[From the history of wound care].

Wound care in ancient times was based on many techniques: Bandages soaked with antibiotics, Sutures continuous or in separate stitches, apply of poultices around the wounds, honey and propolis as antibiotics in the treatment of wounds, surgical drainage of pus with a piece of tin pipe etc. The oldest wound clamp is shown with the jaws of ants holding together the edges of a wound. Finally are discussed the cauterization and the principles of Antisepsis and Asepsis.

Allergic contact dermatitis in patients with anogenital complaints.

OBJECTIVE: Allergic contact dermatitis is a common anogenital disease. STUDY DESIGN: The results of patch testing performed on 1,008 patients evaluated for allergic anogenital contact dermatitis from 21 dermatologic departments in the Information Network of Departments of Dermatology (IVDK) from 1992 to 1997 were analyzed. RESULTS: A final diagnosis of allergic contact dermatitis was made in 351 patients (34.8%). Irritant contact dermatitis was diagnosed in 230 patients (22.8%) and other, nonspecified forms of perianal eczema in 228 patients (22.6%). The remaining cases were due to other distinct dermatologic diseases. Analyzing patch test data from this large group of patients showed that the allergen spectrum resembled that of all patients (54,000) tested from 1992 to 1997. However, dibucaine HCl ranked fourth among contact allergens in the study population. Positive reactions to (chloro-)-methyl-isothiazolinone and to benzocaine were observed more frequently among patients with anogenital complaints as compared to the total IVDK population (3.7% vs. 2.5% and 2.7% vs. 1.5%, respectively). In general, active agents of topical medications and popular remedies, preservatives and ointment bases appeared to cause allergic reactions most frequently. CONCLUSION: Patients with chronic anogenital diseases seem to have a high risk of becoming sensitized. For patch testing we recommend the standard series, dibucaine HCl, propolis, bufexamac and other ingredients from topical preparations according to the patient's history.

Contact allergy to balsam of Peru. II. Patch test results in 102 patients with selected balsam of Peru constituents.

BACKGROUND: In Switzerland, Germany, and Austria, allergic reactions to balsam of Peru (BP) have now made it the third most common contact allergen. OBJECTIVE: A series of 20 single BP constituents (including resorcinol monobenzoate), established in 1995, was used for patch tests in patients with a positive reaction to BP in the standard series. MATERIALS AND METHODS: Between 1995 and 1998, 2,273 patients were tested with the standard series, including BP, fragrance mix (FM), and propolis. Patients positive for BP were requested to participate in a further test using the 19 compounds of the BP constituents and resorcin monobenzoate (BP series); 102 patients agreed and were patch tested. The results of the 72-hour reading were used for the evaluation. RESULTS: A total of 93 patients reacted to 1 or more of the BP series compounds. Positive reactions were seen, in decreasing order, to cinnamic alcohol, cinnamic acid, coniferyl benzoate, benzoic acid, cinnamyl cinnamate, eugenol, resorcinol monobenzoate, coniferyl alcohol, and benzyl alcohol. There were no positive reactions to vanillin or ferulic acid. A correlation between skin lesions and frequent consumption of sweets was found in 7 patients with major positive test reactions to coniferyl benzoate and benzyl alcohol. Most of the reactions to eugenol and isoeugenol had less to do with BP itself than with a primary sensitization to fragrances. Although resorcin monobenzoate (RMB) has up to now not been detected in BP, 16 patients reacted distinctly to this compound. Eleven were strong smokers; the remaining ones had contact with plastic materials that have been reported to contain RMB. RMB is used frequently as an antioxidant in synthetic material. When these patients stopped smoking, the skin lesions cleared. However, consumption of sweets caused recurrences. CONCLUSION: The evaluation of reactions to single constituents of BP by testing with the special BP series facilitates understanding how sensitization may be acquired. The allergen may prove to be BP itself or 1 or more of its constituents. Testing for the constituents of this series may provide patients with a more specific allergen diagnosis and may facilitate improved therapy. BP may function as an important indicator for contact allergy to RMB.

Stimulatory effect of some plant extracts used in homeopathy on the phagocytosis induced chemiluminescence of polymorphonuclear leukocytes.

