The degradation of two mitotic cyclins contributes to the timing of cytokinesis.

BACKGROUND: Cytokinesis occurs just as chromosomes complete segregation and reform nuclei. It has been proposed that cyclin/Cdk kinase inhibits cytokinesis until exit from mitosis; however, the timer of cytokinesis has not been experimentally defined. Whereas expression of a stable version of Drosophila cyclin B blocks cytokinesis along with numerous events of mitotic exit, stable cyclin B3 allows cytokinesis even though it blocks late events of mitotic exit. We examined the interface between mitotic cyclin destruction and the timing of cytokinesis. RESULTS: In embryonic mitosis 14, the cytokinesis furrow appeared 60 s after the metaphase/anaphase transition and closed 90 s later during telophase. In cyclin B or cyclin B3 mutant cells, the cytokinesis furrow appeared at an earlier stage of mitosis. Expression of stable cyclin B3 delayed and prolonged furrow invagination; nonetheless, cytokinesis completed during the extended mitosis. Reduced function of Pebble, a Rho GEF required for cytokinesis, also delayed and slowed furrow invagination, but incomplete furrows were aborted at the time of mitotic exit. In functional and genetic tests, cyclin B and cyclin B3 inhibited Pebble contributions to cytokinesis. CONCLUSIONS: Temporal coordination of mitotic events involves inhibition of cytokinesis by cyclin B and cyclin B3 and punctual relief of the inhibition by destruction of these cyclins. Both cyclins inhibit Pebble-dependent activation of cytokinesis, whereas cyclin B can inhibit cytokinesis by additional modes. Stable cyclin B3 also blocks the later return to interphase that otherwise appears to impose a deadline for the completion of cytokinesis.

Recent advances in the Doppler study of early fetomaternal circulation.

Transvaginal color Doppler has enabled precise investigation of feto-maternal circulation in early pregnancy. Decrease of peripheral impedance, as well as increase of blood flow velocity was found in all segments of the uteroplacental circulation with gestational age. The same finding was observed in umbilical artery, fetal aorta and intracranial blood vessels. A significant decrease of vascular impedance in cerebral blood vessels exists earlier than in fetal aorta or umbilical artery which could be explained by the establishment of cerebral autoregulation mechanisms which ensure adequate oxygen and glucose supply to embryonic brain. There was no significant difference for RI values of luteal blood flow from the 6th to the 12th gestational week. No difference was found between RI values of luteal blood flow in normal and ectopic pregnancy. In threatened, incomplete and missed abortions RI of luteal blood flow was significantly higher in comparison with normal pregnancy. It seems that in the group of patients with threatened abortion, follow up of the luteal flow might give some prognostic value. No difference in peripheral impedance and blood flow velocity of the uteroplacental blood vessels was found between women with normal pregnancies and women with threatened abortions and normal pregnancy outcome. There was no significant difference in terms of RI and PSV in uteroplacental blood vessels between women with threatened abortion and normal pregnancy outcome and women with threatened abortion whose pregnancy terminated with complete abortion and blighted ovum. In 46% of 21 women with visible retrochorionic hematoma RI in spiral arteries was higher and PSV was lower on the side of the hematoma. These values could be explained by compression of the wall of the spiral arteries caused by hematoma. In missed abortion the mean RI value of the uterine artery was lower than the mean value in normal pregnancy, but was not in spiral arteries. Some authors reported RI values in cases of missed abortions around and above the cutoff point value of 0.63. In blighted ovum RI in uterine and spiral arteries was lower in comparison to normal pregnancy, but in the other studies this was not the case. Increased blood flow in radial arteries in patients with pregnancy associated with uterine myomata could be explained by higher levels of estriol hormone metabolized in the placenta. Transvaginal color Doppler could be helpful in diagnosis and differentiation of the different forms of a gestational trophoblastic disease. It seems that this technique is a good substitute for pelvic angiography.(ABSTRACT TRUNCATED AT 400 WORDS)

The first three weeks assessed by transvaginal color Doppler.

