the atony of the uterus and its fatal consequences caused by the inflammatory contents. Since the cervix necessarily has to remain open for the period of treatment the use of a recently developed cervix dilatator (Ludwig Bertram GmbH, Hannover) is described and recommended.

Managing residual trophoblastic tissue. Hysteroscopy for directing curettage.

OBJECTIVE: To describe our experience with selective removal of residual intrauterine trophoblastic tissue via hysteroscopy. METHODS: This is a descriptive report. Eighteen patients, 16 postabortion and 2 postpartum, underwent a hysteroscopic procedure for removal of residual trophoblastic tissue causing continuous bleeding. At hysteroscopy, a cutting loop was used as a curette for selective removal of the adherent residual tissue, while interference with the rest of the endometrial surface was avoided. RESULTS: Complete removal of the suspected residual tissue was achieved in all patients. Histology confirmed the curettings as trophoblastic remnants. No complications were reported during or immediately after the procedure. The median operative time was 10 minutes (range, 8-20). In all cases the bleeding stopped shortly after the procedure. In each patient, postoperative ultrasonography revealed a uterine cavity free of residual tissue. Five of the patients underwent second-look hysteroscopy several weeks later, and no signs of further residual tissue were observed. CONCLUSION: Selective curettage of residual trophoblastic tissue directed by hysteroscopy is an easy and short procedure and might be preferable to conventional, nonselective, blind curettage.

[Descriptive epidemiology of placental retention in intensive dairy herds in Brittany].

The influences of several risk factors (temporal, mechanical, zootechnical and nutritional) on the incidence of placental retention (PR) were tested. The study was conducted in Brittany on 5,240 calvings of Black-Pied females belonging to 47 herds surveyed over a period of four consecutive years. For twin births, the PR-incidence levels were only decreased in the case of calves born alive. Single calvings were associated with PR after more than three artificial inseminations, or when stillbirths, dystocia or shortened gestational periods were observed, but the frequency varied in relation to calving rank. Increased total amounts of concentrate and cake supplied at the end of the gestational period decreased the incidence of PR in heifers. Shorter dry periods and high levels of milk production increased the risk of PR in multiparous cows. The frequency at which PR was observed in both cows and heifers was lower in autumn. Milk production and fertility were decreased in animals having experienced PR. In addition, they were subject to more uterine disorders than other cows during the subsequent lactation, whatever number of calves had been carried. Milk fever in the third lactation and metabolic diseases in the fourth were more frequently observed following PR.

Comparison of intra-amniotic (15S)-15-methyl-PGF2 alpha and intravaginal prostaglandin E2 for second-trimester uterine evacuation.

OBJECTIVE: To compare the efficacy, safety, and side effects of intra-amniotic (15S)-15-methyl prostaglandin F2 alpha (15-M-PGF2 alpha) and intravaginal prostaglandin E2 (PGE2) for midtrimester uterine evacuation. STUDY DESIGN: Ninety-three patients underwent therapeutic midtrimester pregnancy termination by the use of laminaria placement and intra-amniotic injection of 15-M-PGF2 alpha. A matched control group underwent uterine evacuation by laminaria placement and insertion of PGE2 intravaginal suppositories. The main outcomes studied were time to delivery, side effects, and complications. RESULTS: The 15-M-PGF2 alpha group had a shorter time to delivery (12.3 +/- 6.4 hours) compared with the PGE2 group (16.2 +/- 6.6 hours, p < 0.0001). The evacuation rate over time was significantly greater in the 15-M-PGF2 alpha group (p = 0.001). The PGE2 group had a significantly higher incidence of side effects. CONCLUSIONS: The use of intra-amniotic 15-M-PGF2 alpha for therapeutic second-trimester pregnancy termination is safe and is associated with a more rapid evacuation of the uterus and fewer side effects than intravaginal PGF2 suppositories.

[Intrapartum hemorrhages in the maternity ward of the Yalgado Ouédraogo University Hospital Center of Ouagadougou, Burkina Faso].