Some plant extracts on a large range of dilutions as used in Homeopathy were tested on the chemiluminescence emission produced by polymorphonuclear leukocytes. The high stimulatory action was noticed when extracts from Uvae Ursi and Saponaria were tested, as the classical effect exerted by zymosan was exceeded. A moderate stimulatory action comparable with that of zymosan was found when extracts from Echmaceea, Aleo and Prumis were used, as well as in the case of Propolis. The relationship between stimulatory effect and the concentration range is modulated as function of the extract source, several peaks being observed for some dilutions (Saponana), but generally no quantitative relations were obtained. By studying the time when a chemiluminescence peak was observed, it is possible to estimate wether the weight of the NADPH oxidase or myeloperoxidase pathways are involved in the stimulatory effect on polymorphonuclear leukocytes.

Caffeic acid phenethyl ester, an inhibitor of nuclear factor-kappaB, attenuates bacterial peptidoglycan polysaccharide-induced colitis in rats.

Caffeic acid phenethyl ester (CAPE) is an anti-inflammatory component of propolis (honeybee resin). CAPE is reportedly a specific inhibitor of nuclear factor-kappaB (NF-kappaB). The aims of our study were 1) to evaluate the effect of CAPE on cytokine production, NF-kappaB, and apoptosis in two cell lines; 2) to assess the effect of CAPE on NF-kappaB in rats with peptidoglycan-polysaccharide (PG-PS)-induced colitis; and 3) to evaluate the efficacy of CAPE against this colitis. In vitro experiments used rat macrophage (NR8383) and colonic epithelial cell (SW620) lines. NF-kappaB was evaluated by electrophoretic mobility shift assay. Cytokines and apoptosis were measured by enzyme-linked immunosorbent assay. Colitis was induced by intramural injections of PG-PS into the distal colon. CAPE (30 mg/kg) or vehicle was administered once daily to rats by intraperitoneal injection, for 1 week. Various macroscopic and biochemical indices were measured on day 21. CAPE (30 microg/ml) significantly inhibited NF-kappaB and TNF-alpha production in the macrophage cell line. In macrophages, CAPE significantly increased DNA fragmentation. CAPE exhibited generally similar effects in the colonic epithelial cell line. CAPE treatment reduced the mean level of colonic NF-kappaB in rats. CAPE also induced a significant reduction in gross colonic injury. Moreover, colonic cytokine levels (TNF-alpha and IL-1beta) were significantly reduced in CAPE-treated rats. In summary, CAPE inhibits NF-kappaB, causes a reduction of pro-inflammatory cytokine production, and induces apoptosis in macrophages. These mechanisms likely contributed to the attenuation of PG-PS-induced colitis by CAPE.

The use of some immunomodulators and non-antibiotic drugs in a prophylaxis and treatment of mastitis.

Antibiotics are widely used in treatment and prophylaxis of mastitis, but their antibacterial and therapeutic efficacy continues to decrease. New methods of prophylaxis of mastitis with the use of vaccines and biological response modifiers are reviewed. Prophylactic activity of staphylococcal vaccines should be investigated in the future because of difficulties in successful treatment of infections caused by these organisms. Vaccination with J5 bacterin (0111:B4) reduced duration of IMI and local signs of clinical coliform-mastitis. Some immunomodulators proved to be effective in treatment of mastitis during lactating period. Promising results were obtained with the use of giseng saponin, herbal gel, herbal extracts, propolis, lysosubtilin, antibacterial proteins, and lysozyme dimer especially.

Growth inhibition and modulation of antigenic phenotype in human melanoma and glioblastoma multiforme cells by caffeic acid phenethyl ester (CAPE)

The active component of the honeybee hive product propolis, caffeic acid phenethyl ester (CAPE), has been shown to display increased toxicity toward various oncogene-transformed cell lines in comparison with their untransformed counterparts (Su et al., 4: 231-242, 1991). This observation provides support for the concept that it is the transformed phenotype which is specifically sensitive to CAPE. In the present study, we have determined the effect of CAPE on the growth and antigenic phenotype of a human melanoma cell line, HO-1, and a human glioblastoma multiforme cell line, GBM-18. For comparison, we have also tested the effects of mezerein (MEZ), mycophenolic acid (MPA) and retinoic acid (RA), which can differentially modulate growth, differentiation and the antigenic phenotype in these human tumor cell lines. Growth of both cell lines was suppressed by CAPE in a dose-dependent fashion, with HO-1 cells being more sensitive than GBM-18 cells. The antiproliferative effect of CAPE was enhanced in both cell types if CAPE and MEZ were used in combination. Growth suppression was associated with morphological changes in H0-1 cells, suggesting induction of a more differentiated phenotype. CAPE also differentially modulated the expression of several antigens on the surface of the two tumor cell lines. These results suggest a potential role for CAPE as an antitumor agent, an antigenic modulating agent and possibly a differentiation inducing agent.

