[Echomorphology of the prostate].

The echogenic structures of 1983 transrectal sonographies of the prostate were analysed. In addition, 93 dissected prostates were investigated in waterbath in order to correlate sonographic and morphological structures. Compared to the normal gland, hyperplastic nodules are of the same density or slightly echodenser, rarely also with a poorer echo. In about 60% the inner gland can be delineated. Although 52% of the carcinomas show the same echodensity as diffuse hyperplasia, diagnosis is still possible by further criteria. In only 13% no carcinomas were detectable. 18% of the carcinomas were differentiated by poor echogenicity. The diagnostic value is discussed.

[Comparison of prostate sonography and the corresponding large-area histological section].

The present paper reports on a systematic, comparative sonographic and histologic study of 50 fresh cadaver prostates from current autopsy material. Following removal of the performed. Transverse sections of sonographic interest were marked on the specimen using a laterally introduced needle under sonographic control. After appropriate preparation a large-area section from this marked plane was made and compared with the sonogram. With a number of diseases and with regard to the shape and size of the organ sections the correlation between histologic and sonographic findings was generally good.

[Computer-assisted evaluation of ultrasound images of the prostate].

The prostate was examined by sonography to diagnose tumourous changes in the organ. Several visual parameters were found to interpret the pictures. The ultrasonic images can be prepared and improved by means of image-processing algorithm were computed while evolving a method for the automatic evaluation of ultrasonic images. The value of this method and its results are discussed.

[Transrectal ultrasound tomography with an electronic line scanner].

Among the imaging techniques for the morphologic diagnosis of the prostate and seminal vesicles transrectal sonography has assumed a foremost position. With the aid of longitudinal sectional images of the prostate and seminal vesicles obtained by means of a newly developed transrectal linear scanner the typical sonic characteristics associated with morphologic alterations of these organs were well demonstrated in 58 patients. For clarifying sonographic findings suggestive of carcinomas this approach can be used for precisely guided transperineal and above all transrectal puncture. Sonographic imaging of the prostatic urethra has opened new aspects in the assessment of functional processes on the basis of morphologic changes.

[Pancreatic metastases: sonographic and computed tomographic findings].

We analysed sonographic (n = 19) and CT (n = 15) findings in 19 patients with intrapancreatic metastases (solitary: n = 8, multiple: n = 11) from solid tumours. The diagnosis was confirmed either histologically (n = 5) or by the follow-up showing decrease of the size of the lesion due to chemotherapy (n = 3) or progression without specific therapy (n = 11). Clinically or serologically organ-related symptoms were found in 5 cases, whereas the rest of the patients were free of symptoms. Sonographically the metastases appeared hypoechogeneic compared to normal pancreatic parenchyma. In 15 cases CT at the time of sonographic investigation demonstrated hypodense intrapancreatic lesions (8/15) or bulging of the organ contour without differences in density (2/15). In some patients (5/15) sonographically detected metastases measuring less than 2 cm were not discovered via CT. In a patient with known malignancy multiple intrapancreatic lesions must be considered as metastases. Differential diagnosis includes diffusely growing pancreatic carcinoma, haemorrhagic necrotising pancreatitis and focal infiltrates due to malignant lymphoma.

[The image of prostatic adenoma and cancer in transrectal sonography].

The transrectal ultrasonography of the prostate is introduced. The sonographic appearance of the adenoma and carcinoma in 357 histologically proven cases is evaluated. Shape, symmetry and definition of the capsule are more reliable differential criteria than the internal echogenicity. The sensitivity is 96.6% for adenoma and 84.2% for carcinoma, whereas the sensitivity of palpation is 91.7% and 77.5% respectively. 6 early cancers are detected. Since infiltration of the capsule and the seminal vesicles can be seen, staging of carcinoma is considerably improved. Accurate measurement of the prostatic size helps in operative planning. Indication and diagnostic value of the method are discussed.

Urethral patterns on micturition cystourethrography in adult males with subvesical obstruction.

Reexamination of the micturition cystourethrograms in 289 adult male patients with subvesical obstruction proved that the cause of the narrowing can be accurately diagnosed on the pattern of the urethral lumen. The radiologic and the final urologic diagnoses coincided in 87.5 per cent of all cases. The narrowing localized to the bladder outlet associated with short supracollicular urethra correlated well with the urologic diagnosis of bladder neck contracture. In this group 37.9 per cent of the cases presented evidence for an inflammatory etiology. 19 per cent of the same group had patterns which resembled very much nodular hyperplasia. Other possible etiologies for the bladder neck contracture did not appear to produce characteristic patterns.
