[Fatigue in women performing work with predominantly dynamic effort. II. The social working environment and the feeling of fatigue].

The relations between the sense of fatigue and evaluation of social factors demonstrated in the so called organizational climate of a workplace have been investigated. The test groups involved women performing major knitting industry jobs: knitters (94 persons) and sorters (44 persons). A multiple regression analysis demonstrated numerous relationships between the subjective symptoms of fatigue and social climate indices (10 indices have been observed). Among the social factors, especially work organization and interpersonal relationships (among workers themselves and workers in relation to their managers) may affect the magnitude of fatigue in knitting industry-employed women.

[Multi-shift work and work under conditions of jet lag. Questionnaire studies].

The authors discuss the disturbances in the biorhythm - sleep - waking in the workers who are on duty at the posts of command and in communication ouration pilots surpassing the time zones. The questionnaire method was used. The resultant desynchronization of the biological rhythm is exhibited mainly through the subjective feeling of fatigue and sleepiness. Adaptation and readaptation to changing working conditions occurs.

National High Blood Pressure Education Program Working Group Report on Hypertension in the Elderly. National High Blood Pressure Education Program Working Group.

Raised blood pressures in the elderly and the increased prevalence of hypertension in this population are not benign occurrences and should not be viewed as a normal or inevitable consequence of aging. In fact, the relation of systolic and diastolic blood pressures to cardiovascular events is generally more pronounced in people aged 65 years and older when compared with those aged 35 to 64. The relative risk of cardiovascular disease is greater among the elderly at every level of blood pressure. Furthermore, the absolute likelihood that an older individual will have a cardiovascular event is substantially greater than for someone younger, reflecting the increased prevalence of other cardiovascular disease risk factors in this age group. Thus, equivalent blood pressure reduction is likely to produce a greater benefit in the elderly than in younger patients at every level of blood pressure. This report, an update of the 1985 Working Group Report on Hypertension in the Elderly, has two aims: to guide clinicians in their care of elderly patients with hypertension and to assist health care professionals participating in high blood pressure control programs that serve the elderly. The role of lifestyle modifications--weight loss, dietary sodium restriction, alcohol reduction, and exercise--as definitive or adjunctive therapy to drug treatment is discussed. In addition, the report reviews the relative advantages and disadvantages of the specific classes of antihypertensive medications.

[The dust factor of the work and adjacent environments in the working of manganese ore].

The dust professional exposition of the above ground workers in the mines "Obrochishte" at the village of Tsŭrkva, Varna Province has been evaluated as well as the dust pollution in the atmosphere of the village in respect to the hygienic standards for quartz-containing sand and manganic aerosol. The results show that the pollution with quartz-containing dust is in the normal ranges according to all hygienic parameters for fibrogenic dusts in working environment and the workers are not exposed to risk of silicosis. A greater than the standard exposition to manganic aerosol (from 1.1 to 5 PDK) has been found out in some groups of workers in the ore-dressing plant, in truck drivers of the intra-fabric transport, in fadromists and heavy scraper workers. The manganic aerosol is found to be a pollutant of the atmosphere also, and the cause of this is thought to be the mine storages in the open-air and the poor state of the intra-fabric roads.

[Basic-diagnostic-criterions of respiratory insufficiency--results of a community work of the working group "pathophysiology of breathing" (author's transl)].

11 laboratories of the working group "Pathophysiology of Breathing" cooperated with standardized methods to obtain generally admitted practical criterions for the definition of respiratory insufficiency. Function parameters of 2800 examined persons in different diagnostic groups were tested by manual statistical analysis and computed by data processing system. which were assessed with regard to their usefulness as criterions for respiratory insufficiency. The arterial O2-partial pressure proved to be the most crucial criterion of respiratory insufficiency permitting objective evidence of a manifest respiratory insufficiency by hypoxeamia under standard efforts with a bicycle-ergometer. The screening parameters of ventilation such as vital capacity and forced exspiratory volume are suitable as subjective rapid methods for a quantitative estimation of one type of respiratory insufficiency: the ventilation disturbance. Investigations, which were carried out with a higher apparatus expense like the estimation of residual volume, the functional residual capacity, and the arterial CO2-partial pressure permit the further differentiation of causes of the respiratory insufficiency and the separation between respiratory partial- and global insufficiency. The parameters of gas exchange: O2-consumption, respiratory quotient, and the screening parameter of cardiocirculation heart rate are not or only limited useful as criterions of respiratory insufficiency alone.

Cardiac pacing in Canada in 1998: working towards optimal pacing therapy. Canadian Working Group on Cardiac Pacing.

The Canadian Working Group on Cardiac Pacing (CWGCP) was formed in 1996 with the primary goal of promoting optimal pacing therapy in Canada. In 1997, the CWGCP conducted a survey of pacing practices across Canada. Ninety-two of 125 implanting programs (74%) responded. Implant rates vary by province--from 39 per 100,00 population in Ontario to 63 per 100,000 population in Nova Scotia and Prince Edward Island. Variations in regional implant rates persist even after correcting for the age of the population. Physiological pacing was used for 35% of all implants in Canada in 1996/97. There were marked differences across Canada in the mode of pacing selected. In western Canada, 39.5% of pacing systems implanted were physiological compared with 18.2% in Atlantic Canada and 29% in Quebec. There were also differences in follow-up practices. Approximately 40% of centres follow patients with single chamber pacemakers annually, whereas most other centres still follow these patients every six months. Economic constraints, the size of pacing programs and the involvement of committed pacing physicians are factors that may influence the regional differences in cardiac pacing across Canada.