Transvaginal color Doppler has made possible to study ovarian and uterine perfusion in non-pregnant and pregnant patients, thus advancing the understanding of the early human development. RI of follicular blood flow starts decreasing prior to ovulation reaching its nadir at ovulation. It is considered that apart from hormonal factors the angiogenesis is also involved. The mature corpus luteum shows increased blood flow velocity in relation to preovulatory follicle. Comparing RI values of luteal blood flow of normal and ectopic pregnancy no difference was found. But in threatened, incomplete and missed abortions the resistance and pulsatility indices were significantly higher than in normal pregnancy. The follow up of the luteal flow might have a prognostic value in a group of patients with threatened abortion. In women with spontaneous cycles the day preceding the ovulation impedance to uterine flow velocity starts decreasing. Alterations in flow velocity patterns of the radial and spiral arteries in spontaneous ovulatory cycles are paralleling blood flow dynamics of the uterine arteries. In stimulated cycles RI increases the day before ovulation in both the uterine arteries and their branches. It seems that endometrial perfusion presents more accurate noninvasive assay of uterine receptivity than uterine artery perfusion alone. Endometrial receptivity is maximum during the time of peak luteal function during which implantation is most likely to occur. During the pregnancy impedance to blood flow decrease from the main uterine to the spiral arteries as well as with the advancing gestational age. The spiral arteries in pregnancy become the vessels with completely different haemodynamic characteristics in relation to other arteries of uteroplacental circulation. Color Doppler adds new information on perfusion and pathophysiological changes connected with the ectopic trophoblast implantation.

Report of survey: pregnancy outcomes in medical laboratory technologists.

A survey was published and distributed in the CSLT Bulletin. Completed surveys were returned anonymously by mail. Of the surveys returned (n = 2002), 16 were not usable due to incomplete information. Spontaneous abortion (SA) occurred in 19.9% of the total responders' pregnancies. Technologists not employed during pregnancy had a lower rate of SA (13.2%) compared to those working full-time (19.9%) (p < 0.001). Including all types of early (< 28 wk) pregnancy terminations (except induced abortion) the non-employed group had a 17% incidence of fetal loss compared to 23% in the group employed full-time (p < 0.001). The differences in SA rate could not be accounted for by mother's age at pregnancy nor previous obstetric history. The incidence of cesarean section was higher in the full-time employed group (16%) compared to the non-employed group of technologists (9.9%) (p < 0.001). There were 141 reported birth defects representing an overall incidence in responders of 4.8%, comparable to literature values for the general population (6.0%). The incidence of specific birth defects was not different in the working group.

Pulmonary edema possibly developing secondary to the intravenous administration of oxytocin.

Oxytocin, a posterior pituitary hormone, is commonly used for induction of labor, stimulation or reinforcement of labor, management of incomplete or inevitable abortion and control of post partum bleeding. We describe a case of acute pulmonary edema possibly developing secondary to the administration of iv oxytocin. Clinicians should be aware of the potential for pulmonary edema secondary to iv oxytocin. Close hemodynamic monitoring should be done during oxytocin therapy.

Choice of analgesia or anesthesia for pain relief in suction curettage.

One hundred and fifteen patients admitted to Duke University Medical Center from January 1, 1973, to December 31, 1974 inclusive, with the diagnosis of uncomplicated spontaneous incomplete or inevitable abortion were included in a randomized prospective study. All patients underwent suction curettage under either analgesia or general inhalation anesthesia. Patient response was adjudged regarding comfort and cooperativeness in the group receiving analgesia and postprocedure rehabilitation and discharge times in both groups. Fifteen of 59 patients receiving analgesia only were recorded unanimously as failures. Post-operative ambulation was no more rapid in patients receiving analgesia. Procedure-to-discharge intervals were unrelated to medication modality. There appears to be no advantage to analgesia as far as rehabilitation time, procedure-to-discharge time, and medical complications are concerned.

[Ultrasonography in acute pelvic pain].