[History and uses of honey, mead and hive products].

From Antiquity to marketing of cane sugar, and beet sugar in particular, honey was greatly consumed for its sweetening power; it was also employed for its therapeutic properties. Actually, it is always used in feeding and dietetic, and only as popular remedy. The hive products: pollen, propolis, royal jelly, introduced for few decades in dietetic meet show a great commercial passion; the beeswax was excipient in beauty care. These different aspects are developed in this study.

[Most frequent causes of allergic contact dermatitis in farmers: based on material collected in the Nofer Institute of Occupational Medicine, Lodz].

Farmers are exposed to many skin damaging factors. Following dermatological and allergological examinations, in a group of 132 farmers with suspected occupational disease, contact allergy induced by metals (chromates, nickel, cobalt, mercury), disinfectants and their components (formaldehyde, glutaraldehyde, quaternary ammonium compounds, lysol, chloramine), rubber and pesticides, especially pesticides of thiophtalimide and thiocarbamate groups were recorded. Farmers also reacted to other chemical compounds (colophony, lanolin, propolis) rarely responsible for hypersensitivity in other occupational groups. In addition, extensive areas of dermatitis (13%) induced by irritating chemical (fertilizers, pesticides, fodder for breeding animals) and physical (changing temperature, humidity, effect of solar radiation) factors were observed in the study group.

[Microbiologic characteristics of wound infectious process in use of ion-exchange sorbents].

In Wistar rats with modified Danilina's method a purulent wound was simulated. The animals were divided into 5 groups of treatment. In the study groups applications on the wound of ion-exchange sorbents of natural origin (ceolyt including its modification with 1% silver, multicomponent ointment with bentonit, poviargol, propolis and metronidazol) were used. In the control groups gauze bandages and polyethylenoxyde were applied. When modified ceolyt and the ointment were used, earlier reduction of general contamination lower to critical level was seen. Ceolyt modified with silver demonstrated higher antimicrobic effect on gramnegative microflora compared with ointment composition. Ceolyt and its silvered modification were studied in vitro. Zones of growth absence were detected in places of application of silvered ceolyt that testifies to antimicrobic activity of silvered composition.

[Antifungal effect in selected natural compounds and probiotics and their possible use in prophylaxis of vulvovaginitis].

OBJECTIVE: a survey of effective natural compounds and probiotics with antifungal activity and their possible use in vulvovaginitis as a prophylactic or therapeutic drug. DESIGN: review of literature. SETTING: Charles University at Prague, Pharmaceutical Faculty at Hradec Kralove. METHODS: and review of literature and publications from medical data-bases. CONCLUSIONS: certain natural compounds were proved to exert antimycotic effects. The efficiency of garlic extract under in vitro conditions against yeast of C. albicans family becomes manifest within one hour. Essential oils present in the Cayenne pepper (Capsodum frutescens) exert considerable antifungal activity against C. albicans as well. Clove exerts lower antimycotic effects. The effects of Chinese leek are similar to garlic. Propolis also possesses antifungal properties, and sensitivity of the yeast strains decreases in the following order: C. albicans > C. tropicalis > C. crusei > C. guilliermondii. Its efficiency depends on the composition and place of origin. Tea trea oil (Melalauca alternifolia) is inhibiting strains of Candida family (genus) in minimal inhibiting concentration, higher concentrations are necessary for C. crusei and C. norvengensis. In the prophylaxis of vaginal candidoses, oral administration of the strains of Lactobacilus rhamnosus GR-1 and L. fermentum RC-14, B-54 may be recommended. Moreover there are suitable strains of L. brevis CD-2, L. salivarius FV-2, L. gasseri MB 335, L. plantarum, useful in the form of gel capsules (L. plantarum) or vaginal tablets (L. gasseri). The strains of L. acidophilus CRL 1294 and L. salivarius CRL 1328 display the capacity of self aggregation and co-aggregation with Candida. It should be pointed out that the probiotic strain of Lactobacillus (especially L. casei and L. rhamnosus) may, in exceptional cases, cause bacteriemia and sepsis, especially in immunodeficient patients and those who underwent surgery of alimentary tract. No protective effect against yeast infection was detected with vitamin E, any positive effect was not unequivocally confirmed with zinc, but adjusting zinc levels to normal may be recommended.

