Improved automatic analysis of ambulatory blood pressure data based on precise detection of individual night-time from diurnal profile of heart rate.

BACKGROUND: Software programs sold with ambulatory blood pressure monitoring (ABPM) devices are designed to use some set 'typical' night-time (e.g. 2300-0700) to estimate daytime/night-time blood pressure (BP) with limited accuracy. Alternative use of individual periods of sleep/wakefulness from patient diaries is time consuming and subjective. We developed a simple mathematical algorithm for the detection of the 'night-time' as a period of low values in diurnal profiles of heart rate (HR) allowing accurate automatic analysis of daytime/night-time blood pressure. To test this technique we designed a software application allowing automatic analysis of ABPM data based on the different night-time definitions, including the developed algorithm and compared reproducibility of the degree of BP dipping produced by the different methods across two days of 48-h ABPM. METHODS: A 48-h ABPM study was performed in 33 patients with uncomplicated stage II hypertension. Means and standard deviations (SD) of the differences in the degree of BP dipping between two 24-h periods of 48-h ABPM were obtained separately for three methods of night-time definition: automatic detection from individual HR profiles, fixed 2300-0700 h interval and sleep time from patient diaries. RESULTS: Reproducibility of the BP dip estimation across 2 days of BP monitoring was significantly better for night-time detected from individual HR profiles than for the fixed 2300-0700 h interval or sleep time from diary. The SD of the differences was 6.7/8.2 compared with 13.5/18.3 and 13.0/14.8 respectively (systolic BP/diastolic BP, mmHg). CONCLUSIONS: Implementation of the developed method of night-time definition may significantly improve automatic analysis of ABPM data.

Soarian--workflow management applied for health care.

OBJECTIVES: To describe and comment on functionality and architecture of the software product Soarian developed by Siemens, to identify key differentiators to related products, and to comment on predecessor systems and beta versions. This has been done in the framework of a conference on health information systems of the IMIA. METHODS: Analyzing existing literature. Site visit of a predecessor system at Haukeland Sykehus, Bergen. Pilot of a beta version at the Erlangen University Medical Center, elaborating on major characteristics in discussion rounds. RESULTS: Soarian is a functional comprehensive, clinically oriented software product to support health care processes and to be used for health care professional workstations. It is a software product, designed and written completely new. Three major key differentiators were identified in comparison to related software products: Soarian's workflow engine, its embedded analytics, and its 'smart' user interface. The targeted reduced installation time is stated to be 12 months or less. CONCLUSIONS: Soarian has good chances to become one of the major software products for health care professional workstations in the international market to support patient-centered, shared care. Its global design may help to better support and maintain national or language specific versions. The first installations of Soarian will be critical, as they will show how the system will be accepted. To use such software products efficiently, organizational aspects within hospitals as well as between health care institutions have to be considered, e.g. strategic IT planning.

Local elevation: a method for improving the searching properties of molecular dynamics simulation.

The concept of memory has been introduced into a molecular dynamics algorithm. This was done so as to persuade a molecular system to visit new areas of conformational space rather than be confined to a small number of low-energy regions. The method is demonstrated on a simple model system and the 11-residue cyclic peptide cyclosporin A. For comparison, calculations were also performed using simulated temperature annealing and a potential energy annealing scheme. Although the method can only be applied to systems with a small number of degrees of freedom, it offers the chance to generate a multitude of different low-energy structures, where other methods only give a single one or few. This is clearly important in problems such as drug design, where one is interested in the conformational spread of a system.

A visual probe localization and calibration system for cost-effective computer-aided 3D ultrasound.

The 3D ultrasound systems produce much better reproductions than 2D ultrasound, but their prohibitively high cost deprives many less affluent organization this benefit. This paper proposes using the conventional 2D ultrasound equipment readily available in most hospitals, along with a single conventional digital camera, to construct 3D ultrasound images. The proposed system applies computer vision to extract position information of the ultrasound probe while the scanning takes place. The probe, calibrated in order to calculate the offset of the ultrasound scan from the position of the marker attached to it, is used to scan a number of geometrical objects. Using the proposed system, the 3D volumes of the objects were successfully reconstructed. The system was tested in clinical situations where human body parts were scanned. The results presented, and confirmed by medical staff, are very encouraging for cost-effective implementation of computer-aided 3D ultrasound using a simple setup with 2D ultrasound equipment and a conventional digital camera.

New method for rapid characterization of molecular shapes: applications in drug design.

We present a method for the rapid quantitative shape match between two molecules or a molecule and a template, using atom triplets as descriptors. This technique can be used either as a rapid screen preceding the computationally expensive shape-based docking method developed by Kuntz and co-workers or as a stand-alone method to rank compounds in a large database for their fit to a shape template. The merits and limitations of this method are discussed in detail with examples.

Automated pharmacophore identification for large chemical data sets.

The identification of three-dimensional pharmacophores from large, heterogeneous data sets is still an unsolved problem. We developed a novel program, SCAMPI (statistical classification of activities of molecules for pharmacophore identification), for this purpose by combining a fast conformation search with recursive partitioning, a data-mining technique, which can easily handle large data sets. The pharmacophore identification process is designed to run recursively, and the conformation spaces are resampled under the constraints of the evolving pharmacophore model. This program is capable of deriving pharmacophores from a data set of 1000-2000 compounds, with thousands of conformations generated for each compound and in less than 1 day of computational time. For two test data sets, the identified pharmacophores are consistent with the known results from the literature.

Evaluation of a new PCR assay with competitive internal control sequence for blood donor screening.