Acute pelvic pain may be the manifestation of various gynecologic and non-gynecologic disorders from less alarming rupture of the follicular cyst to life threatening conditions such as rupture of ectopic pregnancy or perforation of inflamed appendix. In order to construct an algorithm for differential diagnosis we divide acute pelvic pain into gynecologic and non-gynecologic etiology, which is than subdivided into gastrointestinal and urinary causes. Appendicitis is the most common surgical emergency and should always be considered in differential diagnosis if appendix has not been removed. Apart of clinical examination and laboratory tests, an ultrasound examination is sensitive up to 90% and specific up to 95% if graded compression technique is used. Still it is user-depended and requires considerable experience in order to perform it reliably. Meckel's diverticulitis, acute terminal ileitis, mesenteric lymphadenitis and functional bowel disease are conditions that should be differentiated from other causes of low abdominal pain by clinical presentation, laboratory and imaging tests. Dilatation of renal pelvis and ureter are typical signs of obstructive uropathy and may be efficiently detected by ultrasound. Additional thinning of renal parenchyma suggests long-term obstructive uropathy. Ruptured ectopic pregnancy, salpingitis and hemorrhagic ovarian cysts are three most commonly diagnosed gynecologic conditions presenting as an acute abdomen. Degenerating leiomyomas and adnexal torsion occur less frequently. For better systematization, gynecologic causes of acute pelvic pain could be divided into conditions with negative pregnancy test and conditions with positive pregnancy test. Pelvic inflammatory disease may be ultrasonically presented with numerous signs such as thickening of the tubal wall, incomplete septa within the dilated tube, demonstration of hyperechoic mural nodules, free fluid in the "cul-de-sac" etc. Color Doppler ultrasound contributes to more accurate diagnosis of this entity since it enables differentiation between acute and chronic stages based on analysis of the vascular resistance. Hemorrhagic ovarian cysts may be presented by variety of ultrasound findings since intracystic echoes depend upon the quality and quantity of the blood clots. Color Doppler investigation demonstrates moderate to low vascular resistance typical of luteal flow. Leiomyomas undergoing degenerative changes are another cause of acute pelvic pain commonly present in patients of reproductive age. Color flow detects regularly separated vessels at the periphery of the leiomyoma, which exhibit moderate vascular resistance. Although the classic symptom of endometriosis is chronic pelvic pain, in some patients acute pelvic pain does occur. Most of these patients demonstrate an endometrioma or "chocolate" cyst containing diffuse carpet-like echoes. Sometimes, solid components may indicate even ovarian malignancy, but if color Doppler ultrasound is applied it is less likely to obtain false positive results. One should be aware that pericystic and/or hillar type of ovarian endometrioma vascularization facilitate correct recognition of this entity. Pelvic congestion syndrome is another condition that can cause an attack of acute pelvic pain. It is usually consequence of dilatation of venous plexuses, arteries or both systems. By switching color Doppler gynecologist can differentiate pelvic congestion syndrome from multilocular cysts, pelvic inflammatory disease or adenomyosis. Ovarian vein thrombosis is a potentially fatal disorder occurring most often in the early postpartal period. Hypercoagulability, infection and stasis are main etiologic factors, and transvaginal color Doppler ultrasound is an excellent diagnostic tool to diagnose it. Acute pelvic pain may occur even in normal intrauterine pregnancy. This may be explained by hormonal changes, rapid growth of the uterus and increased blood flow. Ultrasound is mandatory for distinguishing normal intrauterine pregnancy from threatened or spontaneous abortion, ectopic pregnancy and other complications that may occur in patients with positive pregnancy test. Incomplete abortion is visualized as thickened and irregular endometrial echo with certain amount of intracavitary fluid. If applied, color Doppler ultrasound reveals low vascular resistance signals in richly perfused intracavitary area. Transvaginal sonography has high sensitivity and specificity in visualization of uterine and adnexal signs of ectopic pregnancy. Color Doppler examination may aid in detection of the peritrophoblastic flow. Furthermore, it facilitates detection of ectopic living embryo, tubal ring or unspecific adnexal tumor. Corpus luteum cysts and leiomyomas are another cause of pelvic pain during pregnancy, which can be correctly diagnosed by ultrasound. Detection of uterine dehiscence and rupture in patients with history of prior surgical intervention on uterine wall relies exclusively on correct ultrasound diagnosis. In patients with placental abruption sonographer detects hypoechoic complex representing either retroplacental hematoma, subchorionic hematoma or subamniotic hemorrhage. In closing, ultrasound has already become important and easily available tool which can efficiently recognize patients with possibly threatening conditions of different origins.

