Altered PABA pharmacokinetics in cystic fibrosis. Implications for bentiromide test.

The bentiromide test has been proposed as a useful noninvasive method for assessing exocrine pancreatic function in cystic fibrosis (CF) patients. Following oral administration, this peptide is selectively cleaved by pancreatic chymotrypsin liberating PABA which is passively absorbed. Recent studies have suggested that PABA measured in plasma is superior to the more established method of estimating urinary recovery of this marker. However, in using the plasma test in CF patients, one makes the assumption that the PABA marker has similar distribution and elimination patterns in normal and CF subjects. Since many drugs display altered pharmacokinetics in CF patients, we studied the disposition of PABA following ingestion of free PABA in six controls (age 19-28 years) and 18 CF patients (13-18 years; seven steatorrheic and 11 nonsteatorrheic). Elimination of T1/2 of PABA was significantly shorter in CF patients (58 +/- 21 min) compared to controls (93.5 +/- 28) (P less than 0.005). PABA clearance was similar in the control and CF patients (2.99 +/- 1.21 and 3.27 +/- 1.02 ml/min/kg, respectively). PABA distribution volume was smaller, although not significantly so, than in the controls (268 +/- 107 vs 376 +/- 140 ml/kg). Good correlation was found between PABA distribution volume and T1/2 (r = 0.51 P less than 0.02). Our simulation data suggest that altered pharmacokinetics of PABA in CF patients would cause their PABA levels to be 7% lower than controls at 90 min, 18% at 120 min, 29% at 150 min, and 38% at 180 min.(ABSTRACT TRUNCATED AT 250 WORDS)

Activity of serum alpha-amylases in cystic fibrosis.

A new method which uses a differential inhibitor to measure pancreatic and salivary type alpha-amylases (EC 3.2.1.1) was applied to serum samples from 46 cystic fibrosis (CF) patients (age range 4-14 years) and 50 controls of the same age group. The levels of pancreatic type amylase were lower in the CF patients (median 26.5 I.U./1) than the controls (median 81.5) (P less than 0.001). The results for salivary-type enzyme, however, did not support the previously reported finding of higher than usual levels in CF patients. This discrepancy is probably due to differences in analytical methods. It is felt that this procedure will be of value in the investigation of patients for cystic fibrosis and other pancreatic disorders.

Isozymes of ribonuclease in human serum and urine. II. A survey of patients with cystic fibrosis.

A comparison of serum and urine RNase isozymes from CF patients, heterozygotes, and control subjects demonstrated that the major RNase isozymes are not abnormal in CF. Some preliminary evidence indicated that RNase activity of the pancreatic extracts administered to CF patients may appear in the urine and result in a slightly different urine isozyme pattern. In addition, the mean serum RNase activities of the CF patients and heterozygotes were significantly greater than the mean activity of the controls, but there was much overlap in values between the groups.

Serum pancreatic lipase activity in cystic fibrosis.

Patients with cystic fibrosis have been found to have abnormal serum concentrations of immunoreactive trypsin and abnormal activities of pancreatic isoamylase. A study was undertaken to discover whether activity of pancreatic lipase is also altered in cystic fibrosis. Serum from 23 patients with cystic fibrosis was assayed for immunoreactive trypsin and pancreatic lipase. Median serum pancreatic lipase activity was significantly lower in patients with cystic fibrosis than in controls, as was immunoreactive trypsin concentration (p less than 0.0001). Some patients had supranormal lipase concentrations but these were not always associated with absence of malabsorption. Serum pancreatic lipase activity is considerably changed in cystic fibrosis.

[Choice of the method of pancreatodigestive anastomosis after pancreatoduodenal resection].

Immediate results of 129 pancreatoduodenal resections were analyzed. All the patients were divided into 3 groups depending of the diameter of the pancreatic duct and the degree of fibrosis in pancreatic parenchyma. Two original methods of pancreatojejunostomy were used: end-loop and invaginated one. There is a high risk of pancreatic stump-dependent postoperative complications in patients with a narrow duct and loose pancreatic parenchyma. Original invaginated anastomosis reduced the rate of these complications to the number in the group of patients with a wide duct and dense parenchyma. This method has decreased postoperative lethality from 11.3 to 3.0%.
