[Effect of vibration on gamma-aminobutyric acid metabolism in the brain in various functional conditions of the adrenal cortex].

The low-frequency vibration during 30 min (20 Hz, A = 0.4 mm) has been studied for its influence on the level of components of the GABA system and dicarbonic ++amino acids in male rats at hypo- and hyperfunction of the adrenal cortex. It is shown that under these conditions of the experiment the GABA level and glutamate-decarboxylase activity increase. Hyperfunction of the adrenal cortex against the background of vibration causes a relatively less pronounced increase in the GABA content, than the vibration alone or against the background of inhibition of adrenocortical function in the organism.

Chemical shift magnetic resonance imaging: applications in the abdomen and pelvis.

Chemical shift imaging allows unambiguous separation of lipid and water signals. Useful images include opposed-phase (water minus fat) and fat-suppressed images, which can be obtained with spin-echo or gradient-echo methods. Chemical shift techniques can be used to reduce artifacts, to expand dynamic range, to diagnose fatty infiltration of the liver, to distinguish between benign and malignant adrenal lesions, and to diagnose benign teratomas in women.

[Contribution to the study of endocrinopathies in black Africans (author's transl)].

For a long time endocrine pathology was unrecognized or even denied to exist in Black Africans, but this is not true. It seems to be recognized more frequently since medicalization has increased. This pathology does not present any particularities. Hyperthyroidism appears frequently, at least in urban areas. A series of 87 personal cases has permitted a review of the principal clinical and biological aspects; myasthenia seems to be particularly important; radical treatment is preferred over prolonged medical treatment which is not within the financial possibilities of our patients. There have been 30 observations of cortico-adrenal anomalies of which 7 known personally. It must be noted that the Black living in his traditional rural environment presents a certain degree of physiological hypocorticism with respect to whites and especially Blacks of higher social standing. Pathological hypocorticism is essentially due to tuberculosis; the melanodermia is evident to an alert observer. Metabolic hypercorticism produces the usual manifestations. Only 9 cases of Sheehan syndrome have been reported (5 personal). In the Black African woman, it appears earlier than in Europe often due to premature nuptiulity and the higher incidence of tachyparity. One is surprised that this syndrome is detected so rarely; it is, without a doubt, often well tolerated during a long period of time therefore goes undetected.

Adrenal crisis in the setting of high-dose ketoconazole therapy.

We describe a patient with type I diabetes mellitus and hypothyroidism who developed frank adrenocortical insufficiency while receiving a high-dose ketoconazole therapy for keratitis caused by Acanthamoeba species. While impaired cortisol responses to corticotropin and mildly symptomatic hypoadrenalism have been described previously with ketoconazole therapy, to our knowledge, this case represents the first documented article of an actual adrenal crisis associated with this drug. Two reasons are postulated for the development of this complication in our patient: high-dose ketoconazole therapy given in divided doses during the day, and a possibly impaired central response to stress because of hypothyroidism. Our article points to the need to monitor patients treated with high-dose ketoconazole for adrenal insufficiency, particularly if associated illnesses are present that may impair an adequate stress response.

Adrenocortical postirradiation fibrosis.

At autopsy, the adrenal zona reticularis showed linear hyaline fibrosis in 19 cases of cancer, after averages of six months and 5,000 roentgens of irradiation to the abdominal, lumbar, or pelvic regions. The associated roentgen dose was significantly greater than when postirradiation adrenal fibrosis was absent. Primary radiation damage to the reticular vascular plexus is suspected.

[Surgical and endocrinological results after the removal of hormonally inactive pituitary adenomas by means of frontal craniotomy].

The pre- and postoperative endocrinological findings are evaluated in 128 patients suffering from pituitary adenoma without endocrine activity, undergoing surgery in the Department of Neurosurgery, University Hospital, Zürich, in the 10-year period between 1962 and 1971. The preoperative examination showed hypothyroidism in 24% of all cases. 59% of the males showed primary or secondary hypogonadism. All females in the age between puberty and menopause complained of menstrual disturbances. One patient complained of preoperative diabetes insipidus. The postoperative examination showed that the function of a partially insufficient pituitary may improve after surgery. Hypothyroidism and adrenal insufficiency became normal in 2/3 of the cases. Male hypogonadism improved in 33% of the cases. One female gave birth to a normal baby after an uncomplicated pregnancy 6 years after pituitary surgery. 16% of the cases suffered from postoperative permanent diabetes insipidus. The operative complication rate (brain edema, intracranial bleeding, epilepsy) was 32%. The immediate surgical mortality was 7% (9 patients). Recurrent adenomas were found and operated on in 9% of the cases (11 patients). Three patients had two recurrences.
