The wear of oriented UHMWPE under isotropically rough and scratched counterface test conditions.

Unidirectional wear tests of UHMWPE against smooth counterfaces show that molecular chains at the surface of virgin material become oriented parallel to the sliding direction giving low wear rate. It is postulated that under more abrasive conditions and predominantly unidirectional motion as in knee prostheses, it may proof beneficial to provide molecular orientation of the bulk material. Therefore strips of UHMWPE were oriented by die drawing at elevated temperature and the resulting anisotropic material subjected to tensile tests, small punch tests and also unidirectional wear tests both parallel and perpendicular to the draw direction. The tensile tests showed that, in the parallel direction, the oriented UHMWPE became stiffer and less ductile compared to the virgin UHMWPE. In the perpendicular direction, there were reductions in yield stress, 5% proof stress and energy to failure compared to the virgin material. The small punch test showed that the oriented UHMWPE exhibited apparent hardening when tested in both parallel and perpendicular directions but the mechanical behaviour in the perpendicular direction was comparable to the virgin UHMWPE. The wear tests demonstrated that the oriented UHMWPE did not show any significant improvement of wear resistance for sliding against either isotropically rough or scratched counterfaces. There was no clear dependency between the mechanical properties and wear factors of the oriented UHMWPE.

Metabotropic modulation of motoneurons by scratch-like spinal network activity.

Glutamate is the main excitatory transmitter in the spinal motor network. The excitation is to a large extent mediated by ionotropic receptors, but glutamate also activates metabotropic receptors. In motoneurons in spinal cord slices the activation of group I metabotropic glutamate (mGlu1) receptors leads to facilitation of CaV1.3 L-type calcium channels. Here we investigate whether this pathway is activated by motor network activity induced by natural sensory stimuli. The lumbar carapace and spinal cord were isolated from adult turtles. In this preparation, mechanical stimulation in the receptive field for the scratch reflex induced episodes of rhythmic motor network activity. During an episode the excitability of coactivated motoneurons increased. This increase was associated with an increased persistent inward current and was abolished by local blockade of either mGlu1 receptors or CaV1.3 L-type calcium channels near the recording site. We conclude that glutamate released during spinal motor network activity excites motoneurons by parallel activation of ionotropic and mGlu1 receptors. The metabotropic facilitation of L-type calcium channels contributes significantly to this excitation. Our findings establish intrinsic modulation as an active component in the spinal motor network for limb movements.

Septic monarthritis due to Pasteurella multocida after a cat scratch in a patient with rheumatoid arthritis.

A 60-yr-old woman with a history of seropositive rheumatoid arthritis and using a low dose of corticosteroids presented with a very painful, swollen, red coloured knee with limited range of motion. Physical examination revealed a scratch on her left lower leg without discolouration or induration. Synovial fluid cultures yielded Pasteurella multocida. The pathogenetic significance of this commensal of cats and dogs in man is discussed.

[Changes in the segmental reactions in the decerebrate cat during scratching and locomotion].

Experiments on decerebrate cats have shown that N1 component potential of dorsal surface and dorsal root potential elicited by electrical stimulation of paw plantar surface tonically decrease during locomotion and scratching. Phase changes occur against a background of tonic changes in the rhythm of limb movements. The dynamics of the observed changes correlates well with that of primary afferent depolarization during these movements. It is concluded that presynaptic inhibition underlies the changes in segmental reactions.

[Variations in the membrane potentials of motor neurons during the generation of scratching].

The activity of motoneurons of the ankle flexor and extensor muscles was recorded intracellularly during fictitious scratching in decerebrate curarized cats. Before the beginning of rhythmical oscillation a large depolarization and tonic discharge were observed in flexor motoneurons, while the membrane potential of extensor ones did not change. When the rhythmical generation began, a short periodic hyperpolarization and corresponding pause in the discharge lasting for the extensor phase of the cycle appeared in flexor motoneurons. In extensor motoneurons a periodic depolarization increasing in the course of the flexor phase was observed; a maximal depolarization occurred in the extensor phase in which a short burst of spikes was generated.

[Modulation of Renshaw cell activity during scratching].

The activity of the Renshaw cells was recorded during fictitious scratching in 7 decerebrated and 1 spinal curarized cats. The flexor and extensor Renshaw cells were identified according to the responses induced by orthodromic stimulation of muscle nerves. Before the beginning of rhythmical oscillations, the background activity of the flexor Renshaw cells was inhibited. When the rhythmical generation began, both flexor and and extensor cells generated short bursts of spikes at the end of the flexor phase and at the extensor phase of the cycle. The activity of flexor cells began and terminated earlier in the cycle than that of the extensor cells. The results show that the flexor Renshaw cells are active reciprocally with flexor motoneurons, while the extensor cells are coactivated with extensor motoneurons.

[Diagnostic value of glass microfibre-based basophil histamine release test in food allergic children. Comparison with specific IgE antibody and skin scratch test].

We evaluated the diagnostic value of the glass microfibre-based histamine release test (HRT), which allows measurements to be performed using small amounts of whole blood, in 50 children with food allergy case histories. The patients were evaluated by radioallergosorbent tests (RAST), skin scratch tests (ST) and food challenge tests. Of the 50 patients, 39 had a confirmed clinical diagnosis of food allergy from food challenge tests and case histories, and were affected by a total of 60 positive allergens (egg 37, milk 11, soy beans 4, wheat 5, rice 3). The concordance, sensitivity, and specificity of HRT with the clinical diagnosis were 85.3%, 66.7% and 92.1%, those of RAST were 59.4%, 90.0% and 48.2%, and those of ST were 84.7%, 71.8% and 88.7%, respectively. The positive predictive values of HRT, RAST and ST were 75.5%, 38.8% and 66.7%. The false positive ratio of HRT (24.5%) was the lowest among all the tests. There was a significant correlation between HRT and RAST (r = 0.513, p < 0.001). However, the concordance of HRT with respect to RAST was 56.0%. The concordance and specificity of HRT in relation to the clinical diagnosis were higher than RAST and the same as ST. The sensitivity of RAST was higher than that of HRT. From these results, we concluded that RAST is good for the screening of allergens and that HRT is a useful diagnostic method for the confirmation of a clinical allergy.

Pasteurella multocida keratitis and corneal laceration from a cat scratch.

A 24-year-old woman was evaluated 12 hours after she sustained a cat scratch to her left eye. Slit-lamp examination revealed a Seidel-positive corneal laceration with a surrounding dense full-thickness corneal ulcer and severe inflammatory reaction. Since the anterior chamber was well formed, it was decided not to repair the laceration on an emergency basis. She was initially treated with intensive topical fortified tobramycin and vancomycin, and intravenous gentamicin and clindamycin. Cultures of the corneal ulcer revealed Pasteurella multocida and the antibiotic regimen was adjusted appropriately. The laceration healed without surgery, and the infection resolved well, with excellent visual acuity.

Principles of organization of an obstetrical unit from scratch.

The previous discussion has attempted to outline some basic principles in developing a high-risk obstetrical unit from scratch. Needless to say, emphasis in specific areas will be dictated by community needs, financial resources, numbers and training of personnel, and space limitations. The basic functions of patient care, education, and self-evaluation can be accomplished in any high-risk obstetrical unit.