Menstrual regulation by mifepristone plus prostaglandin: results from a multicentre trial. World Health Organization Task Force on Post-Ovulatory Methods of Fertility Regulation.

A multicentre trial was conducted to evaluate whether a combination regimen of a 600 mg dose of the antiprogestogen, mifepristone, followed 48 h later by a 1 mg vaginal suppository of the prostaglandin analogue, gemeprost, could be an effective pharmacological method for menstrual regulation. A total of 228 women with menstrual delay of < or = 11 days were studied in nine centres; 35 of the women (15%) received the treatment without having been pregnant as determined by human chorionic gonadotrophin measurements performed retrospectively. The mifepristone-gemeprost regimen induced vaginal bleeding in all of the pregnant women and in all but two of the non-pregnant women, and a complete abortion in all but four of the 193 pregnant subjects. The four failures included one incomplete and two missed abortions and one continuing live pregnancy. Most women reported lower abdominal pain during the first few hours after gemeprost administration but few, if any, other side-effects were observed. Most non-pregnant women described the induced vaginal bleeding to be comparable with--albeit somewhat longer than--a normal menstruation. In pregnant women the duration of the bleeding was on average longer, and the amount greater, than normal menstruation, but haemoglobin concentrations in these women as well as in the non-pregnant subjects were not affected. The overall success rate (97.4%; 95% confidence interval: 94.4-99.0%) of the regimen employed in this pilot trial strongly suggests that the combination of an antiprogestogen followed by a suitable prostaglandin is potentially useful for menstrual regulation and probably also as a late luteal, once-a-month contraceptive.(ABSTRACT TRUNCATED AT 250 WORDS)

Criminal abortions as seen in the University Teaching Hospital, Lusaka.

Every year a large number of patients who are spontaneously aborting are admitted to the Gynaecological ward of the University Teaching Hospital (Lusaka) at various clinical stages of septic, incomplete, inevitable or threatened abortion. An undetermined number of these patients are induced outside the hospital. Analysis of data revealed that 60% of admissions in the gynaecological admission ward are abortions and a number of them present with features of sepsis. In a 4 year study period there were 15 maternal deaths as a result of septic/criminal abortions. Various methods are used to procure criminal abortions; out of which a case of a patient who used a knitting needle to procure abortion is presented.

Spontaneous abortion: a medical approach to management.

The conventional management of spontaneous abortion is surgical evacuation of the uterus to prevent complications which may arise from retained products of conception (POC). This procedure is not without complications and also demands operating theatre resources. The purpose of this study was to determine the efficacy of a medical approach to the management of spontaneous abortion using the prostaglandin analogue, gemeprost. From an initial 212 women presenting with spontaneous abortion, 4 groups were defined according to what they required in the way of initial management. A group of 20 women were excluded from conservative medical management at presentation because they were bleeding heavily and judged to be unsuitable. Sixty women initially required no further management other than determining that they had a complete abortion using transvaginal sonography (TVS). The 132 women were treated with the prostaglandin analogue, gemeprost. Sixty were judged to need no further management afterwards and were initially discharged. The remaining 72 women underwent evacuation of retained products of conception (ERPC) after the gemeprost treatment as medical evacuation of the uterus appeared to had been incomplete. In each of these 4 groups so defined, there was 1 case where an ERPC was required after the patients have been discharged because of complications attributable to retained POC. We therefore found that no ERPC was necessary in the management of 118 out of 212 (55.6%) women who presented with spontaneous abortion. This has important implications in patient management and economical use of hospital resources.

A comparison of medical management with misoprostol and surgical management for early pregnancy failure.