On 12,175 childbirth registered in 4 years, 200 cases of delivery haemorrhage have been observed giving a rate of 1.6% of these accumulated childbirth. 60 patients (30.5%) have been evacuated from region of the country. The main etiology were: placental retention (54.5%), uterine inertia (24.5%) and coagulation disorders (21%). Blood was not available for emergency transfusion of 72 patients (54.1%) 26 deplorable cases of death raised the rate of lethality by 13%. Instant research of risk factors on all parturients, recycling of all personal on elementary first aid and life saving, also an improvement of sanitary references will enable to lower incidences related to haemorrhage by delivery.

Enzyme activities in placental tissues from cows with and without retained fetal membranes.

The aim of this study was the determination and the comparison of activities of AST, ALT, GGT, 5'-NU and CK in the placental tissues of cows with and without retained fetal membranes. Placental samples were obtained from 16 cows immediately after parturition and divided into 2 groups: A-not retained (n = 10, NRF), B-retained (n = 6, RF). The activities of the examined enzymes were determined in supernatants of homogenized tissues using spectrophotometric methods and ready kits. The activity of AST was statistically significantly lower in the maternal part of the placenta in group B than in group A. There were no differences in ALT activity. The activity of GGT was statistically significantly higher in the maternal part of the placenta in group B than in group A. The activity of 5'-NU was statistically significantly higher in the maternal part than in the fetal part of placenta in both groups examined. The activity of CK did not differ, except for statistically significant lower activity in the fetal part of the placenta in group B. The results can suggest that the metabolism of amino acids is altered to some extend in cases of the retained placenta. Changes in GGT my indicate on imbalance in free radicals generation and neutralisation. Energetic status may not be influenced by the retention of the fetal membranes. Further experiments concerning more frequent sample collecting during whole periparturient period are necessary.

The fine structure of the interhemal membrane of the rat chorioallantoic placenta during prolonged pregnancy.

In placentas from viable rat fetuses which surgically had been retained beyond term in lactating animals for as long as four days, the fine structure of much of the interhemal membrane was quite like that of full-term placentas. Moreover, contrary to what has been reported previously (Jollie, '64a), in retained placentas which were autopsied at term there were few changes in the four cytoplasmic layers of the membrane which could be interpreted as degenerative. With prolongation of the placenta beyond normal delivery the number of pericytes which were associated with the allantoic endothelium increased; the basal lamina separating the endothelium from trophoblast III progressively became more focally split; and the apposed plasma membranes between this latter layer and trophoblast II appeared to dissociate. Trophoblast II became regionally divided in its cytoplasmic organization, its more superficial zone becoming highly infolded and/or vacuolar. As a consequence, the intertrophoblastic space between layers I and II became more extensive and complex. Within this space, a flocculent material appeared; and there was ultrastructural evidence of massive pinocytosis from the space by trophoblast II. With increased prolongation of gestation, a variety of lysosome-like inclusions appeared within this layer of the membrane.

Transfer and metabolism of retinol by the perfused human placenta.

The transfer and metabolism of retinol by human placenta was investigated using an in vitro perfusion system with independent maternal and fetal circulations. 3H-retinol bound to albumin added to the maternal perfusate was rapidly taken up and concentrated by the placenta to levels 16.5 +/- 5.28 times the maternal perfusate. Approximately 8% of the retinol retained in the placenta was esterified. No metabolites were detected in the perfusates. Perfusion of placenta with retinol bound to retinol-binding protein (RBP) reduced the placental concentration to 4.4 +/- 1.72 times the maternal concentration and eliminated evidence of metabolism. The transfer rate of RBP:3H-retinol was less than that of albumin:14C-retinol when measured concurrently in three experiments (clearances 0.11 versus 0.75 mL/min, 0.21 versus 1.7 mL/min, and 0.29 versus 0.48 mL/min, respectively). Transfer of the radioactive retinol was more rapid than 125I-RBP or albumin, indicating that retinol was transferred independently of the proteins. The transfer index of retinol (clearance retinol:clearance L-glucose) was 0.73 +/- 0.085 compared to 2.1 +/- 0.36 for thiamin and 3.4 +/- 0.95 for riboflavin, both water-soluble vitamins with active transport systems. The retinol transferred to the fetal perfusate is not bound to RBP, as demonstrated by gel filtration chromatography and chromatography on a transthyretin affinity column, despite the availability of RBP in the cord serum added to the perfusate. The endogenous retinol in the cord serum is bound to RBP.(ABSTRACT TRUNCATED AT 250 WORDS)
