We report on a male infant born with clinical and radiographic evidence of a lethal form of dyssegmental dysplasia not comparable to Silverman-Handmaker type, who had a prolonged survival of more than eight months. He had ocular and central nervous system abnormalities which have not been previously described. His course included significant feeding and respiratory difficulties, severe physical and psychomotor retardation, and recurrent fever of unknown etiology believed to be of central origin. The relatively long survival of this infant enabled us to focus on the natural history of this rare syndrome. The infant was born to first cousin parents of Druze Lebanese origin supporting an autosomal recessive mode of inheritance for the condition. This is the first documentation of dyssegmental dysplasia Silverman-Handmaker type in a family of Druze Lebanese ethnicity.

We describe the successful management of an elderly husband and wife with Escherichia coli O157:H7 associated hemolytic uremic syndrome (HUS) treated with aggressive therapeutic plasma exchange (TPE) with replacement with fresh frozen plasma. Following twelve TPEs (three 1.5 volume; nine 1 volume), the husband's platelet count increased from 45 x 10(9)/L to 183 x 10(9)/L. Following ten 1.5 volume TPEs, the wife's platelet count increased from 30 x 10(9)/L to 193 x 10(9)/L. This is the first known occurrence of E. coli O157:H7 associated HUS in an elderly husband and wife successfully treated with aggressive TPE. We conclude that early, aggressive TPE should be considered and may be life-saving for E coli O157:H7 associated HUS in the elderly.
A Kurdish family had two children affected with Robinow syndrome. The daughter had short stature, macrocephaly, hypertelorism, hepatosplenomegaly, short forearms and marked vertebral anomalies. Her brother had hypertelorism, hypertrophied alveolar ridges, hepatosplenomegaly, short forearms, rib anomaly and ambiguous genitalia. The karyotype of the affected male sibling showed mosaicism for 45X, 46,X,dicY(q11.22), 47,X,dicY(q11.22),dicY(q11.22).
Congenital cerebellar ataxias are a heterogeneous group of non-progressive disorders characterized by hypotonia and developmental delay followed by the appearance of ataxia, and often associated with dysarthria, mental retardation, and atrophy of the cerebellum. We report the mapping of a disease gene in a large inbred Lebanese Druze family, with five cases of a new form of non-progressive autosomal recessive congenital ataxia associated with optic atrophy, severe mental retardation, and structural skin abnormalities, to a 3.6-cM interval on chromosome 15q24-15q26.
A severe hereditary hemorrhagic diathesis in Simmental cattle has been identified in North America. Platelet numbers and coagulation profiles of affected cattle are normal. We have further characterized the severe dysfunction of platelet aggregation. All agonists tested elicited normal shape change. Aggregations in response to ADP, A23187, and collagen were absent. Aggregations were decreased or required more time for completion in response to PAF and thrombin. No ultrastructural abnormalities were observed in transmission electron micrographs. Dense granule release of ATP in response to PAF was normal. Thrombin-induced aggregation was dependent upon external calcium concentration in normal but not affected animals. Clot retraction in the blood from affected animals was abnormal. The data implicate a defect of Ca++ mobilization or utilization.
Density functional theory has been used to probe the bonding and electronic properties of the homo- and heterobimetallic sp carbon chain complexes (ML(m), = (eta(5)-C(5)R(5))(eta(2)-R(2)PCH(2)CH(2)PR(2))Fe, (eta(5)-C(5)R(5))(NO)(PR(3))Re; z = 0-4). All neutral complexes are best described by MCtbd1;CCtbd1;CM electronic structures, in accord with much experimental data. The singlet dications are best described by cumulenic (+)M=C=C=C=C=M(+) valence formulations. However, the diiron and rhenium/iron dications are found to possess triplet states of nearly identical energy, clarifying experimental magnetic data. Their electronic structures have dominant *(+)MCtbd1;CCtbd1;CM(+)* character, with some spin delocalization onto the carbon chain. The mixed valence monocation radicals exhibit delocalized unpaired electrons, in accord with class III (strongly coupled) and II (weakly coupled) assignments made from experimental data earlier, with some spin density on the carbon chain. An isolable diiron trication has a doublet ground state, but some computational data suggest a close-lying quartet. For the unknown diiron tetracation, a bis(carbyne) or (2+)Fetbd1;CCtbd1;CCtbd1;Fe(2+) electronic structure is predicted. Calculated adiabatic ionization potentials show the iron endgroup to be more electron-releasing than rhenium, in accord with electrochemical data. This polarizes the electronic structures of the rhenium/iron complexes. To help validate the computed model structures, crystal structures of ((eta(5)-C(5)Me(5))Fe(eta(2)-dppe))(2)(mu-C(4)) and [((eta(5)-C(5)Me(5))Fe(eta(2)-dippe))(2)(mu-C(4))](3+) 3PF(6)(-) are determined. Data are analyzed with respect to related diruthenium and dimanganese complexes.

In this study first published in 1972, 218 relatives of 28 schizophrenic and a control group of 34 brain-damaged patients were interviewed and tested with the original Rorschach inkblot cards. In contrast to Rorschach surveys in the general population, the study revealed schizophrenia-like test results in approximately one quarter of the subjects, equally distributed among the relatives of schizophrenic and of brain-diseased patients. On the basis of individual and family history, genetic and psychodynamic hypotheses for these findings are discussed. The study clearly demonstrates that psychologically healthy and well-integrated persons may react in the Rorschach test as schizophrenic patients often do. In addition, neither these subjects nor their offspring show an increased incidence for a schizophrenic psychosis. It is concluded that a schizophrenia-like protocol is not an indicator for a "latent" schizophrenic psychosis.

Four siblings with achalasia, alacrimia and other problems involving the autonomic nervous system involvements are reported. Achalasia and alacrimia were present in all of them. Their parents are first cousins and have four other healthy children. Electrophysiological tests showed that autonomic dysfunction has progressed with age. Blood cortisol levels were normal in all four affected children. Depending on those findings of our case and previous reports, we conclude that triple-A syndrome and achalasia, alacrimia with or without neurological abnormalities could be variable manifestations of the same autosomal recessive gene defect.
A formula is presented for the calculation of the mean number of lethal and abnormal equivalents per person. It has been applied to Brazilian, French, and Japanese data.