BACKGROUND: High-throughput nucleic acid testing for transfusion-relevant viruses by PCR requires contamination-proof methods with high sensitivity and validity. A new PCR reagent kit (TaqMan, PE BioSystems) reduces the risk of carry-over contamination by eliminating post-PCR processing. STUDY DESIGN AND METHODS: Oligonucleotide design was done with software specialized for designing the assays' (TaqMan) primers and probes. A template-derived competitive internal control sequence designed through site-directed mutagenesis was used to reveal failures in amplification. Assay sensitivity was determined for single-donor and single-patient testing and by spiking sample mini-pools. Three seroconversion panels were tested. RESULTS: Sensitivity is high, reaching 300 HBV genomes per mL of single-patient material on direct testing. A detection limit of 1000 HBV genome equivalents per mL of donor plasma is achieved for 96 pooled samples. The window period for HBV infection was reduced by 17, 10, and 63 days from that for HBsAg screening in three seroconverting donors. CONCLUSION: The assay provides sufficient sensitivity to be superior to HBsAg screening in transfusion medicine and will be useful in clinical laboratories because of its ease of handling.

Integrated minimum-set primers and unique probe design algorithms for differential detection on symptom-related pathogens.

MOTIVATION: Differential detection on symptom-related pathogens (SRP) is critical for fast identification and accurate control against epidemic diseases. Conventional polymerase chain reaction (PCR) requires a large number of unique primers to amplify selected SRP target sequences. With multiple-use primers (mu-primers), multiple targets can be amplified and detected in one PCR experiment under standard reaction condition and reduced detection complexity. However, the time complexity of designing mu-primers with the best heuristic method available is too vast. We have formulated minimum-set mu-primer design problem as a set covering problem (SCP), and used modified compact genetic algorithm (MCGA) to solve this problem optimally and efficiently. We have also proposed new strategies of primer/probe design algorithm (PDA) on combining both minimum-set (MS) mu-primers and unique (UniQ) probes. Designed primer/probe set by PDA-MS/UniQ can amplify multiple genes simultaneously upon physical presence with minimum-set mu-primer amplification (MMA) before intended differential detection with probes-array hybridization (PAH) on the selected target set of SRP. RESULTS: The proposed PDA-MS/UniQ method pursues a much smaller number of primers set compared with conventional PCR. In the simulation experiment for amplifying 12 669 target sequences, the performance of our method with 68% reduction on required mu-primers number seems to be superior to the compared heuristic approaches in both computation efficiency and reduction percentage. Our integrated PDA-MS/UniQ method is applied to the differential detection on 9 plant viruses from 4 genera with MMA and PAH of 11 mu-primers instead of 18 unique ones in conventional PCR while amplifying overall 9 target sequences. The results of wet lab experiments with integrated MMA-PAH system have successfully validated the specificity and sensitivity of the primers/probes designed with our integrated PDA-MS/UniQ method.

EView--a workstation-based viewer for intensive clinical electroencephalography.

We describe a workstation-based EEG viewer that satisfies the requirements for high-volume clinical EEG review. It currently supports two 24-h four-bed epilepsy monitoring units and a network of routine, intraoperative, and intensive care monitors, providing paperless review of over 5000 pages/day by a staff of clinical neurophysiologists. The design takes advantage of the development environment, processing and graphics resources, and networking capabilities of high-performance UNIX workstations. The user interface and operational infrastructure have been tailored to the demands of clinical neurophysiologists requiring rapid access to large volumes of EEG waveform data in a large clinical neurophysiology laboratory. This paper describes the functional requirements, system architecture, and implementation details of the current system.

The primer generator: a program that facilitates the selection of oligonucleotides for site-directed mutagenesis.

Site-directed mutagenesis is a powerful tool that has enabled molecular biologists to perform functional analysis of altered nucleic acids and proteins. Newer PCR-based mutagenesis techniques have reduced the process of mutagenesis to as little as one day. While each technique has its advantages, both require a strategy to isolate the desired clone from a population that contains mutagenized and wild-type genes. In this report, we describe a World Wide Web-based computer program that facilitates the design of mutagenic primers such that successfully mutagenized clones can be identified by the presence or absence of a unique restriction site.

A fast communication aid for non-verbal subjects with severe motor handicaps.

A computer-based communication system for non-verbal persons with severely deficient coordination of the upper extremities is described. The device doubles as an environmental control unit. Modular design is chosen throughout. Commercially available subunits are used to reduce cost. Input is by a 3 x 3 keyboard, when the user has some command over his hands. Otherwise a binary 'autoscan' access mode is used, which is controlled by repeated closures of a single binary switch. Output is in written form on a display or acoustical by synthesized speech. Hard-copy output can also be provided. Remote communication is possible by modem or by synthesized speech. Speed of message generation is increased by establishing a vocabulary, which contains, in addition to signs and letters of the alphabet, whole words and stereotyped phrases. The data blocks of the latter are accessed by a single input cycle similar to that used for the selection of isolated letters, symbols, etc.

[A 3D brachytherapy treatment planning system].

Brachytherapy dose prescription and treatment playing lag behind the state-of-the-art for external beam therapy. We have developed a computer program on a SGI Indigo2 graphics workstation to plan the loading or stereotactic implantation of radioactive seeds. The treatment planning system, using data from the pre-treatment diagnostic CT scan or MRI scan, contours target areas on sequential images creating a 3-dimensional target volume, and is based on a dose algorithm of TG-43 formalism. The treatment isodose contour is generated at each scanningplane, reconstructed sagittal and coronal views, and overlaid on the corresponding target contour for 3-dimensional coverage visualization. Volume dose histograms are generated for the target and neighboring normal structures. We also provide two orthogonal digitally reconstructed projected images identifying virtual source positions superimposed on for fluoroscopy guided loading applicators and sources.
