Cytotoxicity of Kuwait weathered lake crude oil on rat hepatocytes: a histological and ultrastructural study.

In the present study, the cytotoxic effects of Kuwaiti weathered crude oil and a potent carcinogen (DMBA) on rat liver cells were examined by light and electron microscopy at each of 4 sampling periods after the start of low dosing (0.5 and 0.2 mg/kg) of feed. Such effects were compared with those of olive oil and uncontaminated food-exposed controls. The results confirm a pronounced cell damage which statistically not significant (p < 0.05). In crude oil, the organelle changes were variable and highly comparable to that of DMBA. The nuclei were mostly disintegrated while the cell showed demarcation of cytoplasmic vacuolization, lipid augmentation, and mitochondrial aberrations. The latter showed a remarkable association with the rough endoplasmic reticulum and lipid droplets, and appeared as decayed and diffused structures within the cell matrix. There was no comparable changes in the hepatocytes of animals fed with uncontaminated food except for the formation of lipid droplets in the olive oil-fed groups. Although the animals food was contaminated with Kuwaiti weathered oil formed in 1991 were exposed to extreme seasonal temperatures, yet the residues of such oil led to severe histopathological alterations in the liver cells which were similar to those of DMBA-treated cells. There is the need to pay attention to potential hazardous effects of the crude oil on environments.

Basic gait parameters: a comparison of reference data for normal subjects 20 to 29 years of age from Kuwait and Scandinavia.

This study obtained measurements of the spatiotemporal gait parameters of healthy young adult Kuwaiti subjects from both genders and compared the data to those collected in a similar study performed in Sweden. Thirty healthy subjects volunteered to participate in the study (which included being asked to walk at their "free," "slow," and "fast" self-selected speeds). We collected the spatiotemporal gait data using an automated system. Descriptive statistics were calculated for each variable measured at each walking condition. The data were then compared to those from the Swedish study. The results indicate several significant differences between Kuwaiti and Swedish subjects in their manner of walking. These results suggest a need to include data from subjects with diverse cultural backgrounds when a database on normal gait is developed or a need to limit the results of the database to a specified ethnic population.

Associations among job satisfaction, optimism, pessimism, and psychosomatic symptoms for employees in the government sector in Kuwait.

The present study examined associations among job satisfaction and optimism, pessimism, and psychosomatic symptoms in a sample of 718 Kuwaiti employees (350 men and 368 women). Men scored significantly higher than women on both the scales of Job Satisfaction and Optimism, while women scored significantly higher than men on both scales for Pessimism and Psychosomatic Symptoms. Job satisfaction scores correlated significantly and positively with Optimism scores and negatively with Pessimism scores. There was also a significant negative correlation between scores on Optimism and Pessimism, and a significant negative correlation between scores on Optimism and Psychosomatic symptoms. Results were discussed within the Kuwaiti context.

Pregnancy outcome after female infertility in Kuwait. Comparison of medical and surgical treatment.

During a 4-year period, 163 patients conceived after treatment of their infertility due solely to a female factor. They were divided into two groups, 108 patients treated medically (Group 1) and 55 treated surgically (Group 2). At the time of conception, the mean age in Group (1) was 24.9 +/- 5.7 years while in Group (2) it was 31.2 +/- 6.1 years. The difference was statistically significant (p less than 0.01). The duration of infertility prior to pregnancy and the treatment-to-pregnancy interval were both significantly longer in Group (2) (p less than 0.05). The rates of miscarriage, multiple pregnancy, and preterm labor were higher in Group (1), though the differences were statistically not significant. In contrast, the rate of ectopic pregnancy was significantly higher in Group (2) (p less than 0.05), as was the need for elective cesarean section (p less than 0.01). Despite these differences, both groups had nearly the same perinatal outcome.

Learning objectives in training residents in general surgery in Kuwait.

An opinion survey of a five-year postgraduate training programme in general surgery was conducted using a questionnaire. The residents who responded indicated that the use of learning objectives needed to be given greater emphasis. They felt that learning objectives should be more clearly defined and be communicated with the learners and the instructors at an early stage in the programme. While the opportunity for learning theoretical concepts in surgery was rated as a strong feature of the programme, learning practical skills, it was pointed out, needed augmenting.

Characteristics of men and women with circulating antisperm antibodies in a combined infertility clinic in Kuwait.

Antisperm antibodies were determined in the sera of 250 infertile couples and 100 puerperal women as controls using the immunofluorescence technique. Couples with significant circulating antisperm antibodies were placed on low-dose prednisolone 5 mg daily for 3-6 months. Initial routine semen analysis and hypoosmotic swelling test were done and repeated after 3 months of therapy. The incidence of antisperm antibodies (ASA) was 18.8 and 17.6% in the men and women, respectively, compared to 4% in the women controls (p < .02). In the men, the main determinants (with incidence) of ASA included smoking (33.9%), past history of sexually transmitted disease (33.3%), surgery to genital tract (28.6%), trauma (27.3%), and unexplained infertility (18.5%). In women whose husbands had antisperm antibodies the incidence of circulating antisperm antibodies was 38.3%, while endometriosis and thyroid dysfunction had incidence of antisperm antibodies of 21.4 and 16.7%, respectively. In the 27 (10.8%) case of unexplained infertility, the incidence of antisperm antibodies was 22.2%. High follicle-stimulating hormone (FSH) in the men and low midluteal-phase progesterone in the women were associated with increased expression of antisperm antibodies. Antisperm antibodies adversely affected quality of sperm. Low-dose prednisolone significantly reduced the titer of antisperm antibodies and improved the sperm parameters and conception rate.

