[Psychological health status of students taking part in medical support for military training on the sea].

OBJECTIVE: To investigate the adaptability and psychological health status of the students from the military medical universities enrolled in the medical support service for military training on the sea, so as to provide more pertinent and specific psychological advice for students in military medical universities. METHODS: The psychological health status of 139 students were assessed by issuing a self-rating symptom scale (SCL-90) before the training and later before the maneuvers commenced, respectively. RESULTS: Except for obsessive-compulsive and interpersonal sensitivity factors, significant increases were recorded in the scores for other factors immediately before the maneuvers as compared with those recorded before the training (P<0.01, P<0.05). The positive symptom rates of various factors measured before the maneuvers were also higher. CONCLUSIONS: It is the essential task for daily psychological health education to help the students foster psychological self-regulation ability and cope with stresses to strengthen their psychological well-being. Building self-confidence, improving the adaptability and conquering fears should be the primary concern of the psychological advisors before military actions.

[Ongoing conflict situations and physical disease].

In most cases psychosomatic patients are not vitally threatened. Nevertheless almost half of the emergency patients have got psychical disturbances (e.g. neurotic heart disease, states of hyperventilation or vertigo). A reason is not only the personal development of early childhood. More often the problems are to be found within strained relations. The physical illness can be conceived as an (unconscious) attempt to master a conflict or loss, as narcissistic reparation, as adaption-effort or even as self destruction Selye's theory of stress presents the linkage between the organ pathology and the influence of the psychosocial situation. Examples of medical cases explain these correlations.

Initial characteristics of psychological development and evolution of the young autistic child.

This longitudinal study assessed multidisciplinary data on 219 children with autistic spectrum disorders from the median age of 5 (Time 1) to 8 years old (Time 2). The evolution of psychological and adaptive data was subjected to cluster analysis. Four clinically meaningful clusters emerged. The first group (21%) demonstrated the most important psychological transformations between the two times of the research. The second group (24%) made progress but less than group 1. The third and biggest group (30%) kept the same developmental slope. The fourth group (25%) showed no significant evolution between the two times of the research. This study highlights important differences among children with autism and the necessity of using a developmental view when considering the autistic syndrome.

Converging on a richer understanding of human behavior and experience through a blending of cognitive and clinical psychology.

McClelland, Kemps, and Tiggemann's (this issue) use of experimental methods typically used in cognitive psychology to reduce the intensity of food cravings formed the basis for maintaining that an understanding of human experiences and behaviors requires a blending of cognitive and clinical psychological approaches. Clinical psychology can adapt cognitive models of information processing to understand the mechanisms underlying clinical phenomena and to create and evaluate effective interventions. Cognitive psychology should broaden its scope to include information relevant to clinical phenomena, such as desire, attitudes, self-regulation, and temperament. Only through a blending of these two fields will we converge on a richer understanding of human behavior and experience.

Blunted cortisol response to a psychosocial stressor in schizophrenia.

Schizophrenia is considered a neurodevelopmental disorder in which vulnerability to stress may be a contributing factor. Coping is an important psychological component of stress processing, and the hypothalamic-pituitary-adrenal system (HPA system) is one of the biological components of stress adaptation. Disturbances of either of these components may make schizophrenic patients more vulnerable to develop a psychosis under stressful circumstances. In this study, 10 schizophrenic men were compared with 10 healthy male controls in their response to a psychosocial stressor, consisting of a public-speaking task. Heart rate was monitored as a measure of autonomic arousal. HPA responses were assessed by measuring salivary cortisol. Coping skills were measured by using the Utrecht Coping List and the Ways of Coping Checklist. The stress of speaking in public increased the heart rate in both patients and controls; however, a significant cortisol response was found in the controls, but not in the schizophrenic patients. The patients used more passive and avoidant coping strategies than controls. The findings provide support for the notion that schizophrenic patients have an impaired ability to adapt, both psychologically and biologically, to their environment.
