Echocardiographic diagnosis of ruptured aortic valve leaflet in bacterial endocarditis.

Aortic valve rupture, secondary to aortic valve endocarditis, was diagnosed echocardiographically and closely followed preoperatively. The ruptured left coronary cusp of the aortic valve was seen as dense irregular echoes, located anteriorly during ventricular diastole, and protruding into the left ventricular outflow tract in an otherwise normally appearing aortic valve. These echocardiographic findings, when correlated with changes in the clinical status of the patient, prompted immediate cardiac catheterization and aortic valve replacement. Early echocardiographic detection of abnormal aortic cusps and variation from normal aortic root echo features should alert the physician to proceed to cardiac catheterization, and aortic valve replacement if necessary.

Bacterial endocarditis with a tolerant staphylococcal organism.

An eight-year-old boy developed staphylococcal endocarditis. He was treated initially with nafcillin and gentamicin, but he remained febrile. The minimum bactericidal concentration (MBC) of nafcillin was 130 times the minimum inhibitory concentration (MIC). Following the addition of rifampin to the antibiotic regimen, the patient became afebrile and recovered. The discrepancy between the MBC and MIC indicated tolerance to nafcillin. Tolerance is a mode of antibiotic resistance that must be considered in treating life-threatening illness.

Septal myocardial abscess and infectious pericarditis in a case of bacterial endocarditis.

A case of acute aortic valve endocarditis is reported, in which the complications of pericarditis and myocardial abscess were diagnosed clinically. Two dimensional and M-mode echocardiography showed large echo-free spaces and a marked thickening of the interventricular septum which had not been detected previously, suggesting pericardial effusion and myocardial abscess. This is the first case in Japan to our knowledge, in which the pericarditis and myocardial abscess were detected preoperatively and successfully treated surgically.