Behçet's disease in Kuwait, Arabia. A report of 29 cases and a review.

Twenty-nine patients with Behçet's disease were studied. Of these, 17 patients were followed up for a mean duration of 37 months and 12 for a mean of 18 months. Male to female ratio was 3.1:1. Of the 29 patients, 26 were Arab, 2 Asian and 1 Black African. The prevalence in the country was 2.1:100,000 population, in Kuwaitis 1.58:100,000, in non-Kuwaiti Arabs, 2.9:100,000 and in non-Arabs, 1.35:100,000 population. The frequencies of the various manifestations were: oral ulcers, 100%; genital ulcers, 93%; skin manifestations, 76%; arthropathy and ocular disease, 69% each; psychiatric disorders, 38%; vascular complications and positive Pathergy test, 34% each; gastrointestinal symptoms, 21%; oesophageal and CNS involvement, 14% each; and peritonism, renal and pulmonary involvement, 7% each. Kuwaiti bedouins seemed to be resistant to the disease, probably due to environmental and/or dietary factors. The hot arid climate of the Gulf had no effect on the course of the disease once it had developed.

Kuwait drug addiction scene: a changing pattern?

Sociocultural considerations and persistence of indiscriminate prescription habits for addicts may have contributed in shifting the pattern of misused drugs from traditional alcohol and illicit drugs to predominantly iatrogenic dependence on benzodiazepines. The clinical profile and the rationale for the long-adopted detoxification treatment of 120 patients were studied. Multiple-drug misusers were found to suffer significantly more anxiety-type symptoms suggestive of benzodiazepine withdrawal. Poor correlation of detoxification medication with patient variables suggests that such treatment lacks the rationale for its continuation and should be replaced by treatments based on proper physical and psychiatric evaluation. Benzodiazepine dependence should be recognized as a clinical entity requiring relevant treatment strategies.

Self-esteem and trait anxiety in relation to drug misuse in Kuwait.

This study was designed to document knowledge about Kuwaiti drug users and to investigate whether or not there is an association between their poor self-concept and high level of anxiety. One hundred and seven incarcerated drug users, 107 individuals serving prison terms for offenses other than drug use, and 107 "normal" individuals were included in this pilot study. The Arabic version of Rosenberg's Self-Esteem Scale and Spielberger's State-Trait Anxiety Inventory were used to measure the subjects' self-esteem and state-trait anxiety, respectively. The results documented revealed that there is a relationship between levels of self-esteem and anxiety in Kuwaiti drug user behavior.

Toxicological screening: a three year experience in poisonings in Kuwait.

The results of toxicological screening of body fluids (urine, blood and gastric lavage) from pediatric patients were analyzed for the period January 1985 - December 1988. Of the 119 cases, approximately one half were positive for at least one foreign substance. In about one fifth of the cases, multiple substances were detected. The most commonly implicated drugs were those acting on the central nervous system including the barbiturates, tricyclic antidepressants, phenothiazines, benzodiazepines and carbamazepine.

Apolipoprotein E polymorphism and lipoprotein levels in a Gulf Arab population in Kuwait: a pilot study.

BACKGROUND: APOE polymorphism is believed to confer susceptibility to coronary heart disease (CHD) and Alzheimer's disease. It is well known that patterns of APOE polymorphisms differ between populations and ethnic groups, although most of the data available so far have been in whites. SUBJECT AND METHODS: We evaluated the frequencies of APOE genotypes and their relationships with serum levels of lipids, lipoproteins and apolipoproteins in two groups of Gulf Arab citizens: a control population of healthy voluntary blood donors (n=106), and a group of patients presenting to the lipid clinic for the first time with combined hyperlipidaemia (CH) (n=41). In both groups, fasting serum total cholesterol (TC), triglycerides (TG), HDL, LDL and apolipoprotein A1 and B levels were measured by routine autoanalyzer methods, and APOE genotyping was performed by validated PCR methods. The lipid and lipoprotein levels were related to the specific APOE allele frequencies. RESULTS: Allele frequencies were 5.7% for *E2, 85.4% for *E3, and 9.0% for *E4 in the healthy blood donor group. An essentially similar pattern was seen in the patients with CH. This APOE allelic distribution conforms to patterns described in Chinese, whites and South Asians. In both the blood donor and CH groups there were no consistent links between specific APOE pattern and serum lipoproteins, as would have been predicted from APO *E2 and APO *E4 frequencies. CONCLUSIONS: We conclude that APOE allelic patterns in healthy Kuwaiti blood donors and a smaller group of patients with CH do not satisfactorily predict circulating blood levels of lipids and lipoproteins.