In vivo effects of caffeic acid phenethyl ester on myocardial ischemia-reperfusion injury and apoptotic changes in rats.

Ischemia/reperfusion (I/R) has been reported to induce apoptotic cellular death in myocardium. This study tested the hypothesis that caffeic acid phenethyl ester (CAPE), one of the active components of propolis, may ameliorate myocardial apoptosis and oxidative myocardial injury. Wistar rats were divided into 4 groups: (i) sham operated, (ii) I/R, (iii) I/R+CAPE, and (iv) I/R+glutathione (GSH). CAPE (10 micromol/kg) was infused iv 10 min before occlusion of the left anterior descending coronary artery (30 min) followed by reperfusion (120 min). GSH (5 mg/kg) was infused iv after the occlusion and immediately before reperfusion. The TdT-mediated in situ nick end-labeling (TUNEL) method was used to evaluate apoptotic activity. I/R resulted in myocardial apoptosis, alterations of antioxidant status, elevation of serum creatine kinase (CK) and aspartate aminotransferase (AST) activities, evidence of lipid peroxidation, and elevated nitric oxide levels, compared to the sham-operation group. No apoptotic cells were found in the myocardial tissue of sham-operated rats. The TUNEL-positive myocardial cells averaged 60%, 30%, and 40% in the I/R, I/R+CAPE, and I/R+GSH groups, respectively. This study demonstrates that pretreatment with CAPE provides cardio-protection from I/R injury. The I/R+CAPE group showed reduced apoptosis, attenuated NO production, elevated myocardial superoxide dismutase (SOD) activity, and diminished serum CK and AST activities, compared to the I/R group.

Effect of various natural medicinals on salivary protein putrefaction and malodor production.

OBJECTIVE: Salivary incubation assays are commonly used in oral malodor studies. Using an in vitro model system, the effect of various natural medicinals (i.e., echinacea, propolis, elder, mastic gum, marigold, sage, lavender, thyme, and chamomile) on salivary protein putrefaction and malodor production was examined. METHOD AND MATERIALS: Malodor production levels were scored by an experienced odor judge. Volatile sulfide levels were measured using a sulfide monitor (Halimeter), and salivary protein degradation was determined densitometrically following electrophoresis on polyacrylamide gel (SDS-PAGE). Microbial population was evaluated by viable counts and microscopy. RESULTS: Whereas all of the various medicinals caused some reduction in malodor production from the incubated whole saliva, echinacea and lavender were the most effective. CONCLUSION: The bioassay utilized in the present study suggests that these herbs may inhibit oral malodor production.

Research in the field of antiviral chemotherapy performed in the "Stefan S. Nicolau" Institute of Virology.

A brief review is made of the research in the field of antiviral chemotherapy performed in the "Stefan S. Nicolau" Institute of Virology during the 35 years since its foundation. The investigations have mainly focused on influenza and herpes virus, but the chemotherapy of other viral infections (mumps, vaccinia, Coxsackie, etc.) has also been approached. Most of the chemotherapy agents assayed have been represented by natural preparations: immunoglobulins, interferon, hormones, vitamins, plant extracts (garlic, horse radish), bee products (propolis, royal jelly); attempts have also been made with numerous synthetic compounds. Stress is laid on the preparations already tested with a view to application in human clinic, and the prospects of chemotherapy research in the Institute of Virology are discussed.

[Effect of pinocembrin on the course of experimental candida infections in mice].

Pinocembrin (5,7-dihydroxy-flavanon)--a component of the bee product propolis--was tested for its in vivo activity against Candida albicans in mice. It was shown that the intravenous infection of AB-Jena mice with 2.5 X 10(5) Candida albicans cells was a very suitable model. Despite of treatment with pinocembrin at daily doses of 100 mg/kg body weight the animals as well as the controls died between the 6th and 24th day after beginning. On the other hand the animals treated with 5 mg/kg amphotericin B survived the test-period of 30 days. The question of effectiveness of pinocembrin in vivo should be cleared up in further pharmacokinetic investigations.