BACKGROUND: Misoprostol is increasingly used to treat women who have a failed pregnancy in the first trimester. We assessed the efficacy, safety, and acceptability of this treatment in a large, randomized trial. METHODS: A total of 652 women with a first-trimester pregnancy failure (anembryonic gestation, embryonic or fetal death, or incomplete or inevitable spontaneous abortion) were randomly assigned to receive 800 microg of misoprostol vaginally or to undergo vacuum aspiration (standard of care) in a 3:1 ratio. The misoprostol group received treatment on day 1, a second dose on day 3 if expulsion was incomplete, and vacuum aspiration on day 8 if expulsion was still incomplete. Surgical treatment (for the misoprostol group) or repeated aspiration (for the vacuum-aspiration group) within 30 days after the initial treatment constituted treatment failure. RESULTS: Of the 491 women assigned to receive misoprostol, 71 percent had complete expulsion by day 3 and 84 percent by day 8 (95 percent confidence interval, 81 to 87 percent). Treatment failed in 16 percent of the misoprostol group and 3 percent of the surgical group (absolute difference, 12 percent; 95 percent confidence interval, 9 to 16 percent) by day 30. Hemorrhage or endometritis requiring hospitalization was rare (1 percent or less in each group), with no significant differences between the groups. In the misoprostol group, 78 percent of the women stated that they would use misoprostol again if the need arose and 83 percent stated that they would recommend it to others. CONCLUSIONS: Treatment of early pregnancy failure with 800 microg of misoprostol vaginally is a safe and acceptable approach, with a success rate of approximately 84 percent.

The experience of termination of second trimester pregnancies using gemeprost vaginal pessaries in a district general hospital.

Experience with the use of Gemeprost pessaries (prostaglandin E1 analogue), in a group of 60 consecutive patients for second trimester termination in a Scottish District General Hospital, is presented. Gestation ranged from 12 to 20 weeks; there were 29 nulliparous (48%) and 31 (52%) multiparous women. Fifty-seven (95%) aborted within 36 hours; 29 (48%) aborted completely and did not require further uterine curettage. Nulliparous patients required more pessaries and the abortion was more often incomplete. Side effects were minimal. There were no serious problems; and antiemetics were required by 17% and parenteral analgesics by 13% of patients.

Prophylaxis against prostaglandin-induced gastrointestinal side effects.

Prostaglandin F2 alpha was administered intravenous drip in 130 patients with missed, incomplete inevitable and septic abortion, intrauterine death and vesicular mole and for therapeutic termination of midtrimester pregnancies. In 84 patients (control group), no prophylactic antiemetic or antidiarrheal drugs were administered, while in 46 patients (study group), an antiemetic (prochlorperazine) and an antidiarrheal (diphenoxylate hydrochloride with atropine sulfate) drug were administered prophylactically before and during prostaglandin infusion. The incidence in vomiting and diarrhea was statistically much less in the study group (P less than 0.0005 for vomiting and P less than 0.005 for diarrhea). There was no statistically significant difference in the success rate of prostaglandin induction in the two groups.

Abortifacient and endocrine effects of metergoline in beagle bitches during the second half of gestation.

The purpose of this study was to investigate the abortifacient effects of high doses of metergoline when administered to pregnant beagle bitches during the second half of gestation and to define the endocrine effects of this treatment as represented by plasma progesterone and estradiol concentrations. Previously, metergoline had been shown to be incompletely luteolytic and induced abortion in only one of eight pregnant bitches when 0.4-0.5 mg/kg were administered twice daily for 5 days from Days 18 to 20 of diestrus. Nine pregnancies in six beagle bitches were used for the present study. Three bitches were treated in each of two consecutive pregnant cycles. Metergoline was administered at a dose of 0.6 mg/kg per os twice daily, starting on Day 28 after the cytological onset of diestrus. Abortion was induced in eight of the nine treated pregnancies and started after 3-23 days of treatment (mean 12.5 days, S.D. 6.4 days). The abortions were completed within 0.5-8 days (mean 2.2 days, S.D. 2.7 days). There were no side effects associated with metergoline treatment and none of the abortions was associated with complications that required intervention. In the single bitch that did not abort, an ovarian granulosa cell tumor was discovered when the single fetus had to be removed surgically at term. Plasma progesterone concentrations declined after the start of metergoline administration in all pregnancies but levels below 4.8 nmol/l were required for successful abortions. Plasma estradiol concentrations showed a tendency to decline and fluctuate concurrently with the plasma progesterone levels. However, suppression of plasma estradiol concentrations by metergoline was not as complete as the suppression of progesterone and did not seem a prerequisite for abortion. The hormone profiles and treatment period required for abortion tended to be similar for both cycles of the three bitches that were treated during two consecutive pregnancies. This suggests a bitch effect on the factors that determine the efficacy of metergoline to induce abortion. The large variation and length of the treatment period that was required until abortion commenced was probably related to the relatively early start of treatment compared to other studies. The results of this investigation suggest that, similar to other prolactin suppressing ergot derivatives, metergoline causes complete luteolysis and can be used to reliably induce abortion only during the last 3 weeks of gestation.

Benign renal angiomyoadenomatous tumor: a previously unreported renal tumor.

We describe a unique benign tumor of the kidney in a 93-year-old man. Microscopically the tumor was composed of an intimate intermixture of epithelial component and smooth muscle tissue, which formed the stroma of the tumor. In some parts the epithelial cells in adenomatous tubular formations were endowed with clear snouts. These clear snouts had a blister-like quality and grew on the secretory cells lining the tubules. No atypias, mitoses, or pleomorphism were present in the tumor. The muscular component consisted of poorly cellular, HMB-45-negative, leiomyomatous bundles, which greatly differed from that of angiomyolipoma. It encircled the whole tumor and intimately intermingled with the epithelial component. These leiomyomatous bands formed focally abortive vessels, which had incomplete and irregular walls and lacked an elastic layer. Even more interesting was a peculiar vascularization of the tumor. All epithelial tubular structures of the tumor revealed an intimate association with small capillaries. A fine labyrinth of the capillaries rimmed the rows of the epithelial cells. To the best of our knowledge this tumor differs from all previously described.

Recurrent sacculation of the pregnant uterus. A case report.

BACKGROUND: Recurrent uterine sacculation is very rare and associated only with pregnancy. It commonly complicates delivery with a retained placenta. CASE: A 24-year-old woman, gravida 4, para 0, with three previous elective abortions, presented in preterm labor at 32 6/7 weeks' gestation. Her second and third abortions were complicated by incomplete evacuation of uterine contents, and uterine sacculation was diagnosed. Preterm delivery and a retained placenta complicated the present pregnancy. At laparotomy, recurrence of uterine sacculation was noted in the left fundal region of a bicornuate uterus. A hysterotomy was performed to remove the placenta from the sacculation. CONCLUSION: In a patient with multiple uterine procedures and retained placenta, uterine sacculation can develop.

Relations of gestational length and timing and type of incomplete pregnancy to ovarian cancer risk.

While the protective nature of parity with respect to ovarian cancer has been well documented, whether a history of incomplete pregnancy affects ovarian cancer risk is uncertain. Data collected from 739 epithelial ovarian cancer cases and 1,313 community controls in the Delaware Valley from 1994 to 1998 were used to evaluate the relation between gestational length and timing of first induced or spontaneous abortion and ovarian cancer risk. Incomplete pregnancy was not associated with ovarian cancer among nulliparous women or among ever-pregnant women either before or after adjustment for relevant confounders (for nulliparous women, odds ratio (OR) = 1.12, 95% confidence interval (CI): 0.66, 1.89; for ever-pregnant women, OR = 0.95, 95% CI: 0.76, 1.18). Among unigravid women, one full-term pregnancy was more protective than an incomplete pregnancy (adjusted OR = 0.29, 95% CI: 0.15, 0.57). These results were independent of the type of pregnancy loss. Among ever-pregnant women, a spontaneous abortion before a first birth provided significant protection (adjusted OR = 0.47, 95% CI: 0.30, 0.75), while no significant effect was found for an induced abortion prior to a first birth (adjusted OR = 0.80, 95% CI: 0.44, 1.47). These data do not support an independent association between incomplete pregnancies, either spontaneous or induced, and ovarian cancer risk.
